ABSTRACT

Objetivo: A má oclusão classe III de Angle se caracteriza por protrusão mandibular, retrusão maxilar ou pela combinação de ambas. Além de prejudicar a estética facial do paciente, essa má oclusão pode causar alterações funcionais e respiratórias. Uma das alternativas de tratamento para esses casos é o uso da máscara de Petit e do aparelho disjuntor de Hyrax. Este estudo tem como objetivo avaliar por meio da cefalometria ortodôntica se o tratamento com expansão maxilar em conjunto com a tração reversa da maxila diminuem os agravos estéticos e funcionais do paciente Classe III. Relato de caso: o relato de caso descrito no presente trabalho é sobre uma paciente que foi submetida a esse tratamento, sendo descrito por meio de análises cefalométricas, exames radiográficos, fotos intrabucais e achados clínicos. Considerações finais: A verificação dos resultados obtidos após o término do tratamento mostrou que a paciente teve uma boa adesão ao uso desses aparelhos e obteve resultados satisfatórios na sua função mastigatória, na sua oclusão e na sua estética facial e dentária.(AU)


Objective: Angle class III malocclusion is characterized by mandibular protrusion, maxillary retrusion or a combination of both. In addition to impairing the patient's facial aesthetics, this malocclusion can cause functional and respiratory changes. One of the treatment alternatives for these cases is the use of the Petit mask and the Hyrax breaker device. This study aims to evaluate, through orthodontic cephalometry, whether the treatment with maxillary expansion in conjunction with the reverse traction of the maxilla reduces the aesthetic and functional problems of Class III patients. Case report: the case report described in the present work is about a patient who underwent this treatment, being described through cephalometric analysis, radiographic examinations, intraoral photos and clinical findings. Final considerations: The verification of the results obtained after the end of the treatment showed that the patient had a good adherence to the use of these devices and obtained satisfactory results in her masticatory function, in her occlusion and in her facial and dental aesthetics.(AU)

ABSTRACT

Abstract The aim of this study was to understand the influence of orthosurgical treatment on oral health-related quality of life (OHRQoL) in patients with Class II and III skeletal malocclusion by conducting a mixed method case series study. Nineteen patients submitted to orthosurgical treatment in a private practice in Brazil were included in the sample. Data were collected retrospectively and interviews were held from March 2020 to July 2021. Patients answered to the Oral Health Impact Profile (OHIP-14) in the first part of the interview and subsequently answered the qualitative questions. The overall mean of OHIP-14 after treatment was 4.21 (SD 4.68). The qualitative data were analyzed according to thematic analysis and four themes emerged from the interviews: a) concept of quality of life, b) pre-treatment life, c) post-treatment life, and d) positive and negative aspects of treatment. Quality of Life was reported by the patients as the absence of feeling pain, having emotional and physical health, having a satisfactory esthetic appearance and self-esteem. Before treatment, most Class II patients used to complain about breathing and sleeping problems, while Class III patients complained more intensely about esthetics. Pain was a common problem reported by both Class II and Class III patients. In general, improvement was perceived in self-esteem, esthetics, function and pain. Complaints about negative aspects of the treatment were restricted to the postoperative period. The orthosurgical treatment was important for improving the OHRQoL of patients in terms of esthetic, functional and psychosocial aspects.

ABSTRACT Introduction: Intermaxillary elastics are orthodontic resources widely used in various malocclusions. Their main advantages are low cost, easy insertion and removal by patients, and application versatility. As main disadvantages, we can highlight the need for cooperation from patients and the side effects normally present in treatments with this resource. Knowledge of the biomechanics involved in the use of intermaxillary elastics is essential to take full advantage of the desired effects and avoid unwanted effects in their use. Objective: Therefore, the objective of this article is to describe the anchorage preparation, connection methods, time and force of use, and side effects involved in the use of intermaxillary elastics for the treatment of anteroposterior, vertical and transverse problems. For that, clinical cases and biomechanics schemes will be presented, in which all these details will be described.


RESUMO Introdução: Os elásticos intermaxilares são recursos ortodônticos amplamente utilizados nas diversas más oclusões. Possuem como principais vantagens o baixo custo, fácil inserção e remoção pelos pacientes, e versatilidade de aplicação. Como desvantagens podemos destacar a necessidade de cooperação dos pacientes e os efeitos colaterais normalmente presentes nos tratamentos com esse recurso. O conhecimento da biomecânica envolvida no uso dos elásticos intermaxilares é fundamental para se obter o máximo de efeitos desejados e evitar os efeitos indesejados na sua utilização. Objetivo: O objetivo desse artigo é descrever o preparo da ancoragem, as formas de conexão, o tempo e a força de uso, além dos efeitos colaterais envolvidos na utilização dos elásticos intermaxilares para tratamento de problemas anteroposteriores, verticais e transversais. Para isso, serão apresentados casos clínicos e esquemas de biomecânica em que todos esses detalhes serão descritos.

