ABSTRACT

Los deportes de combate conllevan un alto riesgo de padecer traumatismos dentoalveolares. Este estudio se evaluó la incidencia de traumatismos y lesiones orofaciales relacionadas con los deportes de combate, junto con la evaluación de las actitudes y hábitos de los deportistas respecto al uso de protectores bucales. Este estudio se realizó mediante sistema de encuestas digitales distribuidas en diferentes clubes deportivos donde se practican deportes de combate y no combate. Los deportistas de combate tienen más probabilidad sufrir alguna lesión orofacial que los no deportistas de combate. El sexo masculino y la experiencia son factores predisponentes en la aparición de traumatismos y lesiones orofaciales, siendo las laceraciones de tejidos blandos las más frecuentes. Los deportistas de combate llevan más protectores bucales que los deportistas en general, siendo el protector tipo II el más usado. Los deportistas que no llevan protectores no consideran que sea necesario. En caso de sufrir una avulsión dental, la mayoría de los participantes consideran que es posible reimplantar un diente avulsionado.


Combat sports carry a high risk of suffering dentoalveolar trauma. This study evaluated the incidence of dentoalveolar lesions related to combat sports, together with the evaluation of athletes' attitudes and habits regarding the use of mouth guards. This study was carried out using a digital survey system distributed in different sports clubs where combat and non-combat sports are practiced. Combat athletes are more likely to suffer a dentoalveolar lesion than non-combat athletes. Male sex and experience are predisposing factors in the appearance of dentoalveolar lesions, with soft tissue lacerations being the most frequent. Combat athletes wear more mouth guards than athletes in general, with the type II protector being the most used. Athletes who do not wear protectors do not consider it necessary. In case of suffering a dental avulsion, most of the participants consider that it is possible to reimplant an avulsed tooth.

La pérdida de superficie dental se define como una condición en la que el desgaste de la estructura dental es excesivo según la edad. Esta pérdida patológica se relaciona comúnmente con una combinación de factores como erosión, abrasión, atricción o parafunción. La técnica de Dahl es una alternativa válida que resuelve los inconvenientes surgidos en este tipo de situaciones como ser la compensación dentoalveolar. Este caso corresponde a un paciente de 63 años que acudió al Hospital Dental de la Universidad de Manchester, derivado por su odontólogo particular. Al momento del examen clínico se constató pérdida moderada a severa de la superficie dental palatina e incisal localizada en el sector anterior de los dientes, sobremordida profunda y compensación alveolar dental. Fueron realizadas restauraciones directas a mano alzada de canino a canino. Luego del relleno incisal se constató una mordida abierta posterior de aproximadamente 1 a 1,5 mm. El contacto oclusal posterior fue restablecido totalmente en 3 meses.


A perda da superfície dentária é definida como uma condição em que o desgaste da estrutura dentária é excessivo de acordo com a idade, essa perda patológica está comumente relacionada a uma combinação de fatores, sendo eles: erosão, abrasão, atrito ou parafunção. Uma abordagem restauradora nestas situações representa um desafio para a reabilitação oral principalmente pela Compensação dentoalveolar. A técnica de Dahl é uma alternativa válida para resolver este problema. Este caso corresponde a um paciente de 63 anos que procurou o Hospital Odontológico da Universidade de Manchester, encaminhado por seu dentista particular. No momento do exame clínico, observou-se perda moderada da superfície dentária no setor anterior, sobremordida profunda e compensação alveolar dentária. Foram realizadas restaurações diretas da mão livre do canino para o canino. Após o enchimento incisal, observou-se mordida aberta posterior de aproximadamente 1 a 1,5 mm. O contato oclusal total foi restabelecido em 3 meses.


Tooth Surface Loss (TSL) is defined as a condition in which the wear of the tooth structure is excessive according to age. This pathological loss is commonly related to a combination of factors such as erosion, abrasion, attrition, or parafunction. The Dahl technique is a valid alternative that solves the problems that arise in this type of situation, such as dentoalveolar compensation. This case describes a 63-year-old patient who attended the Dental Hospital of the University of Manchester, referred by his private dentist. At the time of the clinical examination, moderate to severe loss of the palatal and incisal tooth surface located in the anterior sector of the teeth, deep overbite and dental alveolar compensation were found. Direct free-hand restorations were performed from canine to canine. After incisal filling, a posterior open bite of approximately 1 to 1.5 mm was observed. Posterior occlusal contact was completely restored in 3 months.

