ABSTRACT

Objetivo: Determinar las características sociodemográficas y clínicas asociadas a las actitudes positivas hacia la sexualidad en adultos mayores atendidos en el Hospital Nacional Hipólito Unanue de setiembre a diciembre del 2021. Método: Estudio observacional, analítico y transversal, que incluyó a 227 pacientes adultos mayores de 60 años de consulta externa del servicio de geriatría del HNHU atendidos por teleconsulta. Las actitudes hacia la sexualidad se midieron con la aplicación del Cuestionario de Actitudes hacia la Sexualidad en Adultos Mayores, dividiendo el resultado en actitud positiva y negativa. Resultados: El 86,3% de adultos mayores presentan actitud positiva hacia la sexualidad, la edad de 60 a 79 años se presentó en el 73% de dichos pacientes, la educación superior fue mayor en el grupo con actitud positiva (47%), el 5,6% de los pacientes con actitud positiva refirió tener pareja actual, recibir charlas educativas incrementa en 3,12 veces la probabilidad de actitud positiva hacia la sexualidad. En el análisis bivariado hubo asociación estadísticamente significativa entre la edad de 60 a 79 años (RP: 1,31, IC95%: 1,12-1,52, p<0,001), la educación superior (RP: 1,19, IC95%: 1,08-1,31, p=0,001), el tener pareja actual (RP: 1,32, IC95%: 1,19-1,46, p<0,001), el haber recibido charlas educativas (RP: 1,13, IC95%: 1,03-1.24, p=0,033). En el análisis multivariado se encontró asociación entre el tener pareja actual y la actitud positiva hacia la sexualidad (RPa: 1,177, IC95%: 1,09-1,26, p<0,001). Conclusión: Existe alta frecuencia de actitud positiva hacia la sexualidad en adultos mayores, y el tener pareja actual fue un factor asociado independiente.


Objective: To determine the sociodemographic and clinical characteristics associated with positive attitudes towards sexuality in older adults seen in the outpatient clinic of the Hipólito Unanue National Hospital from september to december, 2021. Methods: Cross-sectional analytical observational study, which included 227 adult patients over 60 years of age from the outpatient clinic of the HNHU geriatrics service attended by teleconsultation. Attitudes towards sexuality were measured by applying the Questionnaire of Attitudes towards Sexuality in Older Adults, dividing the result into positive and negative attitudes. Results: 86.3% of older adults have a positive attitude towards sexuality, the age of 60 to 79 years was presented in 73% of these patients, higher education was higher in the group with positive attitude (47%), 53.6% of the patients with a positive attitude reported having a current partner, having received educational talks increases the probability of a positive attitude towards sexuality by 3.12 times. In the bivariate analysis, there was a statistically significant association between age 60 to 79 years (RP: 1.31, 95%CI: 1.12-1.52, p<0.001), higher education (RP: 1.19, 95%CI: 1.08-1.31, p=0.001), having a current partner (RP: 1.32, 95%CI: 1.19-1.46, p<0.001), having received educational talks (RP: 1.13, 95%CI: 1.03-1.24, p=0.033). In the multivariate analysis, an association was found between having a current partner and a positive attitude towards sexuality (RPa: 1.177, 95%CI: 1.09-1.26, p<0.001). Conclusions: There is a high frequency of positive attitude towards sexuality in older adults, and having a current partner was an independent associated factor.

ABSTRACT

Introdução:A população idosa é suscetível às alterações no estado nutricional devido aos fatores relacionados às modificações socioeconômicas e fisiológicas; porém, estudos que avaliaram a situação alimentar atual de idosos no Brasil ainda são escassos. Objetivo: Avaliar a qualidade da dieta de idosas não institucionalizadas de uma capital do sul do Brasil. Métodos: Trata-se de um estudo de caráter transversal, com 174 idosas selecionadas por conveniência. A avaliação do consumo alimentar foi realizada por meio de registro alimentar de três dias. Avaliou-se a qualidade da dieta por meio do Índice de Qualidade da Dieta ­ Revisado (IQD-R) adaptado para a população brasileira.Resultados: Verificou-seque56,9% (n=99), 40,8% (n=71) e 2,3% (n=4) das participantes apresentaram dieta que "necessita de modificação", dieta "saudável", e dieta "inadequada", respectivamente. A pontuação média obtida por meio do IQD-R foi de 62,5 (± 9,6), com mínimo de 30,2 pontos e máximo de 84,9 pontos.Na análise do consumo dos grupos alimentares, constatou-se baixa pontuação de gordura saturada (9,96±0,50), mono e poli-insaturada (1,08±2,21), cereais integrais (1,7±1,54), sódio (7,31±2,27), e maior pontuação para o grupo de cereais totais (4,61±0,54).Conclusão: A maioria das idosas avaliadas necessitam de melhoria na qualidade da dieta, com ênfase no consumo de cereais integrais e gorduras mono e poli-insaturadas, porém com ingestão adequada de gordura saturada e sódio.


