ABSTRACT

Background: Ovarian cancer is the leading cause of cancer related death amongst women in India. Identification of histological types helps to predict tumor behaviour and further appropriate management. Aims and objectives is to study the histopathological parameters of ovarian tumor.Methods: This retrospective study was done on patients who presented ovarian mass and subsequently underwent surgery in a Regional Cancer Centre, Odisha, over a period of three years from January 2016 to December 2018.  All datas such as age, site, gross findings and histological tumor types were retrieved from pathology and analyzed using MS Excel worksheet.Results: A total 612 cases of ovarian tumor were included for study. Non-neoplastic to neoplastic tumor ratio was 1:7.74. Surface epithelial tumors comprised the majority of tumors, accounting for 452 cases (83.39%). Malignant lesions were predominant in this series 416 cases (76%). Majority of borderline tumors were of mucinous subtype 20 (76.92%). The Mean±SD ages of all benign comprising, borderline and malignant tumors were 47.4±11.9, 44.9±14.3 and 46.9±13, respectively. On the basis of two tired grading system, high grade malignant serous tumors were maximum, 226 (74.34%). Ovarian surface involvement, omental invasion, uterine invasion, LVSI, capsular invasion and pelvic lymph node involvement was observed in 146 (35.26%), 106 (25.6%), 12 (2.89%), 70 (16.9%), 6 (1.44%) and 12 (2.89%) respectively. According to the FIGO staging system, among primary malignant tumor, 58% patients were presented in late stage (III and IV).Conclusions: The high incidence of malignant ovarian tumor with late presentation was observed in our study. So, further study is warranted to elucidate the major factors in our population.

ABSTRACT

Se realizó un estudio descriptivo, retrospectivo y transversal de 87 pacientes con tumores cutáneos no melanoma, atendidos en el Servicio de Dermatología del Hospital Clinicoquirúrgico Docente Dr Joaquín Castillo Duany de Santiago de Cuba, desde enero de 2012 hasta igual mes de 2014 con el objetivo de caracterizarles según variables clínicas y epidemiológicas de interés para la investigación. La información se recogió de las historias clínicas y de los informes de anatomía patológica. Como medidas de resumen para variables cuantitativas y cualitativas se utilizaron la media, la frecuencia absoluta y la relativa. En la serie predominaron el carcinoma basocelular (60,0 por ciento), el sexo masculino, los pacientes de piel blanca y los mayores de 50 años de edad; asimismo, la cabeza resultó ser la región topográfica más afectada y la forma clínica nodular la más frecuente. Según la ocupación, los agricultores y los albañiles estuvieron mayormente relacionados con este tipo de cáncer de piel.


A descriptive, retrospective and cross-sectional study of 87 patients with non melanoma cutaneous tumors, assisted in the Dermatology Service of Dr Joaquín Castillo Duany Clinical Surgical Hospital in Santiago de Cuba was carried out from January, 2012 to the same month of 2014 with the objective of characterizing them according to clinical and epidemiological variables of interest for the investigation. The information was collected from the medical records and from the pathology reports. For quantitative and qualitative variables, means, absolute frequency and the relative frequency were used as summary measures. In the series, the basocelular carcinoma (60,0 percent), male sex, the white skin patients and those older than 50 years prevailed; also, the head turned to be the most affected topographical region and the most frequent nodular clinical type. According to the occupation, farmers and bricklayers were mostly related to this type of skin cancer.

ABSTRACT

Thymic epithelial tumors (TETs) are the most common tumors in the anterior mediastinum, and the staging system and histological classification are complex. CT is the most important imaging examination for TETs. MRI has a certain value in the diagnosis of TETs. PET/CT in classification of TETs is progressing rapidly, but the results are controversial. The research progresses of imaging examinations on staging and typing of TETs were reviewed in this article.