Abstract Objectives: Odontogenic infections are frequent and can spread, leading to complications such as sepsis and the need for admission to an Intensive Care Unit (ICU). The purpose of this study was to perform a computed tomography analysis of the fascial space involvement and correlate with personal data, laboratory tests, length of hospital stays and admission to the ICU in patients with odontogenic infections who required hospitalization. Methods: Patients with odontogenic infections admitted between June 2017 and May 2018 were prospectively evaluated. The fascial spaces involved were studied using computed tomography with contrast. The possible correlations of tomographic findings with comorbidities, nutritional status, the causative tooth, laboratory tests, length of hospital stays and admission to the ICU were analyzed. Results: We identified 66 cases of odontogenic infections which were admitted in the period analyzed. The involvement of primary spaces (86.7%) predominated, followed by secondary ones (8.7%), and cervical (4.6%). The most frequently involved fascial spaces were submandibular (27.1%), buccal (20.8%), sublingual (18.3%), submental (16.7%), and others (17.1%). There were significant differences between laboratory tests (p < 0.001), mean hospital stays (p < 0.001), and admissions to the ICU (p < 0.001) depending on the number of fascial spaces involved. There was no influence of comorbidities, nutritional status, or causative tooth on fascial space involvement. Conclusion: There was a relationship between greater involvement of fascial spaces assessed by computed tomography and higher values of laboratory tests, more extended hospitalization stays and admission to the ICU. ABSTRACT

La fractura coronaria es el traumatismo dentoalveolar (TDA) más frecuente en la dentición permanente. Cuando existe exposición pulpar, se conoce como fractura coronaria complicada (FCC). Actualmente, en estos casos, se recomienda realizar terapias que permitan preservar la vitalidad del tejido pulpar, tanto en dientes maduros como inmaduros. El presente reporte describe el caso de un paciente de 9 años con FCC en diente 2.1, donde se realizó tratamiento con terapia pulpar vital (pulpotomía de Cvek), y adhesión de fragmento dentario. En los controles posteriores al TDA (1 semana, 4 meses, 1 y 2 años) el diente estaba asintomático, se observó formación de puente dentinario y continuación de desarrollo radicular con cierre apical, indicios de que el tejido pulpar se mantuvo vital. En este caso se destaca la importancia del diagnóstico, tratamiento adecuado, junto al seguimiento de un diente con desarrollo radicular incompleto.


Crown fracture is the most frequent dental traumatic injury (DTI) in permanent dentition. When there is pulp exposure, it is known as a complicated crown fracture. Currently, in these cases, it is recommended to carry out therapies that allow preserving the vitality of the pulp tissue, both in mature and immature teeth. This report describes the case of a 9-year-old patient with a complicated crown fracture in tooth 2.1, who underwent treatment with vital pulp therapy (Cvek pulpotomy), and adhesion of the dental fragment. In follow-up sessions (after 1 week, 4 months, 1 and 2 years), the tooth was asymptomatic. It was observed dentin bridge formation and the continuation of root development with apical closure, indications that the pulp tissue has remained vital. In this case, the importance of proper diagnosis and treatment is highlighted, together with the follow-up of a tooth with incomplete root development.

ABSTRACT

El trauma maxilofacial es un problema de salud pública, comúnmente asociado a traumatismo dentoalveolar. Su prevalencia es alta, siendo más frecuente en poblaciones de riesgo, como personal de Fuerzas Armadas, esto por sus actividades laborales, generando gran impacto en el paciente. Caracterizar, según la literatura, el traumatismo dentoalveolar concomitante a trauma maxilofacial en el personal profesional de Fuerzas Armadas. Se realizó una revisión narrativa en cuatro bases de datos, en octubre del 2020. Se incluyeron publicaciones de máximo 5 años de antigüedad, en inglés o español, con resumen disponible, estudios primarios y revisiones sistemáticas. Se excluyó publicaciones no disponibles en texto completo y reportes de casos. Se incluyeron 15 artículos. Existe un déficit de evidencia sobre la asociación que existe entre traumatismo dentoalveolar y traumatismo maxilofacial en la población estudiada. Encontrándose que solo el 6,7 % de estos incluía en su análisis la concomitancia entre ambos tipos de traumas. Sin embargo, los diagnósticos más prevalentes consistieron en fracturas coronarias y mandibulares, respectivamente, asociadas a actividades de entrenamiento y combate. Se establece que el tipo de trauma maxilofacial más frecuente en la población profesional de Fuerzas Armadas es la fractura mandibular y en relación al traumatismo dentoalveolar, la fractura coronaria. En cuanto a la etiología, destacan las heridas de bala, explosivos y accidentes en vehículos, afectando principalmente a personal del Ejército entre 18 a 30 años. Es importante mencionar que los artículos incluidos en esta revisión que hacen referencia a la concomitancia entre el traumatismo dentoalveolar y maxilofacial son escasos y no se encuentran actualizados, por lo que, se necesita continuar investigando en esta temática.


