ABSTRACT

Objective To investigate the relationship between sex hormone binding globulin (SHBG) in serum and diabetic retinopathy (DR) in patients with type 2 diabetic mellitus (T2DM).Methods A total of 160 hospitalized people with T2DM was enrolled into the study. The patients were divided into two groups with or without merged DR.Clinical and laboratorial data were collected, and the correlation was analyzed between sex hormone binding globulin and diabetic retinopathy. Results (1) Compared to the group without DR, patients in T2DM with DR had significant lower SHBG concentration, triglyceride (TG) and total cholesterol (TC) concentration were increased significantly (P < 0.01).(2) The level of SHBG was associated with waist circumference, body weight, fasting blood glucose, and glycosylated hemoglobin (P < 0.05).(3) While the level of SHBG was significantly increased from Quartile 1 to Quartile4, the prevalence of DR was also significantly decreased (A:70%, B:51.2%,C:40.5%, D:29.7%) (P < 0.01).(4) Logistic regressing analysis shows that with the decrease of SHBG level, the increase of triglyceride levels, the risk of DR was significantly increased (SHBG:OR:0.616,95% CI:0.447-0.850,P < 0.01;TG:OR:1.323,95% CI:1.025-1.707,P<0.05).Conclusions With the decrease of SHBG, the prevalence of DR is significantly increased, lower SHBG may be one of the hazards of T2DM patients with DR.

ABSTRACT

Objective To explore the relationship between glycemic controls with periodontal diseases and disease conditions in patients with type 2 diabetes mellitus (T2DM).Methods Glycated hemoglobin A1c(HbA1c) and serum monocyte chemoattractant protein 1 (MCP-1) levels were measured in 34 normal subjects (control group) and 85 T2DM patients. The patients were assigned to four groups according to MCP-1 levels and complicating periodontal diseases. Both Pearson correlation analysis and univariate Logistic regression analysis were used. Results The levels of HbA1c and MCP-1 in T2DM group were significantly higher than those in control group (P <0.01);and the levels of HbA1c and MCP-1 in patients with periodontal disease were significantly higher than those without periodontal disease (P < 0.01).In both normal MCP-1 with periodontal diseases and elevated MCP-1 without periodontal diseases groups,HbA1c level was significantly higher than that in normal MCP-1 without periodontal diseases group (P < 0.01).In elevated MCP-1 with periodontal diseases group, patients had significantly higher HbA1 c levels than that in either periodontal diseases group or elevated MCP-1 group, respectively (P < 0.01).In HbA1c≥7.0% group,MCP-1 levels and the incidence of periodontal diseases were significantly higher than that in HbA1c <7.0% group (P <0.01).HbA1c levels were positively correlated with MCP-1 levels in HbA1c≥7.0% group (R2 =0.6004,P < 0.01).Univariate regression analysis showed that HbA1c ≥7.0% was the risk factor for complicating periodontal diseases in T2DM patients (OR =1.65,95% CI:1.12 ~ 2.56,P < 0.01).Conclusions Poor glycemic control is related to periodontal disease and the disease conditions in T2DM patients.Elevated HbA1c levels can be used as the assessing indicator for periodontal diseases of T2DM patients.

ABSTRACT

Objective To investigate changes of serum silent information regulator 1 (Sirt1) and inflammatory cytokines in type 2 diabetes patients with different stages of diabetic nephropathy. To explore the relationship between serum Sirt1 level and inflammatory cytokines in type 2 diabetic patients with different urinary albumin excretion rates. Methods A total of 436 cases with type 2 diabetes were divided into three groups:normoalbuminuric [D1,n =168],microalbuminuric [D2;n =152],and macroalbuminuric [D3,n =116].Serum Sirt1,hypoxia-inducible factor-1α (HIF-1α),early growth response protein 1 (EGR1),insulin-like growth factors-Ⅰ (IGF-Ⅰ),and monocyte chemotactic protein-1 (MCP-1) were determined by enzyme-linked immunosorbent assay (ELISA).Results The levels of serum Sirt1 in type 2 diabetes patients were significantly lower than that in control group, and with the increase of urinary protein excretion rate, the levels of serum Sirt1 in group D1,D2 and D3 were decreased gradually (P < 0.01).Compared to control, serum inflammatory cytokines (HIF-1α,EGR1,IGF-Ⅰ,and MCP-1) levels were significantly increased in type 2 diabetic patients and gradually increased in the patients of D1,D2 and D3 groups (P <0.01).Furthermore,Serum Sirt1 was negatively correlated with the levels of inflammatory cytokines.Age,duration, fasting blood glucose (FBG),fasting insulin (FINS),homeostasis model assessment insulin resistance index (HOMA-IR),glycosylated hemoglobin (HbA1c),low density lipoprotein (LDL),total cholesterol (TC),triglyceride (TG),serum creatinine (Scr),blood urea nitrogen (BUN),uric acid (UA),HIF-1α,EGR1,IGF-Ⅰ,and MCP-1 were positively correlated with Ln Koc of urinary albumin to creatinine ratio [Ln(ACR)] (P < 0.05);and Sirt1 were negatively correlated with Ln(ACR)(P < 0.01).HIF-1α,MCP-1,IGF-Ⅰ,duration,BUN,Sirt1,UA,LDL,and EGR1 were independent factors that significantly influenced Ln (ACR) (P < 0.05).Conclusions Serum Sirt1 might be a new target for the treatment of diabetic nephropathy.Enhancing serum Sirt1 levels might have a role in delaying the progression of diabetic nephropathy.

