ABSTRACT

Resumo A pandemia de Covid-19 potencializou o já emergente processo de intensificação das conexões virtuais. Para terapeutas de casal e família, essa grave crise de saúde pública acarretou mudanças na forma de atender clientes e interagir com colegas, exigindo a rápida transição da modalidade presencial para a modalidade remota, em muitos casos. O presente estudo sistematizou conhecimentos sobre terapia on-line com casais e famílias, trazendo considerações para a prática e a formação profissional diante da pandemia. Por meio de revisão narrativa da literatura, foram sumarizados resultados de estudos empíricos, bem como recomendações sobre aspectos técnicos, éticos e formativos. Destacaram-se, em particular, potencialidades e desafios para a utilização de tecnologias da informação e da comunicação na prática clínica com casais e famílias, incluindo indicações e contraindicações, recursos mais apropriados e seguros, relação terapêutica on-line, capacitação e supervisão. Entende-se que a Covid-19 representa uma oportunidade para revisão e reflexão da prática clínica e da formação profissional de terapeutas de casal e família no Brasil, país em que as intervenções on-line permaneciam relativamente pouco exploradas até a pandemia.


Abstract The Covid-19 pandemic has boosted the already emerging process of virtual connections. For couple and family therapists, this major public health crisis has led to changes in the way of treating clients and interacting with colleagues, requiring rapid transition from face-to-face to remote mode, in many cases. This study systematized knowledge about online therapy with couples and families, bringing considerations for practice and professional training in the face of the pandemic. By conducting a narrative literature review, we summarized results from empirical studies, as well as recommendations on technical, ethical, and formative aspects. Specifically, we highlighted both the potential and the challenges of using information and communication technologies in clinical practice with couples and families, including indications and contraindications, more appropriate and safer resources, online therapeutic relationship, training, and supervision. Covid-19 represents an opportunity for reviewing and analyzing the clinical practice and professional training of couples and family therapists in Brazil, a country in which online interventions remained relatively little explored until the pandemic.


Resumen La pandemia de Covid-19 potenció el proceso ya emergente de conexiones virtuales. Para terapeutas de parejas y familias, esa grave emergencia de salud pública acarreó cambios en la forma de atender a clientes e interactuar con colegas, exigiendo la rápida transición de la modalidad presencial para la modalidad remota, en muchos casos. El presente estudio sistematizó conocimientos sobre terapia de parejas y familias on line, trayendo consideraciones para la práctica y la formación profesional en la pandemia. Por medio de revisión narrativa de la literatura, fueron obtenidos resultados de estudios empíricos, así como recomendaciones sobre aspectos técnicos, éticos y formativos. Se destacaron, en particular, potencialidades y desafíos para usar tecnologías de la información y de la comunicación en la práctica clínica con parejas y familias, incluyendo indicaciones y contraindicaciones, recursos más apropiados y seguros, relación terapéutica on line, capacitación y supervisión. ABSTRACT

El presente estudio tuvo como objetivo describir cómo participan, en el proceso terapéutico de parejas, las diferencias socioculturales entre terapeutas y parejas consultantes de niveles socioeconómicos (NSE) distintos. Se empleó una metodología cualitativa con un diseño analítico-relacional. La muestra consistió en nueve terapeutas de NSE alto y medio-alto y cinco parejas de NSE bajo y medio-bajo de Santiago de Chile. Se realizaron entrevistas semi-estructuradas y los datos fueron analizados mediante las tres etapas de la técnica Teoría Fundamentada. Los resultados apuntan a que, a pesar de que las diferencias socioeconómicas y socioculturales no se perciben como perjudiciales para el tratamiento cuando se mantiene un vínculo cercano, éstas sí pueden influir las vivencias y tipo de intervenciones de los terapeutas, la alianza terapéutica y simetría de la relación, la evaluación que las parejas hacen sobre el profesional y la terapia, así como la posibilidad de abandono. Aquello depende, en su mayoría, del manejo y características del terapeuta. Los hallazgos adquieren relevancia para la formación y la práctica de terapeutas de pareja, así como de otras modalidades.


