ABSTRACT

@#Dysphagia is most common after medullary infarction, characterized by aspiration, residual pharynx and poor esophageal sphincter opening. The existence and severity of dysphagia can be preliminarily determined according to the site of infarction, and it is more likely to cause dysphagia on rostral, dorsal and lateral part of the medulla oblongata. High resolution manometry can be used to understand the biomechanical characteristics of the pharynx and upper segment of esophagus, and helps to evaluate dysphagia, especially combined with videofluoroscopic swallowing study, to guide the rehabilitation. Compensatory posture, electrical stimulation, transcranial magnetic stimulation and balloon dilatation are effective for the treatment.

@#Dysphagia commonly occurs in Parkinson's disease (PD) at any stage, which may result in serious complications. The mechanism is uncertain. Swallowing Disturbance Questionnaire (SDQ) and Munich Dysphagia Test-Parkinson's Disease (MDT-PD) are often used for screening dysphagia in PD patients. But Non-motor Symptoms Questionnaire (NMSQ) and Unified Parkinson's Disease Rating Scale (UPDRS) underestimate the severity of dysphagia in PD patients. Videofluoroscopic Swallowing Examination (VFSE) and Videoendoscoptic Swallowing Study (VESS) are the gold standards for the dysphagia. There is no systemic management yet. Interventions includes medication, deep brain stimulation, etc. Early diagnosis and management facilitate to decrease the complications.

ABSTRACT

A reabilitação da disfagia orofaríngea ganhou um novo instrumento terapêutico, a eletroestimulação neuromuscular (EENM), sendo que os mais renomados pesquisadores têm estudado a indicação e os resultados desta abordagem. O objetivo deste trabalho foi apresentar revisão bibliográfica sobre a aplicabilidade da EENM na reabilitação da disfagia orofaríngea. Realizou-se amplo levantamento bibliográfico em bases de dados, englobando as duas últimas décadas de pesquisa na área. Este artigo de revisão mostrou que ainda não há consenso sobre o uso da EENM na reabilitação da disfagia. Constatou-se que a maioria dos trabalhos descreveu o uso da EENM de forma isolada, não relatou as técnicas fonoaudiológicas associadas à eletroterapia e utilizou amostras heterogêneas que agrupavam disfagias orofaríngeas mecânicas e neurogênicas. Somente recentemente programas específicos têm sido delineados e testados em populações mais homogêneas.


The rehabilitation of oropharyngeal dysphagia has a new therapeutic tool, the neuromuscular electrical stimulation (NMES), and the most renowned researchers have been studying the applicability and the results of this approach. The aim of this study was to present a literature review regarding the applicability of NMES in the rehabilitation of oropharyngeal dysphagia. An extensive literature review was carried out, considering the last two decades of research in the area. The review showed that there is still no consensus on the use of NMES in the rehabilitation of dysphagia. It was found that most studies described the use of NMES in isolation, did not describe the techniques associated with speech-language therapy associated to electrotherapy, and used heterogeneous samples that clustered mechanical and neurogenic oropharyngeal dysphagia. Only recently specific programs have been designed and tested in more homogeneous populations.

ABSTRACT

@#Objective To investigate the effect of nursing intervention for dysphagia in elderly patients with stroke.Methods 17 patients accepted swallow training individually.Their swallowing functions were assessed before and after training.Results Their swallowing functions improved after 4-week-training(P<0.01).Conclusion Rehabilitation nursing is effective on dysphagia in elderly patients with stroke.

ABSTRACT

@#Objective To investigate the effects of electrical stimulation on dysphagia after stroke.Methods 40 patients with dysphagia after stroke were divided into control group and research group randomly.Both groups were treated with rehabilitation training and medicine.Research group was managed with electrical stimulation in addition.They were assessed with the classification of dysphagia.Results Dysphagia was reduced in both groups,but was better in the research group than in the control group(P<0.05).Conclusion Electrical stimulation can improve the recovery of deglutition.