Objective:To explore the clinical application effect of open vertical helical loop to correct ectopic eruption of the first permanent molar, and to provide reference for the selection of treatment timing and methods for ectopic eruption of the first permanent molar.Methods:A total of 30 patients with unilateral or bilateral ectopic eruption of maxillary first permanent molars, aged from 7 to 8.5 years old, who visited the Affiliated Hospital of Jining Medical University from 2020 to 2021, were retrospectively selected. The first permanent molars were moved to their normal positions by bonding buccal tubes between the second primary molars and the first permanent molars with a vertical loop. We compared and analyzed the mesial inclination angle of the first permanent molar, the length of the lateral dental arch, and the root resorption status of the second deciduous molar before and after treatment.Results:All 30 patients underwent complete orthodontic treatment, with the first permanent molar adjusted to its normal position. The inclination angle of the first permanent molar after treatment was (91.3±5.1)°, which was statistically significant compared to (78.1±6.3) ° before treatment ( t=-10.023, P=0.014); The length of the lateral dental arch after treatment was (34.0±1.0)mm, which was significantly increased compared to (31.61±1.1)mm before treatment, and the difference was statistically significant ( t=-25.96, P=0.007). After treatment, the degree of root resorption of the affected second molar significantly increased compared to that before treatment (χ 2=12.002, P<0.001); There was no statistically significant change in root resorption before and after treatment of the healthy second molar ( P=0.818). Conclusions:The use of open vertical helical loop correction for ectopic eruption of maxillary first permanent molar can effectively adjust the mesial inclination angle of patients with ectopic eruption of maxillary first permanent molar. However, close attention should be paid to the root resorption of the second primary molar, and early detection and treatment should be carried out in clinical practice. If the root resorption of the second primary molar is severe after treatment, a retainer should be made in a timely manner to maintain the arch length.

ABSTRACT

Ilizarov technique is designed based on the "tension-stress principle". Application of the Ilizarov technique can help form a stable mechanical structure through external fixation. It only creates small wounds, without peeling off the surrounding tissue and periosteum. It connects different types of fixation pins and bone or soft tissue to form a traction force, which continuously stimulates the regeneration of bone and tissue and improves the local blood supply and early functional rehabilitation exercises, forming a complete set of minimally invasive orthopedic treatment systems. Ilizarov technique has achieved good results in fracture healing, deformity correction, tissue repair, osteomyelitis, bone nonunion, bone defect, and bone tumor. This paper will summarize the clinical application and research of the Ilizarov technique in orthopedics and explore the key problems of the Ilizarov technique that need to be solved at present.

RESUMEN La problemática del estudio recae en que los tratamientos para niños con escoliosis en Chile carecen de la integración desde el ejercicio físico. El objetivo fue analizar el ejercicio físico para tratamiento de niños con escoliosis idiopática. Se estudiaron las diversas formas de tratamiento físico en la escoliosis idiopática a través de la revisión de documentos oficiales (historias clínicas de los pacientes, programas de ejercicios y métodos de intervención). La investigación se llevó a cabo en el Centro Cubano de Rehabilitación Física de Temuco, Chile. Se aplicó el método empírico indirecto o no interactivo: análisis a documentos oficiales, análisis-síntesis y modelación, que permitieron la sistematización teórica del ejercicio físico en el tratamiento. Como resultado fundamental se diseña el programa para el tratamiento de la escoliosis idiopática mediante ejercicios físicos correctivos en un período de 18 meses.


RESUMO O problema do estudo reside no facto de os tratamentos para crianças com escoliose no Chile carecerem da integração do exercício físico. O objectivo era analisar o exercício físico para o tratamento de crianças com escoliose idiopática. As várias formas de tratamento físico para a escoliose idiopática foram estudadas através da revisão de documentos oficiais (registos médicos dos pacientes, programas de exercício e métodos de intervenção). A investigação foi realizada no Centro Cubano de Reabilitação Física em Temuco, Chile. O método empírico indireto ou não-interativo foi aplicado: análise de documentos oficiais, análise-síntese e modelação, o que permitiu a sistematização teórica do exercício físico no tratamento. Como resultado fundamental, foi concebido um programa para o tratamento da escoliose idiopática através de exercícios físicos corretivos durante um período de 18 meses.


ABSTRACT The problem of the study lies in the fact that the treatments for children with scoliosis in Chile lack integration from physical exercise. The objective was to analyze physical exercise for the treatment of children with idiopathic scoliosis. The various forms of physical treatment in idiopathic scoliosis were studied through the review of official documents (patient medical records, exercise programs and intervention methods). The research was carried out at the Cuban Center for Physical Rehabilitation in Temuco, Chile. The indirect or non-interactive empirical method was applied: analysis of official documents, analysis-synthesis and modeling, which allowed the theoretical systematization of physical exercise in treatment. As a fundamental result, the program for the treatment of idiopathic scoliosis is designed through corrective physical exercises in a period of 18 months.

