ABSTRACT

Resumen Este artículo no tiene como objetivo el presentar una descripción detallada de cada una de las encefalopatías epilépticas y del desarrollo, sino más bien discutir cam bios recientes en la terminología y criterios diagnósticos de ciertas encefalopatías, en base a una revisión actua lizada de los últimos 10 años. Se analizan cambios importantes en definiciones de síndromes específicos y nuevos tratamientos que han demostrado eficacia en el manejo de crisis convulsivas en estos pacientes. En conclusión: Las nuevas terapias de modulación genética, contribuirán no solo a reducir la carga de crisis epilépticas, sino también a mejorar el pronóstico cognitivo, y por lo tanto la calidad de vida.


Abstract It is not the intend of this article to present a de tailed description of each developmental and epileptic encephalopathy, but to discuss recent changes in the terminology and diagnostic criteria of specific disorders, based on an updated review of the last 10 years. Important changes in the definitions of specific syn dromes and new treatments that have shown efficacy in the management of seizures in these patients are analyzed. In conclusion: New gene modulation therapy will likely improve not only seizure frequency, but also cog nitive outcome and therefore quality of life.

ABSTRACT

In the electroencephalography (EEG) monitoring of patients with hypoxic-ischemic encephalopathy, generalized periodic discharges are often monitored abnormal waveforms. When there are some features of generalized periodic discharges (e.g., frequency≥1.5 Hz or plus), it indicates that the patient is at high risk for seizures or has a poor prognosis. Compared with conventional EEG, the time of continuous EEG monitoring is longer, so the detection rate of these waveforms is higher. At present, scholars at home and abroad have studied these waveforms, but there is controversy about the significance of these waveforms. In this paper, the definition and characteristics of these waveforms and their significance in determining prognosis and guiding treatment in patients with hypoxic-ischemic encephalopathy are reviewed.

ABSTRACT

Little is known about the characteristics of electroencephalogram (EEG) findings in epileptic patients in Ethiopia. The objective of this study was to characterize the EEG patterns, indications, antiepileptic drugs (AEDs), and epilepsy risk factors. METHODS: A retrospective observational review of EEG test records of 433 patients referred to our electrophysiology unit between July 01, 2020, and December 31, 2021. Results: The age distribution in the study participants was right skewed unipolar age distribution for both sexes and the mean age of 33.8 (SD=15.7) years. Male accounted for 51.7%. Generalized tonic clonic seizure was the most common seizure type. The commonest indication for EEG was abnormal body movement with loss of consciousness (35.2%). Abnormal EEG findings were observed in 55.2%; more than half of them were Interictal epileptiform discharges, followed by focal/or generalized slowing. Phenobarbitone was the commonest AEDs. A quarter (20.1%) of the patients were getting a combination of two AEDs and 5.2% were on 3 different AEDs. Individuals taking the older AEDs and those on 2 or more AEDs tended to have abnormal EEG findings. A cerebrovascular disorder (27.4%) is the prevalent risk factor identified followed by brain tumor, HIV infection, and traumatic head injury respectively. CONCLUSION: High burden of abnormal EEG findings among epileptic patients referred to our unit. The proportion of abnormal EEG patterns was higher in patients taking older generation AEDs and in those on 2 or more AEDs. Stroke, brain tumor, HIV infection and traumatic head injury were the commonest identified epilepsy risk factors

ABSTRACT

Introducción: El Síndrome Hipertensivo del Embarazo (SHE) es el conjunto de condiciones patológicas derivadas del alza de presión arterial mantenida originadas antes y/o durante el curso de un embarazo. En ocasiones requiere tratamiento hospitalario. El objetivo del estudio es determinar la tasa de egreso hospitalario (TEH) por complicaciones del SHE durante el 2018-2021 en Chile. Materiales y métodos: Estudio descriptivo tipo transversal de las complicaciones por SHE en el periodo 2018-2921 en Chile según grupo etario y días de estadía hospitalaria (n=12.006). Datos obtenidos del departamento de estadística e información en salud. Se calculó TEH. No requirió aprobación del comité de ética. Resultados: La Preeclampsia tuvo más TEH con 61,48 por cada 100.000 habitantes, afectó más a mujeres de 20 a 44 años con 74.03 por 100.000 habitantes. Las complicaciones del SHE reportaron un promedio de 6,4 días de hospitalización. El Síndrome de HELLP registró más días de hospitalización con 7,2 días. Discusión: El envejecimiento poblacional, la migración, el sobrepeso-obesidad podrían influir en mayores TRH por preeclampsia. Se reportó mayores TEH de preeclampsia en mujeres de 20 a 44 años. Esto se debe posiblemente por factores cardiovasculares, maternidad tardía y la hipertensión crónica. El Síndrome de HELLP implica peor pronóstico y reportó mayores días hospitalización debido al manejo médico. Discusión: Hay escasas estadísticas nacionales sobre TEH en relación a SHE. La edad, el riesgo cardiovascular y fenómenos epidemiológicos ofrecen posibles líneas investigativas. Más estudios son requeridos para dilucidar los factores que desencadenan sus complicaciones.


