ABSTRACT

El reposicionamiento labial es un procedimiento quirúrgico mínimamente invasivo que se utiliza para tratar una sonrisa gingival, la cual, es una afección en la que una cantidad significativa de la encía queda expuesta cuando una persona sonríe y puede deberse a una variedad de factores, como un exceso de tejido gingival, un labio superior corto o músculos hiperactivos del labio superior, entre otros. El alargamiento clínico de la corona, por otro lado, consiste en eliminar el exceso de tejido gingival y, si es necesario, el tejido óseo para exponer una mayor parte de la corona natural del diente. Se reporta un caso clínico de paciente femenino de 31 años que presentó una sonrisa gingival provocada por hipermovilidad de labio superior y un exceso de tejido gingival localizado. El plan de tratamiento involucró una combinación de reposicionamiento labial y alargamiento de corona. Los resultados estéticos fueron significativos, con la sonrisa del paciente más equilibrada y proporcionada. Se redujo la cantidad de tejido gingival expuesto cuando la paciente sonreía y la longitud de los dientes fue más visible, lo que dio como resultado una sonrisa de aspecto más natural, además de aumentar su aceptación al sonreír.


SUMMARY: Lip repositioning is a minimally invasive surgical procedure used to treat a gummy smile, which is a condition in which a significant amount of the gum is exposed when a person smiles and may be due to a variety of factors, such as excess gum tissue, a short upper lip or overactive muscles of the upper lip, among others. Clinical crown lengthening, on the other hand, involves removing excess gingival tissue and, if necessary, bone tissue to expose more of the natural crown of the tooth. Clinical case: A clinical case of a 31-year-old female patient who presented a gummy smile caused by hypermobility of the upper lip and an excess of localized gingival tissue is reported. The treatment plan involved a combination of lip repositioning and crown lengthening. The aesthetic results were significant, with the patient's smile more balanced and displayed. The amount of the patient's exposed gum tissue when smiled was reduced and the length of the teeth was more visible, resulting in a more natural-looking smile, as well as increasing their acceptance of smiling.

ABSTRACT

Extrusive tooth movements are an important resource in orthodontic treatment and allow the manipulation of teeth and periodontal tissues. They can be performed quickly or slowly, depending on the patient's need. Rapid extrusion is indicated for cases in need of prosthetic preparation or restoration, where the bone and gingival tissues are intact, such as horizontal and oblique fractures, coronary or external root resorption, iatrogenic perforations (trepanations) and the presence of subgingival caries. The aim of this study is to describe the treatment of a patient who had a coronary fracture of the right upper central incisor, with a limit located 1 mm above the level of the bone crest. Rapid orthodontic extrusion was performed, to restore biologic distances and allow the preparation for prosthesis. It started with partial differentiated bonding of a fixed orthodontic appliance to the upper arch, to allow for a 3mm orthodontic extrusion. At the end of the extrusion, periodontal surgery was performed to increase the clinical crown and endodontic treatment. After these procedures, the case was concluded with the fixed prosthesis installation. The realization of an integrated planning allowed the restoration of aesthetics, with preservation of the functional periodontal limits for the patient.(Au)


Os movimentos dentários extrusivos constituem um recurso importante no tratamento ortodôntico e permitem a manipulação dos dentes e dos tecidos periodontais. Eles podem ser realizados de forma rápida ou lenta, dependendo da necessidade do paciente. A extrusão rápida está indicada para casos com necessidade de preparo protético ou restauração, onde os tecidos ósseo e gengival encontram-se íntegros, tais como fraturas horizontais e oblíquas, reabsorções coronárias ou radiculares externas, perfurações iatrogências (trepanações) e presença de cárie subgengival. O objetivo deste trabalho é descrever o tratamento de uma paciente que apresentava fratura coronária do incisivo central superior direito, com limite localizado 1mm acima do nível da crista óssea. Realizou se extrusão ortodôntica rápida, com a finalidade de restabelecer as distâncias biológicas e permitir o preparo para prótese. Iniciou-se com colagem diferenciada parcial de aparelho ortodôntico fixo no arco superior, para permitir a extrusão ortodôntica de 3mm. Ao término da extrusão, realizou-se cirurgia periodontal para aumento da coroa clínica e tratamento endodôntico. Após esses procedimentos, o caso foi finalizado com a instalação da prótese fixa. A realização de um planejamento integrado permitiu o restabelecimento da estética, com preservação dos limites periodontais funcionais para a paciente. (Au)

