ABSTRACT

ObjectiveTo review the development history of research on people with disabilities, summarize the patterns, characteristics and deficiencies in this discipline, and make suggestions for discipline development in the future. MethodsThe literature about disability from 1986 to 2018 were searched and retrieved on the CNKI. Valid literature were selected based on the title and abstract. Descriptive analyses were used to analyze the development of research on people with disabilities in China. VOSviewer was used to explore the cooperation among researchers and research hotspots in this field. ResultsA total of 2 267 papers were included. Researches on people with disabilities in China started in 1986 and then experienced rapid development driven by survey data, showing obvious stage characteristics. The foundation of academic cooperation networks has been formed initially, showing the comprehensive development of multiple themes. However, in the new stage, the lack of follow-up support for research infrastructure conditions, as well as the slow innovation of research theories and expansion of research contents may become key factors hindering the further development of the discipline. ConclusionThe research foundation should be consolidated in the future, including broadening cooperation and communication channels, strengthening disability statistics, and promoting cross-disciplinary research. Theoretical research should be strengthened by standardizing research methods and finding internalized theoretical innovation points combining the national conditions. Finally, research content should be enriched, especially by closely combining the current changes in the needs of people with disabilities and strengthening the research on disability prevention and control, health promotion, social integration, and social management of the people with disabilities.

ABSTRACT

A demand-capacity mismatch exists between the services for child and adolescent mental health and the needs of society, and the contradiction is becoming increasingly prominent. This study aims to present a thorough analysis of the current status of research in child and adolescent mental disorders and the direction of discipline development, which consists of the epidemiology, etiology and pathogenesis, diagnostic progress, treatment progress and disciplinary development in child and adolescent mental disorders. Mental disorders in children and adolescents are known to have a high prevalence rate, and the disease is most likely to continue into adulthood if it is not identified and treated in a timely manner, posing a serious burden on patients, families and society. Currently, the etiology and pathogenesis of mental disorders in children and adolescents still remain unclear due to its complexity and the numerous interrelated contributing factors such as genetics, environment, stress events and so on. And the diagnosis is limited to symptomatology and lacks objective indicators. Therapeutically, the various treatment options available have shown certain clinical effectiveness, while suboptimal treatment outcomes have been reported among some children, which require multidisciplinary comprehensive intervention. Furthermore, the research on imaging genetics, new drug-target search, wise information technology of med and brain sciences needs to be strengthened, thus providing a broader approach to better address the needs of mental health services for children and adolescents.

ABSTRACT

Analysis of the layout and proportion of classic and innovative surgical procedures of bariatric and metabolic surgery throughout the world, the sleeve gastrectomy is beco-ming the most accepted and respected surgical procedure with its increasingly prominent advan-tages, and its operation number has even exceeded the "gold standard" surgery of the roux-en-Y gastric bypass. This trend is even more pronounced in China. With a small number of physicians lacking experience in gastrointestinal surgery and more and more young physicians joining in the field of bariatric and metabolic surgery, operation related complications and the consequences of insufficient perioperative management experience have occurred successively in clinical practice, such as gastrointestinal leakage, bleeding, lumen and anastomotic stenosis, internal hernia and obstruction, venous thromboembolism, etc. Bariatric and metabolic surgery is a branch of gastro-intestinal surgery, and physicians in the field should have the ability to prevent, predict and manage short- and long-term complications after surgery. The authors believe that through the comprehen-sive evaluation of laboratory indexes, patient signs and related examinations, physicians should have the basic qualities of decisively choosing the surgical timing, choosing the right decision-making, and taking effective measures.

Intensive care unit (ICU) has become an important embodiment of the comprehensive strength of a hospital, especially the affiliated hospital of medical university or a tertiary hospital. This paper aims to discuss the role, status and contribution of critical care medicine in the development and construction of modern medicine and general hospitals, the relationship between hospital management and critical care medicine management, and the future development and construction direction of critical care medicine from the perspective of the managers of third-level first-class general hospitals.

