ABSTRACT

Objective:To analyze the household smoking behavior of smokers in rural Taizhou City of Zhejiang Province, and to provide the basis for the intervention measures to reduce children's second-hand smoke (SHS) exposure. Methods:The investigation started in May 2018. A total of 418 smoking families in Luqiao and Linhai were included. Field face-to-face questionnaire survey was conducted and cotinine level in children's urine was measured. Kernel density estimation was used for curve fitting. The results were analyzed by Wilcoxon rank sum test and chi square test. Results:The age distribution of smokers in the family was bimodal. The younger smoker group was mainly the parents. The older group was mainly the other relatives such as grandfather. The age started smoking in the parent group was younger than that in relative group, but the average number of cigarettes smoked per day in relative group was higher than that in parent group (P<0.05). The parent group had higher proportion than the relative group in having family smoking regulations or agreeing not to smoke in the room in the presence of children (P<0.001).The proportion of the parent group who has heard about third hand smoke and electronic cigarette was higher than that of the relative group (P<0.001).The detection rates of cotinine in urine of children were 91.05% and 86.96% in parent group and relative group, respectively. The urine cotinine level of the children in the parent group was higher than that in the grandfather group (P=0.049). Conclusion:Children are more exposed to SHS when the smoker in the household are parents. In order to reduce children's SHS exposure, we should pay more attention to educate the parents with a family based tobacco control program.

OBJECTIVE@#To prepare monoclonal antibody against cotinine (COT) and to establish immunoassay for detecting COT in human urinary samples.@*METHODS@#BALB/c mice were immunized with synthesized cotinine-bovine serum albumin (COT-BSA) to screen monoclonal antibody with technique of cell fusion. The monoclonal antibody was used for the indirect competitive enzyme-linked immunosorbent assay (ic-ELISA) and colloidal gold immunochromatographic strip assay for the detection of COT in human urine.@*RESULTS@#The monoclonal antibody against COT was identified by ic-ELISA with a 50%inhibitive concentration (IC@*CONCLUSIONS@#The ic-ELISA and colloidal gold immunochromatographic strip assay using the prepared monoclonal antibody against COT have been proved to be reliable for the rapid detection of COT in human urines, which may be used for monitoring of environmental tobacco smoke.

ABSTRACT

Abstract: This study aimed to evaluate the occurrence of the green tobacco sickness (GTS) and its associated factors in tobacco familiar farmers residing in Dom Feliciano, Rio Grande do Sul State, Brazil. A cross-sectional study was conducted evaluating the sickness in 354 small tobacco farmers, between October 2011 and March 2012. The urinary concentration of cotinine, a biomarker of exposure to nicotine, was determined during the tobacco harvest period. Subjects presenting cotinine urinary levels ≥ 50ng/mL, that had contact with tobacco leaves up to 48 hours before the sample collection and reported at least one disease symptom were deemed as cases. A non-conditional logistic analysis was performed to evaluate the association between GTS and the population characteristics. A total of 122 (34.5%) cases were identified, with 39% of them being smokers and 61% being males. The median cotinine urinary concentrations were 75.6ng/mL (74.1ng/mg of creatinine) for non-cases and 755.8ng/mL (632.1ng/mg of creatinine) for the cases (p-value ≤ 0.01). The multivariate analysis showed a positive association between GTS and sunlight exposure time, exposure to pesticides, worse health status, and inverse association with wood cultivation. This study presented a high GTS prevalence and suggest that the use of urinary cotinine is a significant biomarker to determine GTS cases, influencing in the distribution by sex. Once the tobacco production involves health hazards, is important to implement measures to prevent the harm caused to tobacco farmers, as set in articles 17 and 18 of the World Health Organization Framework Convention on Tobacco Control.