ABSTRACT

OBJECTIVE: To investigate the value of HE4,CA125 and ovarian malignant tumor risk prediction models(ROMA)in the diagnosis of ovarian epithelial malignant tumors.METHODS: The clinical and pathological data of 247 patients with ovarian tumors(EOC 139 cases,BOT 18 cases,and benign ovarian tumor 90 cases)and 39 patients with uterine fibroids in Liaoning Tumor Hospital from September 2016 to August 2017 were retrospectively analyzed. The levels of serum CA125 and HE4 were measured before operation. The ROMA values were calculated and the relationship between CA125,HE4,ROMA values and clinical pathological parameters were analyzed. The diagnostic evaluation index was calculated,the receiver operating characteristic(ROC)curve was drawn,and the AUC value was also calculated.RESULTS: The positive rate of HE4 in the ovarian epithelial malignant tumor group was significantly higher than that in other groups before and after menopause,the difference being statistically significant(P0.05).There was no significant statistical difference in the positive rate of ROMA before or after menopause(P>0.05).The sensitivity of CA125 was higher than that of HE4 and ROMA. Specificity of HE4 was higher than that of CA125 and ROMA.Correct diagnosis index of ROMA was higher than that of HE4 and CA125.CONCLUSION: For the diagnosis of ovarian malignant epithelial tumors,the combined detection of serum HE4 and CA125 and ROMA model is superior to the individual detection of HE4 and CA125.

Objective To analyze the MSCT features of subepithelial lesions of the urinary bladder.Methods Thirty-one patients with subepithelial lesions of the urinary bladder proved by pathology were enrolled in this study.The MSCT findings were analyzed. Results Among the 3 1 cases with subpeithelial lesions of the urinary bladder,7 cases were non-epithelial tumors (including 3 cases of leiomyomas,2 cases of paragangliomas,1 case of hemangioma and 1 case of granulocytic sarcoma),and other 24 cases were inflammatory/reactive lesions,all as cystitis glandularis.MSCT findings were as follows:paragangliomas were solitary masses with homogeneous density,smooth border and wide base,with rapid wash-in and wash-out enhancement pattern and peak enhancement in the arterial phase.Hemangiomas and leiomyomas both presented as single round mass with slight enhancement.Granulocytic sarcoma appeared as diffuse bladder wall thickening with pebble-like projections,and slight enhancement.Cystitis glandularis was characterized by local thickening of the bladder wall with nodular or aquatics-like uplift,and was also characterized by diffuse thickening,with slight and progressive enhancement in 22 cases,and significant enhancement in venous phase in 2 cases.Conclusion The subepithelial lesions of urinary bladder may present with tumor-like appearance.The key characteristic feature for cystitis glandularis is local thickening of the bladder wall with nodular or aquatics-like uplift.Rapid wash-in and wash-out enhancement pattern is considered to be diagnostically significant for paragangliomas.However,the rest of the lesions overlap in the imaging appearances,and the diagnosis should be combined with clinical manifestations to improve the accuracy of preoperative diagnosis.

ABSTRACT

Objective To investigate the value of CT and MRI in the differential diagnosis of various subtypes malignant epithelial tumors of the ovary,to improve the understanding of imaging manifestations of ovarian cancer.Methods Fifty four cases with malignant epithelial tumors of the ovary confirmed by clinical operation and pathology were included in this study and preoperative imaging examinations were analyzed retrospectively.Results Thirty four cases were diagnosed as ovary cystadenoma,including 26 cases of serous cystadenoma (48%),8 cases of mucous cystadenoma (14%),10 cases of endometrioid adenocarcinoma (19%)and 10 cases of clear-cell carcinoma (19%).All the tumors appeared as unilocular or multilocular cystic-solid masses,however some differences existed among tumors in calcification in masses,size of solid nodules,locular appearance,with or without associated massive ascites, and adjacent structures involvement .Calcification occurred in 5 cases of cystadenocarcinoma,and no calcification was found in endometrioid adenocarcinoma or clear-cell carcinoma.The proportion of solid component in clear-cell carcinoma was lower,usually presenting as intralumimal nodular protuberance.Endometrioid adenocarcinoma was often associated with endometrial hyperplasia or endometrial carcinoma and was the most prone to peripheral invasion and adhesion.Conclusion The different subtypes of ovary malignant epithelium-derived tumor have different characteristics of multilocular,the size of cyst wall nodule or solid nodule and the calcification.Compare and analysis of these characteristics help us to make a more accurate preoperative diagnosis.

