ABSTRACT

RESUMEN Es probable que los problemas de salud relacionados con los trastornos del sueño se encuentren en aumento en las zonas rurales de América Latina, en parte relacionados con los cambios en el estilo de vida, el trabajo con turnos nocturnos y otros factores. Sin embargo, poco se sabe sobre la prevalencia de estas condiciones en lugares rurales remotos. En el Proyecto Atahualpa, hemos investigado los trastornos del sueño utilizando instrumentos de campo estandarizados asociados con tecnología sofisticada como la polisomnografía. Una mala calidad del sueño se ha asociado con fragilidad y con la presencia de marcadores de neuroimagen de enfermedad de vasos cerebrales y atrofia cortical global. Por el contrario, la ingesta dietética de pescado oleoso se asoció con una mejor calidad del sueño. También se ha investigado la prevalencia y algunos correlatos del síndrome de las piernas inquietas (enfermedad de Willis-Ekbom) y se encontró una prevalencia relativamente superior a la descrita en la mayoría de las regiones tropicales, así como la asociación de esta afección con el estrés psicológico. Finalmente, un informe preliminar muestra una elevada prevalencia de apnea obstructiva moderada a severa en los residentes de Atahualpa de ≥60 años (27%) y su correlación con daño subcortical difuso de origen vascular. Se necesitan más estudios para comprender mejor las consecuencias de los síntomas relacionados con el sueño en dichas poblaciones.


ABSTRACT Health problems related to sleep disorders are likely on the rise in rural areas of Latin America, partly related to changes in life style, shift work, and other factors. However, little is known on the burden of these conditions in remote rural settings. In the Atahualpa Project, we have investigated sleep disorders using standardized field instruments associated with sophisticated technology such as polysomnography. A poor sleep quality has been associated with frailty and with the presence of neuroimaging markers of cerebral small vessel disease and global cortical atrophy. On the contrary, dietary intake of oily fish was associated with a better sleep quality. We have also investigated the prevalence and some correlates of the restless legs syndrome (Willis-Ekbom disease), and found a prevalence relatively higher than that described in most other tropical regions, as well as the association of this condition with psychological stress. Finally, a preliminary report shows a high prevalence of moderate-to-severe obstructive sleep apnea in Atahualpa residents aged ≥60 years (27%) and its correlation with diffuse subcortical damage of vascular origin. Further studies are needed to better understand consequences of sleep-related symptoms in underserved populations.

ABSTRACT

Resumen Objetivo: Determinar los factores asociados a la cantidad de horas de sueño, somnolencia diurna e insomnio antes y durante un periodo de clases y exámenes. Material y métodos: Se evaluó a 384 alumnos de ambos sexos (74,1% mujeres), quienes reportaron su peso y estatura. A cada estudiante se le aplicó el Cuestionario de Insomnio y la Escala De Somnolencia de Epworth en el periodo de clases y periodo de exámenes. Resultados: Al comparar la somnolencia diurna, insomnio, latencia al sueño y cantidad de horas de sueño entre periodo de clases y exámenes se observó un incremento en la latencia al sueño (p<0,05) durante los exámenes. Al realizar la comparación según estado nutricional se observó, en el caso de las mujeres, que aquellas que presentan sobrepeso/obesidad tienen una menor cantidad de horas de sueño en ambos periodos (p<0,05); en hombres, los que presentan sobrepeso/obesidad tienen una mayor somnolencia diurna en exámenes (p<0,01). Al realizar la regresión logística tomando como variable dependiente somnolencia diurna, en mujeres se incrementa el riesgo de somnolencia diurna: OR=3,1 (IC95 % 1,1-8,8); en cambio, la ausencia de insomnio es un factor protector para somnolencia diurna: OR=0,06 (IC95 % 0,01-0,35). Conclusión: En periodo de exámenes se incrementa significativamente la latencia al sueño, en especial en mujeres; en hombres se observa una disminución de las horas de sueño. Finalmente, las mujeres presentan un mayor riesgo de somnolencia diurna; en cambio, la ausencia de insomnio es un factor protector. Ni el estado nutricional ni el consumo de cafeína se asociaron con problemas de sueño.


Abstract Objective: To determine the factors associated with sleep duration, daytime somnolence and insomnia before and after a period of classes and exams periods. Material and methods: 384students (74.1% female) were evaluated, who reported their weight and height. The insomnia questionnaire and Epworth sleepiness scale were administered to each student during class period and exams period. Results: When comparing daytime somnolence, insomnia, sleep latency and the amount of sleeping hours between classes and exam periods an increase in sleep latency (p <0.05) was observed during exams. Fewer hours of sleep were observed in both periods (p <0.05) in women with overweight / obesity. In men, those who are overweight / obese show a higher daytime somnolence during exam periods (p <0.01). By performing the logistic regression analysis using daytime somnolence as a dependent variable, being a woman increases the risk of daytime somnolence (OR = 3.1, 95 % CI 1.1 to 8.8), whereas the absence of insomnia is a protective factor for daytime somnolence (OR = 0.06, 95 % CI 0.01 to 0.35). Conclusion: Sleep latency is significantly increased during exam period, especially in women; in men a decrease is observed in sleep hours. Finally, women present high risk of diurnal somnolence, in difference the absence of insomnia is a protective factor, however the nutritional status and caffeine intake was not associated with sleeping problems.

ABSTRACT

El objetivo de esta investigación fue evaluar en pacientes con Síndrome de Apneas-Hipopneas del Sueño (SAHS), la calidad de vida y el impacto de la somnolencia diurna. Se incluyeron 100 sujetos (50 pacientes con SAHS y 50 sujetos sanos) que respondieron al Cuestionario de Salud (SF-36), Cuestionario de Impacto Funcional del Sueño (FOSQ) y a la Escala de Somnolencia de Epworth (ESE). Respecto al grupo control, el grupo clínico mostró diferencias estadísticamente significativas (p < 0.05) en la mayoría de las variables estudiadas. Los factores más influyentes fueron el índice de masa corporal, el índice de apneas-hipopneas y el índice de desaturación. Este estudio demuestra la importante repercusión que el SAHS tiene en la calidad de vida de los pacientes.


The objective of this research was to evaluate quality of life and impact of daytime sleepiness in patients with Sleep Apnea/Hypopnea Syndrome (SAHS). 100 subjects were included: 50 patients with OSAS and 50 healthy subjects, who responded to the Short Form-36 Health Survey (SF-36), the Functional Outcomes of Sleep questionnaire (FOSQ) and the Epworth Sleepiness Scale (ESS). The comparison between clinical and control group showed statistically significant differences (p < 0.05) in most of the variables studied. The results found that body mass index, apnea-hypopnea index and desaturation index were the most important affecting factors. This study shows the great impact OSAS has on the quality of life of patients.

ABSTRACT

Allergic rhinitis is a disease with an increasing prevalence throughout the world that severely affects the quality of life of individuals suffering from it. Nasal congestion is the most common and bothersome symptom, and is often associated with sleep-disordered breathing, which is thought to be the reason for sleep impairment in individuals with rhinitis. The end result is a decrease in quality of life and productivity and an increase in daytime sleepiness. Treatment with intranasal corticosteroids has been shown to reduce nasal congestion. Data on sleep-related end points from clinical trials of intranasal corticosteroids indicate that this reduction is associated with improved sleep, reduced daytime fatigue, and improved quality of life. Other therapies, such as montelukast, also have a positive influence on congestion and sleep. This review examines nasal congestion and the associated sleep impairment of allergic rhinitis patients. It explores the adverse effects of disturbed sleep on quality of life and how these conditions can be reduced by therapies that decrease congestion.