Introduction: The elderly population is susceptible to changes in nutritional status due to factors related to socioeconomic and physiological changes; however, studies that evaluated the current dietary status of the elderly in Brazil are still scarce. Objective: To evaluate the quality of the diet of non-institutionalized elderly women from a capital city in southern Brazil. Methods: This was a cross-sectional study, with 174 convenience sampled elderly women. Food intake was assessed by means of a three-day dietary record. Diet quality was assessed through the Diet Quality Index - Revised (IQD-R) adapted for the Brazilian population. Results: This work showed that 56.9% (n=99), 40.8% (n=71), and 2.3% (n=4) of the diets of the participants were classified as "needs modification", "healthy", and "inadequate", respectively. The mean score obtained through the IQD-R was 62.5 (± 9.6), with a minimum of 30.2 points and a maximum of 84.9 points. In the analysis of food group consumption, there was a low score for saturated fat (9.96±0.50), mono and polyunsaturated fats (1.08±2.21), whole grains (1.7±1.54), sodium (7.31±2.27), and a higher score for the total grains group (4.61±0.54). Conclusion: Most of the elderly women assessed needed improvement in the quality of their diets, with emphasis on the consumption of whole grains and mono and polyunsaturated fats, but with adequate intake of saturated fat and sodium.

ABSTRACT

Objective: To develop an educational intervention in caregivers and to know its impact on oral hygiene of older adults. Materials and methods: An intervention study was conducted with pre- and post-test design in a group of older adults of a rest house in Guadalajara, Jalisco, Mexico, 2018. The oral hygiene of older adults was evaluated through the simplified index of oral hygiene. Once a week, an educational intervention was performed on oral health issues in caregivers. Results: 6 older adults participated, all of them women (100%). The mean age was 82.50 years (± 9.7). The impact of the intervention improved the level of oral hygiene of older adults by 33.72%. Conclusions: Educational intervention in older caregivers achieved significant changes in oral hygiene. Future intervention programmes should include assessments in the following months to assess long-term effects.


Objetivo: Desarrollar una intervención educativa en cuidadores y conocer su impacto en la higiene bucal de los adultos mayores institucionalizados. Materiales y métodos: Se realizó un estudio de intervención con diseño de pre y postprueba en un grupo de adultos mayores institucionalizados de una casa de descanso en Guadalajara, Jalisco, México, 2018. La higiene bucal de los adultos mayores se evaluó por medio del Índice de Higiene Oral Simplificado. Una vez a la semana se realizaba una intervención educativa sobre temas de salud bucal en los cuidadores. Resultados: Participaron 6 adultos mayores de los cuales todos eran mujeres (100%). La edad media fue de 82.50 años (± 9.7). El impacto de la intervención mejoró el nivel de higiene bucal de los adultos mayores en un 33.72%. Conclusiones:La intervención educativa en los cuidadores de adultos mayores logró cambios significativos en la higiene bucal. Dentro de los futuros programas de intervención debería incluirse el hacer evaluaciones en los siguientes meses para valorar los efectos a largo plazo.

ABSTRACT

This study aimed to evaluate social and economic predictors, as well social behavior and its influence on the quality of life of elders who use the primary health care network of Uberaba, Minas Gerais. Methods: Descriptive, exploratory, cross-sectional and quantitative study, conducted with 248 60 year old or older subjects, from November 2016 to May 2017. A structured questionnaire was applied to evaluate sociodemographic data, health conditions and life habits as well the WHOQOL-Bref and the WHOQOL-Old. The analysis were conducted through relative frequencies and t-test as well multiple linear regression were used to associate variables. The study was approved by the Committee of Ethics in Researches with Human Beings of the Federal University of the Triângulo Mineiro (CEP/UFTM). Results: Most elders were women, catholic, with 4 years or more of formal education, had partners, a sexually inactive life, and were retired. The linear regression analysis, found a positive and light or moderate association between sexual life and the physical, social, and environmental domains, and the past, present and future activities facet. The educational level variable had a positive and light association with the psychological domain, whereas the variable occupation had a negative and light association with the death and dying facet. Conclusions: The results found show an important social impact regarding the studied population, making it clear that public policies should be created and executed as to offer subsidies for actions which are specifically targeted at the QoL of elders.


