ABSTRACT

Children with burns often have uncooperative behaviors such as crying and struggling when changing dressing because of pain and fear, which affects dressing change of medical staff and increases the psychological burden of the family members of children and dressing change personnel. In order to solve the above problems, the author′s team designs and makes the mobile soothing screen for pediatric dressing change. Dressing change personnel are isolated from the children′s sight through the screen′s shielding function, and the children′s favorite program showed on the tablet computer attracts the children′s attention. The wound dressing change is completed by using the disposal window on the screen, which has good clinical application effects.

ABSTRACT

Objective To compare the effect of silver-coated dressing and general aseptic dressing on the wound healing of abdominal operation. Methods A total of 94 patients from the general surgery department of Second Affiliated Hospital of Xi'an Jiaotong University who underwent abdominal operation were divided into two groups by double blind method, 49 cases in silver-coated dressing group, and 45 cases in general aseptic dressing group. The wound healing of the two groups were observed after wound dressing change, and the wound dressing change times, the healing time, the degree of pain, the positive rate of bacterial culture and the incidence rate of scar healing were observed in the two groups. Results The patients in silver-coated dressing and general aseptic dressing group had no significant differences in gender, age, length of incision, type of incision and poor wound healing type (all P>0.05);after two wound dressings change treatment, the wound dressing change times and incision healing timeof the silver-coated dressing group was (6.52 ± 1.52) times and (16.34 ± 5.96) days, which were less than those of the general aseptic dressing group, which were (8.74 ± 2.35) times and (23.32 ± 8.32) days; and the pain severity (NRS) scored 4.13 ± 1.01, which was lower than that of general aseptic dressing group (6.54 ± 0.95), with significant difference between two groups (t=5.482, 4.704, 11.890, P < 0.05). The bacterial culture positive rates of the silver-coated dressing group was 60.00% (21/35), which was higher than that of the general aseptic dressing group, 23.33%(7/30), the difference was statistically significant (P<0.01);at the same time, the scar healing cases was 4 in the silver-coated dressing group, which was less than 11 cases in the general aseptic dressing group. The difference was statistically significant (P < 0.05). Conclusions The silver-coated wound dressing method is better than the general aseptic wound dressing, and has obvious advantages in the treatment of abdominal poor healing incision. The silver-coated wound dressing can promote the healing of incision, reduce scar healing, shorten the hospitalization time, reduce the suffering of patients.

ABSTRACT

Objective: To explore clinical effect of transformation of dressing change process on reducing temporary catheter?related infection in hemodialysis patients. Methods: A total of 60 patients underwent temporary hemodialysis from Apr 2013 to Apr 2014 were randomly divided into intervention group and control group, 30 cases in each. The patients in the control group received conven?tional treatment, the patients in the intervention group received dressing change according to the process, infection rate was compared and analyzed. Results: The indwelling time in the intervention group was (46?35±6?24) days, the total infection rate was 3?33%. The indwelling time in the control group was (39?24±4?64) days, the total infection rate was 13?33% (P<0?05). And there was significant difference in the number of dressing changes, treatment time ( P<0?05) . Conclusion: The scientific, effective dressing process playes an inhibitory effect in temporary catheter?related infection in hemodialysis patients and can prolong the service life of catheter, promote the rehabilitation of patients.

ABSTRACT

Objective To investigate the therapeutic effect of different methods of dressing on the wound after local debridement of dia-betic foot. Methods A total of 53 patients which underwent local debridement of diabetic foot were divided into control group and treatment group. Patients in control group were dressed on traditional measurement,while patients in the treatment group were dressed on the external medicinal wine for the diabetic foot basic on the traditional treatment. The local transcutaneous oxygen partial pressure,ulcer healing rate,av-erage healing time and the amputation rate were observed. Results The healing rate and percutaneous oxygen partial pressure of the treat-ment group were significantly increased than those in the control group (P<0. 05). The average healing time and amputation rate of the treat-ment group were significantly less than those in the control group (P<0. 05). Conclusion Dressing on external medicinal wine after local debridement of diabetic foot can improve the wound healing in the diabetic foot.

ABSTRACT

Objective To investigate the learning curve of peripherally inserted central catheter (PICC) switching operation, for providing a reference basis for the nurses switching operation. Methods The clinical data of 50 cases of switching operation by one nurse of stomach surgery in PICC outpatient service maintenance center were retrospectively analyzed from July to December in 2013. According to the order of operations, every 10 patients was a learning phase, and they were divided into five stages:group A with number 1 to 10, group B with number 11 to 20, group C with number 21 to 30, group D with number 31 to 40, group E with number 41 to 50. The time and score of switching operation were compared among 5 groups. Results The age of the participants was(49.78±14.87) years, and there were no significant differences in sex, age, tube parts, catheter indwelling time among 5 groups, P>0.05. The time of switching operation were (20.17± 0.82),(17.10± 0.47),(15.15± 0.23),(14.27± 0.15),(13.16± 0.09) min in group A, B, C, D, E. The score of switching operation were (80.00 ± 7.88), (91.60 ± 0.56), (93.10 ± 0.53), (93.20 ± 0.44), (93.90 ± 0.43) points in group A, B, C, D, E. There were significant differences in the time and score of switching operation among 5 groups, F=50.978, 23.787,P<0.01. Between two comparison groups, 20 cases, the time of switching operation appeared inflection point (P<0.05), 10 cases, the score of switching operation appeared inflection point (P<0.05). The time of switching operation of the first 20 cases was (19.04±0.51) min,and which of the later 30 cases was (14.45±0.09) min. The score of switching operation of the first 10 cases was (80.00±7.88) points, and which of the later 40 cases was (92.95±1.72) points. Conclusions After a learning curve of 20 switching operation, nurses can overcome the learning curve and master the skills of dressing change of PICC proficiently.