Introduction: The Hypertensive Syndrome of Pregnancy (SHE) is the set of pathological conditions derived from the increase in sustained blood pressure originating before and/or during the course of a pregnancy. Sometimes it requires hospital treatment. The objective of the study is to determine the rate of hospital discharge (TEH) due to complications of EHS during 2018-2021 in Chile. Materials and Methods: Descriptive transversal type of study about the complications of hypertensive pregnancy syndrome during the period from 2018 to 2021 according to age range and number of days in the hospital (n=12.006). Data obtained from the health statistics and information department, which did not require approval of the ethics committee. Results: PE had a higher TEH with 61.48 out of 100,000 people, affecting more women aged 20 to 44 years with 74.03 per 100,000 inhabitants. Complications of hypertensive pregnancy syndrome reported an average of 6.4 hospitalization days. The HELLP syndrome had more days with a quantity of 7.2. Discussion: Population aging, migration, and overweight-obesity might influence the increase in TEH/PE. Higher TEH of PE was reported in women aged 20 to 44 years, possibly due to cardiovascular factors, late maternity, and chronic hypertension. The HELLP syndrome implies a worse prognosis and reported more days at the hospital due to medical treatment. Conclusion: There are few national statistics on TEH in relation to SHE. Age, cardiovascular risk, and epidemiological phenomena offer possible lines of investigation. More studies are required to elucidate the factors that trigger its complications.

ABSTRACT

INTRODUCCIÓN: La pandemia por Covid-19 ha generado cambios en la atención de salud nacional, observándose en este período cambios en las causas de egresos hospitalarios (EH). OBJETIVO: Analizar el impacto del brote de Covid-19 en las causas de EH por enfermedades del Sistema Nervioso Central (ESNC) en población pediátrica durante el primer año de pandemia. MÉTODO: Estudio transversal. Análisis de base de datos del Departamento de Estadística e Información en Salud en pacientes de 0 a 18 años, comparando años 2019 y 2020. RESULTADOS: En 2020 se redujeron EH por ESNC en un 39% comparado con 2019. Disminuyeron principalmente los EH por secuelas de enfermedades inflamatorias SNC, parálisis cerebral, migraña y paraplejia/cuadriplejia, aumentando los EH por isquemia cerebral transitoria, enfermedades desmielinizantes SNC y polineuropatía inflamatoria. El número EH por ESNC mensual se correlacionó con el número de casos Covid-19 (rho -0.774, p0.003) y con la movilidad mensual del país (rho 0.928, p 0.001). CONCLUSIONES: El impacto del brote Covid-19 se asoció con reducción de EH por ESNC, disminuyeron EH por patologías crónicas y aumentaron causas agudas.


INTRODUCTION: The Covid-19 pandemic has been associated with modifications in national health care, with changes in causes of hospital discharges (HD) in this period. OBJECTIVE: To analyze the impact of the Covid-19 outbreak on causes of HD due to Central Nervous System Diseases (CNSD) in pediatric population during the first year of pandemic. METHOD: Cross-sectional study. Analysis of database of the Department of Statistics and Health Information in patients aged 0 to 18 years, comparing 2019 and 2020. RESULTS: In 2020, HD due to CNSD were reduced in 39% compared to 2019. HD causes that mainly decreased were inflammatory CNS disease sequelae, cerebral palsy, migraine and paraplegia/cuadriplegia. The HD that increased were transient cerebral ischemia, CNS demyelinating diseases and inflammatory polyneuropathy. The monthly HD due to CNSD number was correlated with the number of Covid-19 cases (rho -0.774, p0.003) and with the country's monthly mobility (rho 0.928, p 0.001). CONCLUSIONS: Covid-19 pandemic was associated with a reduction in HD due to CNSD, with decrease of EH due to chronic pathologies and increase of acute diseases