ABSTRACT

Objective To investigate the clinical and CT features of non-epithelial malignant tumors of bladder to improve the understanding.Methods Clinical and CT data of 1 1 patients with non-epithelial malignant tumors of bladder were analyzed retrospectively. All patients underwent plain and contrast enhanced CT scan,and confirmed by surgery and pathology.Results Of 1 1 patients,there were 4 rhabdomyosarcomas,3 cancerous sarcomas,2 leiomyosarcomas and 2 small cell carcinomas.The CT performance were mostly cauliflower like or papillary,with wide basis and thickening of adjacent bladder wall.Diffuse thickening of the bladder wall was detected in all rhabdomyosarcoma lesions.The lesions were homogeneous or heterogeneous soft tissue-like density.CT showed calcification in 1 cancerous sarcoma,necrosis in 1 leiomyosarcoma and 1 rhabdomyosarcoma.Contrast enhanced scan showed mild to moderate or significant enhancement.Conclusion Besides the age of rhabdomyosarcoma,other non-epithelial malignant tumors of bladder have no special clinical and CT characteristics.CT examination can provide the information about location,size,surrounding tissue change,which is helpful to make the treatment plan.

ABSTRACT

Undifferentiated carcinoma of the pancreas is a malignant neoplasm that is uncommon among domestic species, especially cockatiels (Nymphicus hollandicus), one of the most popular birds kept as a pet throughout the world. The aim of this study was to describe the occurrence of an undifferentiated carcinoma in the pancreas of a cockatiel. A bird, an adult male that died naturally with swelling in the abdominal region, was referred to necropsy. Macroscopic examination showed poor body condition, the coelomic cavity filled with liquid and a white mass attached to the pancreas and other smaller masses attached to the duodenum. Tissue samples and organs were harvested and fixed in 10% buffered formalin, then routinely processed for histopathology and stained with hematoxylin and eosin. Microscopic analysis demonstrated an epithelial neoplasia with a predominantly solid pattern, lymphatic invasion and involvement of the intestinal serous membrane. These findings indicate the occurrence of an undifferentiated pancreatic carcinoma in a cockatiel that was diagnosed by histopathology.(AU)


O carcinoma indiferenciado de pâncreas é uma neoplasia maligna, incomum entre as espécies domésticas, especialmente em calopsitas (Nymphicus hollandicus), uma das aves mais populares como animal de companhia no mundo. O objetivo deste trabalho foi descrever a ocorrência de carcinoma indiferenciado de pâncreas em Nymphicus hollandicus. Uma ave, macho adulto, com morte natural e com aumento de volume em região abdominal, foi encaminhada para necropsia. Ao exame macroscópico foram observados mau estado corporal, cavidade celômica repleta de líquido e massa esbranquiçada aderida ao pâncreas e outra menor aderida ao duodeno. Amostras de tecidos e órgãos foram colhidas em formol 10% tamponado, processadas rotineiramente para histopatologia e coradas por hematoxilina e eosina. Na microscopia foi observada neoplasia epitelial com padrão predominantemente sólido, com invasão linfática e implantação na serosa intestinal. Com esses achados, comprovou-se a ocorrência de carcinoma indiferenciado de pâncreas em Nymphicus hollandicus, o qual pode ser diagnosticado por meio de histopatologia.(AU)