The maxillofacial injuries are a public health issue commonly associated to dentoalveolar injuries. Its high prevalence in risk population such as the Armed Forces personnel, due to their work activities, generates a great impact on the patient. Characterize, according to the literature, dentoalveolar injuries within the maxillofacial injuries in professional Armed Forces personnel. A narrative research was conducted on October 2020 with four data bases. Only 5-year-old publications were considered both in English and Spanish, including their available summary, primary studies and systematic revisions. Publications without full access or report cases were not included. Fifteen scientific papers were included. There is a deficit of evidence between maxillofacial and dentoalveolar injuries in the target population. Only 6.7 % of the research included a joint analysis between both traumas, however the most prevalent diagnosis consisted in coronaries and mandibular fractures, in that order, associated mainly to training and combat activities. The most frequent maxillofacial injury within the Armed Forces personnel is the mandibular fracture, and in relation with dentoalveolar injuries is the coronary fracture. Regarding the etiology, gunshot wounds, explosives and car accidents are featured affecting mainly between 18 to 30 years old army personnel. It's relevant to highlight that the scientific papers included in this revision about the association between dentoalveolar and maxillofacial injuries are poor and not updated. Further research is needed in this issue.

RESUMEN Se presentó el caso de una paciente femenina de 9 años con historia de traumatismo a nivel de incisivos centrales superiores, por lo cual recibió tratamiento para apicoformación con hidróxido de calcio durante un periodo prolongado, que luego de realizarle la obturación de los conductos radiculares presentó un cuadro de inflamación aguda en labio superior y vestíbulo bucal. El tratamiento incluyó terapia antimicrobiana, desbridamiento quirúrgico amplio bajo anestesia general, apicectomía de ambos incisivos, obturación a visión directa de los conductos y sellado apical de los dientes con gutapercha. El diagnóstico anatomopatológico fue el de osteomielitis crónica agudizada del maxilar. Año y medio después se observó ausencia clínica de signos inflamatorios y adecuada cicatrización del tejido óseo. Se concluyó que, aunque los reportes de osteomielitis crónica del hueso maxilar son escasos, debe estar presente dentro del diagnóstico diferencial cuando se evalúan pacientes en edad pediátrica con historia de traumatismo dentoalveolar y procesos inflamatorios en la región maxilofacial.


ABSTRACT A nine-year old female patient presented with a history of trauma to the upper central incisors. She received treatment for apicoforming with calcium hydroxide for a prolonged period, who after root canal obturation presented acute inflammation of the upper lip and buccal vestibule. Treatment included antimicrobial therapy, extensive surgical debridement under general anesthesia, apicoectomy of both incisors, and direct visual obturation of the canals and apical sealing of the teeth with gutta-percha. The anatomopathologic diagnosis was acute chronic osteomyelitis of the maxilla. One and a half years later, clinical absence of inflammatory signs and adequate healing of the bone tissue were observed. It was concluded that, although reports concerning chronic osteomyelitis of the maxillary bone are rare events, this type of infection should be always present at the time of the differential diagnosis for evaluating pediatric patients with a history of dentoalveolar trauma and inflammatory processes in the maxillofacial region.


RESUMO Apresentamos o caso de uma paciente feminina de 9 anos de idade com histórico de trauma nos incisivos centrais superiores, para a qual ela recebeu tratamento para apicoformação com hidróxido de cálcio por um período prolongado, que após a obturação do canal radicular apresentou inflamação aguda do lábio superior e vestíbulo vestibular. O tratamento incluiu terapia antimicrobiana, desbridamento cirúrgico extensivo sob anestesia geral, apicoectomia de ambos os incisivos, obturação visual direta dos canais e vedação apical dos dentes com guta-percha. O diagnóstico patológico foi uma osteomielite crônica aguda da maxila. Um ano e meio depois, houve uma ausência clínica de sinais inflamatórios e uma cicatrização adequada do tecido ósseo. Concluiu-se que, embora os relatos de osteomielite crônica do osso maxilar sejam escassos, ela deveria estar presente no diagnóstico diferencial ao avaliar pacientes pediátricos com histórico de trauma dentoalveolar e processos inflamatórios na região maxilofacial.