Resumen Introducción: Las enfermedades otorrinolaringológicas presentan una alta prevalencia en la población, lo que obliga continuamente a estudiar sus características epidemiológicas. Una forma de estimarlo a nivel país es a través de los egresos hospitalarios. Objetivo: Describir las principales causas de egresos hospitalarios por enfermedades otorrinolaringológicas a nivel nacional durante el año 2018. Material y Método: Estudio transversal donde se describen las distintas frecuencias de las principales causas de egresos hospitalarios por causa otorrinolaringológica en todo Chile durante el año 2018. Datos obtenidos del Departamento de Estadísticas e Información en Salud (DEIS). Resultados: Se incluyeron 64.715 egresos con una mediana de edad de 22 años. Las patologías con mayor cantidad de egresos fueron la enfermedad crónica de amígdalas y adenoides (28,4%), patología rinosinusal (23,3%), neoplasias de cabeza y cuello (15,2%) y oído (7,8%). Conclusión: La enfermedad crónica de amígdalas y adenoides persiste como la patología con más egresos. Emergen en importancia enfermedades neoplásicas y diagnósticos relacionados con mejora de calidad de vida. Existen errores de codificación de diagnósticos que requieren discusión dentro de la especialidad para mejorar la calidad del registro.


Abstract Introduction: Otolaryngological diseases have a high prevalence in the population, creating the need to continuously study their epidemiological characteristics. One way to estimate prevalence at the country level, is to check hospital discharges. Aim: To describe the main causes of hospital discharges due to otolaryngological diseases at the Chilean level during 2018. Material and Method: Cross-sectional study describing the different frequencies of the main causes of hospital discharge due to otolaryngological diseases throughout Chile during 2018. Data obtained from the Department of Health Statistics and Information (DEIS). Results: 64,715 discharges were included with a median age of 22 years. The pathologies with the highest number of discharges were chronic tonsil and adenoids Disease (28.4%), rhinosinusal (23.3%), head and neck neoplasms (15.2%) and ear (7.8%). Conclusion: Chronic tonsil and adenoids disease persists as the pathology with the most discharges. Neoplastic diseases and diagnoses related to improvement of quality of life emerge in importance. There are errors in the coding of diagnoses that require discussion within the specialty to improve the quality of the registry.

ABSTRACT

OBJETIVO: Describir la tendencia temporal de osteoartritis (OA) según egresos hospitalarios considerando ubicación anatómica, distribución por sexo, edad, condiciones más frecuentes, y ubicación geográfica en Chile entre los años 2012 y 2018. METODOLOGÍA: Estudio descriptivo, retrospectivo, poblacional, que analizó las bases de datos públicas del Departamento de Estadísticas e Información en Salud (DEIS) del Ministerio de Salud de Chile, en particular los registros con codificación de M15 a M19. Se calcularon las tasas por 100 mil egresos hospitalarios, así como también el porcentaje de OA según las categorías analizadas. RESULTADOS: Se encontraron 11.622.605 egresos hospitalarios entre 2012 y 2018. El número total de egresos hospitalarios por OA fue de 78.700. Se observó que la distribución por sexo fue similar a lo largo de los años ( 40% y 60% en hombres y mujeres, respectivamente). El rango de edad más afectado fue el de 65 a 79 años, cuya tasa promedio fue de 2.046 por 100 mil egresos; además, fue el grupo que presentó el mayor aumento (60%) en su tasa de 2012 a 2018. La Región Metropolitana (291,7 egresos) y de Valparaíso (89,6 egresos) presentaron las tasas más altas. La cadera (56,6%) y la rodilla (31%) fueron las ubicaciones más frecuentes de casos de OA, con distribución similar por sexo, edad y geografía en comparación a los egresos totales por OA. CONCLUSIONES: Los egresos hospitalarios por OA en Chile aumentaron entre 2012 y 2018, preferencialmente en casos de OA en las articulaciones de cadera y rodilla en el grupo de mujeres de edad avanzada de la zona centro del país. NIVEL DE EVIDENCIA: Estudio descriptivo.

