ABSTRACT

ABSTRACT Objective: To analyze functional aspects of breastfeeding, self-efficacy, and pain reported by mothers during breastfeeding, in newborns with severe and mild ankyloglossia. Methods: This is an observational study, carried out with 81 babies with ankyloglossia, assessed by the Bristol Tongue Assessment Tool (severe: scores 0-3; mild: scores 4-6) nested in a cohort carried out at the University Hospital of the Federal University of Maranhão, São Luis, Brazil. The functional aspects of breastfeeding were analyzed using the Breastfeeding Observation Form of the United Nations Children's Fund (BOF-UNICEF) and the LATCH Scoring System. Breastfeeding self-efficacy was measured using the Breastfeeding Self-Efficacy Scale — Short-Form. Pain indicators were evaluated by the Short-Form McGill Pain Questionnaire. The significance level adopted was 5%. Results: Maternal age was 26.7±0.8 years, and 64.2% reported high school education. Most babies were male (67.9%), and the birth weight was 3232±60g. A significant association was detected in the sucking aspect evaluated by the BOF-UNICEF [β=0.22 (95%CI 0.07; 0.73), p-value=0.013]. However, the groups did not differ in the assessment of breastfeeding performed by the LATCH scale. The groups had no differences in the assessment of breastfeeding self-efficacy reported by mothers, and in pain scores. Conclusions: Despite the observation of sucking difficulty in infants with severe ankyloglossia., the quality of breastfeeding in general, maternal pain, and self-efficacy reported by mothers do not differ when compared with infants with mild ankyloglossia. Therefore, the severity of ankyloglossia seems not to affect the breastfeeding indicators.


RESUMO Objetivo: Analisar aspectos funcionais da amamentação, autoeficácia e dor relatada pelas mães durante a amamentação, em recém-nascidos com anquiloglossia grave e leve. Métodos: Estudo observacional, realizado com 81 bebês diagnosticados com anquiloglossia avaliados pelo Bristol Tongue Assessment Tool (grave: escores 0-3; leve: escores: 4-6), aninhados em um estudo de coorte realizado no Hospital Universitário da Universidade Federal do Maranhão, São Luís, Brasil. Os aspectos funcionais do aleitamento materno foram analisados por meio do Formulário de Observação da Amamentação do Fundo Internacional para a Infância das Nações Unidas (BOF-UNICEF) e da Escala LATCH. Aautoeficácia foi mensurada por meio da Breastfeeding Self-Efficacy Scale — Short-Form. Os indicadores de dor foram avaliados pelo Short-Form McGill Pain Questionnaire. O nível de significância adotado foi de 5%. Resultados: A idade materna foi 26,7±0,8 anos, e 64,2% concluíram o ensino médio. A maioria dos recém-nascidos era do sexo masculino (67,9%), e o peso ao nascer foi 3232±60g. Foi detectada associação significante no aspecto da sucção avaliado pelo BOF-UNICEF [β=0,22 (IC95% 0,07; 0,73), p=0,013]. No entanto, os grupos não diferiram na avaliação do aleitamento materno realizada pela escala LATCH. Os grupos não apresentaram diferenças na avaliação da autoeficácia em amamentar relatada pelas mães e no escore de dor. Conclusões: Apesar de ter sido observada dificuldade de sucção em lactentes com anquiloglossia grave, a qualidade da amamentação em geral, a dor materna e a autoeficácia relatada pelas mães não diferem quando comparadas com as de lactentes com anquiloglossia leve. Assim, a gravidade da anquiloglossia parece não afetar os indicadores de amamentação.

ABSTRACT Objective: To analyze the validity evidence of the Brazilian version of the Cancer Behavior Inventory - Brief Version. Method: Methodological study, conducted between November and December 2021, with 140 patients undergoing hospital cancer treatment in João Pessoa, Paraíba, Brazil. Psychometric analyses were performed in the adapted version, using exploratory factor analysis and correlation with correlated constructs. Results: A two-factor and 10-item model was evidenced. The cumulative variance explained about 61% the shared variance of the items. Satisfactory values were observed for the factors in the analyses of composite reliability (0.89 and 0.91, respectively), internal consistency (0.86 and 0.91, respectively) and ORION (0.89 and 0.85, respectively). The expected correlations of self-efficacy with quality of life (convergent) and with anxiety and depression (divergent) were evident. Conclusion: The Brazilian version of the instrument showed evidence of validity, being considered reliable to assess the self-efficacy of patients undergoing cancer treatment.


RESUMEN Objetivo: Analizar las evidencias de validez de la versión brasileña del Cancer Behavior Inventory - Brief Version. Método: Estudio metodológico, realizado entre noviembre y diciembre de 2021, con 140 pacientes en tratamiento oncológico hospitalario en João Pessoa, Paraíba, Brasil. Se realizaron análisis psicométricos en la versión adaptada, mediante análisis factorial exploratorio y correlación con constructos correlacionados. Resultados: Se evidenció un modelo de 2 factores y 10 ítems. La varianza acumulada explicó alrededor del 61% de la varianza compartida de los ítems. Se observaron valores satisfactorios para los factores en el análisis de confiabilidad compuesta (0,89 y 0,91, respectivamente), consistencia interna (0,86 y 0,91, respectivamente) y ORION (0,89 y 0,85, respectivamente). Se evidenciaron las correlaciones esperadas de la autoeficacia con la calidad de vida (convergente) y con la ansiedad y la depresión (divergente). Conclusión: La versión brasileña del instrumento mostró evidencias de validez, siendo considerado confiable para evaluar la autoeficacia de pacientes en tratamiento oncológico.