Objective To evaluate the diagnostic value of MSCT in the differentiation of thymic epithelial tumours (TET)with the maximum diameter equal or less than 3 cm.Methods A retrospective analysis of pathological and imaging data of 56 patients with pathologically confirmed TET with the maximum diameter equal or less than 3 cm was performed.According to the 2004 WHO classification,56 TETs were classified as low-risk thymomas(types A/AB/B1),high-risk thymomas (types B2/B3)and thymic carcinomas (type C).The CT manifestations of TET in each group,including shape of tumor,tumor edge (smooth or spiculate protuberance), presence of small nodule around tumor,enhancement degree,pleura invasion and fat space around tumor,were analyzed retrospectively.The differences in the CT manifestations among three types were compared using chi-square test.If the sample number was too small, Fisher 's exact test was used.Results Compared with high-risk thymomas (23 cases)and thymic carcinomas (6 cases),regular round shape was more often observed in low-risk thymomas (27 cases)(χ2 =73,P <0.001;χ2 =116,P <0.001)and the mediastinum-lung interface was more likely to bulge (χ2 = 3.41,P =0.046;χ2 =7.39,P =0.01).Blurred edge,spiculate protuberance and pleural invasion and so on were significantly more common in high-risk thymomas and thymic carcinomas (P <0.001)and they were the most common in thymic carcinomas (χ2 =11.5,P =0.009).There was a significant difference between type B2 thymomas and thymic carcinomas (χ2 =31.52, P <0.001),however there was no significant difference between type B3 thymomas and thymic carcinomas (χ2 =6.96,P =0.07). Conclusion MSCT can accurately show the shape of tumor,tumor edge,presence of small nodule around tumor,enhancement degree,pleura invasion,which can predict the histologic type of thymomas and also can provide information for preoperative diagnosis and prognosis evaluation.

ABSTRACT

Objective To improve the imaging diagnosis and differential diagnosis of non-epithelial ovarian tumor.Methods 34 patients with non-epithelial ovarian tumor confirmed by pathology were analyzed retrospectively.The image features of each patients were analyzed and compared with surgical pathology.Results The results exhibited 13 cases of teratoma,6 cases of ovarian granulosa cell tumor, 6 cases of ovarian fibroma, 5 cases of thecoma, 2 cases of dysgerminoma,and 2 cases of endodermal sinus tumor.All kinds of non-epithelial ovarian tumors had its imaging manifestations and clinical features.Conclusion Non-epithelial ovarian tumors have certain characteristic CT and MR performance,and combining the onset age,clinical symptoms and biochemical examination,can make the qualitative diagnosis.

Objective To investigate the clinical and pathological diagnosis of cutaneous calcifying epithelioma.Methods Thirty cases of calcified epithelioma were diagnosed and treated by our hospital.The clinical features and pathological features were retrospectively reviewed.Results The main pathological features of the tumor were that the main tumor (basophilic cells) basal cells and shadow cells were mixed composition,combined with ossification and calcification and with no recurrence after surgical resection.Conclusion The clinical manifestations of calcifying epithelium are lack of characteristics and diverse pathological features.It is necessary to make the correct pathological diagnosis with the time,location and imaging of tumor growth.

ABSTRACT

BACKGROUND/AIMS: Dual-focus two-stage optical lens technology has been introduced recently. In near-focus mode (NFM), endoscopists can easily examine the mucosal tissue and capillary networks. This study compared the magnified images obtained using NFM and the conventional magnification method (CMM) under narrow-band imaging in patients with gastric epithelial tumors. MATERIALS AND METHODS: An experienced endoscopist performed endoscopy using NFM and CMM in patients with gastric epithelial tumors. We studied 40 images from 40 endoscopy sessions in 20 selected patients. Ten endoscopists rated the image quality independently on a 5-point Likert scale (from poor=1 to excellent=5) in terms of microsurface structure, microvascular structure, and the demarcation line. RESULTS: The gastric epithelial tumors comprised 10 cases of early gastric cancer, 2 of high-grade dysplasia, and 8 of low-grade dysplasia. The median number of magnified images for each method was 11. The mean observation time (+/-SD) for magnification was 99.9+/-64.1 s in NFM and 91.5+/-64.6 s in CMM (P=0.54). The image quality score for the microsurface structure was higher with NFM than CMM (4.09+/-0.39 vs. 3.73+/-0.40, P=0.015), while that for microvascular structure was lower with NFM than in CMM (3.53+/-0.45 vs. 4.29+/-0.45, P=0.001). CONCLUSIONS: Magnification using NFM provides higher-quality images of the microsurface structure, although its optical zoom is limited compared with CMM. Since NFM can obtain magnified images easily by pushing a button on the scope, it is useful for evaluating gastric epithelial tumors.