Este estudo teve como objetivo avaliar os preditores sociais e econômicos, bem como o comportamento social e sua influência na qualidade de vida dos idosos que utilizam a rede de Atenção Primária à Saúde de Uberaba, Minas Gerais. Métodos: Estudo descritivo, exploratório, transversal e quantitativo, realizado com 248 pessoas de 60 anos ou mais, entre novembro de 2016 a maio de 2017. Foi aplicado um questionário estruturado para avaliar os dados sociodemográficos, condições de saúde e hábitos de vida, bem como o WHOQOL-Bref e o WHOQOL-Old. A análise foi realizada através de freqüências relativas e teste t, bem como regressão linear múltipla foram utilizadas para associar variáveis. O estudo foi aprovado pelo Comitê de Ética em Pesquisa com Seres Humanos da Universidade Federal do Triângulo Mineiro (CEP / UFTM). Resultados: A maioria dos idosos eram mulheres, católicas, com 4 anos ou mais de educação formal, tinham parceiros, uma vida sexualmente inativa e eram aposentadas. A análise de regressão linear, encontrou uma associação positiva leve ou moderada entre a vida sexual e os domínios físico, social e ambiental, e facetas de atividades passadas, presentes e futuras. A variável nível educacional teve uma associação positiva e leve com o domínio psicológico, enquanto que a variável ocupação teve uma associação negativa e leve com a faceta de morte e morrer. Conclusões: Os resultados encontrados mostram um impacto social importante em relação à população estudada, deixando claro que as políticas públicas devem ser criadas e executadas de forma a oferecer subsídios para ações especificamente voltadas para a QV dos idosos.

ABSTRACT

Os objetivos deste estudo foram descrever uma proposta do protocolo de treinamento aeróbio intervalado de moderada intensidade e verificar o seu efeito nas funções cognitivas em idosas com o diagnóstico de depressão maior. Trata-se de um protocolo pré-experimental conduzido com 11 idosas, com idade entre 60 e 77 anos (67 ±5,4 anos), clinicamente diagnosticadas com depressão. Foram avaliadas as funções executivas, atenção complexa, habilidades de navegação espacial e desempenho funcional em condições de dupla-tarefa. O protocolo de treino foi constituído por 20 minutos de atividades de coordenação motora, tanto de membros superiores quanto inferiores de forma simultânea, 30 minutos de treinamento aeróbio intervalado e 10 minutos de volta a calma com alongamentos, durante nove semanas de treinamento. A análise estatística empregada foi o teste t para amostras dependentes e a análise do tamanho de efeito. Observou-se 75% de adesão das idosas na intervenção proposta. Em relação à depressão, identificou-se que 54,5% das idosas apresentaram remissão dos sintomas após o programa de treinamento. Conforme os dados pré- e pós-intervenção, observou-se uma redução estatisticamente significativa no tempo médio despendido na realização dos testes de Stroop 1 (20,09 ± 5,07 x 16,27 ± 4,03; p = 0,024; TE = 0,84), de Stroop 3 (34,27 ± 8,39 x 28,36 ± 5,10; p = 0,032; TE=0,88) e de dupla tarefa visuoespacial (10,55 ± 2,07 x 9,45 ± 1,44; p = 0,006; TE = 0,63). O presente estudo verificou um efeito positivo no desempenho das tarefas de atenção, função executiva e dupla tarefa visuoespacial após uma estratégia de treinamento, baseada no exercício aeróbio intervalado de moderada intensidade de nove semanas, em idosas com depressão....(AU)