ABSTRACT

Este trabalho tem como objetivo apresentar um relato de caso no qual a paciente se queixava de desarmonia do sorriso, desta forma o aumento de coroa clínica estética e o retalho reposicionado coronalmente, foram escolhidos juntamente com a reanatomização em resina composta, para restabelecer a harmonia gengival e dentária. A paciente apresentava contorno gengival desigual, no qual uma recessão gengival no dente 22 e coroas curtas e desproporcionais nos demais incisivos anteriores foram observadas, assim como uma vestibularização de todos os dentes superiores anteriores, no qual ela se recusou a tratar com Ortodontia. Foram reanatomizadas e restabelecidas por meio do uso de resina composta pela "técnica de facetas direta". Este caso clínico evidenciou a importância da associação de técnicas de cirurgia periodontal e do trabalho multidisciplinar para que todas as queixas pudessem ser devidamente sanadas e o caso concluído com sucesso dentro dos anseios e expectativas da paciente. This work aims to present a case report in which the patient complained of smile disharmony, and in this way the aesthetic clinical crown increase and the coronally repositioned flap were chosen together with composite resin reanatomization to restore harmony gingival and dental. The patient had an unequal gingival contour, in which a gingival recession on tooth 22 and short and disproportional crowns on the other anterior incisors were observed, as well as a vestibularization of all the anterior superior teeth, in which she refused to treat with orthodontics. They were reanatomized and restored through the use of resin composed by the "direct facet technique". This clinical case evidenced the importance of the association of techniques of periodontal surgery and of the multidisciplinary work so that all the complaints could be duly remedied and the case concluded successfully within the patient's expectations and expectations. ABSTRACT

The purpose of this case report and review is to present the successful management of complicated crown-root fracture of maxillary central incisor with a multidisciplinary approach. A 57-year-old male patient reported with a complaint of unpleasant appearance due to a broken 21. Clinical and radiological examination revealed a complicated oblique crown-root fracture extending below the gingival margin. The tooth was endodontically treated. Surgical crown lengthening with osseous recontouring was performed. After 7 days prefabricated temporary crown was cemented, 6 weeks after surgery fiber post was placed followed by composite core build up. The tooth was restored with a permanent restoration of porcelain fused to the metal crown with gingival porcelain. Examination after 3 months revealed the good periodontal health, esthetic, normal function and patient satisfaction. The key factor in the successful functional and esthetic rehabilitation of a complicated crown-root fracture is weighing of all the available options with a multidisciplinary approach.

A satisfactory understanding of the relationship betweenperiodontal tissues and restorative dentistry is supreme tosafeguard satisfactory form, function, esthetics, and comfortof the teeth and its supporting structures. The tooth and itssupporting structures shall be considered as one biologic unit.Most of the dental surgeons are not aware of the importantrelationship, specific concepts such as biologic width, itsmaintenance and applications of crown lengthening in casesof biologic width violation. In the case of extensive caries,sub-gingival perforation and post and core placement inendodontic therapy, the concept of biologic width gains itsimportance. Biologic width is essential for the preservationof periodontium, which ultimately decides the success ofrestorative procedures. This article reviews the anatomy,categories, evaluation, violation, methods to correct theviolation of biologic width and its relationship to periodontalhealth and restorative dentistry.