Conclusão: A versão brasileira do instrumento mostrou evidências de validade, sendo considerada como confiável para avaliar a autoeficácia dos pacientes em tratamento oncológico.

ABSTRACT Objective: To analyze general self-efficacy beliefs in university students during the COVID-19 pandemic and their correlation with psychological well-being and anxiety and depressive symptoms. Method: Cross-sectional study, carried out with nursing, medicine and psychology students from a higher education institution in the state of São Paulo, Brazil. The sample consisted of 329 students and data collection took place through a questionnaire and scales, from August to December 2020. Mann-Whitney test and Spearman's correlation coefficient were used for analysis of the variables. Results: Participants' self-efficacy was median (34.3±7.5). Higher self-efficacy scores were correlated with better psychological well-being (p<0.001; r= -0.582) and absence of anxiety (p<0.001) and depressive (p<0.001) symptoms. Conclusion: High self-efficacy beliefs were associated with better mental health outcomes. Strengthening self-efficacy in universities can help improve students' health behaviors and prevent mental illness.


RESUMEN Objetivo: Analizar las creencias generales de autoeficacia en estudiantes universitarios durante la pandemia de COVID-19 y su correlación con el bienestar psicológico y la sintomatología ansiosa y depresiva. Método: Estudio transversal, realizado con estudiantes de enfermería, medicina y psicología de una institución de enseñanza superior del estado de São Paulo, Brasil. La muestra estuvo conformada por 329 estudiantes y la recolección de datos se realizó a través de cuestionario y escalas, de agosto a diciembre de 2020. Para el análisis de las variables se utilizó la prueba de Mann-Whitney y el coeficiente de correlación de Spearman. Resultados: La autoeficacia de los participantes fue mediana (34,3±7,5). Las puntuaciones más altas de autoeficacia se correlacionaron con un mejor bienestar psicológico (p<0,001; r= -0,582) y ausencia de síntomas de ansiedad (p<0,001) y depresivos (p<0,001). Conclusión: Las creencias de alta autoeficacia se asociaron con mejores resultados de salud mental. Fortalecer la autoeficacia en las universidades puede ayudar a mejorar los comportamientos de salud de los estudiantes y prevenir enfermedades mentales.


RESUMO Objetivo: Analisar as crenças de autoeficácia geral em universitários, durante a pandemia da covid-19 e sua correlação com bem-estar psicológico e sintomas ansiosos e depressivos. Método: Estudo transversal, desenvolvido com estudantes de enfermagem, medicina e psicologia de instituição de ensino superior do estado de São Paulo, Brasil. A amostra foi de 329 estudantes e a coleta de dados ocorreu por meio de questionário e escalas, de agosto a dezembro de 2020.Utilizaram-se o teste Mann-Whitney e coeficiente de correlação de Spearman para análise das variáveis. Resultados: A autoeficácia dos participantes foi mediana (34,3±7,5). Maiores escores de autoeficácia foram correlacionados a melhor estado de bem-estar psicológico (p<0,001; r= -0,582) e ausência de sintomas ansiosos (p<0,001) e depressivos (p<0,001). Conclusão: Crenças elevadas de autoeficácia foram associadas a melhores desfechos em saúde mental. O fortalecimento da autoeficácia nas universidades pode ajudar a melhorar os comportamentos de saúde dos estudantes e prevenir doenças mentais.

Resumo Objetivo Avaliar a eficácia antimicrobiana de um dispositivo fixo emissor de luz UV-C na desinfecção de diferentes superfícies do ambiente hospitalar e sua eficácia antifúngica na qualidade do ar. Métodos Estudo quase-experimental realizado em uma unidade de internação hospitalar, que utilizou o Bioamostrador de ar Andersen® de seis estágios para análise do ar; e na avaliação das superfícies, utilizaram-se três suspensões de microrganismos (Acinetobacter sp. MDR, Escherichia coli e Klebsiella pneumoniae produtora de KPC) para contaminar o ambiente. Para ambos foram feitas coletas pré (controle) e pós-acionamento da luz UV-C (teste). Resultados Na avaliação do ar houve uma redução importante da contagem de colônias após a luz UV-C e não foram encontrados fungos patogênicos ou toxigênicos em nenhum dos dois momentos. Em relação à desinfecção das superfícies, nenhum crescimento bacteriano foi observado após a intervenção da luz, demonstrando 100% de inativação bacteriana nas condições testadas. Conclusão A utilização da tecnologia com emissão de luz UV-C fixa foi eficaz e pode ser considerada uma intervenção promissora para protocolos de desinfecção de superfícies hospitalares.


Resumen Objetivo Evaluar la eficacia antimicrobiana de un dispositivo fijo emisor de luz UV-C para la desinfección de diferentes superficies del ambiente hospitalario y su eficacia antifúngica en la calidad del aire. Métodos Estudio cuasi experimental realizado en una unidad de internación hospitalaria, en que se utilizó el biomuestreador de aire Andersen® de seis etapas para el análisis del aire. En el análisis de las superficies, se utilizaron tres suspensiones de microorganismos (Acinetobacter sp. MDR, Escherichia coli y Klebsiella pneumoniae productora de KPC) para contaminar el ambiente. En ambos se tomó una muestra antes (control) y después de accionar la luz UV-C (prueba). Resultados En el análisis del aire hubo una reducción importante del recuento de colonias después de la luz UV-C y no se encontraron hongos patógenos ni toxigénicos en ninguno de los dos momentos. Con relación a la desinfección de las superficies, no se observó ningún crecimiento bacteriano después de la intervención de la luz, lo que demuestra un 100 % de inactivación bacteriana en las condiciones analizadas. Conclusión El uso de la tecnología con emisión de luz UV-C fija fue eficaz y puede ser considerada una intervención prometedora para protocolos de desinfección de superficies hospitalarias.


