ABSTRACT

Background: The hospital Emergency department (ED) is one of the most important components of the health delivery system. Objectives: To investigate the public awareness of the ERs in KSA, what the public knows about the provided services, and if they know the difference between the outpatient clinic and ERs. Methods: It is a cross-sectional descriptive community-based study carried out on 977 male and female, young and adult participants from all age groups, in different areas of the Kingdom of Saudi Arabia, during the period from 1st January to 31st July 2019. Data was collected through filling the pre-designed online questionnaire which guided us to the needed data. We utilized the SPSS program version 16. The X2 test was used as a test of significance, and differences considered significant at p-value less than 0.05. Results: Most of the participants (87.5%) reported that they know the difference between the outpatient clinic and ER. The majority (68.1%) of subjects said that ERs is meaning rapid and unplanned medical care, 17.3% said any needed health care is available there, 12.2% said that it means insufficient medical care and only 2.5% said it means availability of physician at any time for any purpose. As regards evaluation to the provided services in ERs; 32.5% of cases said it was very good followed by 28.5% good, 19.8% excellent, 10.2% accepted and 10% reported it was bad services. There were significant relations between the awareness and age (p=0.03) and education level (p=0.003), but no relation was found with the gender of the participant (p>0.5). Conclusion: In our study, Most of the participants reported that they know the difference between the outpatient clinic and ERs. The majority of subjects said that ERs is meaning rapid and unplanned medical care and/or availability of physician at any time for any purpose. There were significant relations between the awareness and age and education level, but insignificant relation was found with the gender of the participant.

Resumen Justificación: ante ciertos desafíos sobre la competitividad de los servicios públicos, resulta crucial mejorar la calidad de la salud. Objetivos: (1) caracterizar el nivel de calidad de servicio en la demanda de urgencias hospitalarias, a partir de la gestión lean seis sigma; y (2) evidenciar la relevancia de ciertos indicadores clave de gestión en la unidad de urgencias de un hospital público. Método: a partir de una propuesta de variables lean-seis-sigma derivadas de la escala SERVQHOS, se aplica una encuesta sobre las percepciones de los usuarios de un servicio de urgencias en un hospital público chileno y se identifican y evalúan sus principales indicadores de gestión. Resultados: enfatizar la importancia de intervenir sobre ciertas variables relevantes en diferentes niveles de actuación de una unidad hospitalaria. Aportación: ilustrar una propuesta para mejorar la atención, enfocándose en la disminución de tiempos y errores en el servicio y agregando valor al paciente.


Abstract Justification: Regarding some challenges of the public services competitiveness, it is crucial to enhance the health quality. Objectives: (1) To characterize the levels of service quality in the emergency rooms based on Lean Six Sigma; and (2) to show the relevance of some key management indicators in the emergency room of a public hospital. Methods: Based on a proposal of Lean- Six-Sigma variables derived from the SERVQHOS scale, a survey was applied to the users regarding their perception about the emergency room services in a Chilean public hospital. The main management indicators thereof were identified and evaluated. Results: To emphasize how it is important to make interventions concerning some relevant variables at different action levels in a hospital Ward. Contribution: To illustrate a proposal to improve the health care, focused on reducing the service times and mistakes and adding values to the patient.


Resumo Justificação: perante certos desafios relacionados com a competitividade dos serviços públicos, resulta crucial melhorar a qualidade da saúde. Objetivos: (1) caracterizar o nível de qualidade do serviço na demanda de urgências hospitalares, a partir da gestão lean- seis-sigma; e (2) evidenciar a relevância de certos indicadores chave de gestão na unidade de urgências de um hospital público. Método: a partir de uma proposta de variáveis lean-seis-sigma derivadas da escala SERVQHOS, aplica-se um inquérito sobre as percepções dos usuários de um serviço de urgências num hospital público chileno e são identificados e avaliados seus principais indicadores de gestão. Resultados: enfatizar a importância de intervir sobre certas variáveis relevantes em diferentes níveis de atuação de uma unidade hospitalar. Contribuição: ilustrar uma proposta para melhorar o atendimento, a focar na diminuição de tempos e erros no serviço e adicionando valor ao paciente.