ABSTRACT

Objective@#To compare the consistency of the biological widths measured by using cone-beam CT (CBCT) and periodontal probe in patients with two different gingival biotypes.@*Methods@#Totally 27 patients [13 males, 14 females, (37.6±13.7) years old], who planned to receive the crown lengthening surgery, were recruited under the inclusion and exclusion criteria in Department of Periodontology, School of Stomatology, The Fourth Military Medical University during November 2017 to June 2018. A total of 40 teeth (14 front teeth, 26 posterior teeth) were involved in this study. The patients were divided into two groups according to their gingival biotypes: thin gingival biotype [5 males, 8 females, (40.2±15.0) years old, 21 teeth] and thick gingival biotype [8 males, 6 females, (35.1±11.9) years old, 19 teeth]. All the teeth were checked before crown lengthening procedures by using CBCT, and the biological widths and sulcus depths were measured during the surgery by using periodontal probes (Hu-Friedy, U S A). The data were recorded and statistically analyzed.@*Results@#There were no significant differences of the biological widths between the two measuring methods amongst all of the 40 teeth [periodonial probe: (1.64±0.26) mm; CBCT: (1.69±0.20) mm], amongst 21 thin gingival biotype teeth [periodontal probe: (1.49±0.19) mm; CBCT: (1.57±0.12) mm] and amongst 19 thick gingival biotype teeth [periodontal probe: (1.80±0.21) mm; CBCT: (1.87±0.18) mm] (P>0.05). There were no significant differences of the biological widths [anterior teeth: (1.59±0.15) mm, posterior teeth: (1.67±0.29) mm, P=0.42] and of the sulcus depths [anterior teeth: (2.00±0.28) mm, posterior teeth: (2.11±0.43) mm, P=0.44] between anterior teeth and posterior teeth. The difference of biological widths, measured by two methods respectively, between thin and thick gingival biotype groups was statistically significant (P<0.01). There were significant differences of the sulcus depths, measured by the periodontal probes, between the thin [(1.93±0.28) mm] and thick [(2.24±0.41) mm] gingival biotype groups (P<0.01).@*Conclusions@#The biological widths measured by CBCT is consistent with those measured by using periodontal probes. The biological widths and the depths of the sulcus of thin and thick gingival biotypes are different.

ABSTRACT

OBJECTIVES: Crown-root fracture and cervical caries in maxillary premolars constitute a challenge in cases of subgingival placement of restoration margins. Surgical extrusion has been practiced successfully in permanent anterior teeth. The aim of the present retrospective study was to assess the clinical outcome of surgical extrusion after orthodontic extrusion in maxillary premolars. MATERIALS AND METHODS: Twenty-one single, tapered root maxillary premolars with subgingival crown-root fracture or caries were included. Presurgical orthodontic extrusion was performed on all teeth to prevent root resorption. Extent of extrusion and rotation was determined based on crown/root ratio. The postoperative splinting period was 7 to 14 days. Clinical and radiographic examination was performed at an interval of 1, 2, and 3 months. RESULTS: After the mean follow-up of 41.9±15.2 months, failure was observed as increased mobility in 3 of 21 cases. No significant difference was observed in the outcome of surgical extrusion based on tooth type, age, sex, 180° rotation, or time for extraction. Furthermore, marginal bone loss was not observed. CONCLUSION: Surgical extrusion of maxillary premolars can be a possible therapeutic option in cases of subgingival crown-root fracture.

ABSTRACT

Periodontal tissue, especially the alveolar bone, are closely associated with the progress and efficacy of orthodontic treatment. Prior to and during orthodontic treatment, dentists should fully evaluate the status of periodontal hard tissues to prevent clinical problems. This article aims to discuss bone issues associated with orthodontic treatment, including gingival papilla absence, alveolar bone insufficiency, excessive cortical resistance, and altered passive eruption, etc. The mechanism and prevention methods of these problems are also described.