Abstract Objective To evaluate a fixed UV-C light emitting device for its antimicrobial effectiveness in the disinfection of distinct surfaces and its antifungal effectiveness on air quality in the hospital environment. Methods This quasi-experimental study was conducted in a hospital inpatient unit, in which a six-stage air Biosampler (Andersen®) was used for air analysis. In the evaluation of surfaces, three suspensions of microorganisms (Acinetobacter sp. multidrug-resistant, Escherichia coli, and KPC-producing Klebsiella pneumoniae) were used to contaminate the environment. In both evaluations, pre- (control) and post-activation of UV-C light (test) collections were made. Results In the air evaluation, an important reduction was observed in the colony count after irradiation with UV-C light, and pathogenic or toxigenic fungi were not found in either of the two moments. Regarding the disinfection of surfaces, no bacterial growth was observed after the application of UV-C light, showing 100% bacterial inactivation under the tested conditions. Conclusion The use of fixed UV-C light emission technology was effective and can be considered a promising intervention for hospital surface disinfection protocols.

ABSTRACT Objective: To assess the evidence regarding the effectiveness of interventions aimed at strengthening self-efficacy beliefs in college students. Methods: Integrative Review conducted on the Lilacs, PubMed, CinahL, Cochrane Collaboration Databases, Scopus, and PsycInfo databases. The methodological quality of the studies was assessed using tools proposed by the Joanna Briggs Institute, and the results were analyzed descriptively. Results: Out of the 10 selected studies, six demonstrated that interventions aimed at strengthening self-efficacy were effective (Levels of Evidence II and III), and four revealed contrary results (Levels of Evidence I and II). Programs aimed at enhancing self-efficacy should include content on positive mental health, psychoeducation strategies, cover a period of eight to twelve weeks, and consider the completion of homework assignments. Conclusion: The synthesis of evidence pointed to pathways for building an effective self-efficacy strengthening program to be implemented in universities.


RESUMEN Objetivo: Evaluar las evidencias sobre la efectividad de las intervenciones para fortalecer las creencias de autoeficacia en estudiantes universitarios. Métodos: Revisión Integrativa realizada en las bases de datos Lilacs, PubMed, CinahL, Cochrane Collaboration Databases, Scopus y PsycInfo. La calidad metodológica de los estudios se evaluó utilizando herramientas propuestas por el Instituto Joanna Briggs y los resultados se analizaron de forma descriptiva. Resultados: De los 10 estudios seleccionados, seis demostraron que las intervenciones para fortalecer la autoeficacia fueron efectivas (niveles de evidencia II y III) y cuatro arrojaron resultados contrarios (niveles de evidencia I y II). Los programas dirigidos al fortalecimiento de la autoeficacia deben incluir contenido sobre salud mental positiva, estrategias de psicoeducación, abarcar un período de ocho a doce semanas y considerar la realización de ejercicios para hacer en casa. Conclusión: La síntesis de evidencia señala el camino para la construcción de un programa efectivo de fortalecimiento de las creencias de autoeficacia que debe implementarse en las universidades.

ABSTRACT

Objetivo: verificar a associação entre o uso da bolsa canguru, desde a maternidade até o domicílio, e aleitamento materno exclusivo, entre o 1o e 2o mês de idade, em bebês a termo saudáveis; identificar o nível de autoeficácia para o aleitamento materno, das mães que utilizaram a bolsa canguru. Métodos: estudo transversal com mães que receberam uma bolsa canguru após o parto, na maternidade, e foram orientadas sobre manterem seus bebês nesta postura, pelo menos uma hora por dia. Entre um e dois meses após o nascimento, foram investigadas a frequência do uso da bolsa canguru, autoeficácia materna da amamentação e tipo de alimentação dos bebês. Para análise foi empregado o teste ꭓ2 ou Exato de Fisher e o coeficiente V de Cramer (p < 0,05). Resultados: participaram 98 mães. Todas utilizaram a bolsa canguru no alojamento conjunto, 39,8% utilizaram três ou mais vezes por semana, no domicílio entre o primeiro e segundo mês após o parto; 70,4% dos bebês permaneceram em Aleitamento Materno Exclusivo (AME), o nível da autoeficácia materna para amamentação foi alto. AME esteve associado ao uso da bolsa canguru no domicílio (p = 0,014). Conclusão: bebês a termo saudáveis podem se beneficiar do uso da bolsa canguru desde a maternidade, estendido até o domicílio.


Objectives: to verify the association between kangaroo slings used from maternity hospital to home, and exclusive breastfeeding among healthy full-term babies between the 1st and 2nd months of age; to identify breastfeeding self-efficacy level of those mothers using a kangaroo sling. Method: this cross-sectional study addressed mothers who received a kangaroo sling after giving birth, at maternity hospital, and were instructed to keep the babies in this posture at least one hour daily. The frequency with which the mothers used the kangaroo sling, breastfeeding self-efficacy, and the type of feeding were investigated between one and two months after delivery. ꭓ2 or Fisher's exact test and Cramer's V coefficient (p < 0.05) were adopted in the analysis. Results: ninety-eight mothers participated in the study. All of them used the kangaroo sling in rooming-in; 39.8% used it three or more times a week at home between the 1st and 2nd months after giving birth; 70.4% of the babies remained on Exclusive Breastfeeding (EBF), the level of breastfeeding self-efficacy was high, and EBF was associated with the use of a kangaroo sling at home (p = 0.014). Conclusion: healthy full-term babies can benefit from using a kangaroo sling, starting at the maternity hospital and extending its use to home.