Resumo: O estudo teve como objetivo avaliar a ocorrência de doença da folha verde (DFV) e fatores associados entre fumicultores familiares no Município de Dom Feliciano, Estado do Rio Grande do Sul, Brasil. Foi realizado um estudo transversal sobre DFV em 354 pequenos produtores de tabaco, entre outubro de 2011 e março de 2012. Foi medida a concentração urinária da cotinina, um biomarcador da exposição à nicotina, durante a temporada de colheita do tabaco. Os casos de DFV foram definidos como indivíduos com níveis urinários de cotinina ≥ 50ng/mL, que tiveram contato com folhas de tabaco em até 48 horas antes da coleta da amostra sanguínea e que relatavam pelo menos um sintoma da doença. Foi realizada uma análise logística não condicional para avaliar a associação entre DFV e as características da população. Foram identificados um total de 122 (34,5%) casos da doença, entre os quais 39% eram fumantes e 61% eram do sexo masculino. As concentrações urinárias medianas de cotinina foram 75,6ng/mL (74,1ng/mg de creatinina) para não-casos e 755,8ng/mL (632,1ng/mg de creatinina) para os casos (valor de p ≤ 0,01). A análise multivariada mostrou uma associação positiva entre DFV e tempo de exposição à luz solar, exposição a pesticidas e pior estado de saúde geral, e uma associação inversa com cultivo de lenha. O estudo identificou alta prevalência de DFV e sugere o uso da cotinina como biomarcador significativo para identificar os casos de DFV, influenciando a distribuição por sexo. Uma vez que a fumicultura envolve danos à saúde, é importante implementar medidas para prevenir os impactos para os fumicultores, de acordo com os artigos 17 e 18 da Convenção-Quadro para o Controle do Tabaco da Organização Mundial da Saúde.


Resumen: Este estudio visó evaluar la ocurrencia de la enfermedad del tabaco verde (GTS por sus siglas en inglés) y sus factores asociados en pequeños agricultores de tabaco, que residen en Dom Feliciano, Rio Grande do Sul, Brasil. Se realizó un estudio transversal, evaluando la enfermedad del tabaco verde, en 354 pequeños agricultores de tabaco, entre octubre de 2011 y marzo 2012. La concentración urinaria de cotinina, un biomarcador de la exposición a la nicotina, se determinó durante el periodo de cosecha del tabaco. Los individuos que presentan niveles de cotinina urinarios ≥ 50ng/mL, que tuvieron contacto con hojas de tabaco hasta 48 horas antes de la recogida de la muestra, e informaron de al menos un síntoma de la enfermedad, fueron considerados casos. Se realizó un análisis logístico no condicional para evaluar la asociación entre GTS y las características de la población. Se identificaron un total de 122 (34,5%) casos, siendo un 39% de ellos fumadores y un 61% hombres. Las concentraciones medias de cotinina urinaria fueron 75,6ng/mL (74,1ng/mg de creatinina) para los no casos y 755,8ng/mL (632,1ng/mg de creatinina) para los casos (valor de p ≤ 0,01). El análisis multivariado mostró una asociación positiva entre el GTS y las horas de exposición a la luz solar, exposición a los pesticidas, un estatus peor de salud, y la asociación inversa con la cultura maderera. Este estudio presentó una alta prevalencia de GTS y sugiere que el uso de la cotinina urinaria es un biomarcador significativo para identificar casos de GTS, influenciando en la distribución por sexo. Debido a que la producción de tabaco implica perjuicios para la salud, es importante implementar medidas para prevenirlos en los agricultores del tabaco, como está estipulado en los artículos 17 y 18 del Convenio Marco para el Control del Tabaco de la Organización Mundial de la Salud.

Background: Secondhand Smoke (SHS) exposure is known to be associated with various cardiovascular and respiratory problems but its effect on pulmonary function remains unexplored. This study was done to evaluate the effect of Secondhand Smoke (SHS) exposure on lung function among non-smoking population.Methods: This cross-sectional study was conducted in Bahour, Pondicherry from 2017-2018. 350 participants, age 40 year and older, with no respiratory symptoms or prior lung diseases were included in this study. Both self-reported history and measurement of urinary cotinine level were used to evaluate the smoking status. Spirometry data, including FVC and FEV1 were used to assess lung function. Diverse variables between groups were compared using T- test and Chi-square test. Analysis of covariance (ANCOVA) adjusted for age, height, alcohol consumption, and level of exercise was used to see any statistical differences in lung function parameters between non-SHS exposed and SHS-exposed groups.Results: Among 350 non-smokers, 120 were SHS-exposed. The urinary cotinine levels clearly distinguished SHS exposure, and the mean urinary cotinine levels were 7±0.3 and 11±0.4 in non-SHS exposed group vs SHS-exposed group, respectively. However, both groups had no significant difference in lung function and was found normal.Conclusions: SHS exposure urinary cotinine is a valuable marker.

