ABSTRACT

Introducción: La diabetes mellitus es una enfermedad crónica con alta prevalencia, que afecta la calidad de vida de la población que la padece, por cuanto identificar el riesgo de desarrollarla permite implementar estrategias para retrasar su aparición y prevenir complicaciones. Objetivo: Identificar el nivel de riesgo de desarrollar diabetes mellitus tipo 2 en una población del Caribe colombiano. Métodos: Estudio cuantitativo, de corte transversal. Participaron 248 individuos del área urbana de un corregimiento de Santa Marta, Colombia, en el primer semestre de 2018, mediante un muestreo estratificado por sectores. Se utilizó el test Finnish Diabetes Risk Score para la recolección de información, así como la medición del perímetro abdominal, el peso, la talla y la tensión arterial. Para el análisis de las variables numéricas se aplicó T de Student para variables independientes y para las variables categóricas se aplicó chi cuadrado. Resultados: Del total de participantes, 36,29 por ciento presentó un riesgo ligeramente elevado y 8,87 por ciento riesgo moderado alto. El grupo poblacional con mayor riesgo fue el de 58 a 65 años (34,78 por ciento). Se encontró significancia estadística entre el riesgo de presentar diabetes en los próximos 10 años y las variables sobrepeso y obesidad, circunferencia abdominal aumentada, antecedentes personales de hiperglucemia, consumo de medicamentos antihipertensivos y antecedentes personales de diabetes mellitus tipo 2 en padres, hermanos e hijos. Conclusiones: El riesgo de desarrollar diabetes mellitus tipo 2 en la población estudiada fue ligeramente elevado en un alto porcentaje de la población(AU)


Introduction: Diabetes mellitus is a chronic disease with high prevalence, which affects the quality of life of the population that suffers from it, since identifying the risk of developing it allows the implementation of strategies to delay its onset and prevent complications. Objective: To identify the level of risk of developing type 2 diabetes mellitus in a Colombian Caribbean population. Methods: This is a quantitative, cross-sectional study. two hundred forty eight individuals from the urban area of a village in Santa Marta, Colombia, participated in the first semester of 2018, through a sampling stratified by sectors. The Finnish Diabetes Risk Score test was used to collect information, as well as to measure abdominal circumference, weight, height, and blood pressure. For the analysis of the numerical variables, Student's t-test was applied for independent variables and chi-square was applied for the categorical variables. Results: Of the total of participants, 36.29 percent had slightly high risk and 8.87 percent moderate high risk. The population group with the highest risk was 58 to 65 years (34.78 percent). Statistical significance was found between the risk of developing diabetes in the subsequent 10 years and the variables overweight and obesity, increased abdominal circumference, personal history of hyperglycemia, consumption of antihypertensive drugs, and personal history of type 2 diabetes mellitus in parents, siblings, and children. Conclusions: The risk of developing type 2 diabetes mellitus in the studied population was slightly elevated in a high percentage of the population(AU)

ABSTRACT

The different pathologies of the prostate, involve the presence of a new microenvironment where inflammatory cells are actively recruited. This research explores the presence of mast cells and eosinophils associated with age and the evaluation of prostate cancer progress (Gleason Index). Forty two biopsies of anonymized patients, with confirmed prostate cancer, were used for histological analysis for eosinophils and mast cells and subsequent determination of Gleason index according to age. The results of the histological analyzes show the presence of eosinophils and mast cells in prostate biopsies with confirmed cancer. In the multiple correlation studies, a high correlation was observed between the presence of lymphocytes and the age of the patient diagnosed with prostate cancer, same correlation was observed between the patient's age and higher Gleason Index (Pearson and Spearman p< 0.05). It is concluded that in prostate biopsies from Chilean patients with confirmed cancer, eosinophilia and tissue mastocytosis were observed. Correlation analyzes show a direct correlation between older patients, higher Gleason index and presence of mast cell. Regarding eosinophilia, only a correlation between age and Gleason index was observed Further studies are suggested to determine that the presence of eosinophils and mast cells can be used as early bioindicators of prostate cancer.