Objetivo: verificar la asociación entre el uso de la bolsa canguro, desde la maternidad hasta el domicilio, y amamantamiento materno exclusivo, entre el 1o e 2o mes de edad, en bebés a término saludables; identificar el nivel de autoeficacia para el amamantamiento materno, de las madres que utilizaron la bolsa canguro. Métodos: estudio transversal con madres que recibieron una bolsa canguro después del parto, en la maternidad, y fueron orientadas sobre mantener sus bebés en esta postura, por lo menos una hora por día. Entre uno y dos meses después del nacimiento, fueron investigadas la frecuencia del uso de la bolsa canguro, autoeficacia materna del amamantamiento y tipo de alimentación de los bebés. Para el análisis fue empleado el test ꭓ2 o Exacto de Fisher y el coeficiente V de Cramer (p < 0,05). Resultados: participaron 98 madres. Todas utilizaron la bolsa canguro en alojamiento conjunto, 39,8% utilizaron tres o más veces por semana, en el domicilio entre el primero y segundo mes después el parto; 70,4%, de los bebés, permaneció en Lactancia Materno Exclusivo (LME); el nivel de la autoeficacia materna para amamantamiento fue alto. El LME estuvo asociado al uso de la bolsa canguro en el domicilio (p = 0,014). Conclusión: bebés a término saludables pueden beneficiarse del uso de la bolsa canguro desde la maternidad, extendido hasta el domicilio.

ABSTRACT

Las enfermedades no transmisibles (ENT) son una creciente preocupación global que afecta tanto a la atención médica como a la calidad de vida. Los programas de automanejo de enfermedades crónicas como el denominado "Tomando Control de su Salud" (TCS), se han vuelto estrategias efectivas para abordar este problema. OBJETIVO: Evaluar el resultado del programa TCS, versión en línea, en las variables automanejo y autoeficacia, en muestras de personas con ENT de México y Perú, durante la pandemia por COVID-19. METODOLOGÍA: El diseño del estudio fue cuasiexperimental con mediciones independientes (intervenidos y controles) y mediciones en línea, al inicio y término de la intervención; evaluadas con Partners in Health Scale y la Escala de Autoeficacia. Los cambios en el tiempo se examinaron mediante la prueba de Wilcoxon. RESULTADOS: De los 114 participantes, la mayoría eran mujeres (83.3%) con una edad promedio de 58.8 años. Antes de la intervención, los grupos fueron iguales en las variables de estudio. No se encontró ningún aumento significativo en las variables de estudio en el grupo control, sin embargo, hubo un aumento estadísticamente significativo en la autoeficacia y automanejo en el grupo intervenido. CONCLUSIONES: El programa TCS en línea contribuyó al aumento de la autoeficacia, el automanejo, el conocimiento de la enfermedad, la adherencia al tratamiento y el manejo de síntomas en participantes con ENT de México y Perú durante la pandemia de COVID-19. Esto respalda la evidencia de que el programa mejora la salud y la calidad de vida de quienes viven con estas enfermedades.


Non-communicable diseases (NCD) are a growing global concern that affects both healthcare and quality of life. Chronic disease self-management programs, such as "Tomando Control de su Salud" (TCS), have become effective strategies to address this problem. PURPOSE: To evaluate the results of the TCS program, online version, in the self-management and self-efficacy variables, in samples of people with NCD from Mexico and Peru, during the COVID-19 pandemic. METHODOLOGY: The study design was quasi-experimental with independent measurements (interventions and controls) and online measurements, at the beginning and end of the intervention; evaluated with the Partners in Health Scale and the Self-Efficacy Scale. Changes over time were examined using the Wilcoxon test. RESULTS: Out of the 114 participants, most were women (83.3%) with an average age of 58.8 years. Before the intervention, the groups were equal in the study variables. No significant increase in the study variables was found in the control group; however, there was a statistically significant increase in self-efficacy and self-management in the intervention group. CONCLUSIONS: The online TCS program contributed to increased self-efficacy, self-management, disease knowledge, treatment adherence, and symptom management in participants with NCD from Mexico and Peru during the COVID-19 pandemic. This supports evidence that the program improves the health and quality of life of those living with these illnesses.

Los cuidadores informales presentan altos niveles de estrés y sobrecarga frente al cuidado de personas a fin de vida. Sin embargo, algunos han logrado afrontar esta situación a través de su autoeficacia. OBJETIVO: El objetivo de esta revisión fue identificar los factores que influyen en el desarrollo de la autoeficacia en cuidadores informales de personas en cuidados paliativos e identificar los efectos de la autoeficacia en dichos cuidadores. MÉTODO: Se condujo una revisión narrativa de literatura científica realizada en las bases de datos Web of Science, PubMed, CUIDEN y CINAHL en base al flujograma de PRISMA, entre septiembre y octubre del 2022, por medio de los descriptores autoeficacia, cuidadores y cuidados paliativos. Los hallazgos de los artículos encontrados fueron sometidos un análisis temático por ambas autoras. RESULTADOS: Se seleccionaron 15 artículos científicos, identificando tres categorías: Factores que disminuyen el desarrollo de autoeficacia de los cuidadores informales (sentimientos de estrés, angustia y ansiedad, y sobrecarga del cuidador); Factores que favorecen el desarrollo de la autoeficacia de los cuidadores informales (esperanza, apoyo social y capacitación de los cuidadores); y los Efectos de la autoeficacia en los cuidadores informales (confianza en el cuidado otorgado, calidad de vida del receptor del cuidado, disminución del estrés y disminución de la sobrecarga del cuidador). CONCLUSIONES: A partir de los hallazgos, se concluye que el cuidador informal debe afrontar importantes retos con prontitud y de manera adecuada al brindar cuidados. Por lo que, la autoeficacia desarrollada a partir de la esperanza, el apoyo social y la capacitación, le permite mejorar la calidad de vida y bienestar tanto de sí mismo como de la persona bajo su cuidado.