The objectives of this study were to describe a proposal of the protocol of moderate intensity interval aerobic training and to verify its effect on the cognitive functions in the elderly with the diagnosis of major depression. It is a protocol and pre-experimental study conducted with 11 elderly women, aged 60-77 years (67 ± 5.4 years), clinically diagnosed with depression. Executive functions, complex attention, spatial navigation skills and functional performance were evaluated in dual-task conditions. The training protocol consisted of 20 minutes of motor coordination activities, both upper and lower limbs simultaneously, 30 minutes of aerobic interval training and 10 minutes of calm back with stretches during 24 training sessions. The statistical analysis employed was the t-test for dependent samples and the effect size analysis. A 75% adherence of the elderly was observed in the proposed intervention. Regardingdepression, it was identified that 54.5% of the elderly had remission of symptoms after the training program. According to the pre- and post-intervention data, a statistically significant reduction in the mean time spent on the Stroop 1 tests (20.09 ± 5.07 x 16.27 ± 4.03; p = 0.024; TE = 0.84), Stroop 3 (34.27 ± 8.39 x 28.36 ± 5.10, p = 0.032, TE = 0.88) and dual visuospatial task (10.55 ± 2.07 x 9.45 ± 1.44, p = 0.006, TE = 0.63). ABSTRACT

El deterioro funcional hospitalario se caracteriza por la pérdida de, al menos, una de las actividades básicas de la vida diaria con respecto a la funcionalidad previa al inicio de la enfermedad aguda. Tiene factores de riesgo asociados que son modificables durante la hospitalización y de ahí la importancia de la identificación temprana y la intervención oportuna. Por eso se debe conocer esta entidad clínica, debido a las complicaciones asociadas que se pueden presentar tanto a corto como a largo plazo, entre las cuales están aumento de la estancia hospitalaria, presencia de complicaciones médicas, mayor riesgo de institucionalización, mayores costos para el sistema de salud y aumento de la mortalidad. El objetivo de esta revisión es conocer la forma de presentación, los factores de riesgo, la identificación temprana y los programas de intervención para mejorar la calidad de atención en los pacientes ancianos hospitalizados por enfermedad médica.


Functional decline in hospitalized elderly adults is charactenzed by the loss of at least one of the basic activities regarding the functionality reo weeks prior to the onset of acute illness. It has associated risk factors that could be modifíed during hospitalization, therefore the importance of the early identification and well'timed intervention. It is important to know the dinical entity's background, due to the short or long term múltiple associated complications that could unexpectedly appear; for example, increased hospital stay, medical complicación presence, increased institutionalization risk, higher costs for the health system and increased mortality rate. The objective of this work is to know about the presence, risk factors, early identification and the intervention programs to improve the caring quality ¿n elderly patients hospitalizad for medical illness.

ABSTRACT

Este estudo visa revisar sistematicamente o efeito do treinamento muscular no desfecho autopercepção dos Músculos do Assoalho Pélvico (MAP) em mulheres idosas (acima de 60 anos) com incontinência urinária. Foram selecionados ensaios clínicos randomizados que avaliaram o treinamento muscular dos MAP com o desfecho da autopercepção dessa musculatura em mulheres acima de 60 anos com incontinência urinária (IU) em 9 bancos de pesquisa no período de janeiro de 1980 a janeiro de 2015. Foram 86 artigos identificados, dos quais apenas 2 estudos foram incluídos. As variáveis encontradas foram: aderência ao tratamento do IIPE (Programa de Intervenção em Incontinência para Idosos), Autopercepção da Continência, Escala de Depressão Geriátrica (GDS), tipo de IU, Avaliação Funcional do Assoalho Pélvico (AFA), Perineometria, Graduação da força de acordo com a Escala de Ortiz e King's Health Questionnarie (KHQ). A informação disponível sobre a temática é escassa. São necessários novos estudos prospectivos para elucidar se o treinamento muscular interfere na autopercepção do MAP em idosas com IU.(AU)


This study aimed to systematically review the effect of muscle training in the outcome self perception of MAP in elderly women (over 60 years) with urinary incontinence. Were selected clinical trials randomized evaluating muscle training of the pelvic floor muscles (MAP) with the outcome of the self-perception of this musculature in women over 60 years with urinary incontinence (UI) in nine databases between January 1980 to January 2015. There were 86 identified articles, but only two studies were included. Unable to perform meta-analysis because of variables and outcome measures are different. The variables were: adherence to treatment of IIPE (Intervention Program for the Elderly incontinence), self-perception of continence, Geriatric Depression Scale (GDS), type of UI, Functional Evaluation of the Pelvic Floor (AFA), perineometry with graduation according to Ortiz Scale and King's Health Questionnaire (KHQ). The available information on the subject is scarce. Being needed new prospective studies to elucidate the muscle training interferes with the perception of MAP in elderly women with UI.(AU)