BACKGROUND: Exposure to secondhand smoke (SHS) has been shown to cause health problems. Recent studies demonstrated an association of SHS with depression and poor mental health. The urinary cotinine concentration (Ucot) is an objective indicator of exposure to SHS. In this study, we evaluated the association between depression and Ucot in adult non-smokers adults in Korea. METHODS: We analyzed the data of 3,417 adults, aged ≥19 years, who participated in the seventh Korea National Health and Nutrition Examination Survey (2016). The eligible subjects were non-smokers. Depression was evaluated using the Patient Health Questionnaire-9 (PHQ-9). The subjects were dichotomized into two groups: the high depressive symptom group (PHQ-9 ≥10 [n=185]) and the low depressive symptom group (PHQ-9 <10 [n=3,232]). The demographic, socioeconomic, and clinical characteristics of the subjects were retrieved from the survey data. Ucot was dichotomized into high-Ucot (≥10 ng/mL) and low-Ucot (<10 ng/mL). The Ucot and other characteristics were compared between the two groups. To adjust for confounding variables, we conducted a logistic regression analysis and determined the difference in Ucot between the two groups. RESULTS: After adjusting for confounders, the high depressive symptom group was found to be associated with high-Ucot (odds ratio, 1.824; 95% confidence interval, 1.020–3.262). Sex, education, socioeconomic status, marital status, occupational status, limitation of activity, and the presence of an underlying diseases (diabetes and dyslipidemia) had a significant effect on depression. CONCLUSIONS: This concluded that depression was associated with high urine cotinine level in adult non-smokers.

OBJECTIVES: To examine the factors affecting passive exposure to secondhand smoke (SHS) in non-smoking student nurses. METHODS: A cross-sectional study was performed in 196 college students who had not smoked cigarettes in the past 12 months. Urinary cotinine levels were examined to identify exposure to SHS, and social factors were identified that influenced exposure to SHS, including requests that smokers extinguish cigarettes. Logistic regression analysis was used to predict the factors influencing SHS. RESULTS: Urinary cotinine measurements showed that 32 students (16.3%) were exposed to SHS. Risk factors that increased exposure to SHS affected 80 students (40.8%) in the previous 7 days. Students who were exposed to SHS were 4.45-times more likely to have increased urinary cotinine levels than those who were not exposed. Students who asked others to extinguish their cigarettes were 0.34 times less likely to test positive than those who did not. CONCLUSION: Urinary cotinine was a useful biomarker for identifying exposure to SHS, with respect to the influence of demographic, health-related, and smoking-related factors. In non-smoking nursing students, avoiding exposure to SHS was attributed to self-assertive behavior by requesting smokers to extinguish cigarettes.

BACKGROUND@#We aimed to examine the association of exposure to environmental tobacco smoke with dental caries among preschool children. Exposure to environmental tobacco smoke was assessed in terms of urinary cotinine concentrations and pack-years of exposure to smoking by parents and other family members at home.@*METHODS@#This cross-sectional study included 405 preschool children aged 3-6 years from two preschools in Japan in 2006. Information on the smoking habits of family members living with the child was obtained from parent-administered questionnaires. Dental examination was conducted to assess dental caries, that is, decayed and/or filled teeth. Urinary cotinine levels were measured using first-void morning urine samples.@*RESULTS@#Overall, 31.1% of the children had dental caries, and 29.5% had decayed teeth. Exposure to current maternal and paternal smoking was positively associated with the presence of dental caries after controlling for covariates. More than three pack-years of exposure to maternal smoking and more than five pack-years of exposure to smoking by all family members were significantly associated with the presence of dental caries as compared with no exposure (odds ratio [OR] = 5.55, 95% confidence interval [CI] = 2.17-14.22, P for trend < 0.001 and OR = 2.00, 95% CI = 1.12-3.58, P for trend = 0.004, respectively). These exposure variables were similarly associated with the presence of decayed teeth (OR = 2.92, 95% CI = 1.23-6.96, P for trend = 0.01 and OR = 1.75, 95% CI = 0.96-3.20, P for trend = 0.03, respectively). As compared with lowest tertile of the urinary cotinine level, the highest tertile of the urinary cotinine level was significantly associated with the presence of dental caries as well as decayed teeth; the ORs for the highest vs. lowest tertile of urinary cotinine levels were 3.10 (95% CI = 1.71-5.63, P for trend = 0.012) and 2.02 (95% CI = 1.10-3.70, P for trend = 0.10), respectively.@*CONCLUSIONS@#These data suggest that exposure to tobacco smoke may have a dose-dependent influence on the development of caries.

