ABSTRACT

The hypothalamus-pituitary-adrenal axis function may be impaired in patients with critical illnesses, especially cases of sepsis, named critical illness-related corticosteroid insufficiency (CIRCI). This study examined the function of the hypothalamic-pituitary-adrenal axis in normal dogs (n = 10) and dogs with critical diseases (n = 16), through determinations of endogenous ACTH (adrenocorticotropic hormone), basal cortisol and cortisol after stimulation in low doses of synthetic ACTH (1.0µg/kg/IV). The stimulation test with ACTH dose tested was verified as effective for evaluation of adrenal function in healthy and sick dogs. Ill dogs differed from healthy dogs by presenting higher basal cortisol values. Eight sick dogs presented a decrease in endogenous ACTH, basal cortisol, or Δ-cortisol. No significant differences were found between the control groups and critically ill dogs for the values of endogenous ACTH, cortisol after stimulation or Δ-cortisol. We concluded that the stimulation test with low-dose ACTH was effective for evaluation of adrenal function, as well as the fact that a considerable portion of critically ill dogs studied here, especially with sepsis, had evidence of inadequate corticosteroid response to stress.(AU)

ABSTRACT

Therapeutic plasma exchange can quickly remove the pathogenic macromolecular sub-stance in vivo, and its application is gradually expanded. With the maturity and development of plasma ex-change technology, the effect of high volume plasma exchange in liver failure, thrombotic thrombocytopenic purpura, sepsis and other diseases is obvious. But there is still controversy about the wide use of high volume plasma exchange in ICU. This review aimed to discuss the use of high volume plasma exchange in patients with critical diseases.

ABSTRACT

Objective:To explore the clinicaleffect of radial arterial puncture cannulationunder ultrasonic guidance in patients with critical diseases. Methods:From December 2016 to May 2017,120 patients under critical conditions in Department of Intensive Care Unit,Xiangya Hospital,Central South University, who received arterial cannulation, were randomly divided into 2 groups:a control group (traditional blind puncture method) and an observation group (ultrasound-guided radial arterial cannulation) (60 cases in each group). The success ratio of radial arterial puncture cannulation by one time, total success ratio of radial arterial puncture cannulation, numbers of puncture, the rate of complications and the time of consumption were compared between the 2 groups. Results:There was no statistically significant difference in the total success ratio of radial arterial puncture cannulationin the 2 groups (P>0.05).The success ratio radial arterial puncture cannulation by one time was significantly greater in the observation group than that in the control group (P<0.05);the numbers of puncture, the rate of complications and the time of consumption in the observation group were lower than those in the control group, with statistically significant (all P<0.05).Conclusion:The application of ultrasound-guided radial arterial puncturecannulation can improve the success ratio of radial arterial puncturecannulation by one time, decrease the numbers of puncture, reduce the incidence of complications and save operation time.

ABSTRACT

Objective To investigate the application of continuous blood purification in the children with critical diseases. Methods Eighteen critical patients aged 1 ~ 15 years in PICU underwent continuous blood purification(CBP). Fourteen with acute renal failure (ARF) were treated with continuous veno-venous hemmofiltration(CVVH) ,2 with Guillain-Barre syndrome and 2 with Raye's syndrome were treated with plasma exchange(PE). The changes of clinical symptoms, blood biochemistry , blood gas, and oxygenation were analyzed before and after CBP. Results After CVVH treatment, the BUN and creatinine of 14 patients with ARF were decreased from (48.6 ± 14. 8) mmol/L to(28. 9 ±5.4) mmol/L and (634. 3 ±258. 2) μ mol/L to (318.4 ± 143.5) μmol/L,K+ and pH of serum were maintained in the normal range, oxygenation was significantly improved. Breathing difficulties and muscle strength in 2 patients with GBS were ameliorated and successfully weaned from ventilator after PE. Serum ALT,AST and ammonia of 2 cases with Raye's syndrome decreased significantly and they discharged after comprehensive treatment including PE. Bleeding in puncture region were found in 3 patients, hypothermia in 2 patients. During the treatment, vital signs of patients were stable, blood pressure and pulse were not fluctuated. Conclusion CBP is an effective and safe method in the treatment of critical diseases in children.

ABSTRACT

Objective To design an integrated emergency medical treatment system mounted on helicopter, which can meet the requirements of field treatment and evacuation in emergency disasters or local wars, the system has immobile form and mobile form. Methods With the characteristics of existing helicopter considered, the components of the system were supposed to include a universal stretchers, overall structure and embedded emergency devices and support transport accessories. Results The system could realize the destination of swift response and air-and ground first-aid, the new model of swift response can made the regulation and principle of first -aid more reasonable, and increases the survival rate significantly. Conclusion Critical diseases have become the latest killer in daily life. The first-aid on spot need the new model of swift response and air -and ground first -aid, the system can decrease the invalidity and mortality.
