ABSTRACT

Traditional thoracic surgery teaching has many problems, such as limited classroom teaching time allocation, many diseases and difficult to fully cover classroom teaching, uneven practical teaching level, and difficulty in updating "big textbooks". The Department of Thoracic Surgery of The Second Affiliated Hospital of Air Force Medical University has gradually applied microlecture to all levels of thoracic surgery teaching, such as undergraduate auxiliary classroom teaching and clinical skills training, grassroots and refresher doctor training, postgraduate education, etc., and has achieved good results of teaching effect.

ABSTRACT

Objective:To explore the value of competency-oriented training mode in the training of professional postgraduate students of thoracic surgery.Methods:A total of 71 professional postgraduate students, who entered thoracic surgery department for rotation training, were divided into experimental group and control group. Among them, 33 students in experimental group accepted competency-oriented training, including medical theoretical knowledge, clinical skills, clinical thinking, scientific information and interpersonal communication, while 38 students in control group received traditional training. Before leaving the department, the two groups of students had a unified assessment, and their results were compared and analyzed. SPSS 19.0 was used to make statistical analysis on the students' assessment results, and the comparison of the two groups of students' results was made by two independent samples t test. Results:The experimental group performed better in theoretical knowledge, clinical skills, clinical thinking and total scores than control group [(14.21±2.03) points vs. (12.97±2.06) points, (24.30±2.35) points vs. (22.37±3.10) points, (24.00±1.85) points vs. (22.71±3.17) points, (78.09±3.53) points vs. (73.34±5.25) points], with significant differences (all P<0.05). Conclusion:Competency-oriented training model can significantly enhance the training quality of professional postgraduates of thoracic surgery. We should make rational use of new media means and various teaching platforms, and give full play to the characteristics and advantages of the discipline, which facilitates to improving the clinical post competence of trainees.

ABSTRACT

Objective To investigate the effect of budesonide suspension combined with terbutaline on the ventilator-associated pneumonia (VAP) and rehabilitation in patients undergoing thoracic surgery. Methods Fifty-five patients underwent thoracic surgery from June 2006 to January 2017 in our hospital. The patients were treated with budesonide suspension combined with terbutaline atomization solution for inhalation. Forty patients with thoracic surgery were selected as the control group. Comparison of the two groups after the blood gas indicators, the incidence of VAP, mechanical ventilation and hospital stay. Results There was no significant difference in white blood cells, neutrophils and CRP between the two groups.The oxygen saturation of the observation group was higher than that of the control group, the arterial oxygen partial pressure was higher than the control group, PaCO2 was lower than the control group, the difference was statistically significant (P<0.05). The VAP of the observation group was lower than that of the control group. The time of mechanical ventilation was shorter than that of the control group. The hospitalization time was shorter than that of the control group, the difference was statistically significant (P<0.05). Conclusion The combination of budesonide suspension and terbutaline atomization can improve the postoperative effect of thoracic surgery and reduce the incidence of postoperative VAP and promote the early rehabilitation of patients.

ABSTRACT

Objective To evaluate the methods of standard patient (SP)combined with case-based learning (CBL ) in humanistic quality cultivation for interns in the department of thoracic surgery. Methods Medical interns in the department of thoracic surgery were divided randomly into experimental(n=30)and control group(n=30)respectively. In experimental group,SP combined with CBL was used while in control group traditional teaching was used. At the end of the study,teaching effect was assessed by the final examination and questionnaires and the data were processed by t test andX2 test. P0.05). Scores of history-taking,medical check-up,paper writing quality in the final examination in experimental group were significantly higher than those in control group(P<0.01). Satisfaction degrees of doctor-patient communication,responsibility and service attitude in experimental group were better than those of control group(X2=16.484,10.335,20.376,32.851,P<0.01). Conclusions SP combined with CBL teaching method can stimulate the students' interests in thoracic surgery practice,improve the doctor-patient relationship and strengthen the ability of human-istic concern. It is a better clinical thoracic practice teaching method and it is beneficial to cultivating medical students with humanistic concern.