ABSTRACT

Critical care medicine is a relatively young, fast-growing discipline, but it also bears the burden of heavy life. In the past 10 years, critical care medicine has made rapid progress. It has been growing to be mature and complete, its environment and equipment have been improved, and the technical ability has been continuously upgraded. However, the rapid improvement and the diversification of management modes have also brought about some concerns about medical quality. Xinyang Central Hospital is a large prefecture-grade tertiary-class A general hospital in Henan Province. As the director of the department of critical care medicine and the discipline leader of critical care medicine in Xinyang City, the author shared his opinion about how to establish a "standardized, institutionalized and procedural"management model from the perspective of department management in order to ensure medical quality and safety. We should "start from the quality control of critical care medicine, implement the concept of critical care through ward rounds, supervise the quality from the self-examination and self-correction of adverse events", in order to establish a management system for critical care patients and lay a foundation for improving medical quality and sustainable developing of critical care medicine.

ABSTRACT

Tianjin No.4 Hospital is a general hospital featured in burns, and the Department of Burns and Plastic Surgery has six secondary clinical departments with 110 hospital beds. After hard working by several generations, the department has successively developed into a key discipline in Tianjin and Tianjin Burn Emergency Center. Tianjin Burn Research Institute was established in 1992, and then the department became master′s granting unit of Tianjin Medical University. In scientific research aspect, many researches have been completed and obtained important results, such as the mechanism and effect of integrated traditional Chinese and Western medicine in prevention and treatment of intestinal failure, early diagnosis and local treatment with fiberoptic bronchoscope, energy metabolism regularity of burn patients, and the development of irradiated biological dressings. In clinic aspect, tens of thousands of complex wounds and chronic refractory wounds were repaired by various kinds of flaps and advanced methods, and the cure rate of patients with large burn area reached 94.1%. In 2016, Tianjin No.4 Hospital became the Affiliated Hospital of Nankai University. Hereafter, a number of research intentions have collaborated between the department and the State Key Laboratory of Academy of Life Sciences of Nankai University. In the future, our department will focus on improving the treatment level of patients with large burn area, carrying out researches on comprehensive anti-shock in the early stage, inhalation injury treatment, infection treatment, comprehensive treatment of multiple organ failure, wound repair, scar treatment, rehabilitation, and the clinical application.

Under the guidance of the predecessors and the efforts of the whole staff, Department of Burns and Plastic Surgery of Wuxi Third People′s Hospital has grown into a well-known regional burn diagnosis and treatment center in China after about 30 years of development. Summarizing the experience, gain, and loss in the course of department growth, following and attaching importance to the discipline development law, consolidating the foundation, strengthening the skills, and being diligent in innovation are the most important. The future trend of discipline development is as follows: wound treatment is still fundamental, burn emergency treatment system should be further improved, interdisciplinary and multidisciplinary cooperation should be further strengthened, and basic scientific research should have the potential to transform to reality.

The department has distinctive disciplinary features in burn care, nutritional support, scar prevention and treatments, standard management of chronic wound, and skin tissue engineering research, making positive contribution to the development of burn medicine in China.

ABSTRACT

Chinese burn medicine began in the " Great Leap Forward" movement, with the mark of the successful rescue of Qiu Caikang in Guangci Hospital (now known as Ruijin Hospital Affiliated to the Medical School of Shanghai Jiao Tong University), Shanghai. With the fading of the " Great Leap Forward" movement in 1962, medical staff of some burn bases such as Southwest Hospital of the Seventh Military Medical University (now known as the First Affiliated Hospital of Army Medical University) made a comprehensive summary on burn treatment, forming a set of original effective treatment protocols, which led to the improvement of burn treatment level. The stamp-like auto-allo intermingled skin grafting was created during that period, followed by the creation of an approach called " to embed small pieces of autogenic skin into the holes of a large sheet of allogenic skin" in Guangci Hospital, Shanghai. Owing to these approaches, the eschar of extensive burns was excised in multiple times, which led to the improvement of the cure level for extensive deep burn. Since then, the cure rate of burns of China continued to be the first in the world. Unfortunately, with the beginning of the " Great Cultural Revolution" , the development of Chinese burn medicine was interrupted and stopped. With the founding of the " Revolutionary Committee" in 1969, the Chinese burn medicine just started to recover. After 1978, the Chinese burn medicine began to rise rapidly again. Since then, the theoretic research on burns has been conducted in China, just as a follower at the beginning, and then as a runner in 1990s′, but as a leader in 2000s′ in some fields such as inhalation injury, sepsis, and wound healing, etc. At present, the clinical cure rate of burns in China is ranked in the leading position in the world, and the theoretical research on burns is also among the advanced ranks in the world.

