ABSTRACT

ABSTRACT

Abstract Objectives To provide a narrative review of the main eating disorders (ED), specifically focusing on children and adolescents. This review also aims to help the pediatrician identify, diagnose, and refer children and adolescents affected by this medical condition and inform them about the multidisciplinary treatment applied to these disorders. Data source The research was conducted in the Scientific Electronic Library Online (SciELO), Medical Literature Analysis and Retrieval System Online (Medline) databases via PubMed and Embase. Consolidated Guidelines and Guidebooks in the area were also included in the review to support the discussion of ED treatment in childhood and adolescence. Data synthesis ED are psychiatric condition that usually begins in adolescence or young adulthood but can occur at any time of life, including in childhood, which has been increasingly frequent. Pediatricians are the first professionals to deal with the problem and, therefore, must be well trained in identifying and managing these disorders, which can be severe, and determine physical complications and quality of life of patients and their families. Conclusion ED has shown an increase in prevalence, as well as a reduction in the age of diagnosed patients, requiring adequate detection and referral by pediatricians. The treatment requires a specialized multidisciplinary team and is generally long-lasting for adequate recovery of affected individuals.

ABSTRACT

Abstract Objective To investigate the relationship between the biopsychosocial environment and eating habits and behaviors that lead to the selection and consumption of certain food from the earliest stages of life. To clarify whether there is an interaction between genetic and epigenetic factors, and how they shape eating habits. Data source A narrative review based on research in PubMed and Web of Science electronic databases was carried out over the last 10 years, searching the title and summary fields using the keywords Children OR adolescents Feeding Behavior eating OR Dietary Habits OR Eating Behavior OR Eating Habits OR Children obesity. Data synthesis The generational transmission of eating habits is related to the home, community, and school environments, mainly during the first years of life, and can exert the modulation of habits during all stages of life. During childhood, the family's role in consolidating eating habits is very broad and ranges from choosing foods to prioritizing family meals, including the lifestyle. Conclusions Eating habits are transmitted from parents to children in different ways: environmental, emotional, social, and educational. In cases of obesity, a greater association of genetic influence can be observed.

ABSTRACT

Literature has emphasized on food environments centrality in food choices and nutritional status. Objective: identify social perceptions of fathers, mothers and guardians, professionals from educational establishments, municipal officials, and stallholders belonging to the southern area of Santiago de Chile, regarding limitations of healthy diet implementation on school environments. Method: six focus groups were carried out grouping 50 key actors linked to educational establishments using a non-probabilistic sample. A content analysis was applied through thematic coding technique seeking to reveal emerging semantic universes. Results: Nine barriers, or semantics, were identified characterizing limitations to healthy eating habits in school environments: family, school kiosks, leaving school establishments, taste, socioeconomic determinants, public policy, lack of knowledge, publicity, and availability of products without seals. Conclusions: limitations associated with domestic food environment have a significantly high representation (47,9%) and contain an individualized responsibility feature. This explanatory culture is opposed to scientific and academic evidence regarding the functioning of eating behavior, and the centrality of food environments facilitating or hindering healthy food consumption.

