ABSTRACT

Objective To know the effects of electronic medical records(EMR) on the training quality of intern doctors' history-collecting and medical-records writing. Methods 195 students from Nantong University,Suzhou University and Xuzhou medical college were chosen as the subjects. For better effects,we divided the 195 students into two groups,81 students of Grade 2007 as the control group(hand-written medical record group) and 114 students of Grade 2008 as study group (electronic medical record group). The control group was arranged to complete EMR by transcribing the teacher's handwriting records into formal medical records after teachers' guidance and modification while the study group was arranged to write and directly modify the records on computer under the guidance of teachers and submit them for formal records. After the teaching, the medical record writing quality of two groups of interns was conducted centralized assessment to analyze the advantages and disadvantages of electronic medical records on the training of interns' collection and writing ability.SPSS 18.0 was used for statistical analysis, with count data using the number(percentage) and measurement data using mean +/-standard deviation.Measurement data of two groups of medical record writing scores were compared with t test, test level for α=0.05.Results The result of handwritten group was significantly better than EMR group(t=6.33,P<0.001),which indicated training effectiveness of intern doctors' ability of writing medical record had digressive tendency on EMR group. Conclusion Negative effects of EMR are shown in intern doctors' history-collecting and medical-records writing.Cultivation quality of intern doctors' history-collecting and medical-records writing should be ensured by the combination of EMR and handwriting-medical training, the combination of daily inspection and key examination of medical record writing quality and establishment of feedback-rectification mechanism.

ABSTRACT

The paper explored the online real-time system built at the hospital to monitor clinical care quality using EMR.The methodology, procedures and index system were established for real-time clinical care quality control. With the online control in place, significant improvements were found in such care indexes as implementation of essential healthcare regulations,EMR quality, rational drug use and rational blood use, making quality control much efficient than before.EMR-based online clinical care quality control can improve quality of care, make it efficient and guide clinical care and quality control as well.

ABSTRACT

Objective To focus on the expressiveness and flexibility of OpenSDE: an application that supports recording of structured narrative data. Methods OpenSDE enabled data entry with (customizable) forms based on trees of medical concepts. The relevant scope for data entry could be tailored per medical domain by construction of a domain-specific tree. OpenSDE was intended for structuring narrative data to make these available for both care and research. Results The OpenSDE application was used at two departments in aspects of radiology, neurology, pediatrics, and child psychiatry. Conclusion OpenSDE is superior to traditional text entry method in aspects of correct and full expression and clinical decision support.

ABSTRACT

Objective To explore the application ways of IHE XDS technology in electronic medical record (EMR) system and develop application system of clinical EMR integration. Methods The clinical EMR integration plan was worked out based on IHE XDS technical framework. Results A EMR solution based on IHE XDS was presented. It was consisted of the components of the clinical Data Entry, XDS Registry, clinical document Repository, EMR application server and the i-node accessing server. The system was tested and the result indicated that the system was effective and scalable. Conclusion IHE XDS profile can be not only used to realize the sharing of EMR documents across multiple hospital, but also support the collection clinical data within a hospital clinical system integration.

ABSTRACT

Objective To further develop SDE for structured documentation of clinical data based on medical ontology. Methods OpenSDE was customized for the broad domain of medical ontology: medical concepts and its descriptors from history taking and physical examination were modeled into a tree structure. Results An expandable EMR system with structured data entry was developed. Conclusion Structured data entry is significant to clinical decision based on EMR.