ABSTRACT

The objective of this study was to evaluate the tooth crown inclination in maxillary and mandibular arches in Class III malocclusion individuals, to identify the presence and magnitude of compensation. The study was conducted on 46 plaster casts of individuals with Class III malocclusion, of both genders (25 males and 21 females), with mean age 21 years and 1 month, with indication for compensatory orthodontic treatment. The dental casts models were scanned and the tooth crown inclinations were measured and compared with standard values by the independent t test. All tests considered a significance level of 5% (p<0.05). According to the results there was extensive variation in tooth inclination in Class III malocclusions. Values distribution for incisors highlighted the significant concentration of maxillary incisors in the area of positive values, compared with a very expressive concentration of mandibular incisors in the area of negative values. Compared with normal standards, in Class III malocclusions, the maxillary posterior teeth exhibited smaller palatal inclination than normal, while the mandibular incisors and second molars presented greater lingual inclination. It was concluded that the analysis of inclinations of all crowns of both dental arches in Class III malocclusions, compared with normal standards, evidenced the presence of natural compensation for maxillary posterior teeth, with reduced palatal inclination, as well as increased lingual inclination in mandibular incisors. The parameters of compensation naturally present in Class III malocclusions described, especially in lower incisors, would help clinicians when compensatory treatment is considered. (AU)


O objetivo deste estudo foi avaliar a inclinação das coroas dentárias nas arcadas superior e inferior em indivíduos com má oclusão de Classe III, para identificar a presença e a magnitude da compensação. O estudo foi realizado em 46 modelos de gesso de indivíduos com má oclusão de Classe III, de ambos os sexos (25 homens e 21 mulheres), com média de idade de 21 anos e 1 mês, com indicação de tratamento ortodôntico compensatório. Os modelos de gesso foram digitalizados e as inclinações da coroa dos dentes foram medidas e comparadas com os valores de normalidade pelo teste t independente. Todos os testes estatísticos adotaram um nível de significância de 5% (p <0,05). De acordo com os resultados, houve grande variação na inclinação dentária nas más oclusões de Classe III. A distribuição dos valores para os incisivos destacou a concentração significativa de incisivos superiores na área de valores positivos, em comparação com uma concentração muito expressiva de incisivos inferiores na área de valores negativos. Comparados aos padrões de normalidade, nas más oclusões de Classe III, os dentes posteriores superiores exibiram inclinação palatina menor que o normal, enquanto os incisivos e segundos molares inferiores apresentaram inclinação lingual maior. Concluiu-se que a análise das inclinações de todas as coroas de ambas as arcadas dentárias nas más oclusões de Classe III, comparadas aos padrões de normalidade, evidenciou a presença de compensação natural para os dentes posteriores superiores, com diminuição da inclinação palatina, bem como aumento da inclinação lingual dos incisivos inferiores. Os parâmetros de compensação naturalmente presentes nas más oclusões de Classe III descritos, especialmente nos incisivos inferiores, podem auxiliar o clínico quando o tratamento compensatório é considerado. RESUMEN: Objetivo: Sintetizar los resultados de estudios epidemiológicos sobre Traumatismo Dentoalveolar (TDA) en la población chilena. Material y métodos: Se realizó una revisión sistemática de la literatura para identificar estudios poblacionales a nivel nacional, regional, provincial y comunal, además de datos de servicios dentales de atención primaria y secundaria sobre TDA en Chile. Resultados: Se incluyeron 2 estudios con representatividad nacional y 8 realizados en centros de atención en salud. A nivel nacional, la prevalencia de TDA a los 6 años fue de un 2.57%, y a los 12 años, 4.97%. En los estudios realizados en centros de salud, el diagnóstico más prevalente en dentición primaria fueron las lesiones de tejidos de soporte y en dentición permanente, la fractura coronaria. Fue más frecuente la afectación de un solo diente, y los incisivos centrales superiores fueron los dientes más afectados. Conclusiones: Los estudios representativos de la población en Chile son escasos, realizados hace más de una década y representativos sólo de la población de 6 y 12 años, lo cual pone de manifiesto la necesidad de mayor información epidemiológica sobre el TDA en la población chilena.


ABSTRACT: Aim: To summarize the evidence on epidemiological studies about traumatic dental injuries (TDI) in Chile. Methods: A systematic literature review was carried out in two databases to identify population studies at national, regional, provincial and community levels, as well as reports from primary and secondary dental care services, regarding TDI in Chile. Results: Two studies with national representation and eight studies from primary and secondary healthcare centers were included in the analysis. National prevalence for 6-year-old children was 2.57%, and 4.97% for 12-year-old children. While the most frequent diagnosis in primary dentition was traumatic injury involving tooth-supporting tissues, the most commonly reported diagnosis in permanent dentition was crown fracture. Single tooth affection was more frequent, and the upper central incisors were the most affected teeth. Conclusions: Data on representative studies about TDIs in Chile are scarce. Few studies, carried out more than a decade ago and only on 6- and 12-year-old children are available. There is a need for further epidemiological information about TDIs in Chile.