Tooth agenesis or hypodontia is a developmental anomaly, where there is absence of one or more dental elements. The absence of maxillary lateral incisor can be uni or bilateral, has an incidence of 20% and is more frequent in females. Is directly related to the establishment of malocclusions and aesthetic dissatisfaction, so it is important to evaluate multidisciplinary parameters for a treatment decision. The objective of the present study is to establish, according to the present literature, clinical criteria for the therapeutic approach of opening or closing dental space of the Maxillary Lateral Incisor Agenesis. A systematized literature review was carried out in online databases: PubMed, Embase and Lilacs. Health descriptors and free terms were combined with Boolean operators to search: "Tooth Agenesis" or "Anodontia" and "Lateral Incisors" and "Treatment" and "Opening Space" or "Space Closure". Clinical studies that presented diagnostic criteria and therapeutic possibilities for Maxillary Lateral Incisor Agenesis, in English and without publication time frame, were included. During the clinical examination, skeletal, dental and periodontal parameters must be carefully observed. The intervention will always consist of a multidisciplinary approach, which may involve orthodontics, prosthetics, dentistry, periodontics and implantology for functional and aesthetic rehabilitation. The interceptive approach is based on early diagnosis and maintenance of the dental space, so orthodontic corrective treatment can be minimized later on. Late intervention includes closing the space with the reanatomization of adjacent dental elements or opening the space for implant and prosthesis installation. (AU)


A agenesia dentária ou hipodontia é uma anomalia de desenvolvimento, onde há ausência de um ou mais elementos dentários. A ausência do incisivo lateral superior possui uma incidência de 20%, mais frequente no sexo feminino e pode ser unilateral ou bilateral e estão diretamente relacionadas ao estabelecimento de maloclusões e insatisfação estética, assim, é importante que os profissionais saibam avaliar os parâmetros multidisciplinares para uma tomada de decisão sobre seu tratamento. O objetivo do presente estudo é estabelecer, segundo a literatura presente, os critérios clínicos para a tomada de decisão terapêutica de abertura ou fechamento de espaço para a Agenesia de Incisivo Lateral Superior. Foi realizada uma Revisão Sistematizada da Literatura nas bases de dados online: PubMed, Embase e Lilacs. Foram combinados descritores em saúde e termos livres com operadores boleanos para realizar a pesquisa nas bases de dados: "Tooth Agenesis" or "Anodontia" and "Lateral Incisors" and "Treatment" and "opening space" or "space closure". Foram incluídos estudos clínicos que apresentaram critérios para o diagnóstico e possibilidades terapêuticas para a Agenesia do Incisivo Lateral Superior, na língua inglesa e sem recorte temporal de publicação. Durante o exame clínico devem ser observados minuciosamente os parâmetros esqueléticos, dentários e periodontais. A intervenção sempre consistirá na abordagem multidisciplinar, podendo envolver ortodontia, prótese, dentística, periodontia e implantodontia para a reabilitação funcional e estética. A abordagem interceptativa consiste em diagnosticar precocemente e manter o espaço dentário, para que possa minimizar terapias ortodônticas corretivas de maior magnitude posteriormente. A intevenção tardia engloba o fechamento do espaço com a reanatomização dos elementos dentários adjacentes e abertura do espaço para instalação de implante e prótese. (AU)

ABSTRACT Introduction: Class II division 1 malocclusion treatment with functional devices offers acceptable results. These devices can be removable or fixed, and the essential difference between them is the need for compliance. It is clinically important to investigate if there are differences in the treatment effects of these devices that present different characteristics. Objective: This retrospective longitudinal study compared the treatment effects of Class II correction with the MARA appliance, Activator-Headgear (AcHg) combination, both followed by multibracket fixed appliances, and an untreated control group. Material and Methods: Each experimental group was composed of 18 patients, with a baseline mean age of 11.70 and 10.88 years, treated for 3.60 and 3.17 years. The control group consisted of 20 subjects with baseline mean age of 11.07 years. The groups were evaluated before (T1) and after (T2) treatment. Lateral radiographs were used to evaluate the treatment changes with treatment (T2-T1), compared to the control group. Intergroup comparisons were performed using repeated-measures analysis of variance (ANOVA), followed by Tukey's test. Results: The AcHg group showed significantly greater maxillary growth restriction than the MARA, while the mandibular changes were due to natural growth. Both devices promoted significantly greater maxillary incisors retrusion, mandibular incisors labial inclination, and improvement of overjet and molar relationships, compared to the control. Conclusions: Both functional devices followed by multibracket appliances were effective to correct Class II malocclusion. Nonetheless, the AcHg combination presents superior skeletal effects, due to significantly greater maxillary growth restriction compared to the MARA appliance. Moreover, the appliances presented similar dentoalveolar effects.