Objetivo verificar, por meio da tecnologia de biomarcador de cotinina urinária, a ocorrência da doença da folha verde do tabaco em trabalhadores que cultivam tabaco do tipo Burley. Método estudo caso-controle pareado, com base no status tabágico e na proporção 1:4, com participação de 20 trabalhadores-caso e 91 controles. Para a coleta de dados foi realizada entrevista por meio de inquérito domiciliar e coleta de urina para exame de cotinina. Foram utilizados os testes t de Student, Mann-Whitney, qui-quadrado de Pearson ou exato de Fisher. Resultados dos 23 casos suspeitos, 20 apresentaram níveis elevados de cotinina, sinais e sintomas de dor de cabeça, irritação de pele, náusea, enjoo e mal-estar geral, principalmente pela manhã. A maioria trabalhou com tabaco molhado decorrente do sereno e o clima estava quente. Conclusão verificam-se sinais sugestivos da doença da folha verde do tabaco nos trabalhadores do tabaco Burley. Faz-se necessária a atuação de profissionais de saúde no desenvolvimento de ações promotoras de saúde e preventivas sobre essa doença relacionada ao trabalho.


Objective using the urinary cotinine biomarker to verify the occurrence of green tobacco sickness in workers who cultivate Burley tobacco. Method paired case-control study, based on smoking status and on the 1:4 ratio, with participation of 20 case workers and 91 controls. Data collection included household surveys and urine collection for cotinine examination. Student's T-Test, the Mann-Whitney test, Pearson's chi-square or Fisher's exact tests were used. Results of the 23 suspected cases, 20 showed elevated levels of cotinine, signs and symptoms of headache, skin irritation, nausea, sickness and general malaise, especially in the morning. Most had worked with tobacco that was wet from the morning dew and when the weather was warm. Conclusion there are signs suggestive of green tobacco sickness in Burley tobacco workers. The action of health professionals is necessary for the development of health promotion and preventive actions addressing work-related illness.


Objetivo certificar, por medio de la tecnología de biomarcador de cotinina urinaria, la ocurrencia de la Enfermedad del Tabaco Verde en trabajadores que cultivan tabaco del tipo Burley. Método estudio caso-control pareado, con base en el status tabáquico y en la proporción 1:4, con la participación de 20 trabajadores-caso y 91 controles. Para la recopilación de datos se realizó una encuesta por medio de cuestionario domiciliar y de recolección de orina para análisis de cotinina. Se utilizaron las siguientes pruebas t de Student, Mann-Whitney, chi-cuadrado de Pearson o exacta de Fisher. Resultados de los 23 casos sospechosos, 20 presentaron niveles elevados de cotinina, señales y síntomas de dolor de cabeza, irritación de la piel, náusea, mareo y malestar general, principalmente por la mañana. La mayoría trabajó con tabaco mojado por el sereno y en un clima caluroso. Conclusión se certifican señales sugestivas de Enfermedad del Tabaco Verde en los trabajadores del tabaco Burley. Se hace necesaria la actuación de profesionales de la salud en el desarrollo de acciones promotoras de salud y preventivas de esta enfermedad relacionado con el trabajo.