ABSTRACT

Crown lengthening is one of the most common surgeries in clinical practice. Under the premise of ensuring the biologic width, the adequate crown is exposed by resecting the periodontal soft tissue and (or) hard tissue to meet the prosthodontic and (or) aesthetic requirements. Considering the various advantages of oral laser, such as safe, precise, minimally invasive and comfort, laser has become a promising technology which can be used to improve the traditional crown lengthening. In this review, the principles and characteristics of laser application in crown lengthening, especially in the minimally invasive or flapless crown lengthening will be reviewed. Its pros and cons will also be discussed.

ABSTRACT

In the oral cavity, the teeth undergo wear and corrosion throughout their lives. Progressive and constant tooth wear is a natural phenomenon of aging, but wear and corrosion due to specific factors are pathological factors. It can cause pathological damage of the occlusal surface, aesthetic problems, dimensional loss and jaw joint disorders. This case is a 26-year-old female patient with general tooth abrasion and erosion on the entire dentition. Diagnostic wax-up was fabricated based on the information including digital facial analysis, physiological stabilization, and evaluation of anterior crown length. Through the digital analysis, the necessary guides for crown lengthening were prepared and the mastication function and esthetics were evaluated by using temporary crowns. Definitive prosthesis was fabricated with the zirconia restorations. The results were satisfactory when they were observed 3 months of follow-up.

RESUMEN: Introducción: Uno de los procedimientos realizados con mayor frecuencia para el restablecimiento de la estética en la zona anterosuperior es el alargamiento de corona, el cual está indicado ante la presencia de una longitud insuficiente de la corona clínica de un diente, cuando existe caries o fracturas subgingivales y para la mejora de la estética en pacientes con margen gingival desigual. Objetivo: El objetivo de este artículo es presentar el alargamiento de corona estético realizado en una paciente que presentaba márgenes discrepantes y pérdida de las papilas en la zona anterosuperior, la cual fue remitida del área de prótesis para poder realizar posterior rehabilitación protésica. Resultados: Mediante el procedimiento quirúrgico se logró la corrección y restablecimiento de los márgenes gingivales. Conclusiones: La zona anterosuperior es el área estéticamente más comprometida, por lo que es indispensable la planeación interdisciplinaria del tratamiento para asegurar el éxito del mismo.


ABSTRACT: Introduction: One of the most frequently performed procedures for aesthetics restoration in the maxillary anterior area is the crown lengthening, which is indicated in the presence of an insufficient length of the clinical crown of a tooth, of cavities or subgingival fractures and for the improvement of aesthetics in patients with uneven gingival margin. Objective: The aim of this article is to present the aesthetic crown lengthening performed in a patient with discrepant margins and loss of the papillae in the maxillary anterior area, referred from the prosthesis department for subsequent prosthetic rehabilitation. Results: Through the surgical procedure, the correction and restoration of the gingival margins were achieved. Conclusions: The maxillary anterior area is the most aesthetically compromised area, which is why the interdisciplinary planning of the treatment is essential to ensure its success.

ABSTRACT

The aim of this case report was to evaluate the clinical outcomes following modified lip repositioning and gingivectomy along with depigmentation in a patient with excessive gingival display. A 22 year old female patient came with the chief complaint of gummy smile and blackened gums. On evaluation, the patient presented with hyperactive lip and moderate vertical maxillary excess. Aless invasive modified lip repositioning procedure was carried out. A partial thickness strip of mucosa was removed leaving the midline frenum intact and the lip mucosa was sutured to the mucogingival line. As the clinical crown length was shorter than the anatomic crown length, crown lengthening by gingivectomy was performed. Laser depigmentation was carried out to treat gingival hyperpigmentation. The patient was evaluated after 2 weeks, 1 and 6 months respectively. An approximate reduction in gingival display of around 3 mm was found and the results were stable at the end of 6 months. Gingival repigmentation had occurred at the end of 6 months, therefore depigmentation procedure was repeated inorder to the maintain the esthetics. In this case, we emphasize on the stable and satisfactory treatment outcome of a non invasive approach made to treat excessive gingival display instead of more invasive surgical treatment.