RESUMO Introdução: O tratamento da má oclusão de Classe II divisão 1 com dispositivos funcionais oferece resultados aceitáveis. Esses dispositivos podem ser removíveis ou fixos, e a diferença essencial entre eles é a necessidade de colaboração. É clinicamente importante investigar se existem diferenças nos efeitos do tratamento desses dispositivos que apresentam características diferentes. Objetivo: O presente estudo retrospectivo longitudinal comparou os efeitos do MARA e da combinação Aparelho Extrabucal-Ativador (AEB-At) no tratamento da Classe II, ambos seguidos por aparelho fixo, adicionando também um grupo controle não tratado. Métodos: Cada grupo experimental foi composto por 18 pacientes; respectivamente, com média de idade inicial de 11,70 e 10,88 anos, tratados por 3,60 e 3,17 anos. O grupo controle foi composto por 20 indivíduos, com idade média inicial de 11,07 anos. Os grupos foram avaliados antes (T1) e após (T2) o tratamento. Radiografias de perfil foram utilizadas para avaliar as alterações do tratamento (T2-T1), em comparação com o grupo controle. As comparações intergrupos foram realizadas por meio da análise de variância para medidas repetidas (ANOVA), seguida pelo teste de Tukey. Resultados: O AEB-At apresentou uma restrição do crescimento maxilar significativamente maior que o MARA, enquanto as alterações mandibulares foram decorrentes do crescimento natural. Ambos os dispositivos promoveram significativamente mais retrusão dos incisivos superiores, inclinação vestibular dos incisivos inferiores e melhora da sobressaliência e relação molar, em comparação com o controle. Conclusões: Ambos os dispositivos funcionais associados ao aparelho fixo foram efetivos na correção da má oclusão de Classe II. No entanto, a combinação AEB-At apresenta efeitos esqueléticos superiores, devido à restrição de crescimento maxilar significativamente maior do que com o aparelho MARA. Além disso, os aparelhos apresentaram efeitos dentoalveolares semelhantes.

ABSTRACT Objective: To emphasize the importance of diagnosis and discuss the therapeutic approaches that can be used in the orthodontic treatment of Class I malocclusion associated with two impacted maxillary canines. The opening of spaces for traction of these teeth by means of rapid maxillary expansion or extraction of maxillary premolars was contraindicated in the case reported. Therefore, it was decided to open spaces with projection of incisors. Results: The obtained results were satisfactory, as a good occlusion was obtained, with adequate functional guides, as well as an improvement in the facial appearance. Conclusion: The projection of the incisors prior to traction of the impacted maxillary canines proved to be a valid option in the case described. Ten years after completion of treatment, the case is stable, maintaining periodontal health.


RESUMO Objetivo: Destacar a importância do diagnóstico e discutir as abordagens terapêuticas que podem ser utilizadas no tratamento ortodôntico da má oclusão de Classe I associada a dois caninos superiores impactados. Relato de caso: A abertura de espaço para tracionar esses dentes por meio de expansão rápida da maxila ou exodontia de pré-molares superiores era contraindicada no caso relatado. Portanto, decidiu-se abrir espaço por meio da projeção dos incisivos. Resultados: Os resultados obtidos foram satisfatórios, pois obteve-se uma boa oclusão, com guias funcionais adequadas, além da melhoria no aspecto facial. Conclusão: A projeção dos incisivos prévia ao tracionamento dos caninos superiores impactados mostrou-se uma opção válida no caso descrito. Dez anos após a finalização do tratamento, o caso apresenta-se estável, mantendo a saúde periodontal.

Introduction:The supply of public orthodontic services is still unable to meet the demand for treatment of malocclusions. The resources available are sometimes mismanaged and significant financial impacts contribute to limited access to the services. Objective:To carry out an integrative review regarding the access and financial impacts of public orthodontic service among the various health systems worldwide, considering social inequalities and the referral and counter-referral systems for services. Methodology:A survey was conducted using the BVS (Biblioteca Virtual em Saúde / Virtual Health Library), PubMed, and Cochrane databases, including articles available from 1970 to 2019, which elucidated the guiding question "How does access work and what are the financial impacts of the provision of orthodontic services in different public health systems worldwide?". Results:211 articles were found, of which 20 were analyzed. The chronological distribution was relatively uniform. The topic 'access to orthodontic services' was most frequently reported and there was a predominance of studies addressing the provision of treatment for children and adolescents. Conclusions:There are significant financial impacts and demographics have a strong influence on access to services. The provision of orthodontic treatments by the private sector predominates, preventive approaches are scarce, and an optimization in the use of available resources is necessary. Primary Care has much to contribute in increasing access, reducing costs, and making the referral / counter-referral system effective (AU).