ABSTRACT Objective: To determine the frequency of active smoking among patients with asthma and individuals without asthma by self-report and urinary cotinine measurement. Methods: This was a cross-sectional study conducted in the city of Salvador, Brazil, and involving 1,341 individuals: 498 patients with severe asthma, 417 patients with mild-to-moderate asthma, and 426 individuals without asthma. Smoking status was determined by self-report (with the use of standardized questionnaires) and urinary cotinine measurement. The study variables were compared with the chi-square test and the Kruskal-Wallis test. Results: Of the sample as a whole, 55 (4.1%) reported being current smokers. Of those, 5 had severe asthma, 17 had mild-to-moderate asthma, and 33 had no asthma diagnosis. Of the 55 smokers, 32 (58.2%) were daily smokers and 23 (41.8%) were occasional smokers. Urinary cotinine levels were found to be high in self-reported nonsmokers and former smokers, especially among severe asthma patients, a finding that suggests patient nondisclosure of smoking status. Among smokers, a longer smoking history was found in patients with severe asthma when compared with those with mild-to-moderate asthma. In addition, the proportion of former smokers was higher among patients with severe asthma than among those with mild-to-moderate asthma. Conclusions: Former smoking is associated with severe asthma. Current smoking is observed in patients with severe asthma, and patient nondisclosure of smoking status occurs in some cases. Patients with severe asthma should be thoroughly screened for smoking, and findings should be complemented by objective testing.


RESUMO Objetivo: Descrever a frequência de tabagismo ativo entre pacientes com asma e indivíduos sem asma, usando questionários padronizados e dosagem da cotinina urinária. Métodos: Estudo transversal realizado em Salvador (BA), com 1.341 indivíduos, sendo 498 com asma grave, 417 com asma leve/moderada e 426 sem asma. O tabagismo foi identificado por meio de autorrelato utilizando questionários e por mensuração da cotinina urinária. Para a comparação das variáveis estudadas, utilizaram-se os testes do qui-quadrado e de Kruskal-Wallis. Resultados: Dos 55 participantes (4,1%) que se declararam tabagistas atuais, 5, 17 e 33 eram dos grupos asma grave, asma leve/moderada e sem asma, respectivamente. Desses 55, 32 (58,2%) eram tabagistas diários e 23 (41,8%) eram tabagistas ocasionais. Observaram-se níveis elevados de cotinina urinária entre não fumantes autodeclarados e tabagistas pregressos, especialmente no grupo asma grave, o que sugere omissão do hábito atual de fumar. A carga tabágica entre os fumantes e a proporção de ex-tabagistas foram maiores no grupo asma grave do que no grupo asma leve/moderada. Conclusões: O tabagismo pregresso esteve associado à asma grave. Tabagismo atual também foi observado em alguns pacientes com asma grave e detectou-se omissão em alguns casos. A investigação de tabagismo deve ser meticulosa em pacientes com asma grave e a entrevista desses deve ser complementada por uma avaliação objetiva.

OBJECTIVES: Smoking is associated with hearing loss, while the correlation between tinnitus and smoking is not fully elucidated. This study aimed to evaluate risk factors of tinnitus in adolescents in terms of smoking, and we identified a rectifiable parameter that can be serially monitored. METHODS: A cross-sectional study was conducted using data from the Korea National Health and Nutrition Examination Survey, with 2,782 participants aged 12 to 18 years, from 2008 through 2011. Participants with history of ear disease, hearing loss, and inadequate responses to questionnaires were excluded. We investigated the prevalence of tinnitus and tinnitus-related annoyance by questionnaire and sought potential risk factors in blood and urine tests and smoking history. RESULTS: The prevalence of tinnitus in the 12- to 18-year-old population was 17.5%, with 3.3% reporting tinnitus-related annoyance. On univariate analysis, the prevalence of tinnitus increased with age (P < 0.001) and was higher among girls (P=0.012). Blood tests and urinalysis showed significant correlation between tinnitus and red blood cell count, alkaline phosphatase levels, and urine cotinine (P=0.002, P < 0.001, P=0.018, respectively). In multivariate analysis, the urine cotinine level was the only parameter associated with tinnitus (odds ratio, 1.000; 95% confidence interval, 0.999 to 1.000; P=0.038). Smoking was also significantly correlated with tinnitus (P=0.043), and amount of smoking with tinnitus-related annoyance (P=0.045). However, current smoking and past smoking were not correlated with tinnitus. CONCLUSION: Urine cotinine may be a rectifiable marker for management of tinnitus in adolescents. This suggests that smoking cessation should be incorporated in the management of tinnitus in adolescents.

