ABSTRACT

Objective To explore the retrograde contamination of drainage bag outlets, and provide basis for the formulation of related guideline for healthcare-associated infection(HAI)management. Methods On October 14,2016,with sterile manipulation,urine,5% glucose solution, glucose normal saline, sterile water, and 0.9% nor-mal saline were injected into anti-reflux drainage bags(anti-reflux group)and common drainage bags(common group)respectively, entrances of bags were sealed and bags were hung in two ways:outlets were 10 cm away from the ground(suspended group)and touched the ground(ground-touching group)respectively, specimens were col-lected from bag outlets to perform bacterial culture every 3 days,a total of 10 times of cultures were performed,re-trograde contamination of drainage bag outlets was observed dynamically. Results Retrograde contamination rate of drainage bag outlets of anti-reflux group was significantly lower than common group(7.7% vs 46.0%,P=0.000);suspended group was significantly lower than ground-touching group(17.9% vs 35.8%,P=0.000). Retrograde contamination rates of outlets of drainage bags filled with different properties of liquid were as follows:urine (54.3%)>5% glucose solution(34.5%)>glucose normal saline(24.3%)>0.9% normal saline(10.8%)>ste-rile water(10.5%),pairwise comparison showed a significant difference(P=0.000).The initial occurrence time of contamination in anti-reflux group and common group was on the 13thday and 7thday respectively, two group was significantly different on the 7thday(P=0.041). There was a medium intensity correlation between the types of drainage bags and liquid properties(PearsonC=0.5).Conclusion Different types of drainage bags, retention time, and liquid property can impact retrograde contamination of drainage bag outlets, regular urine culture during the use of drainage bags should be paid attention in clinical practice,so as to use antimicrobial agents rationally and guide replacement time of drainage bags.

ABSTRACT

Objective To investigate the appropriate time of replacing drainage bags after trans-cervical resection of adhesion(TCRA) surgery. Methods Totally 156 patients underwent TCRA were randomly divided into three groups:Group A,the drainage bags were replaced daily;Group B,the drainage bags were replaced every 3 days;Group C,the drainage bags were never replaced. The drainage bags were removed on the 6th day after TCRA for all groups.Bacterial culture results from the balloon surface of drainage bag, outer cervical orifice, and the connector of drainage bag for 3 groups were observed. The positive rates of bacterial cultures were analyzed statistically. Results The bacterial culture results from the drainage bag connector of 3 groups indicated that the positive rate of Group C was significantly lower than that of other two groups(P<0.05).The bacterial culture from the drainage bag balloon surface also indicated the same result, but the result did not showed statistically significant difference(P>0.05).There was no statistically significant difference in the culture result firom outer cervical orifice and the volume of drainage fluid within three groups(P>0.05).Conclusion It is indicated that the patient may enjoy a lower rate of bacterial infection without replacing any drainage bags connected to balloon drainage tubes placed after TCRA within 6 days.We suggest that the intrauterine drainage hags may be kept without replacement until direct removing the drainage tube on day 6 after TCRA.

ABSTRACT

Objective To compare the effects of adjustable pressure drainage device, negative pressure ball drainage and drainage bag drainage on patients with the total hip arthroplasty. Methods The silicone drainage ball with negative pressure, the urine collection bag, the infusion set′s regulator were used to make adjustable pressure drainage device. 120 cases of total hip arthroplasties from January to December in 2015 were chosed and divided randomly into 3 groups based on the sequence of operation notifications. In 40 cases, drainage bag were used (control group 1), 40 cases with negative pressure ball drainage (control group 2) and 40 cases with adjustable pressure drainage device (experimental group). Total postoperative bleeding, the blood transfusion amount, swelling (the thickness of hematoma) and the nursing workload of the three groups were compared. Results The total postoperative bleeding, the blood transfusion amount, the thickness of hematoma and the nursing workload in the experimental group were (775.1±130.5) ml, (3.5±1.3) mm, (180.2±29.9) ml, (34.5±5.2) min. The total postoperative bleeding, the blood transfusion amount, the thickness of hematoma and the nursing workload in the control group 1 were (889.4 ± 160.8) ml, (5.2 ± 1.1) mm, (285.9 ± 30.4) ml, (40.6 ± 7.4) min and in the control group 2 were (876.2±156.7) ml, (4.3±1.2) mm,(208.3±33.3) ml, (50.3±8.5) min. Compared the data of the experimental group and the control group 1, the difference was of statistically significance (t=3.49-15.68, P < 0.01). Compared the data of the experimental group and the control group 2, the difference was of statistically significance (t=2.86-10.03, P<0.01). Conclusions The effects of adjustable pressure drainage device are better than the drainage bag and negative pressure drainage ball. It can effectively reduce the postoperative bleeding and the swelling degree, and bring a lot of relief to the patients and lots of convenience to the nursing work. The method is affordable, reliable and safe which should be promoted in the primary hospital.

ABSTRACT

Objective To study the appropriate changing frequency of urinary drainage bag for patients with indwelling catheters using anti-reflux drainage bag in urological surgical department. Methods Of 78 in-patients with indwelling catheters using anti-reflux drainage bags 40 patients were selected randomly as the experimental group with no bag change,38 patients as the control group with bag change every day.Bloody urine samples were obtained from each drainage bag for germicuhure ev-ery day. Results Under situations of no difference existed in general information, duration of in-dwelling catheters, volume of drinking and urine and times of opening valve, the positive rate of urine germiculture was 7.5% and 10.5%( P>0.05 ),the incidence of symptomatic urinary tract infection (U-TI) was 2.5% and 2.6% in the experimental group and the control group (P>0.05). Conclusions There is no evidence for the necessity of anti-reflux drainage bag change every day for patients with in-dwelling catheters and bloody urine after operation in the urological surgical department. However the appropriate frequency of urinary drainage bag change needs additional study.