CONCLUSIONS: Hospital discharges due to OA in Chile increased between 2012 and 2018, preferentially in cases of hip and knee-joint OA in the group of elderly women in the central area of the country. ABSTRACT

En las últimas décadas se ha prestado mayor atención a los resultados de la actividad quirúrgica debido a factores económicos, la mejora de los cuidados perioperatorios y la preocupación por la calidad de la asistencia y satisfacción de la población. A este propósito han contribuido el desarrollo y aplicación de guías de práctica clínica. Estas guías reducen la variación en los cuidados del paciente quirúrgico, aumentan la eficiencia de dicho cuidado y proporcionan que los pacientes se beneficien de iniciativas institucionales encaminadas a mejorar la calidad de la asistencia sanitaria. La cirugía de alta precoz es un modelo de atención médico-quirúrgica encaminado a disminuir los costos de la atención médica mediante esfuerzos coordinados que permitan la reducción de la estancia hospitalaria en los pacientes, sin que ocurra incremento de complicaciones posoperatorias, ni insatisfacción de pacientes y familiares con el servicio prestado. Su empleo ha ido incrementándose en afecciones pediátricas. En el hospital pediátrico de Cienfuegos se aplica el modelo hace varios años y la guía fue aprobada en el Servicio de Cirugía Pediátrica en diciembre de 2018. Compartir esta guía mediante su publicación permitiría a otros servicios de cirugía pediátrica del país emplearla como referencia para la aplicación del modelo en sus propias instituciones y beneficiar a un mayor número de pacientes(AU)


In the last decades, there has been more attention on the results of the surgical activity due to economic factors, the improvement of perioperative cares and the concern on the quality of the care and the population´s satisfaction. The development and implementation of the clinical pratice´s guides had contributed to that purpose. These guides reduce the variations in the cares to the surgical patients, increase efficiency in that kind of care and foster that the patients are benefited with institutional initiatives addressed to improve health care quality. The Fast Track Pediatric Surgery is a model of medical-surgical care directed to lower the costs of medical care by means of coordinated efforts that allow the reduction of the hospital stay in patients without increasing the rates of postoperative complications, or dissatisfaction of the patients and relatives with the service given. The use of this procedure has been increasing in pediatric conditions. It has been implemented since some years ago in the Pediatric Hospital of Cienfuegos province, and the guide was approved in the Pediatric Surgery service in December, 2018.To share this guide by publishing it will be a way to allow other pediatric surgery services in the country to use it as a reference for the implementation of that model in their institutions and in that way to benefit a higher number of patients(AU)

ABSTRACT

A 7-year-old lactating Red Sokoto goat was presented to the veterinary clinic, Aliyu Jodi Road Sokoto, with the complaint of inappetance, weight loss, reduced milk output and foul-smellingudder four weeks after kidding. The goat weighed approximately 25 kg. The patient appears dull with rough hair coat, the right mammary gland was necrotic and blue-greenish, atrophied, hardened with a lacerated base, painful to touch with foul smelling. Gangrenous mastitis was diagnosed and animal as scheduled for surgery.The surgery was successfully conducted; the animal was later discharged 16 days post-surgery.

ABSTRACT

Gynecological bacterial infections (GBIs) are prevalent in our environment and as a result pose a number of physical, social and psychological consequences. These infections are acquired through several ways. Treating GBIs is a daunting task making its control the most important strategy to alleviating its physical and psychosocial consequences. To highlight the physical, social, and psychological consequences of gynaecological bacterial infections in our resource limited setting. To highlight the hugely unresolved challenges associated with the management of gynecological bacterial infections in our resource-limited setting. Several databases (Medline, Google Scholar, Pubmed, WHO’s Hinari and Wikipedia) and some selected websites were searched using the following keywords: gynecological infections, vaginal infections and discharges, vaginal flora, sexually transmitted infections, pelvic inflammatory disease, syndromic management and challenges, psychosocial consequences, alternative medicine. A total of 5470 relevant articles were obtained between 1947 and 2018. Out of these only 256 relevant articles on the topic were reviewed. However, 213 were dropped for having an incomplete submission. Forty-three (43) articles were fully accessed and referenced. The high prevalence of GBIs poses a lot of burden on the reproductive and socio-economic lives of our women. This should be matched by behavioral changes, prompt diagnosis and early treatment; facilitated by accessible and affordable health care through improved government funding.