RESUMEN: La parálisis cerebral (PC) es un trastorno neurológico que afecta aproximadamente 2 de cada 1000 nacidos vivos. Se describe como un grupo de trastornos del desarrollo del movimiento, equilibrio y postura, que causan limitación de actividad. Algunos estudios han planteado que la PC podría aumentar el riesgo de traumatismos dentoalveolares (TDA). El objetivo de este estudio fue revisar la evidencia científica disponible sobre prevalencia y tendencias de TDA en pacientes con PC, y compararla con la prevalencia de TDA en sujetos sin PC. Se realizó una búsqueda sistemática en Pubmed, Scielo y Lilacs. Se evaluaron títulos y resúmenes, excluyendo casos clínicos, revisiones sistemáticas, artículos duplicados y/o no atingentes al tema. Se incluyeron artículos que reportan prevalencia de TDA en personas con PC y se extrajo la información sobre el tipo estudio, metodología, prevalencia, tipo de diente, de TDA más frecuente en PC y datos de prevalencia de grupo control. Se identificó un total de 32 artículos, de los cuales se seleccionaron e incluyeron 12 estudios para la revisión sistemática y de ellos cuatro se utilizaron en el metaanálisis. La prevalencia de TDA en personas con PC reportada en estos estudios mostró un rango entre 10,6-76,5 %. Los incisivos superiores fueron el tipo de diente más afectado por TDA. Sólo cuatro estudios comparan con un grupo control. La prevalencia de TDA fue mayor en sujetos con PC que en sujetos sanos (odds ratio 1,58 [95 % IC= 0,66 - 3,77]). Sin embargo, esta diferencia no fue estadísticamente significativa y los estudios incluidos presentaron una alta heterogeneidad.


ABSTRACT: Cerebral palsy (CP) is a neurological disorder that affects approximately 2 out of every 1,000 live births. It is described as a group of developmental disorders of movement, balance, and posture, which cause activity limitation. Some studies have suggested that CP may increase the risk of dental trauma injuries (DTI). The objective of this study was to review the available scientific evidence on the prevalence and trends of DTI in people with CP, and to compare it with the prevalence of people without it. A systematic search was carried out in Pubmed, Scielo and Lilacs. Titles and abstracts were evaluated, excluding clinical case reports, systematic reviews, duplicate articles and/ or not relevant to the topic. Articles reporting the prevalence of DTI in people with CP were included. It was extracted information about the study type, methodology, prevalence of DTI, type of tooth, type of DTI in CP subjects and the prevalence of DTI from the control group, when available. A total of 32 articles were identified, of which 12 studies were selected and included for the systematic review and of these four were used in the meta-analysis. The prevalence of DTI in people with CP reported in these studies showed a range between 10.6-76.5 %. The upper incisors were the type of tooth most affected by DTI. Only four studies compared the DTI prevalence with a control group. The prevalence of DTI was higher in subjects with CP than in people without it (odds ratio 1.58 [95 % CI = 0.66 - 3.77]). However, this difference was not statistically significant and the included studies were highly heterogeneous.

ABSTRACT Objective: This article aims to discuss the multidisciplinary approach required in the treatment of cases of impaction and ankylosis of permanent teeth, associated with a history of trauma, considering the psychological state of the child and family when faced with a traumatic case of bullying, by reporting the complex treatment of a central incisor needing to be orthodontically moved across the midline. Conclusion: This clinical case was a major challenge, which included complex multidisciplinary procedures. Results and stability after 26 months of retention indicated successful orthodontic space closure of two maxillary teeth, without the use of implants or prostheses, in an adolescent patient who had a history of dental trauma, alveolar bone loss, and an uncertain initial prognosis.


RESUMO Objetivo: O presente artigo tem como objetivo discutir a abordagem multidisciplinar necessária no tratamento de casos de impacção e anquilose de dentes permanentes, associados a histórico de trauma, considerando o estado psicológico da criança e da família diante de situações traumáticas de bullying, por meio do relato do tratamento complexo de um incisivo central que precisava ser movido ortodonticamente através da linha média. Conclusão: Esse caso foi um grande desafio, que incluiu procedimentos multidisciplinares complexos. Os resultados e a estabilidade após 26 meses de contenção indicaram fechamento ortodôntico bem-sucedido do espaço de dois dentes superiores, sem o uso de implantes ou próteses, em uma paciente adolescente que apresentava histórico de trauma dentário, perda de osso alveolar e prognóstico inicial incerto.