Las diferentes patologías de próstata, involucran la presencia de un nuevo microambiente donde las células inflamatorias son activamente reclutadas. La presente investigación explora la presencia de mastocitos y eosinófilos asociadas a la edad y la evaluación del progreso del cáncer de próstata según índice de Gleason. Cuarenta y dos biopsias de pacientes anonimizados, con cáncer prostático confirmados, fueron utilizadas para su análisis histológico para eosinófilos y mastocitos y posterior determinación del índice de Gleason según edad. Los resultados de los análisis histológicos, muestran la presencia de eosinófilos y mastocitos en biopsias de próstata con cáncer confirmado. En los estudios de correlación múltiple, se observó una alta correlación entre la presencia de linfocitos, mastocitos y la edad del paciente diagnosticado con cáncer prostático, igual correlación se observó entre la edad del paciente y mayor índice de Gleason (Pearson y Spearman p<0,05). Se concluyó que en las biopsias de próstata de pacientes chilenos con cáncer confirmado, se observó eosinofilia y mastocitosis tisular. Los análisis de correlación muestran una correlación directa entre pacientes de mayor edad, índice de Gleason más alto y la presencia de mastocitos. Con respecto a la eosinofilia, solo se observó una correlación entre la edad y el índice de Gleason. Se sugieren estudios adicionales para determinar que la presencia de eosinófilos y mastocitos puede usarse como bioindicadores tempranos del cáncer de próstata.

ABSTRACT

La inversión uterina obstétrica es una entidad rara, y una de las complicaciones más graves del tercer estadio del parto que pone en peligro la vida de la parturienta. La inversión del útero consiste en un cambio de disposición de sus paredes, la cara interna se convierte en externa y el órgano se invagina en forma de dedo de guante. Es un accidente obstétrico que constituye una verdadera emergencia y es causa de muerte materna por choque hemorrágico. Afortunadamente su incidencia en nuestro país es baja, dado por la escasa cantidad de reportes sobre el tema. El objetivo del trabajo es presentar este caso por la atipicidad en la forma clínica que se diagnosticó durante el puerperio. La paciente acudió al hospital a los 22 días de puerperio, se diagnosticó una inversión uterina puerperal subaguda grado II, se realizan maniobras encaminadas a la reducción manual de la inversión con resultados insatisfactorios. Fue necesario realizar histerectomía total abdominal; se logró conservar ambos anejos y la vida de la paciente. Su tratamiento habitualmente es quirúrgico, sobre todo si no se diagnostica en el tercer estadio del parto(AU)


Obstetric uterine inversion is a rare entity, and one of the most serious complications of the third stage of labor that endangers the life of the mother. The inversion of the uterus is a change of arrangement of its walls, it pulls on the inside surface, and turns the organ inside out shaping a glove finger. It is an obstetric accident which constitutes a true emergency and causes of maternal death from hemorrhagic shock. Fortunately its incidence in our country is low, given the low number of reports on the subject. The aim of this paper is to present this case by its unusualness in clinical form which was diagnosed during the postpartum period. The patient came to the hospital at 22 days postpartum. Sub-acute puerperal uterine inversion grade II was diagnosed. Manual maneuvers were made aiming to reduce the inversion. The results were unsatisfactory. It was required total abdominal hysterectomy; we managed to keep both adnexae and the life of the patient. Treatment is usually surgical, especially if not diagnosed in the labor third stage(AU)

ABSTRACT

The purpose of this study was to determine the accuracy of autofluorescence techniques for diagnosing oral mucosa lesions, using as reference pattern for comparison the visual diagnosis made by a clinical specialist. A pilot study was conducted with 60 patients divided in a control group without mucosal pathology and a study group with known clinical history for mucosal pathology. Both groups were examined by an oral medicine specialist and by a general dentist who used VELscope(r) system, which applies tissue fluorescence visualization to identify oral mucosal abnormalities. Using the VELscope(r) system, the general dentist made overdiagnosis in two cases and underdiagnosis in one case. The sensitivity and specificity for the oral medicine specialist were 1 (95% CI: 0.884 to 1). For the general dentist, the sensitivity did not improve significantly with the use of VELscope(r) system [0.53 (95% CI: 0.343 to 0.717) versus 0.49 (95% CI: 0.406 to 0.773)] and the specificity was 0.80 (95% CI: 0.614 to 0.923). A limitation of the study is the small sample size, which does not fully represent a population and extrapolation of the data should be done carefully. Based on the obtained results, no clinical benefits were obtained using this VELscope(r) system.