There are no studies in Mexico estimating the use of psychoactive substances in the elderly (+69 yrs) and there is a lack of information about the risk factors determining the burden of disease for this age group. The goal of this study is to present prevalences of use of alcohol, tobacco, illicit drugs and nonmedical prescription drugs in beneficiaries of a cash supplement program sponsored by the Institute for the Care of Elderly from Federal District (Instituto para la Atención del Adulto Mayor del Distrito Federal, IAAM-DF). Furthermore, significant sociodemographic factors associated with substance use are reported. From a random sample of the 2 501 users registries of a cash supplement program of IAAM-DF, a total of 2 098 agreed to participate and were interviewed in their homes. The response rate was of 83.9%. We found that 63.5% of all participants ever used alcohol, 45.4% ever used tobacco and 3.5% ever used any drugs. Near one out of every two men and one out of every four women used alcohol during the previous 12 months, one out of every 10 older adults used tobacco and just one out of every 100 used any kind of drug in the last 12 months. Overall prevalence of substance use is higher in men, with the exception of medical drugs. Outstanding associated factors in substance use were sex and higher socioeconomic levels. This is the first study in the country on substance use in dwelling elderly, not living in institutions. Alcohol use, even risky drinking, is not uncommon, smoking tobacco habits do not disappear with age, and medical drug use can be of concern for its effects on other diseases. Therefore, it becomes necessary to establish definitions of risky use for this population and develop appropriate intervention programs for this population group.


No existen investigaciones en el país sobre uso de sustancias psico-activas en poblaciones de adultos mayores (+69 años) y se carece de información sobre los factores de riesgos que son determinantes para la carga de la enfermedad en este grupo. Este estudio tuvo como objetivo presentar las prevalencias de consumo de alcohol, tabaco, drogas ilícitas y drogas médicas usadas sin prescripción en los beneficiarios del programa de pensión alimentaria del Instituto para la Atención del Adulto Mayor del Distrito Federal (IAAM-DF). Adicionalmente, se reportaron factores sociodemográficos asociados al consumo de estas sustancias. De una muestra aleatoria del padrón de usuarios del programa del IAAM-DF de 2501 personas, un total de 2098 aceptaron participar y fueron entrevistadas en sus hogares, obteniéndose una tasa de respuesta de 83.9%. Se encontró que 65.3% de la población consumió alcohol, 45.4% tabaco y 3.5% alguna droga alguna vez en la vida. En los últimos 12 meses, casi uno de cada dos varones y una de cada cuatro mujeres consumió alcohol; uno de cada 10 adultos mayores consumió tabaco; y sólo uno de cada 100 consumió alguna droga. La prevalencia de consumo de sustancias es mayor en hombres, con excepción de las drogas médicas. Los factores asociados que destacaron fueron el sexo y los mejores niveles económicos. Éste es el primer estudio realizado en el país sobre uso de sustancias en población de adultos mayores con domicilio fijo, no institucionalizado. El consumo de alcohol, incluso el consumo riesgoso, no es infrecuente; el hábito tabáquico no ha desaparecido con la edad, y el uso de drogas médicas puede ser un motivo de preocupación por su efecto en otras patologías. Se hace necesario establecer definiciones de uso riesgoso para esta población y desarrollar programas de intervención adecuados para este grupo.

ABSTRACT

El envejecimiento de la población y probables factores ligados a sus estilos de vida parecen ser de-terminantes en la presentación de la Enfermedad de Alzheimer que viene teniendo una tenden-cia de crecimiento en la frecuencia de ocurrencia en la población que resulta afectada no solo en sus desempeños cognitivos sino en condiciones de movimiento que conjuntamente van a limitar la calidad de vida de las personas afectadas por la entidad. Objetivo. Conocer la prevalencia de la enfermedad de Alzheimer entre las entidades neurológicas atendidas en Fisioterapia. Método. Se desarrolló un estudio transversal con los pacientes mayores de 15 años atendidos por causa de afecciones neurológicas en los servicios de Fisioterapia donde se daban Practicas del Programa de Fisioterapia de la Universidad Manuela Beltrán entre febrero y noviembre de 2012, calculando la prevalencia y ocurrencia de la enfermedad de Alzheimer por género. Resultados. Durante el periodo de tiempo del estudio se atendieron un total 1062 personas a causa de enfermedades neurológicas, 19.67% de estas personas registraron como motivo de remisión a Fisioterapia la En-fermedad de Alzheimer (n=209, Error Estándar=0.02), correspondiendo un 77.51% de las personas con esta patología al genero femenino (n=162, Error Estándar=0.01) Conclusiones La enfermedad de Alzheimer tiene una prevalencia importante dentro de las entidades neurológicas, y muestra un predominio en su presentación en las mujeres