ABSTRACT

ABSTRACT: Persistent Idiopathic Dentoalveolar Pain (PIDAP) is an orofacial neuropathic pain, which can be difficult to diagnose and is usually accompanied by increasing anxiety from both the patient and the treating dentist. A case of a 38-year-old female patient is presented, and it is shown the diagnostic process and therapeutic approach. The interdisciplinary management accompanied by several pharmacological lines is highlighted: Botulinum toxin was used as an adjunctive treatment allowing it to decrease systemically administered medications dosing and therefore its possible side effects. This condition usually affects psychosocial aspects of the patient and has a major impact on his quality of life. It is very important before initiating an invasive clinical treatment, obtaining a clear differential diagnosis and assessing in some cases the presence of non-odontogenic pain, such as PIDAP.


RESUMEN: El Dolor Idiopático Dentoalveolar Persistente (DIDAP), es un dolor neuropático orofacial, que puede resultar difícil de diagnosticar y generalmente se acompaña de creciente angustia tanto de parte del paciente como también del odontólogo tratante. Se presenta un caso de una paciente femenina de 38 años en donde se demuestra el proceso diagnóstico y abordaje terapéutico. Se resalta el manejo interdisciplinario acompañado de varias lineas farmacológica: la toxina Botulínica se utilizó como tratamiento coadyuvante para disminuir la dosis de medicamentos administrados por vía sistémica y por ende sus posibles efectos secundarios. Esta condición habitualmente abarca aspectos psicosociales del paciente y tiende a verse sumamente afectada su calidad de vida. Es de suma relevancia antes de iniciar un tratamiento clínico invasivo, obtener un diagnóstico diferencial claro y valorar en algunos casos la presencia de dolor no ontogénico, como el DIDAP.

RESUMEN Se presentó el caso de un niño de 8 años que acudió al Servicio de Estomatología del Policlínico Universitario "Chiqui Gómez Lubián", de Santa Clara, provincia Villa Clara. En el mismo se observó avulsión del incisivo central superior derecho, fractura no complicada de la corona en incisivo central superior izquierdo y una marcada vestibuloversión. Se confeccionó un aparato placa Hawley de acuerdo a las características y necesidades del paciente, con el fin de lograr la recuperación estética, mantener la longitud del arco y corregir la vestibuloversión en el mismo. El aparato permitió obtener un resultado satisfactorio, que ha influido psicológicamente en el paciente al elevar su autoestima y salud bucal.


ABSTRACT A boy of 8 years old was seen in the dental service at the Policlínico Universitario "Chiqui Gómez Lubián", in Santa Clara, Villa Clara. Examination revealed avulsion in the right upper central incisor, uncomplicated crown fracture in left upper central incisor and an improper alignment of the teeth. Taking into account patients´ characteristics and requirements, it was fabricated a Hawley´s retainer in order to achieve a better aesthetic outcome, maintain the length of the dental arch and correct the improper alignment of the teeth. The appliance has achieved a satisfactory outcome, which has had a psychological influence on the patient by improving his self-esteem and oral health.


RESUMO Foi apresentado o caso de um menino de 8 anos que veio ao Serviço de Estomatologia do Policlínico Universitario "Chiqui Gómez Lubián", em Santa Clara, província de Villa Clara. Neste, observou-se avulsão do incisivo central superior direito, fratura não complicada da coroa em incisivo central superior esquerdo e vestibuloversão acentuada. Um dispositivo de placa de Hawley foi confeccionado de acordo com as características e necessidades do paciente, a fim de se obter recuperação estética, manter o comprimento do arco e corrigir a vestibuloversão nele. O dispositivo permitiu obter um resultado satisfatório, o que influenciou psicologicamente o paciente, elevando sua autoestima e saúde bucal.

RESUMEN El trauma dentoalveolar es considerado la segunda causa de urgencias odontológicas, sien los niños y adolescentes los más afectados. La etiología está relacionada a la edad; en niños preescolares, las caídas son una causa muy común de lesiones traumáticas dentales, mientras que en los niños de edad escolar, las lesiones son más frecuentes por actividades deportivas. Suelen suceder en entornos académicos como la escuela, donde el docente es el principal sujeto a quien acuden para la atención preliminar de este tipo de eventos. Por tales argumentos son importantes los conocimientos y actitudes que posean los docentes, debido a que dichas acciones determinarán el pronóstico y tratamiento, más aun, por la relación existente entre el tiempo transcurrido y las acciones realizadas por parte del profesional odontológico. El objetivo de esta revisión sistemática fue describir la existencia de conocimientos y actitudes de docentes de primaria y secundaria sobre el manejo del trauma dentoalveolar; en la cual N=9 publicaciones cumplieron con los criterios de elegibilidad, empleando las bases de datos Pubmed, Scielo y Clinical Keys y descriptores en idioma inglés y español. Se evidencia que existe conocimiento inadecuado, mientras que la actitud del docente frente al trauma dental en la mayoría de los estudios no fue evaluada.