ABSTRACT

Objetivo: Mostrar una alternativa de tratamiento para las fracturas coronorradiculares complicadas, que restablece la función y la estética de las piezas traumatizadas. Caso clínico: Una paciente de 16 años se presentó en la guardia del Centro Odontológico OSEP tras haber sufrido un accidente vial que le produjo un traumatismo dentoalveolar. El diagnóstico fue fractura coronaria sin exposición pulpar de la pieza dentaria 12 y fractura coronorradicular complicada de la pieza 11. El tratamiento incluyó restauración con resina compuesta de la pieza 12 y endodoncia, extrusión ortodóntica, alargamiento coronario y rehabilitación con perno de fibra de cuarzo y corona de resina compuesta de la pieza 11. Conclusiones: El tratamiento de las fracturas coronorradiculares representa un desafío para el odontólogo. Los resultados del tratamiento, en este paciente, fueron excelentes, ya que lograron devolver estética y función (AU)


Aim: To show an alternative treatment for complicated crown-root fractures, which restores function and aesthetics of the traumatized teeth. Case report: This paper reports the case of a 16-yearold girl who attends the OSEP Dental Center, stating that she had suffered a traffic accident resulting in a dentoalveolar trauma. The diagnosis was a coronary fracture without pulp exposure of tooth 12, and complicated crown-root fracture of tooth 11. Treatment included composite resin restoration of tooth 12 and endodontic, orthodontic extrusion, crown lengthening and rehabilitation with quartz fiber post and composite resin crown of tooth 11. Conclusions: The treatment of crown-root fractures poses a challenge for the dentist. The results of the treatment in this patient were excellent, and aesthetics and function were restored (AU)

ABSTRACT

A cirurgia plástica periodontal tem sido indicada para pacientes que apresentam descontentamento com seu sorriso ou desarmonia em relação a sua anatomia gengival. O procedimento tradicional tem sido realizado através da elevação de um retalho para expor o tecido ósseo e subsequente, osteotomia/osteoplastia. Este procedimento exige sutura e um maior tempo de cicatrização, tornando o pós-operatório mais desfavorável em relação às técnicas menos invasivas. Com o avanço das técnicas minimamente invasivas, tem sido proposta a técnica Cirúrgica Periodontal "Flapless", um procedimento minimamente invasivo e inovador, com resultados clínicos extremamente favoráveis. Tem como principal indicação pacientes com o biótipo periodontal fino e intermediário e o planejamento cirúrgico consiste na realização de uma tomografia computadorizada do paciente utilizando um afastador bucal tipo expandex para afastar os tecidos moles e facilitar a visualização da crista óssea vestibular e, desta forma,visualizar e calcular a quantidade de tecido ósseo a ser removido. Após a excisão do tecido gengival, é realizada a osteotomia via sulco gengival, sem abertura de retalho, com utilização de microcinzéis. Um dos principais benefícios é a finalização do procedimento sem a necessidade de suturas, menor desconforto pós-operatório, melhor reparação tecidual e harmonia estética. As evidências clínicas e científicas mostram que o aumento de coroa clínica com a técnica "Flapless" atinge resultados satisfatórios em menor tempo, melhor pós-operatório e previsibilidade (AU)


The increase in clinical crown "Flapless" is a common procedure employed at periodontal plastic to patients with discontent with its gummy smile or with the in harmonious of its gumlines hape. The traditional procedure is performed it the flap lifting to expose the bone and then, osteotomy/ osteoplasty. As a resultof the flap, it requires suturing, making it uncomfortable postoperative. However, with the advancement in a esthetic and constant demands for searching improved results, it was proposed the "Flapless" Technique, which consist of minimally invasive and innov ative procedure, with very favorable clinical out comes. It is mainly indicated to treat patients with thin and intermediate periodontal biotype. Aided by computerized tomography with Expandex, we have a predictability of the amount of bone to be removed. After excision of gingival tissue, gingival sulcus via osteotomy is performed with out opening flap, using micro-chisels. One of the main benef its is the completion of the procedure without sutures, less postoperative discomfort, optimizedt issue repair and perfect as the tic harmony. Clinical and scientific evidence shows that te increase in clinical crown flapless reache satisfactory results in a shorter time, best post operative and predictability in clinical out come. (AU)