Resumo O objetivo deste estudo foi determinar a precisão das técnicas de autofluorescência para o diagnóstico de lesões da mucosa oral, utilizando como padrão de referência para comparação o diagnóstico visual feito por um especialista clínico. Um estudo piloto foi realizado com 60 pacientes, divididos em um grupo controle sem patologia da mucosa oral e um grupo de estudo com história clínica conhecida de patologia da mucosa oral. Ambos os grupos foram examinados por um especialista em medicina oral e por um dentista clínico geral que usou o sistema VELscope(r), que aplica a visualização por fluorescência para identificar anormalidades do tecido da mucosa oral. Usando o sistema VELscope(r), o dentista geral realizou sobrediagnóstico em dois casos e subdiagnóstico em um caso. A sensibilidade e especificidade para o especialista em medicina oral foi 1 (IC 95%: 0,884 a 1). Para o dentista geral, a sensibilidade não melhorou significativamente com o uso do sistema de VELscope(r) [0,53 (95% CI: 0,343 to 0,717) versus 0,49 (95% CI: 0,406 to 0,773)], e a especificidade foi de 0,80 (IC de 95% : 0,614-0,923). Uma limitação do estudo é o pequeno tamanho da amostra, que não representa totalmente a população e a extrapolação dos dados deve ser feita com cuidado. Com base nos resultados obtidos, não houve benefícios clínicos com o uso do sistema VELscope(r).

ABSTRACT

Objective To investigate the value of combined detection of serum alpha-fetoprotein (AFP)and Golgi protein-73 (GP73)in the diagnosis of hepatocellular carcinoma (HCC)and to provide a basis for early diagnosis and differential diagnosis of HCC.Methods A total of 408 patients hospitalized in Baoji Central Hospital from June 2012 to May 2013,as well as healthy persons who had normal test re-sults in physical examination,were included in the study,and their specimens were collected.These patients were classified into HCC group (n=142),chronic hepatitis group (n=156),and liver cirrhosis group (n=110).Serum levels of AFP and GP73 in the three groups were measured by electrochemiluminescence immunoassay and double -antibody sandwich enzyme -linked immunosorbent assay,respectively. Comparison of test results between groups was made by analysis of variance,and comparison of rates was made by chi-square test.The sen-sitivity and specificity of the two indicators for the diagnosis of HCC were calculated using MedCalc statistical software.Results The HCC group had significantly higher serum AFP and GP73 levels than the liver cirrhosis group and chronic hepatitis group (P<0.05);the liver cirrhosis group had significantly higher serum AFP and GP73 levels than the chronic hepatitis group (P<0.05).The sensitivity and speci-ficity of the two indicators for the diagnosis of HCC were 95.8% and 98.6%,respectively,showing significant differences compared with those of each indicator alone (P<0.05).Conclusion Combined detection of serum AFP and GP73 has high diagnostic value and clinical significance for HCC,and they can be used as indicators for early diagnosis and differential diagnosis of HCC.

ABSTRACT

<b>Background:</b> Rapid and accurate diagnosis is essential for containing the novel influenza A/H1N1 pandemic. Polymerase chain reaction (PCR) testing is an accurate diagnostic method, but it is not routinely available worldwide. We herein evaluated the usefulness of pharyngeal “influenza follicles” in diagnosing seasonal influenza and influenza A/2009 (H1N1) pdm.<br><b>Methods:</b> Between August 3 and October 29, 2009, we evaluated 87 patients with influenza-like symptoms. Twenty-three had influenza follicles (22 on initial evaluation; 1 on follow-up) while 64 did not. Considering these two groups, we then compared the positive cases using rapid diagnostic testing (confirmed by PCR). In addition, 419 cases of seasonal influenza diagnosed between 2003 and 2009 were examined for the presence of influenza follicles based on Miyamoto's 2007 definition<sup>9</sup>, and new exclusion criteria were developed.<br><b>Results:</b> Among the 23 patients with influenza follicles, 21 were diagnosed with novel influenza. Of these, follicles were present on initial evaluation in 20 and on follow-up in 1. None of the 64 patients without influenza follicles were diagnosed with influenza (sensitivity 100%, specificity 97%). Among the 419 patients diagnosed with seasonal influenza between 2003 and 2009, influenza follicles occurred in all type A/H3N2, A/H1N1, and B cases (sensitivity 95.46%, specificity 98.42%). Thus, follicles were considered a specific sign of influenza.<br><b>Conclusion:</b> Influenza follicles occur in both seasonal and novel influenza. This identification method has higher diagnostic sensitivity and specificity than rapid diagnostic testing and is a promising clinical tool for diagnosing influenza when PCR is unavailable, or in pandemic situations.