ABSTRACT

Nested stromal and epithelial tumor of the liver is an extremely rare pediatric hepatic tumor. To the best of our knowledge, about 25 cases have been reported in the English literature so far, few of which accompanied with Cushing syndrome. Herein we report our experience with an 8-year-old boy presented with Cushing's syndrome because of ectopic ACTH production by this tumor.

ABSTRACT

The Korean Standard Classification of Diseases (KCD), which reflects the International Classification of Diseases (ICD), is a fundamental coding system for the diagnosis of colorectal epithelial tumors. The KCD coding of colorectal lesions is entirely up to the clinician and is based on pathologic reports. However, coding discrepancies have arisen among physicians using the KCD and pathologists using the ICD for Oncology-3 (ICD-O-3). The Korean Society of Pathologists recently proposed a standardized pathology-reporting format and guidelines for the coding of colorectal cancer to decrease these discrepancies among pathologists. However, ICD and ICD-O are simple classification codes based on pathologic reports, and are neither intended nor suitable for indexing of distinct clinical entities. For appropriate diagnostic coding using the KCD, a corrected coding principle based upon pathologic reports is required, and unified coding between KCD and ICD-O is necessary. A standardized pathologic report format and communication with understanding between physicians and pathologists should be established. Additionally, the private medical insurance system for colorectal cancer should be revised to reduce conflicts among patients, clinicians, and insurance companies over the medical coding system.

ABSTRACT

Objective To determine the maintenance and loss of Epstein-Barr virus (EBV) genome during the clonal expansion of the EBV-infected epithelial cells. Methods The epithelial tumor cell line, 293-EBV, in which the EBV genome was observed with green fluorescent protein (GFP) readout. After a dozen of passages, it contained cells with strong or weak GFP expression, and some with complete loss of EBV genome. The cell growth was then continuously observed under a confocal microscope. The cell dividing and GFP expression were also observed during the clonal expansion by being made into very low density. Results The cells moved around due to adherence and mobility, while the GFP expression remained unchanged in the undivided cells. The cells could form compact or loosen clones. The EBV genome easily persisted in those clones when cells were growing compactly. As the cell number increased, the GFP expression became weak or even died away at the sites of low density in the loosen clones. Conclusion EBV-positive epithelial cells are able to sustain the EBV genome during its clonal expansion. The cells maintain EBV genomes by passing them to the daughter cells after replication. When the cells unsuccessfully inherit the EBV genome, the daughter cells may lose them which is related to the low cell density as well as the epithelial environment.

ABSTRACT

Spindle epithelial tumor with thymus-like differentiation (SETTLE) is a rare malignant thyroid tumor showing thymic or related branchial pouch differentiation. The tumors are composed predominantly of spindle cells along with focal epithelial component and ductular formations. SETTLE occurs in young patients, with indolent growth and a tendency to develop delayed blood-borne metastases. We herein report two cases of SETTLE with a follow-up period of 64 months and 30 months, respectively.

In the continuous search for earlier diagnosis and improved therapeutic modalities against cancer, based on our constantly increasing knowledge of cancer biology, aptamers hold the promise to expand on current antibody success, but overcoming some of the problems faced with antibodies as therapeutic or delivery agents in cancer. However, as the first aptamer reached the market as an inhibitor against angiogenesis for the treatment of macular degeneration, aptamers have found only limited applications or interest in oncology, and even less as radiopharmaceuticals for diagnostic imaging and targeted radiotherapy of tumours. Yet, the chemistry for the labelling of aptamers and the options to alter their pharmacokinetic properties, to make them suitable for use as radiopharmaceuticals is now available and recent advances in their development can demonstrate that these molecules would make them ideal delivery vehicles for the development of targeted radiopharmaceuticals that could deliver their radiation load with accuracy to the tumour site, offering improved therapeutic properties and reduced side effects.