BACKGROUND: Green tobacco sickness (GTS), an occupational disease in tobacco harvesters, is a form of acute nicotine intoxication by nicotine absorption through the skin from the wet green tobacco plant. We carried out a questionnaire survey and measured cotinine concentration, the metabolic product of nicotine, to determine the prevalence, incidence, and risk factors of GTS in Korean tobacco harvesters. METHODS: We measured cotinine concentrations, and administered a questionnaire survey to tobacco harvesters in Cheongsong-gun, Gyeongsangbuk-do, Korea. We repeatedly measured urine cotinine concentration five times with a questionnaire survey. RESULTS: Cotinine concentration at dawn was significantly higher than that at other times; it was significantly lower during the nonharvesting period than during the harvesting period. However, little change in cotinine concentration was detected in the daytime during the harvesting period. Study participants included 20 men and 20 women. The prevalence of GTS was 37.5% and was significantly higher in women than in men (55.0% vs. 20.0%, p < 0.01). GTS incidence according to number of workdays was 3.4 occurrences/100 person days. CONCLUSION: In this study, nicotine exposure and metabolism were experimentally determined from the time of cotinine exposure, and biological monitoring was performed in each season. ABSTRACT

BACKGROUND: This study aimed to analyze environmental tobacco smoking exposure in female nonsmokers by public transportation mode using representative data of Koreans. METHODS: Data from the Second Korean National Environmental Health Survey (2012–2014) were analyzed. Urine cotinine was analyzed by public transport behavior, secondhand smoke exposure, socioeconomic factors, and health-related factors. Participants were 1322 adult females; those with the top 75% urine cotinine concentrations were assigned to the high exposure group. A logistic regression analysis was performed considering appropriate weights and stratification according to the sample design of the Second Korean National Environmental Health Survey. RESULTS: The geometric mean of urine cotinine concentrations differed according to public transportation modes: subway (1.66 μg/g creatinine) bus (1.77 μg/g creatinine), and taxi (1.94 μg/g creatinine). The odds ratio [OR] was calculated for the high exposure group. The OR of the taxi (2.39; 95% confidence interval, 1.00–5.69) was statistically significantly higher than the subway value (reference), and marginally significant after adjusted with life style, sociodemographic factors and involuntary smoking frequency (2.42, 95% confidence interval, 0.97–6.04). CONCLUSIONS: The odds ratio of passengers who mainly used taxis was marginally significantly higher than those of passengers who used subways and buses after adjusted with life style and sociodemographic factors. Implementation of supplementary measures and further studies on exposure to environmental tobacco smoking in taxis are warranted.

ABSTRACT

ABSTRACT OBJECTIVE To describe urinary cotinine levels in tobacco farmers. METHODS A cross-sectional study was conducted in 2,570 tobacco farmers. All participants that reported green tobacco sickness in the week prior to the interview plus a subsample of 492 pesticide applicators were included. We collected urinary samples and information about sociodemographic, behavioral, dietary, occupational characteristics, and pesticide poisoning during their lifetime. Stratification by sex and smoking was performed and the Wilcoxon and Kruskal-Wallis non-parametrical tests were used to analyze cotinine means. RESULTS This study included 582 individuals. There was no difference in urinary cotinine means between green tobacco sickness symptomatic and asymptomatic individuals. Among non-smokers, having picked tobacco in the previous week was associated with higher cotinine means in both genders. Cotinine levels were higher on the first day of symptoms and reduced exponentially with each day in female non-smokers. Male non-smokers had higher levels on the second day and a more gradual reduction. The cotinine level rose up to 15 cigarettes/day of consumption. CONCLUSIONS The urinary cotinine measures exposure to nicotine up to its saturation point; while green tobacco sickness, affected by tolerance, indicates nicotine poisoning. Strategies to reduce nicotine exposure in tobacco production are needed. Mechanization could be an alternative, as long as it overcame the challenge of irregular terrain and did not affect the quality of the leaf. More studies are needed to evaluate the chronic effect of nicotine exposure.