BACKGROUND/AIMS: Primary T-cell lymphoma of the gastrointestinal tract is a very difficult disease entity to diagnose, and has an extremely poor prognosis. The aim of this study was to determine the early diagnostic accuracy for gastrointestinal T-cell lymphoma by gastroenterologists. METHODS: Between January 2000 and October 2010, the clinical features of 15 patients with primary gastrointestinal T-cell lymphomas, including endoscopic findings, radiologic diagnosis, endoscopic biopsy findings, and final diagnosis, were retrospectively reviewed. RESULTS: The most common initial presenting symptoms of primary gastrointestinal T-cell lymphomas was abdominal pain (n=11, 73%). The anatomic location of the primary lesion the small bowel (n=8, 53%), colon (n=5, 33%), and stomach (n=3, 20%). There were no cases of T-cell lymphomas diagnosed based on clinical symptoms, radiologic findings, or endoscopic findings without biopsy alone. Pathologic confirmation of T-cell lymphomas by endoscopic examination was achieved in 7 cases (64%) and the remaining cases (n=8, 53%) were diagnosed with T-cell lymphomas based on pathologic examination after surgery. CONCLUSIONS: All of the patients with primary T-cell lymphomas of the gastrointestinal tract were diagnosed exclusively by endoscopic or surgical pathologic examainations, suggesting that gastroenterologists should scrutinize and suspect this disease with caution due to atypical gastrointestinal ulcers.

OBJETIVO: Revisar criticamente os instrumentos de avaliação mais utilizados na atualidade na literatura para triagem e identificação precoce de anormalidades no desenvolvimento em crianças. FONTES DOS DADOS: Foi realizado um levantamento bibliográfico nas bases de dados no SciELO, plataforma CAPES, PubMed e Google Scholar, com os unitermos "prematuridade", "atraso no desenvolvimento", "paralisia cerebral", "diagnóstico precoce" e "testes de avaliação". SÍNTESE DOS DADOS: Foram listados 455 títulos, sendo selecionados para esta revisão 174 artigos com base em título, relevância temática e resumo. Apenas artigos originais, disponíveis em meio eletrônico, a partir de 1985, com informação sobre a construção, aplicabilidade e propriedades psicométricas dos testes foram usados. CONCLUSÕES: Os testes de triagem podem acelerar o início da intervenção precoce e facilitar o desenvolvimento futuro destas crianças. Vários instrumentos são utilizados para este fim, dentre eles destacam-se, nas pesquisas nacionais, o teste DENVER II e o Alberta Infant Motor Scale. O Movement Assessment of Infant também emerge como teste de triagem utilizado em nosso país. Além desses, dois outros testes são indicados na literatura mundial por sua alta sensibilidade e especificidade em idades precoces: Test of Infant Motor Performance e General Movements.


OBJECTIVE:To review the literature regarding screening psychomotor tests for the early identification of developmental problems. SOURCES: A search on SciELO, PubMed and Google Scholar was performed using the terms "prematurity," "developmental delay," "cerebral palsy," "early diagnosis" and "evaluation tests." SUMMARY OF THE FINDINGS:A total of 455 references were listed, and 174 studies were selected for this review based on title, relevance, and abstract. Only original and electronically available material, from 1985 forward, with information on design, applicability, and psychometric properties of those tests was included. CONCLUSIONS: Screening testS are important to speed the beginning of treatment measures in order to allow for better developmental outcome. Among the many tests that can be employed for this purpose, the DENVER II and the Alberta Infant Motor Scale are the most often used in Brazilian studies. The Movement Assessment of Infants is starting to be used in our country. Two other tests are recommended in the literature due to their high sensibility and specificity: the Test of Infant Motor Performance and the General Movements.