The aging of the population and probable factors related to their lifestyles seem to be decisive in the presentation of Alzheimer's disease that has taken an upward trend in the frequency of occurrence in the population that is affected not only in their cognitive performance but able to move that joint will limit the quality of life of people affected by the entity Objective To know the prevalence of Alzheimer disease between the neurological diseases served in the Physical Therapy Services Methodology developed a cross-sectional study of patients older than 15 years treated because of neurological physiotherapy services in which they practiced the Physiothera-py Program Manuela Beltran University between February and November 2012, calculating the prevalence and occurrence of Alzheimer's disease by gender Results During the study period a total 1062 were met people because of neurological diseases, 19.67% of these people recorded as a reason for referral to Physical Therapy Alzheimer's Disease (n = 209, Standard Error = 0.02), corresponding 77.51% of people with this condition the female gender (n = 162, Standard Error = 0.01) Conclusions the disease Alzheimer's is a significant prevalence in neurological entities, and is predominantly in its presentation in women.

ABSTRACT

O objetivo do estudo foi comparar quatro grupos de idosos com e sem acidente vascular cerebral e quedas. Estudo do tipo caso-controle, desenvolvido na cidade de Fortaleza, em 2010. Dados coletados por entrevista através de um formulário que avaliou variáveis sociodemográficas, percepção de saúde e o índice de massa corporal de 60 idosos. Os grupos eram em maioria mulheres (66,7%), com média de idade de 69,86 a 71 anos. Os que sofreram acidente vascular cerebral apresentaram uma média de 1,4 eventos (±0,67), em um tempo médio de 5,6 anos (±7,5). Quanto aos grupos com quedas, o número de eventos foi ligeiramente maior naqueles com acidente vascular cerebral (1,4 ±0,63). A maioria referiu um estado de saúde regular e encontrava-se com sobrepeso ou obesidade. Conclui-se que os fatores de risco para quedas devem ser continuamente avaliados por profissionais qualificados, no intuito de manter favoráveis as condições de saúde do idoso.


The study aimed to compare four groups of elderly with and without stroke and falls. A case-control study that was performed in Fortaleza city, 2010. Data collected by interview through a form that evaluated demographic variables, health status and body mass index in 60 elderly. Most of the groups were  composed by women (66.7%), mean age of 69.86 to 71 years. Those who had suffered a stroke had presented an average of 1.4 events (±0.67) at a time of 5.6 years (±7.5). The number of stroke was slightly higher (1.4 ±0.63) at the groups with fall history. The  majority reported a regular health  condition and was overweight or obese. This way, the risk factors for falls should be continuously evaluated by qualified professionals, in order to maintain favorable conditions for elderly health.


El objetivo del estúdio fue comparar cuatro grupos de ancianos con y sin accidente cerebrovascular y caídas. Estudio de caso-control, desarrollado en 2010, en la ciudad de Fortaleza-CE-Brasil. Datos recogidos mediante entrevista através de un formulario que evaluó variables sociodemográficas, percepción de salud y el índice de masa corporal de 60 ancianos. Los grupos eran formados por mayoría de mujeres (66,7%), edad media de 69,86 a 71  años. Aquellos con accidente cerebrovascular presentaron una media de 1,4 eventos (±0,67), en un tiempo medio de 5,6 años (±7,5). Cuanto a los grupos con caídas, el número de eventos fue ligeramente superior en personas con accidente cerebrovascular (1,4 ±0,63). La mayoría informó salud regular y tenía sobrepeso u obesidad. Se concluye que los factores de riesgo para caídas deben ser continuamente evaluados por profesionales cualificados, con el objetivo de mantener condiciones favorables para la salud del anciano.

