ABSTRACT

RESUMEN El quiste dentígero (QD) es un quiste odontogénico de malformación y origen epitelial,constituye el segundo tipo más común dentro de este grupo y abarca cerca del 20% de todos los quistes encontrados en los maxilares. Radiográficamente el QD se manifiesta como un área radiolúcida unilocular bien definida asociada a la corona de un diente no erupcionado. El QD es considerado una patología rara en niños, por lo cual se decidió presentar el caso de un paciente de 5 años de edad que ha sido atendido en el Servicio de Cirugía Maxilofacial del Hospital Pediátrico Universitario de Cienfuegos "Paquito González Cueto".El paciente fue visto en consulta inicialmente, por presentar aumento de volumen indoloro en la región retromolar inferior izquierda, que tras la exploración clínica y radiográfica sugirió la presencia de un quiste dentígero; quirúrgicamente para realizar la enucleación del mismo, obteniéndose excelentes resultados postoperatorios. En esta ocasión pudimos corroborar que por muy rara que puedan ser algunas entidades, pueden presentarse, por lo que debemos tenerlas presentes al plantear los diferentes diagnósticos diferenciales.


ABSTRACT The dentigerous cyst is an odontogenic cyst of malformation and epithelial origin, being the second most common type of cysts of this group, comprising about 20% of all cysts found in the jaws. Radiographically, it manifests as a well-defined unilocular radiolucent area associated with the crown of a non-erupted tooth. It is considered a rare pathology in children. We present the case of a 5-year-old patient who has been followed up in the Maxillofacial Surgery consultation of the "Paquito González Cueto" University Pediatric Hospital of Cienfuegos, after having undergone surgery to perform the enucleation of a dentigerous cyst.

ABSTRACT

Un-erupted maxillary incisors can have a major impact on dental and facial aesthetics and when they don’t erupt at the expected time it is prudent for the clinician to determine the etiology and formulate an appropriate treatment plan. Lasers such as the argon, diode, Nd: YAG, CO2 and erbium have enabled dentists to reduce patient stress and fear during dental treatment and hence lasers represent a phenomenal change in pediatric dentistry. This article provides an overview of the diode laser in guiding the un-erupted upper incisors into occlusion in mixed dentition.

ABSTRACT

Post-endodontic restorations are both important and challenging for clinical success in endodontically treated posterior teeth. Several options have been proposed to restore endodontically treated molars.In pediatric dentistry, restoration using conventional single crowns, especially for partially erupted molars with insufficient retentive tooth structure, has proven to be difficult. However, the endocrown presents a conservative and esthetic restorative alternative to conventional crowns with post-and-core, as it acquires additional retention within the pulp chamber. The tooth preparation consists of a circular, equigingival, butt-joint margin and a central retention cavity in the pulp chamber that helps to construct both the crown and core as a single unit.This case report describes the esthetic and conservative endocrown restorations of erupting permanent first molars with extensive coronal destruction.

ABSTRACT

ABSTRACT The teeth become very close to each other when they are crowded, but their structures remain individualized and, in this situation, the role of the epithelial rests of Malassez is fundamental to release the EGF. The concept of tensegrity is fundamental to understand the responses of tissues submitted to forces in body movements, including teeth and their stability in this process. The factors of tooth position stability in the arch - or dental tensegrity - should be considered when one plans and perform an orthodontic treatment. The direct causes of the mandibular anterior crowding are decisive to decide about the correct retainer indication: Should they be applied and indicated throughout life? Should they really be permanently used for lifetime? These aspects of the mandibular anterior crowding and their implication at the orthodontic practice will be discussed here to induct reflections and insights for new researches, as well as advances in knowledge and technology on this subject.


RESUMO Os dentes ficam muito próximos quando estão apinhados, mas suas estruturas permanecem individualizadas e, nessa situação, o papel dos restos epiteliais de Malassez é fundamental para liberar o EGF. A tensigridade é um conceito chave para compreender as respostas dos tecidos submetidos às forças nos movimentos corporais, incluindo os dentes e sua estabilidade nesse processo. Os fatores da estabilidade de posição de um dente na arcada dentária — ou tensigridade dentária — devem ser considerados quando se planeja e finaliza um caso na prática clínica ortodôntica. As causas diretas do apinhamento dentário anteroinferior são determinantes para se refletir se a contenção deve ser mesmo indicada e aplicada por toda a vida e se, necessariamente, deve ser usada de forma permanente. Esses aspectos do apinhamento dentário anteroinferior e suas implicações na prática clínica serão aqui abordados para induzir reflexões e insights de novas pesquisas, bem como avanços no conhecimento e tecnologia sobre esse assunto.

