ABSTRACT

Objective To observe the effects of tetramethylpyrazine on intraoperative autotransfusion of blood quality , analysis of ligustrazine in autotransfusion of clinical value of blood quality control.Methods 68 patients undergoing elective surgery in our hospital from February 2015 to May 2016 were selected as the research object,and were randomly divided into control group and experimental group,each group of 34 cases.Two groups of patients in the intraoperative autologous blood transfusion and the control group according to conventional autologous transfusion in operation ,test group of patients with intravenous injection of ligustrazine injection in the recovery of blood before adding ligustrazine injection in the recovery process of blood.The two groups were collected before blood transfusion,observe the morphology of red blood cells, red blood cell fragments under the microscope and compared;the two groups were collected after reinfusion of blood two hours,one and five days in peripheral venous blood, the determination of superoxide dismutase (SOD), malondialdehyde (MDA), T lymphocyte subsets level of two groups were compared before and after one and five days. Results The erythrocyte deformability and erythrocyte debris of the two groups were higher than those before operation (P<0.05).The blood red blood cell deformability and the number of red blood cell debris in the blood transfusion group were significantly higher than those in the control group Significantly lower than the control group (P<0.05).The levels of SOD were significantly lower at two hours and one day after operation than those before operation, and the serum SOD was still lower than that before operation in the control group,The SOD in the experimental group was significantly higher at two hours, one and five days in the control group,MDA was lower than the control group(P<0.05).The CD3 +,CD4 +, CD4 +/CD8 +levels were significantly lower at two hours and one day after operation than those before operation (P<0.05).CD3 +,CD4 +and CD4 +/CD8 +in the control group were still lower than those in the control group at five days after operation(P<0.05),the levels of CD3 +, CD4 +, CD4 +/CD8 +were significantly higher in the experimental group than those in the control group at two hours,one and five days after treatmen (P <0.05).There was no significant difference between the two groups in the incidence of adverse reactions.Conclusion The addition of preoperative intravenous injection of ligustrazine,blood recovery process,can effectively protect the transfusion of red blood cell integrity,reduce the effect of transfusion of blood and blood antioxidant capacity in patients with T lymphocyte immune function ,to improve the quality of blood transfusion patients has important clinical value .

ABSTRACT

BACKGROUND: Improvements in the iron status of a population as result of food fortification are expected at the long term. In Brazil, the effectiveness of mandatory flour fortification with iron has been evidenced mostly from surveys on gestational anemia after 1 or 2 years from its implementation, in 2004. Our aim was to assess hemoglobin (Hb) concentrations and the prevalences of anemia and linked erythrocyte morphology patterns among pregnant women in 2006 and 2008.METHODS: The study design was retrospective and cross-sectional. The analysis was based on secondary data in 546medical records from women at the times of their first prenatal attendance in 13 public primary healthcare units of the Butantan Administrative Region from São Paulo (SP), Brazil. Anemia was evaluated from Hb concentration (cut-off <11.0 g/dL) and erythrocyte morphology patterns from mean corpuscular volume (MCV), mean corpuscular Hb concentration (MHC), and red cell distribution widths (RDW). Recorded sociodemographic and obstetric data included maternal age, gestational age, ethnoracial self-classification, and residence type. Student's t tests, analysis of variance, Chi-squared tests, and multiple linear and logistic regressions were employed in the statistical analysis using a significance level of 5 %.RESULTS:The prevalence of anemia was 9.7 % in 2006 and 9.4 % in 2008 (P= 0.922), with no significant difference in mean Hb concentrations (P= 0.159). Normocytosis (normal MVC), normochromia (normal MHC), and anisocytosis (high RDW) were found in most anemia cases, suggesting that the low Hb concentrations resulted from mixed causes. In multiple regression analysis, gestational age at the first prenatal attendance was an independent predictor of low Hb and of having anemia. Moreover, black ethnoracial self-classification was associated with lower Hb. CONCLUSIONS: The prevalence of gestational anemia was low among women in both of the studied periods, representing a mild public health problem. Our results highlight the importance of early prenatal care as a means of reducing gestation-associated risks. The erythrocyte morphology pattern found in most cases of low Hb levels suggests that, besides iron deficiency, hemoglobin pathies and nutritional deficiencies of folate and vitamin B12 are common complicating factors of gestational anemia in our setting.