ABSTRACT

Objective To evaluate the effect of improved cleaning and disinfection method for subclavian venipunc-ture dressing change to prevent catheter-associated infection. Methods 120 hospitalized patients with right subcla-vian venipuncture at an intensive care unit in January-June 2012 were divided into control group (n= 60)and experi-mental group(n= 60)according to venipuncture date. Control group adopted conventional dressing change :disin-fected skin at and around puncture point by 2% iodine tincture,then used 75% alcohol for deiodination;experimen-tal group adopted improved dressing change :Wiped skin at and around puncture point three times by normal saline, cleaned catheter,disinfected skin around puncture point (avoid puncture point)three times by 75% alcohol,disin-fected skin at and around puncture point three times by 0 .5% iodine tincture ,then disinfected catheter . Associated infection rate between two groups were compared . Results Focal infection rate and CRBI rate of experimental group were both lower than control group(5.00% vs 16.67% ;1.67% vs 13.33% ),the difference was statistically different (P< 0.05 ). Conclusion Effect of improved dressing method is better than the conventional dressing method,it can effectively prevent occurrence of focal infection and CRBI.

ABSTRACT

Objective To explore the relationship between frequency of dressing change and PICC insertion site infection in neonates. Methods A prospective study was undertaken. 95 neonates were randomized into 3 groups, group 1 changed dressing every 3 days, group 2 changed dressing every 5 days, group 3 changed dressing every 7 days. Insertion site infection was observed and underwent pathogenic detection,complications associated with dressing change were also observed in all the 3 groups. Results 31 neonates were randomized to group 1 ,there were 3(9.68%)episodes of insertion site infection,34 neonates were randomized to group 2,there were 2(5.88%)episodes of insertion site infection, 30 neonates were randomized to group 3, there were 5(16.67%)episodes of insertion site infection. Rates of insertion site infection did not differ in the 3 groups.8 strains of staphylococcus epidermidis(2 of them were methicillin resistant staphylococcus epidermidis)were founded in 10 neonates who developed insertion site infection,positive rate was 80%. Skin excoriation occurred in 4 of 31 neonates in group 1,no neonates in the other two groups developed skin excoriation. Conclusions We suggest change dressing of neonatal PICC every 7 days,replace dressing if the dressing becomes damp or loosened at any time.

ABSTRACT

Objective To study the application of individualized dressing change on tumor patients with PICC. Methods We made different methods of dressing change in accordance with the status of the patients and the seasons, and observed the color and the integrity of the local skin around the puncture points. Results With the exception of 2 cases requested extubation because of tetter on skin around the puncture point, the local skin infection or systemic infection didn't occur in the other 97 cases by individualized dressing change. Conclusions It can reduce the rate of local infection, extends time of catheter retaining, and improves quality of life of tumnor patients with PICC by individualized dressing change

ABSTRACT

Objective To observe the effect of shuanghuang liquid on bum wound healing. Meth-ods 120 cases of patients with moderate bums adopted shuanghuang liquid (the treatment group) and gentamyein (the control group) for wound dressing change, wound pain and wound healing effect were com-pared between the two groups. Results The two groups showed difference in reducing wound pain and shortening time of wound healing. Conclusions Shuanghuang liquid was superior to gentamycin in wound healing and wounds pain.

OBJECTIVE To observe the effect of the silver sulfadiazine-impregnated hydrocolloid dressing on the pain of nail extraction wound during dressing change and the healing time of wound. METHODS Forty eight patients with nail extraction were randomly divided into two groups: in the study group,whose wound was covered with silver sulfadiazine-impregnated hydrocolloid dressing;in the control group,whose wound was applied vaseline gauze when the nail had been extracted and the wound was applied antibiotic gauze during dressing change.The pain scores of two groups were compared.Two groups were compared with healing time and the times of dressing change. Objective:To study the effect of dressing with insulin on the expression of HIF-1?and VEGF in deepⅡ degree burn wound healing of diabetic rats and explore the possible mechanisms. Methods:42 rats,either male or female,were established models of deepⅡ burn wounds with diabetes,which was induced by streptozotocin (STZ). They were completely randomly divided into two groups,that is the insulin group and the control group. Dressing changes with insulin and saline dressings twice daily,respectively. ABSTRACT

OBJECTIVE To monitor the hand contamination after dressing change for the infected wound and to explore an effective way for hand hygiene. METHODS Caregivers were divided into three groups after applying dressing on infected wound:group 1(no hand-washing),group 2(regular hand-washing),and group 3(standard hand-washing),the pathogens in hands were monitored and the effects of two ways of hand-washing were compared. RESULTS The rate of hand-carrying pathogens after dressing change for the infected wound was 100.0%,the qualified rate of regular hand-washing was only 47.9% and that of standard hand-washing (six-steps method)was 97.9%. CONCLUSIONS The standard hand-washing is more effective than regular hand-washing. Correct hand hygiene should be emphasized to prevent and control nosocomial infection.