Introducción: Los dientes supernumerarios son anomalías donde hay un aumento de la fórmula dental normal. Ocasionalmente provocan complicaciones, como la formación de un quiste dentígero. Por lo infrecuente del hallazgo, y más aún del reporte de esta asociación, se realizó la presentación del caso. Objetivo: Describir el manejo de un supernumerario distomolar en rama mandibular asociado a un quiste dentígero, enfatizando en la importancia de los estudios imaginológicos preoperatorios y el abordaje quirúrgico endobucal. Presentación del caso: Paciente femenina de 34 años, que se extrajo los terceros molares en la adolescencia, y que 3 meses atrás presentó una celulitis facial por la que se le realizó una ortopantomografía, y se detectó un molar en la rama mandibular. Al examen bucal se encontró ligera expansión de la tabla vestibular de rama mandibular izquierda. Se constató en la ortopantomografía la presencia de una estructura dentaria del grupo molar asociada a área radiolúcida, lo que se corroboró en una tomografía. Se planificó la exéresis de ambos mediante abordaje endobucal bajo anestesia general. No se presentaron complicaciones. La paciente fue egresada a las 24 horas y fue seguida evolutivamente por consulta externa durante 6 meses. El diagnóstico histopatológico indicó la presencia de un quiste. Conclusiones: La detección de un diente supernumerio distomolar en rama mandibular asociado a quiste dentígero es una situación clínica infrecuente que permite explotar los beneficios de estudios imaginológicos digitales y crear un plan de tratamiento que culmine en la exéresis exitosa por vía endobucal, sin complicaciones(AU)


Introduction: Supernumerary teeth are anomalies in which there is an increase of the normal dental formula. They occasionally cause complications, such as the formation of a dentigerous cyst. Because of the unusual finding, and even more because of the report of this association, a case presentation is carried out. Objective: To describe the management of a supernumerary distomolar in the mandibular ramus associated with a dentigerous cyst, making emphasis on the importance of the preoperative imaging studies and the endo-buccal surgery approach. Case presentation: 34 years old female patient from whom the third molars were removed during adolescence, who presented facial cellulitis 3 months before the molars removal, which was a reason to perform an orthopantomography. Then, a molar in the mandibular ramus was detected. During oral examination, a light vestibular table expansion in the left mandibular ramus was found. The presence of a dental structure from the molar group which was associated with a radiolucent area was confirmed by orthopantomography, and was also confirmed by CT-scan. Exeresis of both molars under general anesthesia was planned, with an endo-buccal approach. There were no complications. The patient was discharged 24 hours later, and was treated in the outpatient consulting room for 6 months. Histopathological diagnosis revealed the presence of a cyst(AU) Conclusions: The finding of a supernumerary distomolar tooth in the mandibular ramus associated with a dentigerous cyst is an infrequent clinical situation that allows to make good use of the benefits of digital imaging studies, and implement a treatment plan that ends in a successful exeresis via an endo-buccal route, without any complications(AU)

Los odontomas son clasificados por la OMS como tumores odontogénicos benignos compuestos por esmalte, dentina, cemento y pulpa, considerados lesiones hamartomatosas más que verdaderos neoplasmas. En casos excepcionales erupcionan hacia la cavidad bucal.En este trabajo se realiza la presentación de un caso de odontoma que se expone espontáneamente a la cavidad bucal, en un paciente masculino de 22 años, entidad que se conoce con el nombre de “odontoma erupcionado”. Se presenta también una revisión de la literatura entre los años 1980 y 2015, de los casos existentes de esta patología.


Odontomas are classified by the WHO as odontogenic tumors composed of enamel, dentin, cement and pulp, considered hamartomatous lesions rather than true neoplasms. In exceptional cases, odontomas erupt into the oral cavity.This study reports a case of odontoma spontaneously exposed to the oral cavity, which is known as “erupted odontoma”, in a 22-year-old male patient. A review of the literature between 1980 and 2015 of the existing cases of this pathology is also presented.

ABSTRACT

ABSTRACT Odontoma is the most common type of odontogenic tumor and is most often diagnosed in the second decade of life. This benign tumor has a slow growth and is asymptomatic, which favors its persistence in the bone until some clinical sign emerges or the tumor or be detected in a routine radiograph. The aim of this case report was to present a clinic case of a 13-year-old male, who presented delayed exfoliation of the mandibular right primary canine. The radiographic exam revealed a radiopaque image in the anterior region of the mandible produced by structures that resembled the formation of teeth surrounded by a radiolucent area. Adjacent to the tumor, the impacted mandibular right canine was identified with deviation of its eruption axis. The clinical diagnosis was compound odontoma. Treatment consisted of the removal of the tumor and the surgically-assisted eruption of the tooth without orthodontic traction. After three years of follow up, the patient has exhibited positive results. It is possible to surgically assist the eruption of an impacted tooth, with a deviated axis, associated with an odontoma through the removal of the tumor and of the physical barrier (ostectomy) without orthodontic traction.


