ABSTRACT

Objective To compare the clinical value of XELOX and FOLFOX6 in the treatment of elderly patients with colorectal cancer. Methods Clinical data of 68 elderly patients(aged>60 years)with colorectal cancer were retrospectively analyzed. The patients were divided into XELOX group(n =32)and FOLFOX6 group(n =36) according to the chemotherapy regimen. The response evaluation criteria in solid tumors, progression free survival, 2 -year mortality, chemotherapy related adverse reactions and health related quality of life in the two groups were compared. Results Compared with the FOLFOX6 group, the neurotoxicity incidence in the XELOX group was signifi-cantly lower (3.13% vs.22.22%,χ2 =5.380,P =0.020).The neutrophils reduced incidence was significantly lower(9.38% vs.30.56%,χ2 =4.649,P =0.039).The hand foot syndrome complications significantly increased (34.38% vs.8.33%,χ2 =7.027,P =0.008).There was no significant difference in the incidence of nausea,vomi-ting,diarrhea,oral mucosal ulcer, thrombocytopenia and elevation of transaminase in the two groups(χ2 =0.001, 0.000,0.008,2.102,1.071,1.851,all P >0.05).The quality of health related quality of life was significantly higher in the XELOX group than in the FOLFOX6 group at 6 months after the operation[(67.94 ±12.49)points vs. (57.39 ±13.59)points,t =3.198,P =0.015].There were no significant differences in the curative effect grade at 6 months after the operation,2 -year mortality rate and progression free survival time between the two groups(χ2 =0.949,0.008,t =0.573,all P >0.05).Conclusion Although XELOX increases the incidence of hand foot syn-drome in elderly patients with colorectal cancer, but it significantly reduces the incidence of neurotoxicity and neutro-phil,and improves the quality of life of patients after surgery.

ABSTRACT

Objective To observe the efficacy and untoward reactions of FOLFOX4 regimen and XELOX regimen in the treatment of patients with advanced colorectal cancer. Methods Fifty four advanced colorectal cancer patients were randomly divided into two groups.Group A (FOLFOX4):28 patients were treated with oxaliplatin 85 mg/m2 on day 1,LV 200 mg/m2 as a 2h infusion followed by bolus 5 -FU 400 mg/m2 and a 24h infusion of 5 -Fu 600 mg/m2 ,repeated for consecutive days every 2 weeks. One cycle was 14 days and efficacy was evaluated after 4 cycles.Group B(XELOX):26 patients were treated with oxaliplation 130mg/m2 on day 1,capecitabine 1 000 mg/m2 orally daily for 14 days. One cycle was 21 or 28 days and efficacy was evaluated after 2 cycles.Efficacy of the two groups was evaluated at 2 cycles. Results All 54 patients were available for evaluating objective response. In A group, the overall response rate was 46.4%,and the overall response rate was 42.3% in B group. The overall response rate of A group was slightly higher than B group, but there was no statistically significant difference(χ2 =0.093,P =0.791 ).Adverse reactions:thrombocytopenia incidence rate and the peripheral nerve toxicity had no significant differences between the two groups(χ2 =0.552,0.108,P =0.550,0.787).The incidence rates of nausea, vomit, diarrhea and leucocytopenia in B group were lower than those in A group(χ2 =9.495,4.862,6.273,P =0.003,0.033,0.108).The incidence rate of hand -foot syndrome was higher in B group than in A group(χ2 =13.389,P =0.000),but the severity was less(grade Ⅰ ~Ⅱ).Conclusion The response rate in the treatment of advanced colorectal cancer was similar between XELOX and FOLFOX4 regiments, but XELOX regiment has the advantages of more convenience, better safety. Thus,XELOX regiment is worth recommending as a first -line therapy for the treatment of patients with advanced colorectal cancer.

ABSTRACT

Objective To explore the clinical therapeutic effects of postoperative intracranial infection in neurosurgery. Methods Between January 2014 and January 2014,50 patients with postoperative intracranial infection in the department of neurosurgery in our hospital were selected in this study.Among the 50 patients,25 patients were treated by intravenous antibiotic therapy,15 patients were treated with intrathecal and intravenous antibiotics treatment,10 cases were treated by intrathecal injection, intravenous antibiotics and lumbar pool catheter drainage way for treatment, and then we observed the clinical therapeutic effect of three groups of patients. Results In 50 patients,37 patients were cured,3 cases had marked effect,8 patients had effect,2 patients had no effect. Conclusion For patients with neurosurgical operations,to maximally reduce the risk of intracranial infection,it is necessary to take antibiotics according to the actual situation of patients, and make different treatment schemes according to the difference of infection.

ABSTRACT

Objective To research the position transition training effect of general practitioner of northeast district of Chongqing, and find out the problems and their reasons. Methods 326 students and 150 unit leaders were chosen to make a questionnaire survey(recycled 307,150).The Kirkpatrick model was also used for data analysis from 4 levels. The result was described in percentage. Results Reaction Level:42.02%(129/307) of the trainees were not satisfied with the time arrangement of clinical medicine training; Study Level:3 courses were at high failure rate 19.54%(60/307),22.8%(70/307),17.92%(55/307); Behavior Level:81.43%(250/307)of the trainees considered the training effective; Result Level:94.00%(141/150)unit leaders considered the training had a positive impact. Conclusion The time arrangement of clinical medicine training is mostly unsatisfied and the ability of instrument using and result interpretation is weak. Most people consider the position transition training is effective.