ABSTRACT

ObjectiveTo identify factors related to the occurrence of choking and coughing while eating in the disabled elderly and conduct importance matrix analysis to provide a reference basis for controlling choking on food in the disabled elderly. MethodsA convenience sampling method was used to select 80 disabled elderly individuals in a hospital between October 2019 and April 2022 as the study population. The occurrence of choking and coughing while eating was recorded, and a questionnaire was administered to collect general information. Additionally, assessments were conducted using the intelligent mental status examination scale (MMSE), oral health checklist (BOHSE), eating assessment tool⁃10 (EAT-10), and chewing function evaluations. Univariate and multifactorial analyses were conducted to analyze the influencing factors of choking and coughing while eating in the elderly with disabilities. ResultsThe incidence of choking and coughing while eating was 52.50% (42/80) among the 80 disabled elderly. The degree of disability (OR=2.895, 95%CI: 1.352‒6.201), age (OR=4.040, 95%CI: 1.121‒14.562), BOHSE score (OR=2.473, 95%CI: 1.002‒6.102), EAT-10 score (OR=5.345, 95%CI: 2.112‒13.527), and chewing function score (OR=3.453, 95%CI: 1.247‒9.562) were identified as risk factors for choking and coughing while eating in the disabled elderly. The MMSE score (OR=0.343, 95%CI: 0.135‒0.869) was identified as a protective factor. The importance matrix analysis indicated that EAT-10 score, MMSE score, and chewing function had high importance with slightly lower difficulty in improvement, and were thus listed as items in the priority improvement area. Age and degree of disability had high importance and high difficulty in improvement, and therefore belonged to the suboptimal improvement area. BOHS score was slightly lower both in importance and difficulty of improvement, entering the alternative improvement area. ConclusionAge, degree of disability, BOHSE score, EAT-10 score, and chewing function score are risk factors for the occurrence of choking and coughing while eating, while the MMSE score is a protective factor. The importance matrix analysis can provide a basis for targeted intervention in clinical practice.

ABSTRACT

Abstract Medium chain acyl-coA dehydrogenase deficiency (MCADD), the most common fatty acid oxidation disorder, has been regarded as a relatively benign condition with low risk of mortality in patients with a known diagnosis, if adequate caloric intake is met. However, inadequate energy provision, as occurs in eating disorders, significantly amplifies the risk of metabolic decompensation. This case series describes four patients with MCADD and a concomitant eating disorder and aims to raise awareness of the potentially under-recognised coexistence of these conditions. All patients were female with signs of disordered eating in adolescence and young adulthood though latency in diagnosis was apparent. Three of the patients had low body mass index (BMI) and the other was overweight. Metabolic decompensation and hospitalisation occurred in three of four patients secondary to extreme risk-taking behaviour with caloric restriction. The coexistence of MCADD and eating disorders is of significant concern, placing the patient at substantial risk of decompensation in an otherwise relatively stable metabolic condition. Awareness of disordered eating in this population is paramount, as early recognition of signs and symptoms of eating disorders in the MCADD population may facilitate prompt intervention and avoidance of morbidity and potential mortality.

ABSTRACT

SUMMARY OBJECTIVE: The precise relationship between obesity and eating habits, attitudes, and emotion regulation is still ambiguous. The purpose of this study was to investigate possible correlations among body mass index, challenges related to managing emotions, and attitudes toward eating among adult participants with known psychiatric diagnoses. METHODS: The body mass indices of participants were calculated, and data on eating styles were collected using the Dutch Eating Behavior Questionnaire. The level of difficulty in managing emotions was evaluated using the Difficulties in Emotion Regulation Scale. RESULTS: The research findings indicated a meaningful positive association. An observation was made between body mass index and results from the Eating Attitude Test-40, as well as the restrained eating subdimension of the Dutch Eating Behavior Questionnaire. Conversely, a meaningful reverse relationship was identified between the scores of the "strategies" subdimension of the Difficulties in Emotion Regulation Scale. No meaningful differences in eating attitudes and emotion regulation were found between non-obese and obese patients. CONCLUSION: While a partial and meaningful correlation was observed among body mass index, eating attitudes, and emotion regulation difficulties, it is suggested that factors such as patients' age, disease duration, current body mass index, and the simultaneous presence of depression and anxiety should be considered.