ABSTRACT

Investigación cuantitativa, descriptiva y correlacional cuyo objetivo fue determinar la presencia de estrés laboral crónico o burnout y su probable relación con factores sociales y laborales, en los profesionales de enfermería que trabajan en Unidades de Emergencias (UE) y Servicios de Atención Médica de Urgencia (SAMU) de la Octava Región, Chile. Se trabajó con el universo conformado por 91 enfermeras(os), siendo 57 de UE y 34 de SAMU. Se utilizaron dos instrumentos recolectores de datos; el primero elaborado por la autora, que recogió información sobre variables sociodemográficas y laborales. El segundo correspondió a la Escala de Maslach Burnout Inventory (MBI); que midió el desgaste profesional producto del estrés laboral crónico y los tres aspectos del síndrome de Burnout, que corresponden a cansancio emocional, despersonalización y realización profesional. Los resultados más relevantes del estudio mostraron que más del 50 por ciento de estos profesionales corresponden a adultos jóvenes, solteros(as) y sin hijos; presentando menos de 10 años de experiencia laboral, la mayoría realiza cuarto turno. El grupo de profesionales evidenció presentar burnout en una intensidad intermedia, que estaría principalmente influenciado por variables laborales como la percepción de recursos insuficientes y la realización de exceso de turnos, al mismo tiempo el grupo de mayor edad evidenció más cansancio emocional y los viudos o separados presentaron menos despersonalización que el resto de los encuestados.


This cross sectional study is a correlational research whose principal objective was determinating the presence of chronic labour stress or burnout and its probably relationships with labour and social factors of nursing professionals working in Emergency Rooms and UMAS (Urgency Medical Attention Service) at the Eighth Región of Chile. We work with the total universe constituted by ninety-one nursing professionals: fifty-seven nurses or male nurses of emergency unities and thirty-four of UMAS. We utilized two instruments to collect the information. The first one was made by the author, and their purposes were collecting information about socio-demographical, and labors variables. The second questionnaire correspond to the Maslach Burnout Inventory (MBI) that measures emotional tiredness, dispersonalization, and professional fullfiment between nursing professionals as an outcome of stress at the working conditions. The most outstanding results of this study showed that more than 50 percent of nursing professional working in the crash unity are single young-adults, without children, and having not more than 10 years of work experience; most of them often working on night-turn. These ER and UMAS group of nursing professionals displayed middle-intensity burnout; the principal influence acting on this illness coming from work variables, like the perception of insufficient resources in the unities and the excess of night turns; The older group showed more emotional tiredness while separated and widowers professional presented less depersonalization than the rest of these professionals.

ABSTRACT

To determine the causes of and related factors to childhood injuries, the emergency room records and inpatient medical records were reviewed for 4,849 injured children out of 15,790 pediatric patients(<15 years old) who visited the emergency rooms of 3 university hospitals and 2 general hospitals in Taegu from 1 January to 31 December 1987. Out of total injured children, 54.6% were 3-8 years old and the male to female ratio of the total injured children was about 2 : 1. The leading causes of injury were falls and slips (29.1%) and traffic accident(28.2%). The frequency of injury was higher in May-October than the rest of months and 51.6% of the injuries occurred between 15 and 20 o'clock. Falls and slips took place most frequently at the stairway(25.7%). The most common interpersonal violence was inflicted injuries(85.6%) and there were 11 child rapes. Dog bites accounted for 67.6% of all biting injuries and it occurred 2.9 times more in male than in female. CO intoxication was the most common cause of poisoning (45.3%) and scalding accounted for 85.2% of all burns. Common places of drownings were river (32.2%), swimming pool (22.6%) and construction site(19.3%). To prevent childhood injuries, it is recommended to eliminate the hazardous environmental factors, to provide safe playgrounds, to educate the children for safety from kindergarten and the general public through mass communication, to establish a strict safety standard for houses, public buildings and facilities, and playgrounds.