Na contínua busca do diagnóstico precoce e melhores modalidades terapêuticas contra o câncer, com base no constante aumento do conhecimento da biologia do câncer, aptâmeros têm a promessa de expandir o atual sucesso dos anticorpos, e superar alguns dos problemas enfrentados com os anticorpos, como agentes terapêuticos ou como agentes que chegam ao processo neoplásico. Entretanto, como o primeiro aptâmero chegou ao mercado como um inibidor da angiogênese para o tratamento da degeneração macular, aptâmeros tem encontrado apenas aplicações ou interesse limitados em oncologia, e ainda menos como radiofármacos para o diagnóstico por imagem e radioterapia direcionada para tumores. A química para a marcação de aptâmeros e as opções para alterar suas propriedades radiofarmacocinéticas, para torná-los mais adequados para uso como radiofármacos, é agora disponível e os avanços recentes no seu desenvolvimento podem demonstrar que essas moléculas poderiam ser ideais como veículos para o desenvolvimento de radiofármacos sítio-dirigidos que poderiam levar radiação com precisão para o tumor, oferecendo melhores propriedades terapêuticas e reduzidos efeitos indesejados.

ABSTRACT

BACKGROUND: Thymic epithelial tumors are the most common tumors affecting the anterior mediastinum. The aim of this study is to investigate clinical features of the patients who were diagnosed with thymic epithelial tumors at Pusan National University Hospital. METHODS: We retrospectively reviewed the records of thirty-seven patients who were diagnosed with thymic epithelial tumors from Jan. 1997 to Jan. 2007. The pathological classification and clinical stage of the thymic epithelial tumors were based on the WHO classification and Masaoka's staging system. A total 37 patients were enrolled: 23 were males and 14 were females, and their mean age was 51.3 years. RESULTS: Thirty patients presented symptoms at the time of diagnosis and their symptoms were as follows: chest pain (53%), dyspnea (23%), and cough (17%). Myasthenia gravis was diagnosed in five patients. With respect to the tumor staging, three cases (8%) were stage I, 10 cases (28%) were stage II, 12 cases (32%) were stage III, 6 cases (16%) were stage IVA and 6 cases (16%) were stage IVB. Twenty-four cases (67%) displayed stage III or IV disease. The pathological types according to the WHO classification were as follows: B1 (32%), C (23%), B3 (20%), B2 (16%), AB (6%) and A (3%). Twenty-four patients underwent thymothymectomy and four of these patients relapsed. Stage III or type B3 was common in the relapsed patients. Five patientsexpired. Stage IV or type B3 and C were common in the expired patients. CONCLUSION: In this study, stage III or IV disease and type B3 or C were common at the time of diagnosis and these findings might contribute to postoperative recurrence and a poor outcome.

ABSTRACT

OBJECTIVE: Phosphate and tensin homolog deleted on chromosome 10 (PTEN) is a potent tumor suppressor gene, localized to chromosome 10q23, and shows extensive homology with auxilin and tension. PTEN has variety roles involved in cell proliferation, invasion, and migration in tumorigenesis of solid tumors. In this study, the expression of the PTEN in the ovarian epithelial tumors, including benign, borderline malignancy, and adenocarcinomas was investigated. METHODS: Immunohistochemical expression of PTEN were analyzed in formalin fixed tumor tissues of 20 benign cystadenomas, 22 borderline tumors, and 49 malignant ovarian cancer. In the same tissue extracts, single nucleotide polymorphism were studied. RESULTS: Most of benign and borderline ovarian tumors revealed strong positive reaction, but a few cases showed negative reaction or weak positive reaction. In adenocarcinomas, 33% of cases was negative, and 43% was focal weakly staining, grade 1. The remainder of adenocarcinomas showed strong nuclear staining. In SNP assay, A/A allele of rs1234213 shows low frequency, but A/G allele reveals high frequency. C/C allele of rs701848 shows high frequency, and rs9651492 is not detected polymorphism. CONCLUSION: These results suggest that loss of PTEN expression is associated with tumorrigenesis of ovarian epithelial tumors, and is related with single nucleotide polymorphism.
