@#Introduction: The social lockdown measures imposed to contain the COVID-19 pandemic, have had profound effects on the healthcare systems across the world and India has been no exception to it. The study was aimed to evaluate the impact of COVID-19 on orthopaedic practice in India during the lockdown period and assess the preparedness of orthopaedic surgeons for resuming clinical practice after the initial lockdown was lifted. Materials and Methods: An online survey of 35 questions was conducted to evaluate impact on (i) general orthopaedic practice (ii) hospital protocols (iii) out-patient practice (iv) surgical practice (v) personal protective equipment (PPE) use and (vi) post-lockdown preparedness. Results: A total number of 588 practising orthopaedic surgeons from India completed the survey. Majority (88.3%) found severe impact (>50%) on trauma surgery and nontrauma surgery with significant reduction in out -patient attendance compared to corresponding time in 2019. There were significant changes made in individual hospital protocols (91.7 %). Appropriate required PPE was available in majority of the hospitals (74.3%). No remodelling or upgrading of the existing operating theatre infrastructure was done by most surgeons (89.5%). Conclusion: This pan India survey of orthopaedic surgeons has indicated that COVID-19 has had a profound impact on their outpatient and surgical trauma and non-trauma practice, due to the lockdown and resulted in significant changes to hospital protocols. Preparedness to resume clinical and surgical practice was associated with anxiety in two-thirds of the respondents. Majority of the orthopaedic practitioners felt that they would continue to conduct pre-operative COVID-19 screening and use PPE even after the lockdown is over.

ABSTRACT

Objetivo: discutir as experiências e contradições no controle da pandemia de Covid-19 sob a perspectiva da epidemiologia e das políticas públicas brasileiras. Conteúdo: o debate apresentado divide-se em duas seções: a primeira traça a reflexão dos parâmetros epidemiológicos sobre a curva epidêmica do SARS-CoV-2, bem como as experiências internacionais para o controle deste fenômeno. A segunda, ancorada nas políticas públicas brasileiras, apresenta uma análise comparada das experiências nacionais e internacionais, discutindo os potenciais reflexos sobre a pandemia, considerando algumas especificidades sociais. Conclusão: frente à impossibilidade de redução de pessoas susceptíveis por meio de estratégias vacinais, a redução da velocidade da curva epidêmica precisa ocorrer por meio de ações de isolamento físico social. Ademais, a construção de políticas públicas que visem a proteção ao trabalhador e a ampliação do investimento no setor saúde são medidas urgentes.


Objective: to discuss experiences and contradictions in the control of the Covid-19 pandemic from the perspective of epidemiology and Brazilian public policies. Content: the discussion presented is divided into two sections: the first one present epidemiological parameters on the epidemic curve of SARS-CoV-2, as well as the international experiences for the control of this phenomenon. The second, anchored in the Brazilian public policies, presents a comparative analysis of national and international experiences, discussing the potential impacts on the pandemic, considering some specific social issues. Conclusion: in the face of impossibility of reducing susceptible people through vaccination strategies, the reduction of the speed of the epidemic curve needs to occur through actions of social distance. In addition, the construction of public policies aimed at protecting workers and expanding investment in the health sector are urgent measures.


Objetivo: discutir las experiencias y contradicciones en el control de la pandemia Covid-19 desde la perspectiva de la epidemiología brasileña y las políticas públicas. Contenido: el debate presentado se divide en dos secciones. El primero traza la reflexión de los parámetros epidemiológicos en la curva epidémica del SARS-CoV-2, así como las experiencias internacionales para el control de este fenómeno. El segunda seccione, anclada en las políticas públicas brasileñas, presenta un análisis comparativo de las experiencias nacionales e internacionales, discutiendo los posibles impactos en la pandemia, considerando algunas especificidades sociales brasileñas. Conclusión: dada la imposibilidad de reducir a las personas susceptibles a través de estrategias de vacunación, la reducción de la velocidad de la curva epidémica debe ocurrir a través de acciones de aislamiento social y físico. Además, la construcción de políticas públicas destinadas a proteger a los trabajadores y ampliar la inversión en el sector de la salud son medidas urgentes.

ABSTRACT

Ensaio teórico reflexivo que teve por objetivo refletir na temática da Covid-19 articulado as lições deixadas por Florence Nightingale. Para tanto, literatura como livros, artigos, relatórios e imagem foram utilizados como articuladores da reflexão ao dialogar com o micro e macro no espaço sociocultural. Os resultados foram organizados em três momentos do desenvolvimento da proposta: Florence Nightingale e Guerra da Crimeia, Florence Nightingale produto de sua época e Florence Nightingale e as lições deixadas para aplicação em tempos de COVID-19, como desenvolvimento reflexivo para se traçar as conclusões. Nesta, com base nas lições deixadas por ela para o combate da Covid-19, em tempos de pandemia, deixamos a reflexão no sentido de quando o período pandêmico irá terminar.


Reflective theoretical essay that aimed to reflect on the theme of Covid-19 articulated as lessons left by Florence Nightingale. Therefore, literature such as books, articles, reports and images used as articulators of refl ection when dialoguing with micro and macro in the socio-cultural space. The results were organized in three stages of the proposal's development: Florence Nightingale and Crimean War, Florence Nightingale product of its time and Florence Nightingale and as lessons left for application in COVID-19 times, as a reflective development to trace how to result. In this one, based on the lessons left by her for the combat of Covid-19, in times of pandemic, we leave a reflection in the sense of when the pandemic period will end.


Ensayo teórico reflexivo que pretendía como objetivo reflexionar sobre el tema del Covid-19 articulado como lecciones dejadas por Florence Nightingale. Por tanto, la literatura como libros, artículos, reportajes e imágenes se utilizan como articuladores de la reflexión al dialogar con lo micro y macro en el espacio sociocultural. Los resultados se organizaron en tres etapas del desarrollo de la propuesta: Florence Nightingale y Crimean War, Florence Nightingale producto de su tiempo y Florence Nightingale y como lecciones dejadas para su aplicación en tiempos de COVID-19, como un desarrollo reflexivo para rastrear cómo resultar. En éste, basado en las lecciones que dejó ella para el combate del Covid-19, en tiempos de pandemia, dejamos una reflexión en el sentido de cuándo terminará el período pandémico.