The aim of the present study was to describe how sociocultural differences between therapists and consultant couples of different socioeconomic levels (SEL) participate in the couple's therapeutic process. A qualitative methodology with a relational-analytic design was used. The sample consisted of nine therapists of high and middle-high SEL and five couples of low and middle-low SEL of Santiago de Chile. Semi-structured interviews were carried out and data were analyzed by the three stages of the Grounded Theory technique. Results indicate that despite the socioeconomic and sociocultural differences are not perceived as damaging for the treatment when a close bond is held, they can influence the therapists' experiences and interventions, the therapeutic alliance and symmetry of the relationship, the couples' evaluation of the therapist and the possibility of attrition. This mostly depends on the guidance and characteristics of the therapist. The findings become relevant for the training and practice of couple's therapists, as well as other modalities.


Este estudo teve como objetivo descrever como afetam o processo de terapia de casal, as diferenças socioculturais entre terapeutas e casais de nível socioeconômico (NSE) diferente. Foi utilizada uma metodologia qualitativa com um desenho analítico-relacional. A amostra foi composta por nove terapeutas de NSE alto e médio-alto e cinco casais de NSE baixo e médio-baixo de Santiago de Chile. Foram realizadas entrevistas semiestruturadas e os dados foram analisados utilizando os três estágios da Grounded Theory. Os resultados sugerem que, apesar de as diferenças socioeconômicas e socioculturais não serem percebidas como prejudiciais para o tratamento quando uma ligação estreita seja mantida, elas podem influenciar o tipo de experiências e intervenções de terapeutas, a aliança terapêutica e a simetria da relação, a avaliação que os casais fazem do profissional e da terapia, bem como favorecer o abandono. Isso depende principalmente das características e da gestão do terapeuta. Os resultados tornam-se relevantes para a formação e prática de terapeutas de casais e outras modalidades.

ABSTRACT

Este é um artigo de revisão de literatura onde é feito uma reflexão sobre as relações conjugais, primeiramente contextualizando as relações de gênero, num segundo momento trazendo a Teoria do Espaço Consciente, que explora como as mulheres são mantidas presas pelas normas culturais em barreiras sexistas. São encadeadas reflexões com a Psicologia feminista e o empoderamento das mulheres. A meta foi estabelecer relações entre a hierarquia de gênero com a manutenção da vulnerabilidade feminina presentes nas configurações conjugais, objetivando estimular terapeutas de casais a ajudarem na construção de relações conjugais mais igualitárias, já que estas caminham juntas com a satisfação conjugal, segundo os achados.(AU)


This is a literature review article where is make a reflection on marital relations, firstly contextualizing gender relations, secondly bringing the Theory of the Mindful Space, which says that women are held in spaces by cultural norms in sexist barriers. Then were made reflections with feminist psychology and women's empowerment. The aim was to establish links between gender hierarchies and the maintenance of female vulnerability present in conjugal settings, looking for encourage marital therapists to help to build more egalitarian marital relations, since those go together with marital satisfaction, accordingly the findings.(AU)

ABSTRACT

Background There are many techniques being followed in marital or couples therapy for the resolution of marital discord. In the particular module of Conjoint collaborative therapy where joint sessions with the couple run concurrently with individual sessions for each partner, an additional issue crops up which may influence the outcome independently of the marital therapy: resolution of individual psychopathology may have beneficial effects in strengthening the marriage, but may also have the opposite effect. Individual ego strengthening may disturb the homeostasis which has kept the dysfunctional marriage together. Objective: To study individual ego strengthening as a dynamic factor that influences the course and outcome of marital therapy. Discussion: It may be that many marriages which are held together because of the pressures from without rather than the desire from within may find the dynamics changed by therapy. If the individual sessions are going well, it may be that the benefits there may outpace those in the joint sessions or the progress in the relationship. Conclusion: With the resolution of individual crisis, one or both partners may no longer be willing to carry the relationship forward and a separation is the result. The individual may benefit from the ego strengthening, but it proves deleterious for the relationship. This paper explores the dynamics that may be involved in this event.

ABSTRACT

La alianza terapéutica (AT) es uno de los factores comunes que ha recibido mayor soporte empírico en relación con el resultado terapéutico. En terapia de familia y pareja resulta más complejo desarrollar la AT que en la individual. Mediante entrevistas semiestructuradas a consultantes y terapeutas, se exploraron los factores que ellos relacionaban con el desarrollo de la AT. El análisis de contenido mostró que la participación voluntaria y comprometida de los consultantes, el asumir responsabilidad por el problema, el confiar en la terapia, el desear cambiar y la congruencia entre intervención y expectativas hacia la terapia, entre otros factores, favorecen la AT.