ABSTRACT

Objective To explore the in vivo exposure levels of cigarette smoking ( CS) by measuring the biomar-kers nicotine and cotinine. Methods One hundred and sixty male SD rats were divided into 15 cigarette exposure groups (10, 20 and 30 nonfilter cigarettes/day, for 2, 4, 6, 8 and 12 weeks) and a control group (without CS exposure). The rats were sacrificed at different time?points. The concentration of plasma nicotine and cotinine were measured by GC?MS/MS. Results The CS?exposed rats displayed decreased locomotor activity, ataxic gait, irregular respiration, nasal noise, and salivation after smoking exposure for 3 weeks. Rats in the CS exposure groups had lower body weight, and the reduction of body weight was time and dose related (P<0. 01). The retention time of nicotine was 7. 5 to 8. 5 min. The concentra?tion of plasma nicotine in the CS exposure groups was higher than control group (155 ± 56. 65) ng/mL. The retention time of cotinine was 11. 5 to 12. 5 min, the concentrations of plasma cotinine in CS exposure groups were higher than control group (340 ± 41. 97) ng/mL, the increase of plasma cotinine in CS groups was time?related (P<0. 05), and the exposure concentration and duration had synergistic effect on the level of plasma cotinine ( P<0. 05 ) . Conclusions CS exposurecauses structural damages in male rats. The plasma concentration of cotinine can effectively reflect the in vivo exposure lev?els of cigarette smoking, and well presents a dose?response relationship.

ABSTRACT

Introduction: One of the known factors that hindered smoking cessation is nicotine dependence. Measurement of the nicotine dependence is important to better understand cigarette smoking addiction dependence and ways to overcome it. Among methods of nicotine dependence measurement are self-reported Fagerstrom Test for Nicotine Dependence (FTND) and biochemical assessment such as saliva cotinine. Biochemical assessment can be used to measure the accuracy of the self-reported measurement of nicotine dependence. Objective: To explore the correlation between the FTND and the saliva cotinine of the smokers in three different timeline. Methods: A total of 61 male smokers who currently smoke cigarette on daily basis were recruited. The study used the one-group pretest-posttest study design and the data were collected three times. The self-reported measurement were measured by using FTND and the biochemical assessment measured by using saliva cotinine from Saliva Bio oral swab (SOS) with the sensitivity of 0.15ng/ml. Data analysis was conducted by using Pearson correlation. Results: There was a significant association between the FTND score and saliva cotinine level of the smokers at baseline, second and third data collection (p=0.014, p=0.003, p<0.001). Conclusion: Both the self-reported measurement of nicotine dependence and biochemical assessment of the smokers are correlated and it could provide reliable information of the nicotine dependence.

ABSTRACT

Unintentional environmental exposure to toxicants is associated with an aggravated health status of the general population. Involuntary smoking (IS) exposure is one of the main routes to involuntary toxicants exposure. However, few studies have attempted to understand the environmental cadmium exposure by IS exposure in the general, non-smoking population. The purpose of the current study was to examine the relationship between blood cadmium level and IS level according to gender and age. We used the Korea National Health and Nutrition Examination Survey (KNHANES) IV–VI data that included heavy metal and urine cotinine sampling with IS exposure history. The final analysis comprised 3,493 adults (1,231 males and 2,262 females) and 395 adolescents (210 males and 185 females). Linear regression was performed to estimate the association between self-reported IS exposure with urine cotinine level and blood cadmium level in non-smokers with gender and age group stratification. In final regression model, the effect values (B) (standard errors [SE]) between blood cadmium and urine cotinine level in men was 0.0004 (0.0001) and 0.0006 (0.0002) in adults and adolescents, the B (SE) in women was 0.0006 (0.0002) and 0.0016 (0.0006) in adults and adolescents. Our study revealed, for the first time, a significant association between blood cadmium and IS exposure in non-smokers. Greater efforts are needed to improve environmental justices of the general population from IS, considering the severe harmful effects of involuntary exposure to even a low level of cadmium.