ABSTRACT

Generalized periodic discharges (GPDs) with triphasic morphology are an electroencephalographic (EEG) pattern traditionally associated with encephalopathy and coma, although they have been observed in a wide array of neurological disorders. The clinical significance of these waveforms and their relationship to seizures and prognosis has been debated, and differentiation between interictal patterns, patterns associated with seizures, and patterns representing nonconvulsive status epilepticus can at times be a challenge. The most established literature suggests that GPDs, including those with triphasic morphology, are associated with the development of electrographic seizures, but that in the absence of clinical information, distinguishing waveforms based on morphology alone may not be clinically useful. Recent work has advocated for a more proactive approach in evaluating GPDs with triphasic morphology. Further studies of nonsedating antiseizure drugs in patients with GPDs with triphasic morphology that incorporate continuous EEG monitoring will be useful in tailoring therapy to optimize long-term clinical outcomes and recovery.

ABSTRACT

Approximately two-thirds of patients with epilepsy become seizure-free with the adequate antiepileptic drug therapy.It is generally believed that antiepileptic treatment might be gradually withdrawn until stopped after a minimum period of two years of seizure freedom,as well as making a careful evaluation of the risk/benefit ratio.In the antiepileptic drugs discontinuance literature,the most consistent risk factors predictive of relapse are remote symptomatic causes,abnormal electroencephalogram and special epilepsy syndrome.The impacts of abnormal electroencephalogram,especially interictal epileptiform discharges on the seizure relapse after antiepileptic drugs withdrawal are not completely clear for now.In this article the literature describing the predictive value of interictal epileptiform discharges in different withdrawal periods is reviewed.

ABSTRACT

RESUMEN Las aguas de desecho dispuestas en una corriente superficial, lagos, ríos, mar, sin ningún tratamiento, ocasionan graves inconvenientes de contaminación, que afectan la flora y la fauna. Estas aguas residuales, antes de ser vertidas en las masas receptoras, deben recibir un tratamiento adecuado, capaz de modificar sus condiciones físicas, químicas y microbiológicas, para evitar que su disposición cause la alteración y la degradación de los ecosistemas asociados y problemas de salud pública. La investigación tuvo como objetivo evaluar el efecto de mezclas de quitosano y extracto acuoso de la cáscara de naranja, a diferentes concentraciones, en el proceso de coagulación-floculación, de aguas residuales, para lo cual, se realizó una prueba de jarras con agitación rápida y lenta, para evaluar turbidez (NTU), demanda química de oxígeno (DQO), demanda bioquímica de oxígeno (DBO), sólidos suspendidos totales (SST) y SS (sólidos sedimentables). Los resultados mostraron que todos los tratamientos presentaron diferencias significativas (p<0,05), con la muestra control y fueron eficientes en la reducción de turbidez, DBO, DQO, SST, SS. El tratamiento que combinó quitosano y extracto acuoso de cáscara de naranja (50-50%), a un pH de 5,5, disminuyó significativamente (p<0,05) la turbidez, en 79%, demostrando, de manera preliminar, que el extracto acuoso de la cáscara de naranja acidifica la mezcla e incrementa la formación de flóculos aglomerados en una fase liviana de las muestras, convirtiéndose en un agente eficiente, para ser usado en el tratamiento de aguas residuales.


ABSTRACT Waste water disposed in a superficial current, lakes, rivers, the sea, without any treatment, causes great inconvenient pollution that affects the flora and fauna of a region. This wastewater, before being poured into the receiving body of water, must receive the appropriate treatment, in order to modify its physical, chemical and microbiological conditions, to avoid that its disposal causes the alteration and degradation of the associated ecosystems and problem with public health. This research had as an objective to assess the effect of chitosan and aqueous extract of orange peel shells at different concentrations on the coagulation-flocculation process of wastewater. In order to do this, a jar test with fast and slow pull was carried out, assessing turbidity (NTU), chemical oxygen demand (COD), biochemical oxygen demand (BOD), total suspended solids (TSS) and SS (solid sediments). The results showed that all the treatments presented significant differences (p<0.05) to the control sample, and were efficient in the decrease of NTU, DQO, DBO, SST, and SS. The treatment that combined chitosan and aqueous extract of orange peel (50-50%) at a pH or 5.5, decreased significantly (p<0.05) the turbidity in a 79%, showing in a preliminary way that the aqueous extract of orange peel shells acidifies the mix and increases the formation of agglomerated floccules in a light phase of the samples, becoming an efficient agent to be used in the treatment of wastewater.