ABSTRACT Dentoalveolar trauma is considered the second cause of dental emergencies, with children and adolescents being the most affected. The etiology is related to age: in preschool children, falls are a very common cause of traumatic dental injuries, while in school-age children, injuries are more frequent from sports activities. Dental falls usually happen in academic environments such as school, where the teacher is the main subject to whom ones go for the preliminary attention of this type of event. For such reasons, the knowledge and attitudes possessed by teachers are important, because their actions will determine the prognosis and treatment, especially due to the relationship between the time elapsed and the actions carried out by the dental professional. The objective of this systematic review was to describe the existence of knowledge and attitudes of primary and secondary school teachers on the management of dentoalveolar trauma. In the revision we evidence N = 9 publications met the eligibility criteria, using the Pubmed databases, Scielo and Clinical Keys and descriptors in English and Spanish. It is evident that there is inadequate knowledge, while the teacher's attitude towards dental trauma in most of the studies was not evaluated.

ABSTRACT Intrusion is defined as displacement of a tooth into the alveolar bone. In this report, we presented a patient with 15 years of delayed diagnosis of maxillary teeth intrusion into the nasal cavity. A 30-year-old male presented to our clinic with nasal discharge. He had been in a traffic accident 15 years ago. Radiographic examination revealed intrusion of two anterior teeth into the nasal cavity. In patients with maxillofacial trauma, the possibility of having an accompanying dentoalveolar trauma must always be kept in mind. While a computerized tomography (CT) scan is useful in diagnosis of such traumas, it must be performed in all patients with maxillofacial trauma to avoid misdiagnosis.

ackground: Maxillofacial fractures are one of the mostfrequent and complicated problems of oral and maxillofacialsurgery. Within these fractures, mandible is one of the mostaffected bones. The most important aim of the treatment is toprovide healing on both functional and cosmetic aspects. Thisretrospective study was undertaken to analyse and assess thepattern of mandibular factures in emergency.Materials and Methods: 80 patients who had receivedtreatment for mandibular fractures were included in this study.All the demographic details of the patients were obtained fromthe hospital records. Patients with a history of congenitalosseous defects and deranged vitamin D and calcium levelswere excluded from this study. The patients were divided intothree age groups. SPSS software was used for statisticalanalysis.Results: In the current study, 57 out of the 80 patientsselected retrospectively were males (71.25%) and 23 werefemales (28.75%). It was seen that most of cases of fracturewere seen in the young age group of 20-30 years (42.50%).36.25% and 21.25% cases were seen in the age group of 31-40 years and more than 40 years respectively. This studyobserved that maximum cases of single mandibular fractureswere dentoalveolar fractures comprising 28 out of 80 cases(35%). Similarly, maximum fracture cases were observed in theyoung age group of 20-30 years and even this co-relation wasnot significant statistically with P-value of .088.Conclusion: Young males presented with a greaterpercentage of fracture cases and the most common singlemandibular fractures were the dentoalveolar fractures.

La odontología deportiva es la rama de la medicina deportiva que se ocupa de la prevención y el tratamiento de las lesiones bucodentales y las enfermedades orales asociadas al deporte y el ejercicio. Por lo que se hace necesario la intervención de profesionales de la salud bucodental para hacer evaluaciones del deportista y el deporte que practica para ver los riesgos que puede tener de desarrollar y sufrir lesiones bucodentales como traumas dentoalveolares y lesiones no cariosas. Y de este modo realizar las recomendaciones a cada paciente que practica deporte, de los elementos que deben usar como equipamiento bucodental para evitar o reducir el daño dentoalveolar (AU)


Sports dentistry is the branch of sports medicine that deals with the prevention and treatment of oral injuries and oral diseases associated with sports and exercise. Therefore it is necessary the intervention of oral health professionals to make evaluations of the athlete and the sport he practices to see the risks that may have to develop and suffer oral injuries such as dentoalveolar trauma and non-carious lesions. And in this way make the recommendations to each patient who practices sports, of the elements that should be used as oral equipment to avoid or reduce dentoalveolar damage (AU)