RESUMO Os odontomas são considerados os tipos mais comuns de tumores odontogênicos e frequentemente diagnosticados na segunda década de vida. Possuem crescimento lento, comportamento benigno e assintomático, favorecendo sua permanência intraóssea por muito tempo até demonstrarem algum sinal clínico ou serem detectados em radiografias de rotina. O objetivo deste relato foi apresentar um caso clínico de um paciente de 13 anos de idade, sexo masculino, que apresentou atraso na esfoliação do canino decíduo inferior direito. No exame radiográfico, verificou-se uma imagem radiopaca na região anterior da mandíbula, produzida por estruturas que lembravam a formação de dentes, circundada por uma área radiolúcida. Adjacente à lesão, na parte basal, identificou-se a presença do canino permanente inferior direito incluso, apresentando desvio do seu eixo de erupção. O diagnóstico clínico foi definido como odontoma composto. O tratamento consistiu na remoção da lesão e erupção do dente incluso associado, por meio de intervenções cirúrgicas, sem tração ortodôntica, com três anos de acompanhamento e resultados positivos. Existe a possibilidade de erupção cirurgicamente guiada de dente retido, com desvio do seu eixo de erupção e associado à odontoma, através da remoção da lesão e da barreira física (ostectomia) sem tração ortodôntica.

Los odontomas están compuestos de esmalte, dentina, cemento y tejido pulpar. Histológicamente se clasifican en 2 grupos: compuestos y complejos, mientras que clínicamente se clasifican en 3 tipos: odontoma central (intraóseo), odontoma periférico (extraóseo o de los tejidos blandos) y odontoma erupcionado. Epidemiológicamente son reportados como los tumores odontogénicos más frecuentes, sin embargo la erupción de este tipo de lesión es poco común, produciéndose en el 1,6% de los casos. Por lo general son asintomáticos, siendo comúnmente un hallazgo radiográfico, y en algunos casos pueden estar asociados con alteraciones de la erupción dentaria. Este estudio corresponde a un caso clínico de un odontoma complejo de gran tamaño que erupciona en la cavidad oral asociado a un molar retenido.


Odontomas consist of enamel, dentin, cementum, and pulp tissue. Histologically, they are classified into 2 groups: compounds and complexes. They are also clinically classified into 3 types: Central odontoma (intraosseous), peripheral odontoma (extraosseous or soft tissue odontoma) and erupted odontoma. Epidemiologically they are reported as the most frequent odontogenic tumor, however, the eruption of this type of lesion is rare, and odontoma occurs only in 1.6% of cases.

ABSTRACT

Odontomas are hamartomas, being of two types: Compound and complex and are nonaggressive hamartomatous developmental malformations or tumors of odontogenic origin, which consists of enamel, dentin, cementum and pulpal tissue. They are usually asymptomatic and diagnosed on routine radiological studies in the second and third decades of life. Compound variety is more often found in anterior maxilla; complex occurs in molar areas of either jaw. Mean age of occurrence is 12-16 years. Radiographically, they present as radiopacities surrounded by a radiolucent rim in complex variety and tooth like structures of different size and shape surrounded by radiolucency in compound variety. Here, we have discussed a rare case of erupting complex odontoma associated with an impacted mandibular third molar.

ABSTRACT

Odontomas are hamartomatous malformations of odontogenic origin composed of all the structures that form the tooth. Pigmentation associated with odontomas is rare, with only two cases reported in the literature. The genesis of this composite malformation is attributable to the organizational failure of the developing odontogenic apparatus leading to abnormal morphodifferentiation. Although some odontomas erupt into the oral cavity, it defies the existing views of normal physiologic tooth movement thus raising questions over the use of this term. Here, we review a case of large odontoma in a 23-year-old male surfacing into the oral cavity from its intraosseous location. The unusual occurrence of melanotic pigmentation in the odontogenic epithelial rests was demonstrated by Mason Fontana special stain. The histogenesis of this unusual entity and explanation to its possible occurrence is discussed.

ABSTRACT

Introduction: Odontomas are the most common odontogenic tumors of the jaws which are benign, slow growing and nonaggressive. They are usually asymptomatic and are diagnosed on routine radiological examination. The eruptions of odontomas are uncommon and very few cases are reported in the literature. Objective:To report two rare cases of erupted compound odontomas. Case report:Two cases of erupted compound odontomas are reported in middle aged patients. Erupted miniature teeth-like structure were seen on clinical examination. The condition was diagnosed on the basis of conventional intraoral radiographs and histopathological examination. Both cases were treated by surgical excision under local anaesthesia without any complications. Conclusion: Odontomas are benign tumors with uncertain etiology. Odontomas erupting in the oral cavity is an infrequent situation. Surgical excision is the treatment of choice in all such cases followed by histopathological examination to confirm the clinical diagnosis.