Resumen Introducción: Las enfermedades de origen infeccioso son importantes productoras de morbilidad y mortalidad. Objetivo: Describir la tendencia temporal de las tasas de los episodios de hospitalización por enfermedades infecciosas en Chile entre los años 2004 y 2014. Métodos: Estudio descriptivo usando el Registro Nacional de Egresos hospitalarios, identificando los episodios de hospitalización por enfermedades infecciosas incluyendo parasitarias. Se calcularon las tasas de egresos hospitalarios y se estandarizaron por edad. Mediante regresión de Poisson se estimó el porcentaje de cambio anual (PCA). Resultados: Los episodios de hospitalización por enfermedades infecciosas han disminuido (PCA de −4%). Las mujeres presentan menos episodios que los hombres. La magnitud de las tasas según grupos etarios, es mayor para personas de 0-9 años, y de 80 años y más. Conclusiones: En la última década hubo una reducción de episodios de egreso hospitalario por enfermedades infecciosas.


Background: Infectious diseases are important causes of burden of illness and mortality. Aim: To describe the temporal trends of hospital discharges due to infectious in Chile from 2004 to 2014. Methods: Descriptive study. National Hospital Discharge Registry were used. We included all hospital discharges with main diagnosis of infectious diseases including parasitic diseases. Rates were calculated and standardized. The anual percentage change (APC) was estimated by Poisson regression. Results: During a stated period of time in hospitalization of infectious diseases decreased (APC −4%). Women had fewer hospital discharged compared to men. Patients aged between 0 and 9 years and those who aged 80 years and older showed greater rates than other age groups. Conclusions: Chile has experimented a reduction in hospital discharges due to infectious diseases.

ABSTRACT

Objective To investigate and study the relationship between abnormal monitoring and prognosis of long range video electroencephalogram(VEEG)in patients with intractable epilepsy. Methods 100 patients from March 2012 to May 2017 in our hospital whose diagnosis and treatment of refractory epilepsy were the research object,all patients were given VEEG monitoring records,epilepsy typing characteristics and antiepileptic drug(AEDs)use survey,prognosis of all patients and the correlation analysis. In 100 cases,GTCS 60 cases,CPS 30 cases,CPS-GTCS 10 cases;56 cases treated with sodium valproate,lamotrigine treatment in 44 cases. Results VEEG monitored 80 cases of epileptiform discharge in 100 patients and 20 cases without epileptiform discharge. There was no significant difference in the incidence of epileptiform discharges between different epileptic patients in VEEG monitoring(P > 0.05),and there was no significant difference in the incidence of epileptiform discharges between VEEG patients in different medication groups(P > 0.05). The total effective rate of treatment in the VEEG epileptiform discharge group was 91.3%,which was significantly lower than that of the VEEG non epileptic discharge group (100%,P < 0.05). The main adverse reactions during treatment were drowsiness , abnormal behavior, abnormal sensation and gastrointestinal reaction. There was no significant difference in monitoring epileptiform discharges between VEEG patients (P > 0.05). All adverse reactions were improved after symptomatic treatment. Conclusion The VEEG plays an important role in guiding the patients with refractory epilepsy for epilepsy typing characteristics and AEDs treatment ,so it has significant correlation to the prognosis , and it also has an important significance in assessing the prognosis.