Informal caregivers present high levels of stress and overload in the face of end-of-life care. However, some have managed to cope with this situation through their self-efficacy. OBJECTIVE: This study had the objective of identifying the factors that influence the development of self-efficacy in informal caregivers of people in palliative care and identify the effects of self-efficacy on these caregivers. METHOD: A narrative review of scientific literature was carried out in the databases Web of Science, PubMed, CUIDEN, and CINAHL based on the PRISMA flowchart, between September and October 2022, using the descriptors self-efficacy, caregivers, and palliative care. The findings of the articles were subjected to a thematic analysis by both authors. RESULTS: 15 scientific articles were selected, identifying three categories: Factors that decrease the development of informal caregivers' self-efficacy (feelings of stress, distress and anxiety, and caregiver overload); Factors that favour the development of informal caregivers' self-efficacy (hope, social support, and caregiver empowerment); and the Effects of self-efficacy on informal caregivers (confidence in the care given, quality of life of the care recipient, decreased stress and decreased caregiver overload). CONCLUSIONS: It was concluded that the informal caregiver must meet important challenges promptly and appropriately when providing care. Therefore, the self-efficacy developed from hope, social support, and training enables them to improve the quality of life and well-being of both them and the cared-for person.

ABSTRACT

OBJECTIVES@#To optimize the oxygen therapy regimens for infants with pulmonary diseases during bronchoscopy.@*METHODS@#A prospective randomized, controlled, and single-center clinical trial was conducted on 42 infants who underwent electronic bronchoscopy from July 2019 to July 2021. These infants were divided into a nasal cannula (NC) group and a modified T-piece resuscitator (TPR) group using a random number table. The lowest intraoperative blood oxygen saturation was recorded as the primary outcome, and intraoperative heart rate and respiratory results were recorded as the secondary outcomes.@*RESULTS@#Compared with the NC group, the modified TPR group had a significantly higher level of minimum oxygen saturation during surgery and a significantly lower incidence rate of hypoxemia (P<0.05). In the modified TPR group, there were 6 infants with mild hypoxemia, 2 with moderate hypoxemia, and 1 with severe hypoxemia, while in the NC group, there were 3 infants with mild hypoxemia, 5 with moderate hypoxemia, and 9 with severe hypoxemia (P<0.05). The modified TPR group had a significantly lower incidence rate of intraoperative respiratory rhythm abnormalities than the NC group (P<0.05), but there was no significant difference in the incidence rate of arrhythmias between the two groups (P>0.05).@*CONCLUSIONS@#Modified TPR can significantly reduce the risk of hypoxemia in infants with pulmonary diseases during electronic bronchoscopy, and TPR significantly decreases the severity of hypoxemia and the incidence of respiratory rhythm abnormalities compared with traditional NC.

ABSTRACT

BACKGROUND@#The application of programmed cell death 1 (PD-1)/programmed cell death ligand 1 (PD-L1) antibodies has greatly improved the clinical outcomes of lung cancer patients. Here, we retrospectively analyzed the efficacy of PD-1 antibody therapy in locally advanced non-surgical or metastatic lung cancer patients, and preliminarily explored the correlation between peripheral blood biomarkers and clinical responses.@*METHODS@#We conducted a single center study that included 61 IIIA-IV lung cancer patients who received PD-1 antibody treatment from March 2020 to December 2021, and collected the medical record data on PD-1 antibody first-line or second-line treatment. The levels of multiple Th1 and Th2 cytokines in the patient's peripheral blood serum, as well as the phenotype of peripheral blood T cells, were detected and analyzed.@*RESULTS@#All the patients completed at least 2 cycles of PD-1 monoclonal antibody treatment. Among them, 42 patients (68.9%) achieved partial response (PR); 7 patients (11.5%) had stable disease (SD); and 12 patients (19.7%) had progressive disease (PD). The levels of peripheral blood interferon gamma (IFN-γ) (P=0.023), tumor necrosis factor α (TNF-α) (P=0.007) and interleukin 5 (IL-5) (P=0.002) before treatment were higher in patients of the disease control rate (DCR) (PR+SD) group than in the PD group. In addition, the decrease in absolute peripheral blood lymphocyte count after PD-1 antibody treatment was associated with disease progression (P=0.023). Moreover, the levels of IL-5 (P=0.0027) and IL-10 (P=0.0208) in the blood serum after immunotherapy were significantly increased compared to baseline.@*CONCLUSIONS@#Peripheral blood serum IFN-γ, TNF-α and IL-5 in lung cancer patients have certain roles in predicting the clinical efficacy of anti-PD-1 therapy. The decrease in absolute peripheral blood lymphocyte count in lung cancer patients is related to disease progression, but large-scale prospective studies are needed to further elucidate the value of these biomarkers.