Introduction: Odontomas are the most common odontogenic tumour. They are considered to be hamartomasrather than neoplasms. They are generally asymptomatic and are discovered on routine radiographicexamination. Odontomas might be associated with disturbance in tooth eruption. Other symptoms includeretention of deciduous teeth, pain, swelling, expansion of cortical bone, tooth displacement. Eruption ofodontoma in the oral cavity is rare with only 11 cases being reported about erupted compound odontomas.Case report: An unusual case of erupted compound odontome associated with pain and impacted maxillaryleft second premolar is reported. We have also discussed the clinical, radiographic and histopathologicfeatures of this rare condition.


Introdução: Odontomas são os mais comuns entre os tumores odontogênicos. Eles são considerados hamartomas,ao invés de neoplasias. Eles geralmente são assintomáticos e descobertos em exames radiográficos derotina. Odontomas podem estar associados com distúrbio na erupção dentária. Outros sintomas incluem retençãode dentes decíduos, dor, inchaço, expansão do osso cortical e deslocamento do dente. A erupção de um odontoma na cavidade oral é rara, com apenas 11 casos relatados de odontomas compostos erupcionados.Relato de caso: Um caso incomum de odontoma composto associado à dor e segundo pré-molar superior esquerdoimpactado é relatado neste artigo. Os aspectos clínicos, radiográficos e histopatológicos dessa doençarara também são discutidos.

El análisis de Tanaka-Johnston, mundialmente empleado para la predicción del espacio de caninos y premolares no brotados, ha sido muy cuestionado por la variabilidad en el tamaño dentario entre las diferentes poblaciones, y por haber sido realizado en un grupo étnico no aplicable a todos. Se realizó un estudio descriptivo transversal, con el objetivo de determinar el ancho mesiodistal de incisivos, caninos y premolares mandibulares, así como de caninos y premolares maxilares, y comparar la suma real de estos dientes con los estimados por Tanaka-Johnston, según el sexo y la arcada. El universo de esta investigación estuvo constituido por 50 estudiantes de la Facultad de Estomatología de La Habana, 25 del sexo femenino y 25 del sexo masculino, con oclusión normal, sin antecedentes de tratamiento ortodóncico; todos descendientes de cubanos. En los resultados se observó que los anchos mesiodistales de los incisivos, caninos y primeros premolares inferiores y de caninos y segundo premolar izquierdo superiores fueron significativamente mayores en el sexo masculino que en el femenino. La suma real de caninos y premolares superiores e inferiores fue mayor en el sexo masculino que en el femenino. Cuando se comparó la suma real de caninos y premolares, con lo estimado según la ecuación de Tanaka-Johnston, se halló que la esta sobrestimó los valores de caninos y premolares, ya que los resultados fueron significativamente menores(AU)


The Tanaka-Johnston analysis used at worldwide to predict the space of non-erupted canines and premolars has been questioned due to the variability in the teeth size among different populations and also to be carried out in non-applicable whole ethnic group. A cross-sectional and descriptive study was conducted to determine the mesiodistal width of mandibular incisives, canines and premolars, as well as of danines and premolar-maxillary and to compare the real sum of these theeth with the estimates by Tanaka-Johnston, according to sex and the arcade. Universe of this research included 50 students of Stomatology Faculty of havana (25 males and 25 females) presenting with normal occlusion, without a history of Orthodontics treatment all of them descendants of Cubans. In results it was noted that the mesiodistal widths of incisives, canines and first lower premolars and of canines and second left premolars upper are greater in male sex than in the female one. The real sum of upper and lower canines and premolars was greter in male sex than in the female one. Comparing the real sum of canines and premolars with that estimated according Tanaka-Johnston, it was noted that it overestimated the values of canines and premolar since the results were significantly lower(AU)

ABSTRACT

Os odontomas constituem-se como os tumores odontogênicos mais comuns. São considerados harmartomas(anomalias do desenvolvimento), sendo compostos por vários tecidos dentários, como esmalte, dentina, cemento e polpa. Podem ser classificados em complexos, quando apresentam desorganização tecidual; ou compostos, quando os tecidos dentários se apresentam de forma organizada. O presente trabalho relata um caso clínico em paciente do gênero feminino, com trinta anos, que procurou atendimento odontológico em virtude de lesão na boca, na região de molares inferiores esquerdos, associada a aumento de volume extrabucal na região. Confrontando-se características clínicas, radiográfica se o relato da paciente sobre a evolução da lesão, esta foi diagnosticada como odontoma do tipo complexo, tido como atípico por se mostrar erupcionado na cavidade bucal. A remoção da lesão ocorreu de forma cirúrgica, tendo-se obtido sucesso no tratamento