ABSTRACT

This year is the 60th anniversary of both Chinese burn medicine and Institute of Burn Research of Southwest Hospital of Army Medical University. The institute is one of the earliest burn centers in China founded in 1958 by professor Li Ao, one of the pioneers of Chinese burn medicine and the academician of Chinese Academy of Engineering. With unremitting efforts of several generations, the institute consists of three main sections at present, including clinical section which is composed of national key clinical department, research section which is composed of national key laboratory, and two editorial offices of Chinese Journal of Burns and Burns & Trauma. During the 60 years of development, we made lots of remarkable achievements on discipline construction, scientific research, clinical treatment, and talent cultivation and made great contribution to the development of burn medicine of China, with many generous supports and help from the experts and units at home and abroad.

ABSTRACT

Preliminary epidemiological data showed that the overall incidence of burns in China is decreasing. How does burn medicine develop in our country under this circumstance? I put forward my personal opinion of " treating burns in wartime and cutaneous wounds in peacetime" . " Treating burns in wartime" refers to the occurrence of significant mass burn incident in wartime and peacetime. In order to meet the national demand in emergency medical rescue in major public health events, staffs of burn discipline should rescue the victims and minimize mortality. At the same time, in the absence of significant mass burn incident and confronting decreasing incidence of burns, chronic refractory cutaneous wounds could be the important alternative targets for burn discipline in daily work, which should be regarded as the opportunity for the transformation development and survival of burn discipline.

ABSTRACT

This article briefly introduces the development history of Department of Burns and Cutaneous Surgery in Xijing Hospital of Air Force Medical University (the former the Fourth Military Medical University) since its establishment more than 60 years ago. The medical staff of the department have been adhering to the treatment concept of the older generation of experts and professors, applying the principles and methods of plastic surgery to the treatment of deep burn wounds. Furthermore, the new techniques in the field of flap surgery, microsurgery, and minimally invasive surgery are applied to repair various complex wounds after burn/trauma and severe scar deformities. Our department has formed our own distinctive technical features and contributed to the development of burn cause in China.

ABSTRACT

Department of Burns and Plastic Surgery of Ruijin Hospital Affiliated to the Medical School of Shanghai Jiao Tong University is derived from the group of severe burn treatment in Surgical Department of Kuang-Ci Hospital in Shanghai in 1958. In three score years, they created and developed distinctive technologies in treatment of massive deep burns, including early eschar excision on deep burn wound by stages and batches, taking the scalp as a donor site, wound covering with the large sheet of allo- or xeno-skin graft with small holes covered by small pieces of autoskin, the Ruijin formula for fluid resuscitation in early stage post burn and the " hibernation remedy" , which achieved actual effects in clinic, and they summed up academically the experiences of their clinical practice and researches. These technologies with other creations by Chinese burn surgeons have been named as " Chinese method" of burn treatment.

As an initial member of burn medicine in China, I am lucky to participate in the activity of writing paper for the journal to celebrate the 60th anniversary of the founding of burn medicine in China. It is a chance to introduce my clinical work, laboratory study, and teaching. I review the development of burn surgery to make some points in the transformation and development of burn surgery, some suggestions for better coordination between burn surgery and critical care medicine during emergency support for mass burn casualties. Furthermore, I wish that some of the differences in opinions and experience in burn care could be studied and conformed in order to form relatively uniform opinion which might benefit the successors.

In order to celebrate the 60th anniversary of Chinese modern burn medicine, the advancing history of burns in the world and in China were reviewed. Moreover, the rescuing abilities and levels of burns were compared between abroad and in China, clarifying the advantages and disadvantages of Chinese burn medicine. Finally, the challenges and development in the future of Chinese burn medicine were analyzed and addressed in this article.
