ABSTRACT

Objective To establish the Polock indexes (PI) of singleton neonates in different gestational age (GA),so as to provide a reference data for evaluating physical fullness and symmetry in neonates at birth. Methods A total of 16 887 live singleton neonates at 27-42 weeks of GA from two hospitals were measured at birth by site survey method using cluster samples by a cross sectional time in Shenzhen City, from 2013 to 2015 in this study,to establish PI of singleton neonates in different GA.Results The PI mean and the percentile curves(3rd,10th,25th,50th,75th,90th,97th)of singleton neonates at 27-42 weeks of GA (male, female, and unisex three groups) were established in 2015 in Shenzhen,China. The fullness and nutrition status of neonates at birth can be evaluated by these PI curves.According to the 3rd,10th,25th-75th,90th,97th curves, the down, mid down, middle, mid upper, upper levels were divided in turn. When the PI values >97th curve it was overweighted or fatty. When the PI values < 3rd curve it was malnutrition. The lowest values of PI were at 27 weeks of GA,and the highest values appeared at 42 weeks of GA.The PI values were increasing with GA growth, which indicated that the GA increased the body density and fullness. The 50th percentile curve of male PI was higher than that of the female, and the male ratio increased by 61.2-89.5 at the gestational age of 27-34 weeks;between 35 and 42 weeks of fetal age, males increased by 104.8-149.1,which had a statistical difference (P < 0.001).Conclusions The PI of singleton neonates rises with GA increase, which shows the GA increases the body density and fullness.PI is higher in male than in female singleton neonates.

ABSTRACT

OBJECTIVE:To observe the efficacy and safety of insulin in the treatment of gestational diabetes in different gesta-tional age. METHODS:The data of 121 women with gestational diabetes were retrospectively analyzed, and divided into observa-tion group (63 cases) and control group (58 cases) based on first use of insulin in different gestational age. All patients received dirt control [(calorie control in 30-35 kJ/(kg·d),55% carbohydrates,25% fat and 20% protein)];based on it,Biosynthetic hu-man insulin injection was injected before meals and at bedtime by 4∶2∶3∶1 with 4-6 units before pregnant 30 weeks according to the FPG and postprandial 2 h blood glucose(2 h PG)levels in observation group, the blood glucose of women with gestational dia-betes was monitored to prevent the occurrence of complications such as hypoglycemia. Control group received Biosynthetic human insulin injection(the same dosage with control group)when no less than 30 weeks. The treatment course for both groups was 15 d. FPG and 2 h PG levels when first diagnosed and delivery, pregnancy outcomes and neonatal conditions in 2 groups were observed, and the incidence of adverse reactions was recorded. RESULTS:When first diagnosed, there were no significant differences in FPG and 2 h PG levels in 2 groups(P>0.50). When delivery,FPG and 2 h PG levels in 2 groups were significantly lower than first di-agnosed,and observation group was lower than control group, the differences were statistically significant (P0.05). There were no obvious adverse reactions during treatment. CONCLUSIONS:Based on diet control, insulin in the treatment of gestational diabetes before pregnant 30 weeks can effectively control blood glucose level and improve pregnancy outcomes, with good safety.

ABSTRACT

Objective To observe the characteristics of electrocardiographic parameters of the right heart in the early newborns with different gestational age.Methods Seventy-five cases of early newborns from January 2014 to January 2015 were selected.They were divided into 4 groups according to gestational age,15 cases in group A (28-30 weeks),18 cases in group B (>30-33 weeks),17 cases in group C (>33-37 weeks) and 25 cases in group D (> 37-40 weeks).All newborns were examined by the synchronous 12-lead ECG machine,and the changes in ECG time limit detection results,QRS amplitude and RV1 amplitude were compared among groups.Results (1) There was a significant difference in P-R intervals among group A [(0.090 ± 0.011) s],group B [(0.095 ± 0.012) s],group C [(0.098 ± 0.013) s] and group D [(0.105 ± 0.014) s] (F =7.458,P < 0.05).The P-R intervals of group A,group B and group C were much shorter than those of group D (P < 0.05);the comparison of QRS duration and QTc among groups had no statistical significance (F =1.626,1.569,all P > 0.05).(2) In group A:QRS amplitude was:(4.430 ± 1.380) mV,RV1 amplitude:(0.850 ± 0.420) mV;in group B:QRS amplitude was:(4.800 ± 1.350) mV,RV1 amplitude:(1.012 ± 0.425) mV;in group C:QRS amplitude was:(5.200 ± 1.600) mV,RV1 amplitude:(1.210 ± 0.520) mV;in group D:QRS amplitude was:(5.800 ± 1.800) mV,RV1 amplitude:(1.509 ± 0.525) mV.There were significant differences in QRS and RV1 amplitude among 4 groups (F =7.002,16.870,all P < 0.05).The QRS amplitude changes in group A and group C were far less than those in group D (P < 0.05),the RV1 amplitude changes in group A,group B,and group C were far less than those in group D (P < 0.05),and there was no difference between among other groups (all P > 0.05).Conclusions There are correlations between the electrocardiographic parameters of right heart and the gestational age of newborn infants.These parameters,including the P-R intervals,the amplitude of QRS and RV1,increase with the gestational age.