Prior research shows that there is a robust relationship between the Therapeutic Alliance (TA) and the results of therapy. Developing the TA is more difficult in family and couples therapy than in individual therapeutic processes. Semi-structured interviews with patients and therapists were used to explore the factors they believed to be associated with the development of TA. Content analysis showed that the following factors, among others, foster TA: the voluntary and committed participation of the patients, assuming responsibility for the problem, trusting the therapist, wishing to change, and the coherence between intervention and expectations.


A aliança terapêutica (AT) é um dos fatores comuns que vem recebendo maior suporte empírico com relação ao resultado terapêutico. Acaba sendo mais complexo desenvolver a AT na terapia de família e de casal do que na individual. Mediante entrevistas semiestruturadas a consultantes e terapeutas, exploraram-se os fatores que eles relacionavam com o desenvolvimento da AT. A análise de conteúdo mostrou a participação voluntária e comprometida dos consultantes, o assumir responsabilidade pelo problema, o confiar na terapia, o desejar mudar e a congruência entre intervenção e expectativas à terapia, entre outros fatores, favorecem a AT.

ABSTRACT

PURPOSE: The purpose of this study was to examine effects of the Permission, Limited Information, Specific Suggestions, Intensive Therapy (PLISSIT) model sexual health enhancement program on, and development in, sexual function, sexual distress, marital intimacy, and subjective happiness of women with gynecologic cancer and their husbands. METHODS: The comprehensive program (4 session, 90 minutes per session) was developed based on the PLISSIT model. Participants were 43 couples, 21 assigned to the experimental group who attended the 4-week program, and 22 to the control group. Sexual function, sexual distress, marital intimacy, subjective happiness of the women, marital intimacy, subjective happiness of husbands were determined by a questionnaire that was completed by the participants before and after the program. The control group received the intervention post experiment. Chi-square test, t-test, Fisher's exact test were used to test the effectiveness of the program. RESULTS: Post intervention results showed significant differences between the groups for sexual function, sexual distress, and marital intimacy in the women and for subjective happiness in the husbands. CONCLUSION: Results indicate that the sexual health enhancement program is effective in improving sexual function, lowering sexual distress, increasing marital intimacy, and subjective happiness in women with gynecologic cancer and their husbands.

ABSTRACT

Por 11 anos desenvolvemos uma pesquisa envolvendo uma proposta interventiva com famílias que buscam atendimento em um serviço-escola, com demanda localizada na criança. Por meio da psicoterapia psicanalítica com o casal procuramos atingir a remissão dos sintomas do(s) filho(s). O objetivo deste estudo é refletir sobre a manutenção das mudanças psíquicas ocorridas nesse tipo de clínica, que abrange a esfera conjugal e familiar, ao longo do tempo. Com base na metodologia de estudo de caso e no referencial psicanalítico, foram analisados dois casos clínicos enfatizando-se as sessões de follow up: dois casais que estiveram em processo terapêutico, respectivamente, por três anos e por dois anos e meio, sendo realizadas duas sessões de acompanhamento após o término da terapia. Esse tipo de intervenção mostrou-se efetiva para a elucidação de conflitos conjugais latentes e como promovedora de um ambiente saudável e favorecedor do desenvolvimento emocional da criança.


For eleven years we have been conducting a research that consists in a proposal of intervention with families that look for therapy for a child. Through the psychoanalytical psychotherapy of couples, we try to release the child's symptoms. The aim of this article is to reflect about the maintenance over time of the psychic changes obtained in this clinical practice, which evolves conjugal and familiar levels. Making use of the case study methodology and the psychoanalytic theory, two clinical cases were analyzed, with emphasis in follow up sessions: two couples that had been in therapy for 3 years and 2,5 years, and each couple has participated in two follow up interviews after the end of the therapy. This intervention showed up to be effective on the elucidation of latent conjugal conflicts and also as a promoter of a healthy and supportive environment in the emotional development of the child.