ABSTRACT

Abstract Objectives: to assess the association between pregnant women's consumption of ultra-processed foods and newborn body weight. Methods: prospective study with pregnant women (n=214) selected from all Basic Health Units in the city of Pinhais, Paraná. Socioeconomic, demographic, and health data were collected. Food consumption data were assessed using a 24-hour dietary recall and tabulated with GloboDiet software. Daily relative energy intake of ultra-processed food was estimated and logistic regression analysis was utilized. The infuence of covariates on the association analysis was also explored (e.g., income and education). Results: ultra-processed foods contributed to 26.9% of pregnant women's total energy intake. About 5.7% of newborns were classifed as small-for-gestational-age and 10.7% as large-for-gestational-age. A borderline statistically signifcant association was observed between large-for-gestational-age newborn weight and maternal consumption of ultra-processed foods (OR= 1.027; p=0.048). Additionally, family income was associated with the consumption of ultra-processed foods (OR= 0.144; p=0.008). With each additional 1% consumption of ultra-processed foods, mothers' likelihood of having large-for-gestational-age babies increased by about 2.7%. Conclusions: the study reveals a trend of positive association between the weight of large-for-gestational-age newborns and the consumption of ultra-processed foods by pregnant women, but not for small-for-gestational-age children.

ABSTRACT

ABSTRACT Objective Evaluate the food consumption of the participants of the Cohort of Universities of Minas Gerais, according to the degree of processing, and its relationship with socioeconomic, behavioral, and individual factors. Methods A total of 4,124 individuals from the baseline of the Cohort of Universities of Minas Gerais (2016 and 2018) participated in this study. Food consumption was self-reported by completing an online Food Frequency Questionnaire. The foods were divided into 3 groups: Group 1, in natura, minimally processed foods, culinary ingredients and culinary preparations; Group 2, processed foods; Group 3, ultra-processed foods. A hierarchical multiple linear regression model was used to verify the associated factors. Results Regarding the factors associated with food consumption, it is noteworthy that Group 1 was positively associated with the practice of physical activity, female gender, age, "non-white" skin color, and the presence of diabetes Mellitus; and negatively with "not married/without stable union" marital status, alcohol abuse, tobacco use, obesity, and depression. Considering Group 2, it was positively associated with alcohol abuse, tobacco use, and age; and negatively with physical activity, female gender, and "non-white" skin color. As for Group 3 it was positively associated with a marital status of "not married/without stable union", obesity, and depression; and negatively with physical activity, age, "non-white" skin color, and presence of diabetes Mellitus. Conclusion The factors that are in at least one of the final hierarchical linear regression models stand out: marital status, physical activity, alcohol abuse, tobacco use, sex, age, skin color, obesity, diabetes mellitus, and depression.

ABSTRACT

ABSTRACT Objective To assess dietary intake and weight status changes among Brazilian adults. Methods In this dietary survey, data from the food consumption modules of the 2008-2009 (n=21,003 adults) and the 2017-2018 (n=28,153 adults) Household Budget Survey were evaluated to estimate the mean consumption (g/day) of 20 food groups. The body mass index was calculated to classify the weight status of adults and estimate the prevalence of overweight and obesity. Differences between surveys were identified when the 95% confidence intervals were not interspersed. All analyses were stratified by gender and considered the sample weight and the complexity of the sample design. Results The prevalence of overweight increased both among men (38.4%; 95% CI: 36.8-40.0, in 2008-2009 vs. 42.2%; 95% CI: 40.9-43.5, in 2017-2018) and women (29.5%; 95% CI: 58.0-30.9 vs. 35.2%; 95% CI: 34.0-36.4, respectively). Mean consumption of poultry and eggs (57.6g/day vs. 77.9 g/day in men and 43.5g/day vs. 57.3g/day in women, p<0.05) and fast foods (31.3g/day vs. and 48.7g/day in men and 25.3g/day in 2008-2009 vs. 34.8g/day in women, p<0.05) increased between the two surveys, while the mean consumption of rice, beans, fruits, coffee and tea, fish and seafood, processed meats, milk and dairy products, sweets and desserts, sugary drinks, and soups declined. Conclusion The Brazilian food consumption pattern follows the increased prevalence of overweight and reinforces the need to encourage healthy patterns that revive our country's food culture and eating habits.