ABSTRACT

PURPOSE: The aim of this study was to assess the long-term clinical outcomes of the tension-free vaginal tape (TVT) procedure for stress urinary incontinence (SUI) in elderly women and to identify the factors influencing failure in these cases. MATERIALS AND METHODS: Women with SUI who underwent a TVT procedure were studied. "Cure" was defined as no urine leakage at all in any circumstances and "improvement" was defined as some urine leakage but a score of over 4 points out of 5 in a satisfaction inquiry. Patients were divided into two groups (middle-aged, or =65 years) for comparison of clinical outcomes. In the elderly group, patients were subdivided into two groups (cure and no cure groups) and were compared to identify the factors influencing failure. RESULTS: A total of 136 women (middle-aged group, 106; elderly group, 30) were enrolled in the study. The mean ages of the patients in the 2 groups were 53.5+/-5.9 and 72.0+/-5.0 years and the mean follow-up times were 50.5+/-9.4 and 48.8+/-9.1 months, respectively. The cure and improvement rates in the middle-aged and elderly groups were 80.2% vs. 66.7% and 4.7% vs. 3.3%, respectively (p>0.05). The satisfaction scores in the middle-aged and elderly groups were 3.8+/-1.1 vs. 3.3+/-1.5 points (p>0.05). In the elderly group, the body mass index of the cure and no cure groups were 24.6+/-3.3 kg/m2 and 26.6+/-1.0 kg/m2, and body mass index was the only factor that differed significantly between the two subgroups (p=0.028). CONCLUSIONS: Our long-term results suggest that TVT is an effective treatment even in elderly women. However, elderly women who are obese should be counseled carefully about the success rate.

ABSTRACT

Pesquisas recentes demonstram que a população mundial de idosos está em crescimento e que o avanço tecnológico tem proporcionado uma melhoria da qualidade de vida dessas pessoas, ocasionando mudanças comportamentais dos mesmos, os quais passaram a adotar um estilo de vida mais ativo. Contudo esses fatores, tem levado essas pessoas a uma maior exposição a agentes agressores, fato constatado pelo aumento do número de idosos que sofreram algum tipo de traumatismo. Tem-se observado o aumento gradativo desses traumas decorrentes de agressões físicas, fato compatível com a atualidade do sistema socioeconômico desigual e com fatores de exclusão social. A escolha do tratamento dos traumatismos em idosos deverá levar em consideração o paciente em particular e não apenas a idade, podendo ser realizada tanto por métodos conservadores quanto por técnicas cirúrgicas invasivas, considerando-se os aspectos risco-benefício. Este trabalho tem por objetivo relatar um caso de trauma facial em uma paciente geriátrica, vítima de agressão física, enfocando os aspectos clínicos e conduta frente a tal caso.


Recent researches show of mondial elderly population and the technologic advancement of medicine has brought a better quality of life combined with many seniors citizens leading more active lifestyle. However, these factors make them more susceptible of damage agents. It is a fact confirmed by the increased number of eldery that suffered traumatisms. It has been observed a great number of these traumatisms by physical aggressions and compatible with socioeconomic system desfavorable and social exclusion. The choice of management in elderly has to take in consideration the medical condition of the patient and not only the age and treatable with both surgical measures and more conservation approaches, considering risk-benefit. This paper objectives report a case of facial trauma in a elderly victim of physical aggression, focusing clinical aspects and the conducts taken.

ABSTRACT

Objetivo: Devido a crescente e maior expectativa do nú­mero de idosos, e conceituando-se aqueles da 4a idade, ou seja, após os 80 anos, decidimos pesquisar as queixas dermatológi­cas por consulta espontânea, no Serviço de Dermatologia do Hospital do Servidor Público Estadual de São Paulo. Métodos: Foram selecionados dois consultórios médicos para o registro das queixas e diagnóstico dos pacientes, pelo período de um ano. Resultado: Foram examinados 210 pacientes, sendo a proporção de duas mulheres para um homem, com média de idade de 84 anos (variação de 80 a 97 anos). Os resultados se rela­cionaram à predominância das lesões pré-cancerosas e câncer de pele. Conclusão: Após análise dos resultados consideramos que estes pacientes necessitam de uma conduta adequada e reco­mendações terapêuticas especiais.

ABSTRACT

PURPOSE: This study was conducted to compare the level of cognitive and physical function by the level of social disengagement in institutions and community dwelling elderly. METHOD: The study subjects were 209 people whose age ranged from 65 to 94. The data were surveyed with social disengagement index, MMSE-K, and ADL, and analysed by SPSS 11.0 using frequency, t-test, ANCOVA and ANOVA. RESULTS: 1) The level of social disengagement was 1.67 in institution and 3.25 in community elderly, and showed statistically significant difference (F=41.07, p<.001). 2) There were statistically significant difference in MMSE-K (F=15.51, p<.000), and activity of daily living (F=9.65, p=.002) between two residence after control the age, gender, religion, allowance and number of children. 3) There were statistically significant difference in MMSE-K (F=6.55, p=.002) by social disengagement in community dwelling elderly. CONCLUSIONS: Programs for successful aging to reduce social disengagement should be developed that stimulate social activities and then improve cognitive function for the elderly. For these, establishing social policies in nation and local government is needed. Especially, these policies should be conducted among the elderly with low-income living in institutions.