Introdução:A oferta de serviços ortodônticos públicos ainda não consegue suprir a demanda de tratamento de má oclusões. Os recursos disponíveis, por vezes, mal alocados e os impactos financeiros significativos contribuem para a restrição do acesso aos serviços.Objetivo:Realizar uma revisão integrativa pertinente ao acesso e aos impactos financeiros da provisão ortodôntica pública nos diversos sistemas de saúde mundiais, sob a luz das iniquidades sociais e dos sistemas de referência e contrarreferência de serviços.Metodologia:Foi realizado um levantamento nas bases de dados BVS (Biblioteca Virtual em Saúde), PubMed e Cochrane Reviews, incluindo os artigos disponíveis no período de 1970 a 2019, que elucidavam a questão norteadora "Como se dá a provisão de serviços ortodônticos nos diversos sistemas públicos de saúde mundiais?".Resultados:Foram encontrados 211 registros, dos quais 20foram analisados. A distribuição temporal foi relativamente uniforme. A temática "acesso aos serviços ortodônticos" foi mais frequentemente relatada e houve um predomínio de estudos abordando a oferta de tratamentos a crianças e adolescentes. Conclusões:Há impactos financeiros significativos e os determinantes sociais exercem forte influência sobre o acesso aos serviços. Predomina a oferta de tratamentos ortodônticos pelo setor privado, abordagens preventivas são escassas e é necessária uma otimização no uso de recursos disponíveis. A Atenção Básica tem muito a contribuir para aumento do acesso, diminuição de custos e efetivação do sistema dereferência/contrarreferência dos serviços (AU).


Introducción: La provisión de servicios públicos de ortodoncia aún no puede satisfacer la demanda de tratamientos de maloclusión. Los recursos disponibles a veces están mal asignados y los impactos financieros significativos contribuyen a restringir el acceso a los servicios.Objetivo: Realizar una revisiónintegradora pertinente al acceso y los impactos económicos de la provisión pública de ortodoncia en los diferentes sistemas de salud a nivel mundial, a la luz de las inequidades sociales y los sistemas de derivación y contrarreferencia de servicios.Metodología: Se realizó una encuesta en las bases de datos de la BVS (Virtual Health Library), PubMed y Cochrane Reviews, incluidos los artículos disponibles desde 1970 hasta 2019, que dilucidó la pregunta orientadora "¿Cómo es la prestación de servicios de ortodoncia en los diferentes sistemas públicos?".Resultados: Se encontraron 211registros, de los cuales se analizaron 20.La distribución temporal fue relativamente uniforme. El tema "acceso a servicios de ortodoncia" fue el que se informó con mayor frecuencia y hubo un predominio de estudios que abordan la provisión de tratamientos a niños y adolescentes.Conclusiones: Hay impactos económicos significativos y los determinantes sociales tienen una fuerte influencia en el acceso a los servicios. Predomina la oferta de tratamientos de ortodoncia por parte del sector privado, los abordajes preventivos son escasos y es necesario optimizar el uso de los recursos disponibles. La Atención Primaria tiene mucho que contribuir para aumentar el acceso, reducir costos e implementar el sistema de derivación / contrarreferencia de servicios (AU).

Objective To provide direction for the improvement of quality control of hospital preparations and ensure the safety for clinical use by analyzing the hospital preparation deviations in recent three years. Methods A retrospective analysis on 59 minor hospital preparation deviations from 2017 to 2019 was conducted. Brainstorming, fishbone drawing and, Minitab software were used to analyze the root causes of deviations from five aspects: personnel, machine, materials, methods and environment. The preventive and corrective measures were implemented. The results were evaluated. Results 1 significant deviation (1.7%), 24 major deviation (40.7%), and 34 minor deviation (57.6%) were identified among the 59 casses of preparation deviation. With the implementation of preventive and corrective measures, the total number of deviations in 2018 was significantly reduced compared to that in 2017. The total number of deviations in 2019 was about the same as that in 2018. The human factors need to be focused. Conclusion The pharmaceutical preparation deviations in our hospital have been reduced. The further quality improvements for pharmaceutical preparations will be carried out by following the regulations of pharmaceutical production quality management standards and pharmaceutical production supervision and administration measures.

ABSTRACT

Objective:To study the facial aesthetic effect of different periodontal splints combined with orthodontic treatment for patients with sector displacement of periodontitis teeth.Methods:A total of 186 patients with sector displacement of periodontitis teeth admitted to Hunan Second People′s Hospital from April 2018 to April 2020 were prospectively selected as the research objects. 186 patients were divided into observation group and control group by random number table method, with 93 cases in each group. Both groups were treated with orthodontic correction. The observation group was treated with orthodontic combined with super fiberglass periodontal splint, while the control group was treated with orthodontic combined with conventional periodontal splint. The treatment effect, serum inflammatory factors [tumor necrosis factor-α (TNF-α), C-reactive protein (CRP), interleukin-6 (IL-6) ], periodontal pocket depth, bleeding index and plaque index before and after treatment were compared between the two groups, and the satisfaction of the two groups was compared.Results:At half year and 1 year after treatment, the effective rate of the observation group was 91.40%(85/93) and 100%(93/93), which were significantly better than those of the control group [84.95%(79/93), 95.70%(89/93), all P<0.05]. The levels of serum TNF-α, CRP and IL-6 in the two groups at half year and 1 year after treatment were significantly lower than those before treatment (all P<0.05); the levels of serum TNF-α, CRP and IL-6 in the observation group was significantly lower than the control group, with statistically significant difference (all P<0.05). The periodontal pocket depth, bleeding index and plaque index of the two groups after treatment were also significantly lower than those before treatment (all P<0.05); the periodontal pocket depth, bleeding index and plaque index of the observation group was significantly lower than the control group, with statistically significant difference (all P<0.05). At half year and 1 year after treatment, the patients′ satisfaction in the observation group reached 91.40%(85/93) and 94.62%(88/93), respectively, which was also significantly better than that in the control group [81.72%(76/93), 89.25%(83/93), all P<0.05]. Conclusions:Compared with the conventional periodontal splint, the combination of super fiberglass periodontal splint can significantly improve the treatment effect and patient′s satisfaction.