ABSTRACT

ABSTRACT Objective To investigate the anthropometric variables and body composition of children and adolescents with cerebral palsy based on the type of enteral diet received. Methods A case-series study involving 38 individuals with spastic quadriparetic cerebral palsy, aged four to 18 years, fed only by the enteral route, followed up at a Reference Hospital in the city of Recife (PE), Brazil. One group received an exclusively industrialized enteral diet, while the other received a mixed diet (industrialized and homemade). Weight, stature, arm circumference, and arm muscle area were measured. Body composition was assessed using bioelectrical impedance analysis. Results There was no significant difference between the groups receiving industrialized and mixed diets, with a high stature deficit frequency (63.6% versus 68.7%; p=0.743), excess fat mass (93.3% versus 58.3%; p=0.060), and fat free mass deficit (73.3% versus 66.7%; p=1.000) observed in both groups. Regarding the nutritional composition of the enteral diet, 54.5% and 53.8% of the individuals in the industrialized and mixed diet groups, respectively, received an industrialized enteral diet with a hypercaloric, hyperlipidic, and hypoproteic nutritional composition. Conclusion It was concluded that there was no difference in anthropometric parameters and body composition based on the type of diet received. It should be considered that the nutritional composition of the most commonly used industrialized diet among the individuals in this study may have influenced the unfavorable outcomes, such as the high frequency of low muscle mass and excess fat mass. This highlights the need for the formulation and availability of an enteral diet that meets the nutritional needs of this population.

ABSTRACT

Summary: Objective: to learn the relation between the food consumption profile (frequency and type) and the prevalence, extension and distribution of cavity lesions in children of 18-35 months of age from the city of Montevideo, Uruguay. Material y Methods: a cross-sectional study was carried out in 50 children, of both sexes. Parents answered a questionnaire regarding sociodemographic data, eating habits and oral hygiene. Two examiners recorded caries lesions on each tooth surface according to ICDAS-epi criteria, visible plaque index and PUFA index. Descriptive analysis of the variables were performed. The diet analysis considered food type, incorporation time and frequency through percentages. Results: children examined had an average age of 26.6 ± 5.9 months of age. The cavity prevalence was 34%, with an extension of 2.3 and 4.3 for ceo-d and ceo-s respectively. For the most part, severe untreated injuries were observed located in the anterior superior and posterior-inferior sector. The PUFA index was 8%. 30% consumed breast milk, 54% milk with sugar, 100% consumed beverages and sugary foods, mostly before 11 months and daily. Children who consumed sugary drinks (non-milk) in a bottle presented greater extent and severity of cavity lesions (ceo-d modificated= 3.5 and ceo-s modificated = 7.8). Conclusions: we need to deepen our knowledge of feeding patterns in early childhood. The population studied showed the early incorporation of sugar into the diet, warning about the presence of a common risk factor for various non-communicable diseases (cavities, diabetes, obesity, cardiovascular diseases).

ABSTRACT

Introduction: The Covid-19 pandemic led to various consequences for people's mental and physical health. The context of social isolation exacerbated problems related to body image, especially in individuals with eating disorders. Objective: To analyze the repercussions of Covid-19 on anthropometric variables regarding weight perception and body satisfaction, comparing women with eating disorders to those without this condition. Methods: Observational, descriptive, comparative, and cross-sectional study. Women with eating disorders in treatment (EDG) were recruited from five services in the South and Southeast regions of Brazil. The counterpart, the control group (CG), consisted of women without eating disorders recruited online and with a negative score on the EAT-26 test. Online questionnaires were applied between June 2020 and January 2021. Statistical analyses included Pearson's Chi-square or Fisher's Exact test and logistic regressions, performed using the SPSS 23.0 program. Results: Participants were 174 young women (31.94±9.35 years of age), with higher education (71.3%) and with a partner (61.5%), who were in social isolation. The EDG consisted of 58 women, with116 in the CG. The majority (55.2%) of the EDG noticed an increase in body weight while being dissatisfied with body image (81.0%), a statistically different result from the CG. The association between women with eating disorders and low body satisfaction was significant (p=.0010). Those in social isolation and overweight or obese were more likely to perceive weight gain. Conclusion: The Covid-19 pandemic intensified body dissatisfaction, especially among women with eating disorders. Those in isolation who were overweight or obese perceived more weight gain, indicating the need for intervention strategies for these groups.