ABSTRACT

The prevalence of undernutrition in hospital populations is known to be high. The presence of malnutrition is associated with depression, infections, sarcopaenia, falls, fractures, reduced autonomy and increased mortality. This study specifically examined the prevalence of malnutrition in patients aged 65 or older at the time of admission as determined by the Mini Nutritional Assessment (MNA) which has been a frequently used nutritional risk screening tools in detecting undernutrition in old people. This study was done for one hundred eight hospitalized geriatric patients in Seoul National University Bundang Hospital, Seoul, Korea. On admission baseline history, anthropometrics measurements, laboratory data and nutritional status by MNA were assessed. Length of hospital stay was obtained by reviewing medical charts. We used one-way analysis of variance to compare the differences in variables. Spearman's rank correlation coefficients were calculated for associations between MNA and variables. On admission, 22.3% of patients were malnourished and 40.7% were at risk of malnutrition according to the MNA. Percent of ideal body weight, anthropometrics data, albumin, and hemoglobin were lower in the malnourished patients (p < 0.05). The malnourished patients stayed in the hospital 7.3 days longer, as compared with well nourished patients (p < 0.05). Percent of ideal body weight, albumin, hemoglobin and total cholesterol were correlated inversely with nutritional status according to MNA (p < 0.05). MNA can be used for nutritional assessment in Korean old people, because MNA significantly correlated with other nutritional assessment parameters, such as, anthropometric and laboratory data in hospitalized geriatric patients. The high prevalence of malnutrition in the elderly was observed and the presence of malnutrition on admission predicted a significant increase in the length of hospital stay in this study. Therefore further studies are needed to determine whether nutritional interventions in old people with low MNA scores can improve clinical outcomes during the hospital course.

ABSTRACT

PURPOSE: In order to improve self-management of elderly hypertensives in this study, the researcher examined the effect of blood pressure regulation by an abdominal relaxed breathing training which regulates physical response to stress and lessens the activity of the sympathetic nerve. METHOD: A quasi-experimental pre-test and post-test design was used. Thirty elderly hypertensive patient ; Sixteen in the abdominal breathing group and fourteen in the control group participated in this study. The abdominal relaxed breathing training consisted of eight sessions twice a week for four weeks. There was no intervention for the control group. RESULT: 1. There was a significant decrease in systolic and diastolic blood pressure between the experimental group and control group over three different times, and interaction by groups and over time. 2. There was no significant difference in the level of total stress response, physical stress response, behavioral-cognitive stress response between the experimental group and control group over three different times, and interaction by groups and over time. CONCLUSION: Findings indicate that this study will contribute to develop nursing strategies for the regulation of blood pressure for the elderly, which is easy for the elderly to learn as a nonpharmacologic approach.

ABSTRACT

Kerion celsi, a severe inflammatory type of tinea capitis, which occurs chiefly in children between the ages of 4 and 14 years. We report a case of kerion celsi occuring in an elderly woman. She is a 67-year old wonman, presented with inflammatory, boggy mass studded with broken hairs, and oozing purulent materials from follicular orifices on the parietal scalp, which had been extending over almost entire scalp for 2 weeks. Kerin celsi caused by M. canin with atypical microscopic findings of the colony on the Sabouraud's dextrose agar was diagnosed by PCR analysis using PAPD method. This case is reported with two interesting facts: Kerion celsi is occuring rarely in old persons; Clinically kerion celsi progessed rapidly after onset in this case.

ABSTRACT

Purpose:To study the surgical treatment of the cases with cancer of esophageal and gastroesophageal conjunction by thoracotomy. Methods:Total of 43 patients were selected. Squamous cell carcinoma of esophagus were found in 26 patients,among them 23 received radical resection and 3 failed due to massive invasion of the tumor.The rest were diagnosed as adeno carcinoma of cardia,13 of them undergoing radical resection and 4 failed.Results:The results of the whole group were surgical resection 93.0 %,complication 41.9% and operative mortality 4.7%.Conclusions:The authors conclude that selecting patients with precise indications as well as treating complication correctly was very important for surgical procedures of elderly patients.The pre-operative preparation,nutrition and approach for resection are also important.