ABSTRACT

Objective:To investigate the effects of bracket-free invisible orthodontic technique versus traditional fixed orthodontic technique on the levels of inflammatory factors in the gingival crevicular fluid of orthodontic adolescents. Methods:A total of 67 orthodontic patients who received their first orthodontic treatment in Huzhou Central Hospital from June to September 2020 were included in this study. They were randomly divided into an observation group ( n = 34) and a control group ( n = 33). The control group was treated with traditional fixed orthodontic treatment. The observation group was treated with invisible orthodontic treatment without brackets. At 0, 1, 2, 4, and 6 months of treatment, the level of inflammatory factors and periodontal index were compared. Results:At 0 and 1 month of treatment, there were no significant differences in the levels of interleukin-1β (IL-1β), interleukin-6 (IL-6), and tumor necrosis factor-α (TNF-α) in the gingival crevicular fluid between the observation and control groups (all P > 0.05). At 2, 4, and 6 months of treatment, the levels of IL-1β, IL-6, and TNF-α in the observation group were significantly lower than those in the control group (IL-1β: t2 months = 5.56, P = 0.042; t4 months = 8.14, P = 0.019; t6 months = 9.87, P = 0.002. IL-6: t2 months = 7.15, P = 0.029; t4 months = 7.94, P = 0.021; t6 months = 9.16, P = 0.007. TNF-α: t2 months = 6.87, P = 0.039; t4 months = 7.65, P = 0.026: t6 months = 9.89, P = 0.001). In each group, gingival index (GI), sulcus bleeding index (SBI), probing depth (PD), and plaque index (PLI) increased significantly after treatment. At 0 and 1 month of treatment, there were no significant differences in GI, SBI, PD, and PLI between the two groups (all P > 0.05). At 2, 4, and 6 months of treatment, GI, SBI, PD, and PLI in the observation group were significantly lower than those in the control group (GI: t2 months = 3.62, P = 0.073; t4 months = 8.16, P = 0.018; t6 months = 8.54, P = 0.016. SBI: t2 months = 5.65, P = 0.042; t4 months = 7.56, P = 0.027; t6 months = 8.15, P = 0.019. PD: t2 months = 5.652, P = 0.042; t4 months = 7.56, P = 0.027, t6 months = 8.15, P = 0.019. PLI: t2 months = 9.57, P = 0.006; t4 months = 9.98, P = 0.002. t6 months = 9.94, P = 0.010). In addition, at 1 month of treatment, PLI in the observation group was significantly lower than that in the control group ( t = 9.99, P = 0.001). Conclusion:Bracket-free invisible orthodontic treatment provides better protection on periodontal tissue and has less impact on periodontal health than traditional fixed orthodontic treatment.

Malunion is a common complication following a calcaneal fracture which was not treated or treated inappropriately.It is a therapeutic target and a great challenge as well to relieve pain, correct deformity and restore the function of the affected foot in clinical treatment of calcaneal malunion. As a result of researches by scholars at home and abroad focusing on the biomechanical mechanisms underlying the symptoms caused by calcaneal malunion, a variety of corrective calcaneal osteotomy has been widely applied in clinical practice to specifically correct the calcaneal deformity and restore normal calcaneal morphology. This review expounds on the techniques, outcomes, indications and complications of corrective calcaneal osteotomies commonly used in clinic.

