ABSTRACT

ABSTRACT Objective: The present article aims at reporting the clinical case of a patient who suffered trauma at two years of age, causing almost complete apical displacement of the deciduous maxillary left central incisor and of the permanent incisor. Methods: Ectopia secondary to intrusion was minimized by surgical removal of the ectopic tooth, and the left permanent canine was submitted to orthodontic traction to replace the extracted tooth. Results: The treatment period lasted 36 months, resulting in correct occlusion and a good aesthetic outcome. Conclusions: Dental transposition carried out by means of orthopedic traction is a good alternative in cases of a very unfavorable ectopic tooth position.


RESUMO Objetivo: o presente relato de caso descreve uma paciente que sofreu trauma aos dois anos de idade, causando deslocamento apical quase completo do incisivo central esquerdo decíduo e do incisivo permanente. Métodos: a ectopia resultante da intrusão foi minimizada pela remoção cirúrgica do dente ectópico, e o canino permanente esquerdo foi tracionado ortodonticamente para substituir o dente extraído. Resultados: o período de tratamento durou 36 meses, obtendo-se uma oclusão correta e um bom resultado estético. Conclusões: a transposição dentária realizada através da tração ortopédica é uma boa alternativa em casos de dentes ectópicos em posições muito desfavoráveis.

ABSTRACT

La transposición dental es una alteración de la posición de dos o más dientes en el arco dentario. Su etiología, aún desconocida, se asocia entre otros factores, a la herencia, al trauma dentario y a la persistencia del diente deciduo. El diente donde más se observa es en el canino superior, transposicionado con el primer premolar. Puede ser unilateral o bilateral, y completa o parcial. Esta anomalía puede provocar diversas complicaciones de la estética y la función del aparto estomatognático; por lo que requiere de diagnóstico precoz y en muchas ocasiones, de un enfoque multidisciplinario que permita la evaluación individual de cada caso para su correcto tratamiento. Se presentó un caso clínico de transposición bilateral de caninos superiores, en un paciente con antecedentes de traumatismo dentario no tratado, se tornó complejo su tratamiento debido a las características individuales del paciente.


The dental transposition is an alteration of the position of two or more teeth in the dental arch. Its etiology, still unknown, is associated to heritage, dental trauma and the persistence of the deciduous tooth, among other factors. The tooth where it is observed the most is the upper canine, transposed with the first pre molar. It may be unilateral or bilateral, and complete or partial. This anomaly could cause different complications of the stomatognathic system esthetics and function; therefore, the precocious diagnosis and, in many cases, a multidisciplinary approach are needed to allow the individual evaluation of each case for its correct treatment. It was presented a clinical case of upper canine bilateral transposition in a patient with antecedents of non-treated dental trauma. The treatment became complex due to the patient´s individual characteristics.

ABSTRACT

A 12-year-old girl was referred to our clinic for evaluation of an unaesthetic dental appearance. All permanent teeth were erupted, while the deciduous maxillary right canine was retained. Cone-beam computed tomography revealed a complete transposition of the maxillary left canine and first premolar involving both the crowns and the roots. Initial cephalometric analysis showed a skeletal Class III pattern, with a slight maxillary retrusion and a compensated proclination of the upper incisors. The patient's teeth were considered to be in the correct position; therefore, we decided to attempt treatment by correcting the transposition and using only orthodontic compensation of the skeletal Class III malocclusion. After 25 months of active orthodontic treatment, the patient had a Class I molar and canine relationship on both sides, with ideal overbite and overjet values. Her profile was improved, her lips were competent, and cephalometric evaluation showed acceptable maxillary and mandibular incisor inclinations. The final panoramic radiograph showed that good root parallelism was achieved. Two-year follow-up intraoral photography showed stable results.

INTRODUCCIÓN: el objetivo de esta investigación fue caracterizar la transposición dental en una población que asistió a la consulta de odontopediatría en Cali, Colombia, entre enero de 1997 y octubre de 2011 y describir las anomalías dentales asociadas. MÉTODOS: estudio descriptivo de tipo retrospectivo. Un total de 1.809 radiografías panorámicas se revisaron estableciendo la presencia de la transposición dental y las anomalías dentales asociadas. Las siguientes variables se registraron: edad a la cual se diagnosticó, sexo, antecedentes familiares, médicos y estomatognáticos, trauma a la dentición temporal y permanente, localización y presencia de otra anomalía dental asociada. RESULTADOS: de 1809 pacientes revisados, cuatro presentaron transposición dental (0,22%) todos de sexo femenino. El rango de edad fue de 7 a 15 años. No se encontraron antecedentes familiares, médicos ni estomatognáticos relevantes. Tres de los casos se presentaron en el maxilar, uno en la mandíbula. No hubo predominio de lado de la transposición. Uno en el maxilar fue bilateral, los otros dos fueron unilaterales. La anomalía dental encontrada fue la persistencia de los caninos temporales en todos los casos. CONCLUSIONES: en este estudio se encontró prevalencia de la transposición dental del 0,22% (4 casos en 1809 pacientes), todos los casos se presentaron en pacientes de sexo femenino y el 75% de los casos fueron en el maxilar superior.


INTRODUCTION: the purpose of this study was to characterize dental transposition in a population consulting a pediatric dental service in Cali, Colombia, between January 1997 and October 2011 and to describe associated dental anomalies. METHODS: this was a retrospective descriptive study. A total of 1809 panoramic radiographs were reviewed in order to establish the presence of dental transposition and associated dental anomalies. The following variables were recorded: age at diagnosis; gender; family, medical, and stomatognathic histories; trauma to the primary and permanent dentition; location and presence of other associated dental anomalies. RESULTS: out of the 1809 patients reviewed, four had dental transposition (0.22%), all of them females. The age range was 7-15 years. No relevant family, medical or stomatognathic histories were found. Three of the cases occurred in the maxilla and one in the mandible. There was no side predominance for the transpositions. One of the mandible cases was bilateral and the other two were unilateral. The associated dental anomaly usually found was persistent temporary canines in all the cases. CONCLUSIONS: this study found a prevalence of dental transposition of 0.22% (4 cases in 1809 patients); all of the cases occurred in female patients and 75% of them were located in the upper maxilla.

ABSTRACT

La transposición dentaria es una anomalía caracterizada por el cambio de posición entre 2 dientes o más en el arco dentario. Esta anomalía puede provocar diversos problemas funcionales y estéticos y por estas razones necesita de un correcto diagnóstico para un tratamiento precoz, que involucra generalmente un abordaje multidisciplinario. El objetivo de este artículo fue relatar 3 casos donde fue diagnosticada la presencia de transposición dentaria. Se discutieron los posibles abordajes terapéuticos y se hizo énfasis en la importancia del diagnóstico precoz(AU)


The dental transposition is an anomaly characterized by the change in position among 2 or more teeth in the dental arch and it may to provoke different functional and esthetic problems being needed a correct diagnosis and an early treatment involving by general, a multidisciplinary approach. The aim of present paper is to relate the history of 3 cases in which the presence of a dental transposition was diagnosed and also to discuss on the possible therapeutical approaches emphasizing the significance of the early diagnosis(AU)