ABSTRACT

Objective: To discuss the therapeutic effect of related periodontal surgical treatment and aesthetic rerepair in the patient with residual root and crown of anterior teeth induced by unfitting prosthesis and to clarify the curative effect of periodontal surgical treatment and aesthetic re-repair in the anterior teeth area, and to provide guidance for making the program of clinical treatment. Methods: Two years after repair of the anterior teeth of a patient, the restorations loosed; on the one hand the patient had a toothache of chewing food accompanying gingival bleeding, on the other hand the upper labial frenum was too high, furthermore the upper left incisor apical area showed low density images. After the consultation of periodontics, endodontics, prosthodontics and other disciplines doctors, anterior teeth root canal re-treatment and periodontal basic treatment were adopt as well as crown lengthening combined with labial freneetomy to restore the normal biological width, additionally apply not only guided bone regeneration (GBR) but also platelet rich fibrin (PRF) to repair the defects of apical bone. One month later, provisional restorations were put into use and then three months later permanent repairs were conducted. Results: The corresponding gingival and gingival papilla shape recoverd perfectly and the height of gingiva coordinated with that of adjacent teeth, in the meantime the upper labial frenum attached normally and there was not obvious space between upper incisors; the morphology and function examination of restoration in maxillary anterior region were both well. The follow-up X-ray showed that the bone density of the defect area was the same as that of the peripheral bone without obvious boundaries. Conclusion: The aesthetic repair of thin biotype referring to anterior area, especially in the complicated re-treatment cases, the intervention of the appropriate periodontal surgical treatment is not only beneficial to obtain the obvious short-term effect, but also positive to stabilize longterm effects.

It is reported that the causes of unaesthetic proportion of anterior teeth vary widely. Especially, when the unaesthetic tooth proportion of the mandibular incisors arises due to the wear of the anterior teeth accompanied by the compensation of the alveolar bone, it may cause serious functional and aesthetic problems. In such case, it should be considered that the evaluation of vertical dimension and tooth proportion as well as smile line, soft tissue and hard tissue morphology. And, increase of vertical dimension or clinical crown lengthening followed by prosthodontic restorations is needed to improve the interdental mesial/distal, width/length ratio considering the anterior guidance. This case report demonstrates functional and aesthetic improvements through systematic diagnosis and treatment procedures in a 48-year-old male patient with unaesthetic anterior teeth proportion because of tooth wear accompanied by the compensation of alveolar bone and defect of several central incisors due to chronic periodontitis.

ABSTRACT

Clinical crown lengthening procedure would be an effective method for overcoming adverse clinical condition such as short abutment length. There are three kinds of methods in clinical crown lengthening, those are, surgical crown lengthening, orthodontic extrusion and surgical extrusion. Clinicians have to try their best to choose a proper method among those for favorable results. This report aims to review the considerations in each method with various cases and to suggest a decision flow for appropriate selection.

ABSTRACT

Objective:To discuss the therapeutic effect of related periodontal surgical treatment and aesthetic rerepair in the patient with residual root and crown of anterior teeth induced by unfitting prosthesis and to clarify the curative effect of periodontal surgical treatment and aesthetic re-repair in the anterior teeth area,and to provide guidance for making the program of clinical treatment.Methods:Two years after repair of the anterior teeth of a patient,the restorations loosed;on the one hand the patient had a toothache of chewing food accompanying gingival bleeding,on the other hand the upper labial frenum was too high,furthermore the upper left incisor apical area showed low density images.After the consultation of periodontics,endodontics,prosthodontics and other disciplines doctors,anterior teeth root canal re-treatment and periodontal basic treatment were adopt as well ascrown lengthening combined with labial freneetomy to restore the normal biological width,additionally apply not only guided bone regeneration (GBR) but also platelet rich fibrin (PRF) to repair the defects of apical bone.One month later,provisional restorations were put into use and then three months later permanent repairs were conducted.Results:The corresponding gingival and gingival papilla shape recoverd perfectly and the height of gingiva coordinated with that of adjacent teeth,in the meantime the upper labial frenum attached normally and there was not obvious space between upper incisors;the morphology and function examination of restoration in maxillary anterior region were both well.The follow-up X-ray showed that the bone density of the defect area was the same as that of the peripheral bone without obvious boundaries.Conclusion:The aesthetic repair of thin biotype referring to anterior area,especially in the complicated re-treatment cases,the intervention of the appropriate periodontal surgical treatment is not only beneficial to obtain the obvious short-term effect,but also positive to stabilize longterm effects.