Abstract Orthodontic-surgical treatment with the "Surgery First Approach" provides immediate facial aesthetic improvements and significantly reduces the patient's orthodontic treatment time, avoiding the transient worsening of the facial profile due to dental decompensation that occurs in surgical cases. Thus, this clinical case describes the retreatment of a 22-year-old female leukoderma patient, whose main complaint was related to the proclination of upper and lower incisors. The patient used a mio-relaxing plate for 30 days, which evidenced the skeletal mandibular deficiency and the ½ bilateral Class II malocclusion. Orthognathic surgery first approach associated with the extraction of the 4 premolars was chosen considering the patient's aesthetic demand. The use of a mio-relaxing plate in the diagnostic stage was essential for the real diagnosis of mandibular deficiency and the technique employed made it possible to conclude the treatment avoiding aesthetic commitment, with excellent results. Resumo O tratamento ortodôntico-cirúrgico por meio do benefício antecipado proporciona melhorias estéticas faciais imediatas e reduz de maneira significativa o tempo de tratamento ortodôntico do paciente, evitando a piora transitória do perfil facial devido à descompensação dentária que ocorre em casos cirúrgicos. Assim, este caso clínico descreve o retratamento de uma paciente com 22 anos de idade, leucoderma, sexo feminino, que apresentava queixa principal relacionada à inclinação vestibular dos dentes anteriores. Após uso de placa miorrelaxante por 30 dias, verificou-se a presença de Classe II esquelética com deficiência mandibular e ½ Classe II dentária bilateral. Considerando a demanda estética da paciente, optou-se pela abordagem ortodôntico-cirúrgica com Benefício Antecipado associada à extração de 4 pré-molares para correção da inclinação dentária anterior. O uso da placa miorrelaxante foi fundamental para o diagnóstico real da deficiência mandibular e a técnica empregada possibilitou concluir o tratamento evitando o comprometimento estético pré-cirúrgico, com obtenção de excelentes resultados. La maloclusión clase III se considera un reto en la práctica de todo ortodoncista. Una de las principales dudas al respecto reside en ¿cuándo es el mejor momento para intervenir? Exis- ten dos enfoques en el manejo ortodóntico del paciente: 1) la ortodoncia interceptiva; y 2) la ortodoncia correctiva. La or- todoncia interceptiva busca la prevención del establecimiento de la malolcusión. En este grupo, se encuentra el uso de más- cara facial con disyunción maxilar y el de aparatología fija (2x4 o 2x6). Por otro lado, la intervención correctiva hace re- ferencia al camuflaje de las características que trae consigo la maloclusión clase III ya establecida; dentro de este enfoque se encuentran las extracciones de piezas, el uso de minitornillos extraalveolares y la filosofía MEAW. Se puede concluir que el adecuado manejo de la maloclusión clase III radica en el oportuno y correcto diagnóstico, que debe realizarse a través de la minuciosa inspección de las características y hallazgos intra y extraorales de los pacientes (AU)


Class III malocclusion is considered a challenge in the practice of every orthodontist. One of the main questions is: when is the best time to intervene? There are 2 approaches to the orthodontic management of the patient: 1) interceptive orthodontics, and 2) corrective orthodontics. Interceptive or- thodontics seeks to prevent the establishment of malocclusion by means of the use of a facial mask with maxillary disjunc- tion, or the use of fixed appliances (2x4 or 2x6). Corrective intervention refers to camouflaging the characteristics of a Class III malocclusion that is already established. This ap- proach uses tooth extraction, extra-alveolar mini screws or the MEAW philosophy. To conclude, proper management of Class III malocclu- sion is based on timely, correct diagnosis, which must be made through careful inspection of the characteristics and intraoral and extraoral findings in patients (AU)

ABSTRACT

A Odontologia moderna busca atualmente tratamentos que restaurem função e estética dos elementos dentários, levando também em consideração os fatores biológicos envolvidos na saúde bucal. O tratamento reabilitador protético pode ser desafiador, principalmente em pacientes adultos, com perdas dentárias e problemas periodontais. Nesse sentido, a busca por uma somatória de bons resultados leva a equipe a unir especialidades como Periodontia, Dentística, Prótese, Implantodontia, Cirurgia Bucomaxilofacial e Ortodontia. Esta última vem sendo uma grande aliada para a reabilitação protética em etapas de diagnóstico e plano de tratamento, proporcionando melhores prognósticos estéticos e funcionais. O presente estudo trata-se de uma revisão da literatura realizada por meio de busca nas bases de dados PubMed e Google Scholar e tem como objetivo abordar os aspectos mais recentes do tratamento ortodôntico pré-protético, enfatizando as principais vantagens, indicações e as formas de estabelecer um plano de tratamento, bem como movimentações ortodônticas em pacientes com agenesia dentária anterior e a utilização de mini-implantes como auxiliares no plano terapêutico. O tratamento ortodôntico pré-protético é um tema considerado atual, e a individualidade de cada paciente faz com que a literatura concentre mais artigos de relato de caso, sendo, por isso, necessários mais estudos que englobem protocolos e indicações para os possíveis casos que venham a surgir na prática clínica. Conclui-se que a Ortodontia sendo inserida nas etapas de diagnóstico e plano de tratamento da reabilitação protética traz benefícios de grande valia na busca de um ambiente reabilitador mais fácil e com maiores garantias de sucesso estético e funcional.


