ABSTRACT

Objective:To evaluate the effect of Mongolian medicine Eerdun Wurile on postoperative cognitive function in elderly patients undergoing thoracoscopic lobectomy.Methods:Sixty elderly patients of either gender, aged ≥60 yr, of American Society of Anesthesiologists Physical Status classification Ⅱ or Ⅲ, with body mass index≤28 kg/m 2, with preoperative Mini-Mental State Examination (MMSE) score ≥27 points, undergoing thoracoscopic lobectomy under general anesthesia, were divided into 2 groups ( n=30 each) using the random number table method: control group (group C) and Eerdun Wurile group (group E). Eerdun Wurile 15 capsules per day were taken for 7 consecutive days starting from 3 days before surgery in group E, and placebo was given instead in group C. The peripheral venous blood samples were collected before medication at 3 days before surgery (T 1) and 24 and 72 h after surgery (T 2, 3) for determination of the concentrations of serum brain-derived neurotrophic factor (BDNF), interleukin-1beta (IL-1β), tumor necrosis factor-alpha (TNF-α) and Tau proteins by enzyme-linked immunosorbent assay. The patients′ cognitive function was assessed by MMSE at T 1 and 5 days after surgery (T 4), and the occurrence of cognitive decline (MMSE score < 27) was recorded. Results:Compared with group C, the serum BDNF concentration was significantly increased at T 2 and T 3, and the concentrations of IL-1β, TNF-α and Tau protein were decreased, the MMSE score was increased at T 4, and the incidence of cognitive decline was decreased in group E ( P<0.05). Conclusions:Mongolian medicine Eerdun Wurile can improve postoperative cognitive function in elderly patients undergoing thoracoscopic surgery, and the mechanism may be related to reduction of systemic inflammatory responses and promotion of nerve cell repair and regeneration.

Objective@#To summarize the clinical application of Zhenbao pills (Eerdun-Wurile) in recent years, so as to clarify its clinical efficiency and provide an effective basis for future research.@*Methods@#The clinical application of Zhenbao pills (Eerdun-Wurile) was classified and summarized by consulting the relevant literature.@*Results@#Mongolian medicine Eerdun-Wurile consists of many active ingredients such as <i>Sandalwood Padauk</i>, pearl, <i>Myristica fragrans, Calculus bovis</i>, Bufallo horn concentrated powder, <i>Gardenia jasminoides Ellis, Glycyrrhiza uralensis Fisch, Euphorbia humifusa, Catsia tora Linn, Chingma Abutilon Seed</i>, Resina Liquidambaris, <i>Carthamus tinctorius L, Eugenia caryophyllata Thunb, Cardamoj amomum, Semen Nigellae, Cuminum cyminum L, Piper longum Linn, Lygodium japonicum</i>, crab, medicine terminalia fruit, <i>MeLia toosendan Sieb.etZucc</i>, musk, white sandalwood, <i>Cinnamomum cassia Presl, Aucklandia lappa Decne, Inula helenium L, Amomum tsao-ko Crevostet Lemaire etc</i>. Mongolian medicine Eerdun-Wurile used for treat cardiovascular and cerebrovascular diseases, cerebral infarction, cerebral infarction and ischemic stroke, rheumatic heart disease, coronary heart disease, angina pectoris, rheumatism, kind rheumatism, cervical spondylosis, arthritis, insomnia, high blood pressure, epilepsy, skin diseases, diabetes, eye diseases.@*Conclusion@#Mongolian Medicine Eerdun-Wurile has remarkable curative effect on cardiovascu- lar and cerebrovascular diseases, brain infarction, cerebral infarction, ischemic stroke, diabetic diseases, rheumatism, bone diseases and so on. In this paper, the curative effect of this drug was summarized to provide reference for its clinical application.

ABSTRACT

The present study aimed to explore the effects of Eerdun-Wurile on brain-derived neurotrophic factor (BDNF) and nerve growth factor (NGF) expressions in the prefrontal cortex of MCAO / R injury rats.Sixty SD male rats of SPF grade were selected to establish the model of MCAO / R with Zea-Longa thread occlusion,and divided into five groups at random:the sham operation group,the model group,the nimodipine group and the Eerdun-Wurile group.After modeling,rats were anesthetized for preparing the brains.The pathomorphological changes of the brains were evaluated by immunohistochemical techniques,such as HE staining and SP.The protein and mRNA expressions of BDNF and NGF in the prefrontal cortex of rats were detected by ELISA and RT-PCR,respectively.As a result,compared with the model group,it was found that the number of necrotic cells in the prefrontal cortex were significantly reduced in the Eerdun-Wurile group (P < 0.05),while the mRNA and protein expressions of BDNF and NGF were significantly increased (P < 0.05).In conclusion,the BDNF and NGF expressions in the prefrontal cortex were up-regulated for stimulating the activation of astrocytes and protecting the neurons with the treatment of Eerdun-Wurile in MCAO / R injured rats,which may be the mechanism of the treatment of Eerdun-Wurile for white vein disease.