ABSTRACT

Objective To investigate the effect of subclinical epileptiform discharges (SED) on the cognition of adult patients with epilepsy,exploring the mechanism of SED that leads to cognitive impairment in adult patients with epilepsy to raise physicians' attention about SED.Methods Patients were collected in the Department of Neurology,China-Japan Union Hospital of Jilin University from March 2016 to March 2017.Sixty adult patients with epilepsy without clinical episodes in the last three months were selected as SED group and 40 healthy volunteers as control group.Medical history of the SED group was recorded in detail.All patients were examined by Self-rating Anxiety Scale,Self-rating Depression Scale,Pittsburgh Sleep Quality Index scale in order to exclude organic brain disorders,metabolic diseases,anxiety,depression,sleep disorders and drug-induced cognitive dysfunction.Subjects in the two groups received Montreal Cognitive Assessment (MoCA),electroencephalogram and blood oxygen level dependent functional magnetic resonance imaging examination.Finally,the results were compared between the two groups.Results ①SED had different effects on cognitive function in adult patients with epilepsy,and the MoCA score (26(22,27)) showed statistically significant difference compared with the control group (29 (28,29),Z =-6.26,P =0.00).②Different discharges indexes showed different effects on cognitive function aspects.Cognitive impairment was significant when the discharges indexes were > 10% (discharges indexes 1%-10%:MoCA score 26(26,28),discharges indexes 10%-50%:MoCA score 22(19.5,25),Z =-4.74,P =0.00).③The cognitive function of epilepsy patients was positively correlated with the duration of education (r =0.41,P =0.00) and the time interval to recent seizure (r =0.31,P =0.02),and negatively correlated with SED (r =-0.57,P=0.17).There was no correlation between cognitive function and duration of disease and onset age.The SED was the main influencing factor of cognitive function in epilepsy.④Compared with healthy people,epilepsy patients with SED showed differences in resting brain function network connection,with strong connective regions at the right inferior temporal gyrus,right hippocampus,bilateral thalamus,with weak connective regions at the double medial upper frontal gyrus,lateral dorsal frontal gyurs.Conclusions SED had an effect on the cognitive function of adult patients with epilepsy.The mechanism of cognitive impairment in adult epilepsy with SED may be related to abnormal brain function in cognitive-related areas.

RESUMEN El objetivo del presente trabajo es actualizar el análisis de los egresos hospitalarios (EH) por cáncer genitourinario (CGU), específicamente cáncer de próstata, testículo, vejiga y riñón en nuestro país. Para estos efectos se obtuvieron os datos del Boletín de Egresos Hospitalarios del Ministerio de Salud (MINSAL) del año 2010 y 2015, utilizándose los códigos de la clasificación internacional de enfermedades de la Organización Mundial de la Salud (OMS CIE-10). Los distintos diagnósticos fueron caracterizados según su composición geográfica y demográfica, comparándose con la información publicada en los reportes anteriores.(AU)


ABSTRACT The aim of the study is to update the analysis of hospital discharges for genitourinary cancer, specifically prostatic, testicular, bladder and renal cancers in our country. For this purpose, data was obtained from the registers of the Chilean Ministry of Health (MINSAL) for the years 2010 and 2015, using the international classification codes of diseases from the World Health Organization (WHO ICD-10). The different diagnoses were characterized according to their geographic and demographic distribution, comparing them to the information published in the previous reports.(AU)

ABSTRACT

<p><b>OBJECTIVE</b>To explore the effect of scalp acupuncture for children with cerebral palsy whose video-electroencephalogram(VEEG) showed epileptiform discharges.</p><p><b>METHODS</b>A total of 184 children with cerebral palsy whose VEEG showed epileptiform discharges or those combined with epilepsy were randomly assigned into a combination group (99 cases) and a rehabilitation group (85 cases). All the cases were treated with the original antiepileptic drugs. The conventional physical training and massage were applied in the rehabilitation group for 3 courses with 20 d at the interval, once a day, 5 times a week and 15 times as one course. Based on the treatment as the rehabilitation group, scalp acupuncture was used in the combination group for 3 courses with 15 d at the interval, once the other day and 10 times as one course. Shenting (GV 24), Benshen (GB 13), Sishencong (EX-HN 1) were selected as the main acupoints, combined with motor zone, foot motor-sensory area, balance zone,and temple-three-needle etc. Clinical onset and VEEG results were observed before and after treatment.</p><p><b>RESULTS</b>After treatment in the combination group, 27 cases improved; 47 cases had no effect; 25 cases aggravated. While in the rehabilitation group, 11 cases improved; 46 cases had no effect; 28 cases aggravated. There was no statistically significance between the two groups (>0.05). As for the cases with epilepsy onset in the combination group, 8 cases improved; 4 cases had no effect; 4 cases aggravated. In the rehabilitation group, 4 cases had no effect; 7 cases aggravated. The result in the combination group was better than that in the rehabilitation group (<0.05). As for the cases with epileptiform discharges in the combination group, 19 cases improved; 43 cases had no effect; 21 cases aggravated. In the rehabilitation group, 11 cases improved; 42 cases had no effect; 21 cases aggravated. There was no significance between the two groups (>0.05).</p><p><b>CONCLUSIONS</b>Scalp acupuncture therapy does not increase the risk of onset or epileptiform discharges in the children with cerebral palsy combined with epilepsy or epileptiform discharges. Scalp acupuncture combined with rehabilitation is better than simple rehabilitation for thosewith cerebral palsy and epilepsy onset.</p>