Modern dentistry is currently seeking treatments that restore the function and aesthetics of dental elements, also considering the biological factors involved in oral health. Prosthetic rehabilitation treatment can be challenging, especially in adult patients with tooth loss and periodontal problems. In this sense, the search for a sum of satisfactory results leads the team to integrate specialties such as Periodontics, Dentistry, Prosthodontics, Implantology, Maxillofacial Surgery and Orthodontics. The latter has been a great ally for prosthetic rehabilitation in stages of diagnosis and treatment plan, providing better aesthetic and functional prognoses. The present study is a literature review performed by searching the PubMed and Google Scholar databases and aims to address the most recent aspects of preprosthetic orthodontic treatment, emphasizing the main advantages, indications, and ways of establishing a treatment plan, as well as orthodontic movements in patients with anterior tooth agenesis and the use of mini-implants as an aid in the therapeutic plan. Preprosthetic orthodontic treatment is considered a current topic, and the individuality of each patient means that the literature concentrates more in case report articles, thus further studies are needed that encompass protocols and indications for possible cases that may arise in clinical practice. We conclude that including orthodontics in the stages of diagnosis and treatment plan of prosthetic rehabilitation brings benefits of great value in the search for an easier rehabilitation environment with greater guarantees of aesthetic and functional success.

Introduction: The Dental Aesthetic Index (DAI) is considered an essential tool in the association of the aesthetic and clinical elements of occlusion. It plays a key role in the timely and essential orthodontic treatment. Objective: to determine the orthodontic treatment required in students of a Peruvian public institution using the DAI. Material and Methods: A descriptive, observational, cross-sectional study was carried out during the months of June to July 2016 in a Peruvian educational institution. The sample consisted of 120 students. The use of the DAI allowed to assess the orthodontic treatment required, through the 10 occlusal conditions and regression indicators that constitute a linear formula, with the following components: no treatment required, elective treatment, desirable treatment, and priority treatment, according to the severity of the malocclusion as normal, defined, severe, and very severe, respectively. Results: 53.3% (64) of the students required priority orthodontic treatment due to presenting DAI=43.03. Between the ages of 12-14 years, 56.7% (34), 8.3% (5), and 28.3% (17) required priority, desirable, and elective orthodontic treatment, respectively. The need for priority orthodontic treatment was more prevalent in females accounting for 57.6% (38). 95% (57) of the students from rural areas required orthodontic treatment. Conclusion: The need for orthodontic treatment in a Peruvian sample using the Dental Aesthetic Index was priority orthodontic treatment, mostly in females with ages ranging between 12-14 years.

Resultados: Estudio de diseño descriptivo, observacional, con corte transversal, ejecutado durante los meses junio a julio de 2016, en una institución educativa peruana. La muestra fueron 120 estudiantes. Introducción: Los minitornillos de ortodoncia son dispositivos temporales de anclaje utilizados ampliamente en ortodoncia correctiva. Sin embargo, se ha reportado en la literatura que su tasa clínica de éxito no es completa debido a que pueden existir fallas relacionadas con la pérdida de su estabilidad. Varios factores pueden comprometer el anclaje y están relacionados con el tipo de implante, su manejo quirúrgico y el hospedero. Dentro de este último, se encuentra la inflamación del tejido periimplantario o mucositis que, aunque reversible, es necesario su tratamiento temprano para evitar su progresión a la afectación de tejido duro (periimplantitis). Objetivo: Describir un caso clínico de mucositis periimplantaria asociada a un dispositivo de anclaje ortodóntico temporal. Presentación del caso: Paciente femenina que acudió a la consulta por presentar dolor en la zona anterior del maxilar superior. Durante el examen intraoral se observó que usaba aparatos de ortodoncia con mecanismo de anclaje: dos minitornillos de ortodoncia ubicados al nivel de los caninos superiores. Se realizó la remoción quirúrgica de los minitornillos de ortodoncia, se le indicó el uso de antibióticos y se realizó estudio histopatológico para descartar malignidad. Se diagnosticó con mucositis periimplantaria. Conclusiones: La remoción quirúrgica de los minitornillos de ortodoncia es una excelente alternativa para evitar la progresión de la lesión hacia los tejidos duros. A través de esta técnica lograron eliminar signos de dolor, enrojecimiento y sangrado(AU)


Introduction: Orthodontic miniscrews are temporary anchorage devices widely used in corrective orthodontics. However, it has been reported in the literature that their clinical success rate is not high, due to failures related to the loss of stability. Anchorage may be affected by factors associated to the type of implant, its surgical management and the host. Host-related factors include peri-implant tissue inflammation or mucositis, which though reversible, its early treatment is necessary to prevent expansion into and damage to the hard tissue (peri-implantitis). Objective: Describe a clinical case of peri-implant mucositis associated to a temporary orthodontic anchorage device. Case presentation: A female patient attends clinical consultation with pain in the anterior zone of her upper maxilla. Intraoral examination revealed the presence of orthodontic appliances with an anchorage mechanism: two orthodontic miniscrews located at the level of the upper canines. Surgical removal was performed of the orthodontic miniscrews, antibiotics were indicated and histopathological testing was conducted to rule out malignancy. The patient was diagnosed with peri-implant mucositis. Conclusions: Surgical removal of orthodontic miniscrews is an excellent alternative to prevent the progress of the lesion into hard tissues. Pain signs, reddening and bleeding were all eliminated with the technique applied(AU)