ABSTRACT

OBJECTIVE To systematically evaluate the effects of C3435T polymorphism in ABCB1 gene on lipid-lowering efficacy of statins. METHODS Retrieved from PubMed, Web of Science, the Cochrane Library, CNKI and VIP, the cohort studies on the use of statins were collected from the inception to November 1, 2023. After literature screening, data extraction and quality evaluation, meta-analysis was performed by using RevMan 5.4 software. RESULTS A total of 11 literature involving 1 575 patients were included. The results showed that under the dominant genetic model, the reduction of low-density lipoprotein cholesterol (LDL-C) [MD=-1.87, 95%CI (-3.62, -0.13), P=0.04], total cholesterol (TC) [MD=-1.42, 95%CI (-2.80, -0.04), P=0.04] in patients with CT+TT genotype was significantly higher than CC genotype. There was no significant difference in the increase of high-density lipoprotein cholesterol (HDL-C) [MD=-0.65, 95%CI (-2.48, 1.18), P=0.49] or the decrease of triglyceride (TG) [MD=-0.05, 95%CI (-2.94, 2.84), P=0.97] between patients with CT+TT genotype and CC genotype. Under the recessive genetic model, the reduction of TC [MD=2.26, 95%CI (0.97, 3.56), P=0.000 6] and the increase of HDL-C [MD=2.38, 95%CI (0.42, 4.35), P=0.02] in patients with TT genotype were significantly higher than CC+ CT genotype. There was no significant difference in the reduction of LDL-C [MD=1.53, 95%CI (-0.10, 3.15), P=0.07] or TG [MD=0.06, 95%CI (-2.98, 3.10), P=0.97] between CC+CT genotype and TT genotype. Under the additive genetic model, the reduction of TC [MD=2.98, 95%CI (1.27, 4.69), P=0.000 6] and LDL-C [MD=2.84, 95%CI (0.67, 5.01), P=0.01] in patients with TT genotype were significantly higher than CC genotype. There was no significant difference in the increase of HDL-C [MD=2.40, 95%CI (-0.17, 4.97), P=0.07] or the decrease of TG [MD=0.97, 95%CI    (-2.93, 4.87), P=0.63] between patients with TT genotype and CC genotype. CONCLUSIONS The reduction of LDL-C and TC in patients with dyslipidemia treated with statins may be related to the heterozygous and homozygous mutation of C3435T in ABCB1 gene, and the reduction of LDL-C and TC in patients with CT or TT genotype is more obvious, compared with patients with CC genotype. The elevation of HDL-C may be related to homozygous mutation, and the effect of HDL-C elevation may be more obvious in patients with TT genotype, compared with CC+CT genotype. However, the change of TG may not be related to the C3435T polymorphism in ABCB1 gene.

ABSTRACT

AIM: To compare the efficacy of intravitreal injection of ranibizumab(IVR)and intravitreal injection of conbercept(IVC)in children with retinopathy of prematurity(ROP).METHODS: Retrospective study. A total of 1 100 eyes with ROP treated with intravitreal anti-VEGF at our hospital from January 2015 to June 2023 were included. According to the different therapeutic drugs, the children were divided into two groups: IVR group and IVC group. According to the degree of ROP, the patients were divided into three groups: aggressive ROP(A-ROP), Zone Ⅰ type 1 ROP and Zone Ⅱ type 1 ROP. The reactivation and retreatment between the two groups were compared after propensity score matching(PSM)analysis, and they were followed-up for at least 3 mo after surgery.RESULTS: In Zone Ⅱ type 1 ROP, there was a statistically significant difference in the rates of reactivation and retreatment between the IVR and IVC groups(P&#x003C;0.05); however, in A-ROP and Zone I type 1 ROP, there were no statistically significant differences in the rates of reactivation and retreatment between the two groups(P&#x003E;0.05). The risk of reactivation and retreatment of Zone I type 1 ROP was higher than the Zone II type 1 ROP. Furthermore, the use of drugs and corrected gestational age of first treatment were influencing factors of lesion recurrence and retreatment.CONCLUSION: There is a significant difference in the initial cure effect between the two drugs in Zone II type 1 ROP, with the reactivation and retreatment rates of the IVC group being much lower than those of the IVR group.

Objective@#To evaluate the clinical efficacy of invisible orthodontic appliances without brackets for the distal movement of maxillary molars to improve the ability of orthodontists to predict treatment outcomes.@*Methods@#Web of Science, Cochrane Library, Embase, PubMed, Wanfang Database, CNKI Database, and VIP Database were searched for studies investigating the efficacy of invisible orthodontic appliances for distal movement of maxillary molars in adult patients and published from database inception to August 1, 2023. A total of three researchers screened the studies and evaluated their quality and conducted a meta-analysis of those that met quality standards.@*Results@#This study included 13 pre- and postcontrol trials with a total sample size of 281 patients. The meta-analysis revealed no significant differences in the sagittal or vertical parameters of the jawbone after treatment when compared with those before treatment (P>0.05). The displacement of the first molar was MD=-2.34, 95% CI (-2.83, -1.85); the displacement was MD=-0.95, 95% CI (-1.34, -0.56); and the inclination was MD=-2.51, 95% CI (-3.56, -1.46). There was a statistically significant difference in the change in sagittal, vertical, and axial tilt of the first molar before and after treatment. After treatment, the average adduction distance of the incisors was MD=-0.82, 95% CI (-1.54, -0.09), and the decrease in lip inclination was MD=-1.61, 95% CI (-2.86, -0.36); these values were significantly different from those before treatment (P<0.05).@*Conclusion@#Invisible orthodontic appliances can effectively move the upper molars in a distal direction and control the vertical position of the molars. When the molars move further away, there is some degree of compression and distal tilt movement, which is beneficial for patients with high angles. The sagittal movement of incisors is beneficial for improving the patient's profile.