ABSTRACT

Objective To explore the association between serum vaspin levels and the severity of the lower extremity vascular lesions in type 2 diabetes mellitus (T2DM) patients. Methods According to the lower extremity artery plaque, stenosis and intima-media membrane thickening severity score,92 cases of T2DM patients were divided into three groups:simple diabetes group (DM1 group) (32 cases),mild diabetic lower limb vascular lesion group (DM2 group) (33 cases),and moderately severe diabetic lower limb vascular lesion group (DM3 group) (27 cases).Twenty-six age-and gender-matched apparently healthy controls (control group) were recruited as well.Systolic blood pressure (SBP),ankle-brachial index (ABI),insulin resistance index (HOMA-IR),and other indicators were determined, serum vaspin was measured by enzyme-linked immunosorbent assay. Results After adjustment for age and gender, the DM1 group had a significantly higher serum vaspin level than control group (P <0.01),while for the DM2 and DM3 groups, serum vaspin levels were significantly lower than the DM1 group (P < 0.01).Partial correlation analysis showed that serum vaspin was inversely associated with HOMA-IR (r =-0.461,P =0.001)and positively correlated with ABI (r =0.462,P =0.001).Logistic regression analysis demonstrated that SBP,ABI and fasting serum vaspin levels were significantly associated with the presence of the lower extremity vascular lesions in type 2 diabetes. Conclusions Serum vaspin levels were significantly elevated in simple type 2 diabetic patients while the reduction of its level might be associated with the formation of lower extremity vascular lesions in type 2 diabetic patients.

ABSTRACT

Objective To investigate association of pancreatic fat content and lipid levels in individuals with newly diagnosed type 2 diabetes. Methods Fifty four subjects with newly diagnosed type 2 diabetes and 22 volunteers without type 2 diabetes were enrolled in this study.Pancreatic fat content was determined with magnetic resonance imaging.Lipid levels were measured. Results The pancreatic fat content in patients with newly diagnosed type 2 diabetes and control subjects was (17.13 ± 10.35) % and (11.62 ±7.15)%, respectively (P =0.025).Median pancreatic fat content of subjects with newly diagnosed type 2 diabetes correlated negatively with total cholesterol level (r =-0.361, P =0.007) and triglycerides level (r =-0.346, P =0.010).Conclusions Increased levels of serum total cholesterol and low density lipoprotein cholesterol might relieve the extent of pancreatic fat content.

ABSTRACT

ObjectiveTo explore the relationship between plasma A-FABP and obesity type and degree, glucose and lipid metabolism parameters and insulin sensitivity.MethodsPlasma adipocyte fatty acid binding protein (A-FABP) was detected by double antibody sandwich ELISA in fasting status in subjects with normal glucose regulation and normal weight (NW-NGR,n =44),overweight or obese subjects with normal glucose regulation (OB-NGR,n =36),newly-diagnosed type 2 diabetes mellitus (T2DM) and its subgroups of T2DM with normal weight ( NW-T2DM,n =89 ) and T2D)M with overweight or obesity (OB-T2DM,n =44).And glucose, lipids, insulin levels as well as anthropometrical parameters such as body mass index ( BMI ),fat content ( Fat% ),waist circumference (WC) and waist hip ratio (WHR) were measured.Meanwhile insulin sensitivity was assessed by HOMA-IR.ResultsAfter age and sex adjustment, plasma A-FABP levels in OB-NGR,NW-T2DM,and OB-T2DM were significantly higher than that of NW-NGR [11.32(6.54 - 15.43)μg/L,14.60(10.35 -20.10) μg/L,18.25(12.85 -26.65) μg/L vs 9.32(3.72 - 14.00) μg/L,all P <0.05].There was no difference in plasma A-FABP levels between OB-NGR and NW-T2DM [ 11.32 (6.54 - 15.43 ) vs 14.60 ( 10.35 -20.10) μg/L,P > 0.05 ],but the plasma A-FABP levels in OB-NGR and NW-T2DM were significantly lower than that of OB-T2DM [ 11.32(6.54 - 15.43) μg/L,14.60( 10.35 -20.10) μg/L vs 18.25( 12.85 -26.65 ) μg/L,P <0.01 ].In a multiple stepwise regression analysis,HOMA-IR,sex,WC and age were the most significant independent determinants for plasma A-FABP concentration (all P < 0.05 ).Conclusions Plasma A-FABP is strongly associated with the abdominal obesity and insulin resistance, measurement of plasma A-FABP concentrations might be useful for diagnosis of abdominal obesity and type 2 diabetics.

ABSTRACT

Objective To investigate the effect of HbA1c meeting the standard or not on microalbuminuria, blood lipids and liver enzymes in patients with type 2 diabetes. Methods A retrospective analysis was performed on 457 subjects who had type 2 diabetes. They were divided into substandard group and standard group according to HbA1c result. The general information and relevant laboratory indicators of patients were.collected and compared between two groups. Results The microalbuminuria, serum triglyceride and liver enzymes (glutamyl transpeptidase, alkaline phosphatase, aspertate aminotransferase) were significantly different between two groups [ (189.8 ± 235.3) mg/dl vs (38.9 ± 85.5) mg/dl,(2.64 ± 2.99) mmol/L vs (2.02 ± 1.50)mmol/L,(41.7 ±52.9)U/L vs (29.7 ±24.9)U/L,(83.6 ±28.6) U/L vs (74.3 ±25.8)U/L,(26.7 ±19.1)U/L vs (22.0 ±10.5) U/L,P <0.05].HbA1c level was positively correlated with microalbuminuria, glutamyl transpeptidase and alkaline phosphatase (r =0.209,0.115,0.11,P <0.01).The microalbuminuria was an independent risk factor of affecting HbA1c to reach the standard (OR = 1.009,P <0.05).Conclusions HbA1c meeting the standard or not can influence many factors except blood glucose.