Las pruebas de laboratorio son un complemento importante para que el médico homeópata recopile la totalidad de los datos con que se manifiesta la enfermedad de sus pacientes. Uno de los exámenes más habituales a los que se puede recurrir es la biometría hemática, el cual ofrece una orientación conveniente sobre los mecanismos que se activan en el organismo ante un estímulo nocivo determinado,mediante información detallada sobre los elementos formes de la sangre: serie blanca, serie roja y plaquetas. Se presentan a continuación las consideraciones más importantes sobre las variables que se estudian mediante un hemograma, pues aunque ninguno delos valores que nos puede proporcionar se encuentra en los repertorios clásicos,es evidente que nos ofrece información sobre signos tan importantes o comunescomo anemia, hemorragias, trombosis, tromboflebitis o abscesos, todos ellos ensus diferentes modalidades.


Laboratory tests are an important complement to the homeopath. They help to collect all the data manifested by the patient´s disease. One of the most common tests to which the homeopath can turn to is the Cell Blood Count, which offers a convenient guidance of the mechanisms that are activated in the body to a determined noxious stimulus, through detailed information about the blood cells: white,red and platelets count.The following are the most important considerations of the variables that are studied by a CBC, as though none of the values that we can find is in the classical repertoire, it is evident that offers very important information on common signs such as anemia, bleeding, thrombosis, thrombophlebitis or abscess, all in their different ways.

ABSTRACT

As anemias microcíticas e hipocrômicas atingem grande parcela da população mundial. Entre estas, significativa porcentagem de casos se deve à deficiência do ferro, enquanto em algumas regiões a frequência de talassemia menor se torna importante. Por outro lado, a anemia de doença crônica é a causa mais comum de anemia em pacientes hospitalizados. O diagnóstico diferencial destas doenças é clinicamente importante, e é atualmente realizado através dos exames padrão ouro envolvendo a avaliação do metabolismo do ferro e dosagem de HbA2. Embora dotados de grande utilidade, estes testes podem apresentar uma metodologia mais demorada e onerosa que, em casos de concomitância de doenças, comuns na prática clínica, não conseguem proporcionar um correto diagnóstico. Na tentativa de otimizar e direcionar o diagnóstico destas anemias, o uso de alguns parâmetros derivados dos modernos contadores automáticos tem sido sugerido. Neste estudo, o papel do RDW, parâmetros plaquetários (número de plaquetas, PDW, VPM) e morfologia eritrocitária como parâmetros diferenciadores, foi avaliado em um grupo de 159 pacientes portadores de anemia microcítica e hipocrômica devido à deficiência do ferro, anemia de doença crônica e talassemia beta menor, comprovado pelos exames padrão ouro. Foi possível observar que o RDW não se mostrou um bom discriminante, enquanto o índice plaquetário PDW pode ser um parâmetro auxiliar no diagnóstico diferencial das anemias microcíticas e hipocrômicas. Com relação às alterações morfológicas dos eritrócitos, o pontilhado basófilo foi um achado bastante comum apenas em pacientes portadores de talassemia beta menor, com indícios de potencial utilidade na elucidação de casos de microcitose.


Microcytic and hypochromic anemias affect many people worldwide. A significant percentage of cases are due to iron deficiency, while in certain regions the frequency of thalassemia minor is important. On the other hand, anemia of chronic disease is the most common cause of anemia in hospitalized patients. Differential diagnosis between these anemias, currently established by using gold standard tests involving evaluation of iron metabolism and measurement of HbA2, is of clinical importance. Although very useful, these tests are time consuming and onerous. In cases of concomitant diseases, a common finding in the clinical practice, these tests are unable to provide a correct diagnosis. In an attempt to optimize diagnosis of these anemias, the use of some parameters derived from modern automated blood count analyzers has been suggested. In this study, the role of RDW, platelet parameters (platelet number, PDW, MPV) and erythrocyte morphology as differentiating parameters were evaluated in a group of 159 patients diagnosed as carriers of microcytic or hypochromic anemias due to iron deficiency, anemia of chronic disease and â-thalassemia minor, as confirmed by gold standard tests. The RDW did not prove to be a good discriminator, while the platelet index, PDW, may be helpful in the differential diagnosis of microcytic and hypochromic anemias. Regarding the red cell morphologic alterations, basophilic stippling was a quite common finding just in patients with â-thalassemia minor, suggesting it to be a potential marker for elucidating cases of microcytosis.