ABSTRACT

ObjectiveTo investigate the functions and characteristics of traditional Chinese medicine (TCM) from plant skin and their Chinese patent medicines and explore the related laws of the medicinal tissue, property, taste, efficacy, and clinical application, so as to strengthen the theory of identifying symptoms for qualities and provide references for the development and utilization of TCM from plant skin and their Chinese patent medicines. MethodBy reviewing the 2020 edition of the Chinese Pharmacopoeia and some local pharmacopeias, TCM from plant skin and their Chinese patent medicines were screened out, and the characteristics, functions, and precautions of TCM from plant skin and their Chinese patent medicines were summarized. Statistical analysis was carried out with Excel. ResultA total of 62 TCM from plant skin were found, mainly distributed in one kingdom, three phyla, and 31 families. In terms of the family genus, Rutaceae>Leguminosae>Cucurbitaceae. In terms of the medicinal tissue, bark>root bark>fruit bark>seed bark. In terms of property and taste, warm>cold>plain>cool>hot, and bitter>sweet=pungent>acidic. In terms of meridians, lung>liver>spleen>heart>colorectal>kidney>stomach=bladder. In terms of TCM classification, most of them belong to the category of heat-clearing medicines. There were 485 types of Chinese patent medicines from plant skin, with the most Chinese patent medicines containing Citri Reticulatae Pericarpium. Among the forms of administration, pills were the most predominant. In terms of the tastes of the medicines, bitter and sweet flavors predominated. In terms of functions, medicines for strengthening the body resistance were the most. For the precautions, contraindications during pregnancy were the most common. ConclusionThere is a correlation among medicinal tissue, property, taste, efficacy, and clinical application of TCM from plant skin. It is also necessary to pay attention to the contraindications of the medicines and rationally choose TCM from plant skin and their Chinese patent medicines under the guidance of TCM theory based on syndrome differentiation and treatment.

ObjectiveThe relevant laws among the biological characteristics, medicinal parts, growth environment, and medicinal properties and efficacy of traditional Chinese medicines (TCM) from fungi were excavated, so as to strengthen the theory of distinguishing symptoms for quality and provide a reference for the development and utilization of TCM from fungi. MethodThe medicinal parts, meridians for properties and tastes, heterotrophic mode, and efficacy of commonly used TCM from fungi were summarized. By consulting the Compendium of Materia Medica, Shennong Materia Medica, Flora of China, and literature, the TCM from fungi indexed in the 2020 edition of the Chinese Pharmacopoeia and some local pharmacopeias were checked. ResultA total of 28 common TCM from fungi were selected. Different TCMs from fungi have different meridians for properties and tastes, medicinal parts, habits, and growth environments. The relevant information was counted. Among the four properties, plain>cold>warm. Among the five tastes, sweet>bitter>light>pungent=salty. In terms of medicinal parts, fruiting body>sclerotia>complex>spermia=outer skin=other. In terms of meridians, lung>liver=heart>spleen=kidney>stomach. In terms of habits, parasitism>saprophysis>symbiosis=facultative parasitism=facultative saprophysis. ConclusionTCM from fungi are mainly parasitic and saprophytic, and the plain property and sweet taste the most. The meridians are mostly lung, heart, and liver. Nourishment and diuresis are the main efficacy. There is a certain correlation between the color, habit, medicinal parts, and growth environment of TCM from fungi and their properties, tastes, and efficacy, providing comprehensive literature reference and theoretical basis for their in-depth research, clinical use, and resource development.

ABSTRACT

ObjectiveTo investigate the correlation among the botanical characteristics, biological characteristics, chemical composition, and medicinal properties and efficacy of traditional Chinese medicines (TCM) from latex-containing plants, so as to strengthen the theory of "identifying symptoms for qualities" and provide a reference for the development and utilization of the latex-containing plant resources. MethodStatistics on the meridians for properties and tastes, efficacy, medicinal parts, family and genus, and chemical components of TCM from latex-containing plants were carried out. A total of 53 TCM from latex-containing plants included in the 2020 edition of the Chinese Pharmacopoeia were screened by mining the Chinese Botanical Journal, Chinese Materia Medica, Dictionary of Traditional Chinese Medicines, and related literature. In addition, their meridians for properties and tastes, medicinal parts, chemical components, and TCM classifications were summarized and statistically analyzed by using Excel 2013 and ChiPlot 2023.3.31 software. ResultIt was found that latex-containing plants were mainly distributed in one kingdom, one phylum, two classes, and 20 families, and most of the TCM from latex-containing plants belonged to Dicotyledonaceae under Angiosperms. In terms of properties and tastes, plain>cold>warm>cool>hot and bitter>pungent>sweet>sour>salty. In terms of meridians, liver>lung>kidney>spleen=large intestine=stomach>heart>bladder=gallbladder=small intestines. In terms of medicinal parts, roots (root, rhizomes, tuberous root, and root bark)>resin>seed>whole herb (whole herb and above-ground part)>stem (stem and branch)>fruit>leaf>flower=skin. In terms of research on chemical components, they were mostly glycosides. In terms of TCM classification, they were mostly medicines for activating blood circulation and removing blood stasis. ConclusionThe TCM from latex-containing plants is mainly plain, with a uniform warm and cold distribution. The tastes are mainly bitter and pungent, and the major meridians are the liver and lung. The roots and resins are mainly used as medicines. The components mostly contain glycosides, alkaloids, and volatile oils, and most of them are medicines for activating blood circulation and removing blood stasis, as well as for removing heat and toxins. There is a certain degree of correlation among the growth habits, medicinal parts, chemical components, and the properties, tastes, and efficacy of the TCM from latex-containing plants. It may provide a reference for resource development and utilization of TCM from latex-containing plants.

