ABSTRACT

@#With an ageing population, the number of pre-frail and frail elderly in the community is on the increase. Favourable policies will encourage and enable elderly to continue to be engaged and cared for, with appropriate services in the community, for the community, and by the community. This article provides an overview of suitability of currently available community centre-based care services in Singapore based on the Clinical Frailty Scale, to work towards a frailty-friendly community. Keys to building a successful frailty-friendly community will also be discussed.

Home care is an integral part of primary community care. It is a multi-disciplinary holistic service providing personalised care to patients who are homebound. This encompasses both the medical and psychosocial aspects and allows patients to be treated in the comfort of their home, with reliable support from healthcare professionals, the community and caregiver. This article seeks to explore and discuss the provision of home care in Singapore.

ABSTRACT

Concept of coordinated care delivery is clearly defined,before a comparative analysis is made between such a concept and the integrated care delivery concept which is popular overseas and similar.On such basis,a theory framework is established for building and implementing such care delivery in line with the integrated theory of care delivery systems.Coordinated care delivery system now in place in China aims at rationally delivering medical resources,services and patients to primary institutions,by means of efficiently distributing resources and service systems and integrating care delivery flows,thus offering patients with cost effective and appropriate services and optimizing the care delivery system.

ABSTRACT

This paper summarized practices and experiences of other countries in coordinated care delivery system building,and described the care delivery systems used in various healthcare fundraising patterns.It is found that most countries have defined duties and service coverage of healthcare institutions at all the levels and measures to ensure rational patient flow.In the end,the paper concluded found experiences of these countries and inspirations for China.

ABSTRACT

Authors described the patterns,core measures and mechanisms of coordinated care delivery in the localities and analyzed initial outcomes of this practice.It is found that such practice has optimized distribution of regional healthcare resources,implemented first visit to primary institutions, and the care delivery to separate emergency from outpatient service.In addition,the paper analyzed problems found in localities in terms of policy,public hospitals and primary institutions,and proposed elements required for coordinated care delivery system to succeed,such as enhanced policy support, enhanced multi-departmental coordination,and public hospital reform to explore feasible approaches.

ABSTRACT

Coordinated care delivery is faced with various challenges to carry out in China,which are major decisions to make for decision makers,including how to push forward,how to deliver resources, services and patients to primary institutions,and how to ease the difficulties and affordability of seeing doctors.Based on analysis of roadblocks in coordinated care delivery in China,the authors provided near-term and long-term policy recommendations on the system,including how to build a scientific and reasonable healthcare delivery system,coordinated care delivery by stages,building incentives and constraint mechanisms,and enhancing public awareness and patient-doctor education.

ABSTRACT

A gestão estratégica é introduzida nos hospitais franceses no final da década de oitenta, num contexto de ineficácia e necessidade de controle das despesas de saúde. Este conceito tem inspirado uma série de reformas do governo, da organização e da dinâmica dos hospitais e do modo de regulação do sistema de saúde francês, pressupondo uma verdadeira mudança da cultura profissional. As mudanças no contexto econômico e no modo de financiamento dos hospitais, no comportamento dos usuários, o envelhecimento da população, o desenvolvimento das patologias crônicas e a intensificação da concorrência, constituem novos desafios a serem enfrentados. O envolvimento dos atores é fundamental e a gestão estratégica, baseada na mobilização dos atores, torna-se, hoje, um modo de administração particularmente adaptado à área da saúde. Este artigo tem por objetivo apresentar o impacto do conceito de gestão estratégica na evolução do meio hospitalar francês e as consequências desta evolução sobre as perspectivas de difusão da gestão estratégica no setor da saúde. Para isso, examina-se a evolução do contexto francês até 2010, os novos desafios que os hospitais franceses devem enfrentar e, por fim, as consequências destes desafios sobre a maneira de conceber os serviços, de administrar as relações entre os atores do sistema de saúde e de organizar o funcionamento operacional dos hospitais.


The strategic management was introduced in French hospitals in the late 80s, in a context of inefficiency and the need to control healthcare expenditure. This concept has inspired a series of government reforms, the organization and dynamics of hospitals and the mode of regulating the French health system, assuming a real change of professional culture. The changes in the economic context and in the finance of hospitals, the behavior of users, the population aging, the development of chronic diseases and increased competition, are the new challenges to be faced. The involvement of stakeholders is crucial and the strategic management, based on mobilizing these actors, is today, a method of administration particularly well adapted to the health area. This article aims to present the impact of the concept of strategic management in the evolution of French hospitals and the consequences of these developments on the outlook for the dissemination of strategic management in the health sector. To do so, it is examined the evolution of the French context until 2010, the new challenges that French hospitals must face and, finally, the consequences of these challenges on how to design services, manage the relationships between the actors of the health system and organize the operational functioning of hospitals.