ABSTRACT

Objective To analyze the characteristics and some related influences of interictal epileptiform discharges (IEDs) for symptomatic temporal lobe epilepsy(TLE) and extratemporal lobe epilepsy(ETLE) patients in whom with focal lesion on structural imaging .Methods The electrophysiological and clinical data of 257 epilepsy patients were retrospectively analyzed , all of whom had a focal lesion revealed by structural imaging .Patients were divided into 2 groups according to the locaton of the lesion:TLE group and ETLE group , patients were also divided into 3 subgroups according to the relationship between the location of IEDs and the lesion :TLE-1/ETLE-1 subgroup with a norm interictal EEG; TLE-2/ETLE-2 subgroup with IEDs absolutely located in brain lobes in which lesion located;TLE-3/ETLE-3 subgroup with IEDs exceed or absolutely not located in the lobes in which lesion located . Results The proportion of TLE-1 was significantly lower than ETLE-1 ( P<0.01 ) , while the proportion of TLE-2 was significantly higher than ETLE-2(P<0.01).The proportion of the TLE-3 subgroup increased along with a longer duration, and the proportion of ETLE-3 subgroup decreased along with a lower seizure frequency , and also the older the age at onset .Conclusions The positive rate of IEDs and its positioning accuracy are significantly higher in symptomatic TLE than that in ETLE patients .The distribution of IEDs is more likely to be affected by epilepsy duration in TLE , while it is more easily to be affected by seizure frequency and age at onset in ETLE .

Introducción: Las enfermedades de origen otorrinolaringológico (ORL) tienen una elevada prevalencia en atención primaria de salud, sin embargo, la información epidemiológica en pacientes hospitalizados es bastante escasa. Objetivo: Obtener características demográficas y prevalencia de enfermedades en los pacientes hospitalizados del Servicio de ORL del Hospital Clínico de la Universidad de Chile (HCUCh). Material y método: Estudio de tipo descriptivo y retrospectivo, llevado a cabo en el Servicio de ORL del HCUCh, donde se revisaron los registros electrónicos, existentes de grupos relacionados a diagnóstico (GRD) de egresos, entre los años 2007 y 2014. Resultados: Se incluyeron 7.353 egresos, con un promedio de edad de los pacientes de 28,24 años. La causa de hospitalización más frecuente fue la patología de faringe. Del total de los egresos 87,88% presentaron una intervención quirúrgica donde la amigdalectomía con adenoidectomía alcanzó el 15,7% de las cirugías realizadas. Conclusión: Este estudio nos entrega información epidemiológica sobre los pacientes hospitalizados en un servicio de ORL de un hospital de referencia a nivel nacional.


Introduction: Diseases ofotolaryngology (ENT) origin have a high prevalence in primary health care, however, the epidemiological information on hospitalized patients is almost null. Aim: To obtain demographic and disease prevalence information in hospitalized patients of the ENT Department at the Hospital Clínico de la Universidad de Chile (HCUCh). Material and method: A descriptive and retrospective study, was carried out at the ENT department of the HCUCh, reviewing existing records of diagnostic related groups (GRD) for discharges between 2007 and 2014. Results: We review a total of 7353 discharges, with an average age of patients of 28.24 years. The most frequent cause of hospitalization was pharynx pathology. An 87.88% of discharges had surgery performed, where tonsillectomy with adenoidectomy reached 15,7% of the total. Conclusion: This study provides us with epidemiological information on patients hospitalized in an ENT department in a nationwide referral hospital.