ABSTRACT

Cheilitis is a group of chronic diseases of the vermilion border that are diverse in etiology, pathogenesis and clinical picture. The development of various forms of cheilitis is facilitated by defects of the architectonics of the lips, unhealthy lifestyle, anatomical features of the lip structure, adverse meteorological effects, decrease in specific and non-specific factors for protecting the oral cavity, the patient's psycho-emotional state, immuno-allergic factors and accompanying general somatic and dental pathologies. The issues of prevention and comprehensive treatment of lip diseases are especially relevant in dental practice. The article presents data on the prevalence of cheilitis and dentoalveolar anomalies and the need for their treatment in young people. Furthermore, the authors provide a composition justification and develop an ointment for the treatment of cheilitis in this category of patients.

@#Traumatic dental injuries are frequent problems among teenagers, affecting aesthetics, functionality and quality of life. With regards to the present case, there was an inadequate restorative space for anterior restorations due to the dentoalveolar compensation two years after a traumatic event and was particularly challenging. The use of the Dahl concept to create restorative space is well documented particularly in patients with localized tooth wear. However, there are no evidence to justify the feasibility of Dahl concept application on the fractured teeth. The purpose of this article is to provide a detailed description regarding this technique to create the necessary restorative space of the fractured teeth.

ABSTRACT

ABSTRACT Odontogenic cysts are usually intraosseous and are classified into developmental and inflammatory. Residual cysts are inflammatory in nature and result from the extraction of a tooth with a radicular cyst, but with no removal of the lesion remnants in the same surgical procedure. They are mainly located in the maxilla, show a slight predilection for males, and are usually asymptomatic. This article reports the case of a 64-year-old man who consulted for an increase in bulk in the anterior edentulous area of the maxilla several years after the extraction of dental organs 11 and 12. The treatment consisted of buccal wall osteotomy, enucleation and curettage of the residual cyst, using a Newman flap from teeth 13 to 23. Postoperative controls were periodically performed, showing adequate healing.


RESUMEN Los quistes odontogénicos suelen ser intraóseos y se clasifican en quistes del desarrollo e inflamatorios. Los quistes residuales son de tipo inflamatorio y resultan de la extracción de un diente con un quiste radicular, pero sin remover los remanentes de la lesión en el mismo procedimiento quirúrgico. Se localizan principalmente en el maxilar, muestran ligera predilección por los hombres y generalmente son asintomáticos. Se presenta el caso de un hombre de 64 años de edad, quien consulta por presentar un aumento de volumen en la zona edéntula anterior del maxilar, años después de la exodoncia de los órganos dentarios 11 y 12. El tratamiento realizado consistió en osteotomía de la pared vestibular, enucleación y curetaje del quiste residual, usando un colgajo de Newman de los dientes 13 a 23. Se realizaron controles posoperatorios periódicamente y se observó buena cicatrización.

ABSTRACT

Objetivo: Mostrar una alternativa de tratamiento para las fracturas coronorradiculares complicadas, que restablece la función y la estética de las piezas traumatizadas. Caso clínico: Una paciente de 16 años se presentó en la guardia del Centro Odontológico OSEP tras haber sufrido un accidente vial que le produjo un traumatismo dentoalveolar. El diagnóstico fue fractura coronaria sin exposición pulpar de la pieza dentaria 12 y fractura coronorradicular complicada de la pieza 11. El tratamiento incluyó restauración con resina compuesta de la pieza 12 y endodoncia, extrusión ortodóntica, alargamiento coronario y rehabilitación con perno de fibra de cuarzo y corona de resina compuesta de la pieza 11. Conclusiones: El tratamiento de las fracturas coronorradiculares representa un desafío para el odontólogo. Los resultados del tratamiento, en este paciente, fueron excelentes, ya que lograron devolver estética y función (AU)


Aim: To show an alternative treatment for complicated crown-root fractures, which restores function and aesthetics of the traumatized teeth. Case report: This paper reports the case of a 16-yearold girl who attends the OSEP Dental Center, stating that she had suffered a traffic accident resulting in a dentoalveolar trauma. The diagnosis was a coronary fracture without pulp exposure of tooth 12, and complicated crown-root fracture of tooth 11. Treatment included composite resin restoration of tooth 12 and endodontic, orthodontic extrusion, crown lengthening and rehabilitation with quartz fiber post and composite resin crown of tooth 11. Conclusions: The treatment of crown-root fractures poses a challenge for the dentist. The results of the treatment in this patient were excellent, and aesthetics and function were restored (AU)