Hace once años hemos desarrollado una investigación que consiste en una propuesta de intervención en familias que buscan atención con una demanda localizada en los niños. Por medio de la psicoterapia psicoanalítica con la pareja, intentamos alcanzar la remisión de los síntomas de lo(s) hijo(s). El objetivo de este artículo es reflexionar sobre la manutención de los cambios psíquicos ocurridos en ese tipo de clínica, que abarca la esfera conyugal y familiar, a lo largo del tiempo. A partir de la metodología del estudio de caso y del referencial psicoanalítico, dos casos clínicos fueron analizados, enfatizándose las sesiones de "follow up": dos parejas asistidas en psicoterapia, por 3 y 2,5 años, con los cuales se realizaron dos sesiones de acompañamiento después del término de la psicoterapia. Ese tipo de intervención fue efectivo en la detección de los conflictos conyugales latentes, proporcionando un ambiente favorecedor del desarrollo emocional del niño.

ABSTRACT

O objetivo deste trabalho foi realizar uma revisão dos estudos sobre os marcadores e preditores da construção e dissolução da conjugalidade em uma perspectiva sistêmica, apontando suas implicações para o campo da psicoterapia de casal. As bases de dados consultadas foram PsycINFO e SciELO, utilizando-se as seguintes palavras-chave, em múltiplas combinações, em português e inglês: revisão; terapia de casal; conjugalidade; dissolução da conjugalidade; preditores; marcadores. Foram consultados 132 trabalhos publicados até agosto de 2009. A satisfação conjugal é ressaltada como o principal construto, marcador e preditor envolvido no estudo da formação e manutenção da conjugalidade. São apresentados modelos que descrevem o divórcio como um processo complexo e longo, ressaltando-se que o uso de modelos não lineares pode melhorar a previsibilidade de dissolução da conjugalidade, a partir de fatores psicossociais. Conclui-se que os estudos interacionais trazem importantes contribuições para a clínica de casais, ao sugerir direções e intervenções. Todavia, o simples uso de marcadores e preditores, como variáveis a serem utilizadas em modelos terapêuticos, não tem se revelado útil para melhoria da eficácia terapêutica, pelo fato de serem apenas indicadores de um dado padrão em andamento. Preditores e marcadores seriam, assim, melhor compreendidos como indicadores de processos sistêmicos de estabilidade e mudança.


The goal of this paper was to review studies about markers and predictors of conjugality construction and dissolution from a systemic perspective, emphasizing the implications for couples’ therapy. PsycINFO and SciELO databases were consulted, with the following keywords (in different combinations of Portuguese and English): literature review, couples’ therapy, conjugality, conjugality dissolution, predictors, and markers. We consulted 132 studies published until August 2009. Conjugal satisfaction is pointed out as the main concept, marker and predictor involved in both the construction and maintenance of conjugality. Models describing divorce as a complex and long process are presented, emphasizing those non-linear models may improve the possibility of prediction of conjugal dissolution, based on psychosocial factors. We conclude that interactional studies bring important contributions to couples’ therapy in suggesting directions and interventions. However, the simple use of markers and predictors as variables in therapeutic models has not been improving therapeutic efficacy, because these are only indicators of certain ongoing patterns. Thus, predictors and markers would be better understood as indicating systemic processes of stability and change.

ABSTRACT

Purpose: Both the diagnosis of prostate cancer (PCa) and the physiologic outcomes of surgical treatment impact the male’s psychological sphere. However, current research advocates a refocusing of outcomes directed to the PCa “couple”. Herein we acquire insight into perspective and concordance regarding male physiological function from the standpoint of a couple recovering from PCa surgery. Materials and methods: Couples whereby the male partner had undergone primary surgical treatment for PCa were mailed a Retrospective Sexual Survey (RSS) packet consisting of male and female partner questionnaires. RSS questions surveyed physiological changes in libido, foreplay, erection and arousal, orgasm and ejaculation in addition to perceived psychological impact. Patients’ and partners’ scores were evaluated to determine the concordance of both individual items as well as domain sums. Results: Twenty-eight couples completed the questionnaires. Only about 40 percent of men and women were happy with their levels of sexual interest with 82 percent concordance. Urine loss during orgasm was reported by 43 percent of men; the majority of participants were bothered by it. Ejaculation changes were observed by 96 percent of men (concordance 96 percent) with most reporting anejaculation. A change in orgasm experience was noted by 86 percent of men (and 36 percent of their female partners, p < 0.0001). Despite the change, the majority of men and women reported being satisfied with their ability to climax. Conclusion: Our results indicate that patients and their female partners may interpret differently the same physiological outcomes of PCa surgery. This information could be useful to better counsel the PCa couple and help patients and partners adjust after surgery.