ABSTRACT

ObjectiveTo evaluate the efficacy and safety of Lianhua Qingke tablets in the treatment of acute bronchitis in children with the syndrome of phlegm-heat obstructing lung. MethodA randomized, open, parallel controlled, and multi-center clinical study was conduted. Children with acute bronchitis (syndrome of phlegm-heat obstructing lung) were randomly assigned to an observation group and a control group. The control group received routine basic treatment, and the observation group was treated with Lianhua Qingke Tablets on the basis of routine basic treatment. After 7 days of treatment, the clinical efficacy, TCM efficacy, time to symptom disappearance, time to cough disappearance, and clinical safety were compared between the two groups. ResultA total of 248 children were included (124 in the observation group and 124 in the control group). After 7 days of treatment, the total response rate in terms of clinical efficacy in the observation group was 96.8% (120/124), which was higher than that (90.3%, 112/124) in the control group (Z=-5.034, P<0.01). The total response rate in terms of TCM syndrome in the observation group was 97.6% (121/124), which was higher than that (93.5%, 116/124) in the control group (χ2=-5.326, P<0.01). The scores of physical signs and TCM symptoms in the observation group were lower than those in the control group at the time of taking medicine for 3 days and 7 days (P<0.01). The time to symptom disappearance and the time to cough disappearance in the observation group were shorter than those in the control group (P<0.01). Drug-related adverse reactions occurred in neither group. ConclusionLianhua Qingke tablets demonstrate a definite effect on acute bronchitis in children with the syndrome of phlegm-heat blocking lung. The tablets can significantly shorten the course of disease and relieve cough and TCM symptoms, with high safety, which is worthy of clinical application and promotion.

Colorectal cancer (CRC) is one of the most common malignant tumors recorded worldwide. This condition has high morbidity and mortality and seriously endangers people's health. Traditional diagnostic models fail to meet people's current needs for real-time monitoring of tumors. Compared with traditional detection methods, ctDNA detection is not only noninvasive but can also attain real-time detection of comprehensive genomic information of tumors. The advancement of detection technology has gradually highlighted the potential of ctDNA detection in the clinical treatment of CRC. This article reviews the advancements on the clinical application of ctDNA in early screening, minimal residual disease detection, and guidance on individualized treatment of CRC patients.

ABSTRACT

AIM: To observe the real world efficacy of continuous subcutaneous insulin infusion (CSII) with insulin pump in short-term intensive treatment of patients with type 2 diabetes (T2DM) and explore the influencing factors associated with efficacy. METHODS: A total of 369 hospitalized T2DM patients were treated with CSII short-term intensive therapy. Based on the blood glucose levels on the 5th day of treatment, they were divided into two groups, namely the qualified blood glucose control group and the non-qualified blood glucose control group. The differences of glucose metabolism, physiology, biochemistry and other indicators between the two groups were compared, and the influencing factors of insulin pump efficacy were analyzed by regression statistics. RESULTS: After 4 days of CSII short-term treatment, 225 patients (61%) achieved blood glucose control standards, while 144 patients (39%) did not achieve blood glucose control standards. Compared with the qualified blood glucose control group, the non-qualified blood glucose control group was older and with higher levels of total daily insulin dose per kilogram of body weight (TDD), fasting blood glucose (FBG), fasting insulin, fasting C-peptide, and insulin resistance index (HOMA-IR), while lower levels of blood phosphorus, insulin growth factor 1 (IGF1). In terms of biochemical indicators, there was no significant difference between the two groups as a whole, including blood lipid levels, liver function, and kidney function. The results of univariate regression and multivariate stepwise regression analysis showed that 2-hour postprandial insulin, TDD and blood phosphorus were the key factors influencing the efficacy of Insulin pump. Among them, qualified blood glucose control was positively correlated with blood phosphorus, and negatively correlated with 2-hour postprandial insulin and TDD. CONCLUSION: CSII with insulin pump is an ideal method for short-term intensive treatment of type 2 diabetes. In order to achieve a more satisfactory rate of qualified blood glucose control, more attention should be paid to the patient's blood phosphorus, 2-hour postprandial insulin and TDD in clinical use.

ABSTRACT

OBJECTIVE To investigate the effects of GSTP1, XRCC1, ABCB1, MTHFR gene polymorphisms on efficacy and toxic effect of chemotherapy regimen containing oxaliplatin in patients with stage Ⅲ and Ⅳ colorectal cancer patients. METHODS Clinical data of 76 patients with stage Ⅲ and Ⅳ colorectal cancer who received chemotherapy regimen containing oxaliplatin (XELOX,FOLFOX) were collected from the Second Affiliated Hospital of Soochow University from September 2018 to March 2020. The correlation of genotypes with progression-free survival (PFS) and toxic effect was analyzed by using univariate and multivariate COX regression model. RESULTS Carriers of the ABCB1 3435T>C locus C allele (TC/CC) had a significantly higher risk of progression compared to TT genotype patients [HR=2.39, 95%CI (1.05,5.50), P=0.038]. The risk of progression in patients at stage Ⅳ was significantly higher than those at stage Ⅲ [HR=8.11, 95%CI(3.39,19.40), P<0.001]. Chemotherapy regimen, Karnofsky performance status score and tumor site had no significant effect on disease progression (P>0.05). Mutations in gene loci were not correlated with adverse reactions (P>0.05). CONCLUSIONS Patients carrying ABCB1 TC/CC and receiving chemotherapy regimen containing oxaliplatin have a higher risk of disease progression, which may be associated with longer PFS in patients (TT genotype) with stage Ⅳ colorectal cancer receiving the chemotherapy, while GSTP1, XRCC1, and MTHFR gene polymorphisms have no significant impact.