ABSTRACT

Objective@#To observe the long-term clinical treatment outcome and the influencing factors of the outcome for the teeth receiving modified crown lengthening surgery combined with root canal treatment and post-core crown restoration. To summarize the clinical guidelines of modified crown lengthening surgery in selection of indications and for mulation of treatment planning.@*Methods@#Fifty-seven patients with a total of 67 teeth receiving modified crown lengthening surgery combined with root canal treatment and post-core crown restoration for at least a 6 months' follow-up period between July 2004 and July 2013 were recruited in this retrospective study by phone call interviews. The patients' clinical outcomes were evaluated by the combination of clinical examination, radiograph and questionnaire regarding patient-reported outcome of the last follow up (≥9 months post modified crown lengthening surgery and ≥6 months after definite crown restorations). All of the treated teeth were classified into two groups, group A (teeth with good clinical treatment outcome) and group B (teeth with poor clinical treatment outcome), based on the defined criteria including patients' satisfaction with the function and esthetics of the teeth and absence of periodontal, endodontic and prosthodontic complications. The potential influencing factors of clinical treatment outcome were also determined by Logistic regression analysis.@*Results@#Vertical root fracture in 1 tooth was found on its periapical film and the tooth was deemed hopeless. Thus, the survival rate is 99% (66/67) for the multidisciplinary treatment approach. Seventy-two percent (48/67) of the teeth achieved good clinical treatment outcome and 28% (19/67) of the teeth developed one or several complications. In group B (teeth with poor clinical treatment), 16 out of teeth exhibited periodontal complications with bleeding on probing (BOP) positive mostly found. Logistic regression analysis demonstrated that plaque control (OR=21.392, P=0.014), edge form (OR=7.610, P=0.011), and smoking experience (OR=7.315, P=0.018) were the risk factors influencing the clinical treatment outcome of modified crown lengthening surgery combined with root canal treatment and post-core restoration.@*Conclusions@#Modified crown lengthening surgery combined with root canal treatment and post-core restoration has a good and stable clinical effect in the observational time of 6-114 months. Plaque control, smoking status and edge form of the tooth appeared to be the influencing factors of this multidisciplinary treatment approach.

ABSTRACT

Introduction: The crown lengthening surgery and the transurgical restoration are alternative procedures to enable the restoration of teeth inaccessible to the conventional techniques and restore the lost biological space. Objective: This study aimed to present clinically the reconstruction of a right maxillary second molar through crown lengthening surgery associated with transurgical restoration with composite resin in a patient treated in the Integrated Clinics of the University of Ribeirão Preto. Case report: Patient E.H.C.V., female, aged 51 years, attended the dental clinic of the University of Ribeirão Preto. The anamnesis showed a favorable health history and the clinical examination showed great tooth crown destruction of tooth #17. The treatment planning included the crown lengthening periodontal surgery with regularization of alveolar bone on the buccal surface of tooth #17, rubber dam isolation, the restauration of the compromised area with composite resin, and suture in the same session. After 7 days, the sutures were removed and the restoration polished. Conclusion: It can be concluded that the use of this technique is a viable, fast, and satisfactory solution, with good results, favoring the health and aesthetics, returning function and better social life to the patient.