BACKGROUND: Enquiry into smoking status and recommendations for smoking cessation is an essential preventive service. However, there are few studies comparing self-reported (SR) and cotinine-verified (CV) smoking statuses, using medical check-up data. The rates of discrepancy and under-reporting are unknown. METHODS: We performed a cross-sectional study using health examination data from Healthcare System Gangnam Center, Seoul National University Hospital in 2013. We analyzed SR and CV smoking statuses and discrepancies between the two in relation to sociodemographic variables. We also attempted to ascertain the factors associated with a discrepant smoking status among current smokers. RESULTS: In the sample of 3,477 men, CV smoking rate was 11.1% higher than the SR rate. About 1 in 3 participants either omitted the smoking questionnaire or gave a false reply. The ratio of CV to SR smoking rates was 1.49 (95% confidence interval [CI], 1.38-1.61). After adjusting for confounding factors, older adults (≥60 years) showed an increased adjusted odds ratio (aOR) for discrepancy between SR and CV when compared to those in their twenties and thirties (aOR, 5.43; 95% CI, 2.69-10.96). Educational levels of high school graduation or lower (aOR, 2.33; 95% CI, 1.36-4.01), repeated health check-ups (aOR, 1.45; 95% CI, 1.03-2.06), and low cotinine levels of <500 ng/mL (aOR, 2.03; 95% CI, 1.33-3.09), were also associated with discordance between SR and CV smoking status. CONCLUSION: Omissions and false responses impede the accurate assessment of smoking status in health check-up participants. In order to improve accuracy, it is suggested that researcher pay attention to participants with greater discrepancy between SR and CV smoking status, and formulate interventions to improve response rates.

ABSTRACT

Objective To explore the effect of different family tobacco control patterns on cotinine level and acute respiratory infections among infants.Methods A total of 300 infants were included,and were divided into 3 groups based on the tobacco control patterns:strictly tobacco -controlled group (97 cases),partly tobacco -controlled group (88 cases)and tobacco -uncontrolled group (1 1 5 cases).Urinary cotinine was measured in all participants.All participants were prospectively followed -up for 1 year,and the incidence of acute respiratory infections was recorded during the follow-up.Results The cotinine level of strictly tobacco -controlled group [0.45 ±0.21 (μg/L)]was significantly lower than the other two groups [1 .01 ±0.49(μg/L),1 .1 6 ±0.48(μg/L),P <0.05],and no significant differences were detected between the partly tobacco -controlled group and tobacco -uncontrolled group.The incidence of lower respiratory tract infection,not the upper respiratory tract infections,was significant different among the 3 groups (strictly tobacco -controlled group:1 8.75%;partly tobacco -controlled group:32.1 8%;tobacco -uncontrolled group:37.72%)(P <0.05).The number of upper respiratory tract infections and lower respiratory tract infections was significantly different among the three groups.The difference was significant between strictly tobacco -controlled group and partly tobacco -controlled group and between strictly tobacco -controlled group and tobacco -uncontrolled group (P <0.05).Conclusion Strict tobacco control could reduce the prevalence of passive smoking and the incidence of respiratory infections among infants.

BACKGROUND: Cotinine has been widely used as an objective marker to identify current smokers. We conducted this study to address the absence of Korean studies investigating the efficacy of immunoassays and liquid chromatography-tandem mass spectrometry (LC-MS/MS) for the detection of serum cotinine and to determine the optimal serum cotinine cut-off level for differentiating current smokers from nonsmokers. METHODS: Serum specimens were obtained from 120 subjects. They were randomly chosen to represent a broad distribution of urine cotinine levels based on a retrospective review of questionnaires and results of urine cotinine levels. We determined serum cotinine levels using the IMMULITE 2000 XPi Immunoassay System (Siemens Healthcare Diagnostics Inc., USA) and LC-MS/MS (API-4000, Applied Biosystems, USA). Correlation was analyzed between IMMULITE serum cotinine, urine cotinine, and LC-MS/MS serum cotinine levels. ROC curve was analyzed to identify the optimal IMMULITE serum cotinine cut-off level for differentiating current smokers from nonsmokers. RESULTS: IMMULITE serum cotinine levels correlated with both urine cotinine and LC-MS/MS serum cotinine levels, with correlation coefficients of 0.958 and 0.986, respectively. The optimal serum cotinine cut-off level for distinguishing current smokers from nonsmokers was 13.2 ng/mL (95.7% sensitivity, 94.1% specificity) using IMMULITE. CONCLUSIONS: This is the first study to investigate the use of LC-MS/MS for the measurement of serum cotinine and to determine the optimal serum cotinine cut-off level for the IMMULITE immunoassay. Our results could provide guidelines for differentiating current smokers from nonsmokers in the Korean population.
