ABSTRACT

Objective@#To explore the effect of the public welfare training of esophageal speech on the psychological condition of the laryngectomees.@*Methods@#Using experimental research method and convenient sampling method to select 96 laryngectomees as research object by inclusion and exclusion criteria in Hubei province. The public welfare training was used to guide the laryngectomees to learn the esophageal speech. Then, laryngectomees' restoring vocalization, self-rating anxiety and self-rating depression would be assessed in a month before training, a month after training and half a year after training. The results were analyzed by descriptive analysis, chi-square test, F-test and so on.@*Results@#Ninety-six laryngectomees participated in the public service esophageal language training one month before, one month after and half a year after the training. The number of patients recovered completely from 3 to 39 to 72 in the recovery of pronunciation function. There was significant difference in the distribution (χ2=195.579, P < 0.01). The score of anxiety and depression was (54.84±9.19), (47.41±8.16), (42.92±7.16) points and (59.00±8.87), (52.48±8.16), (47.56±7.41) points. There were significant differences (χ2=79.772, 82.705, P<0.01).@*Conclusions@#Most laryngectomees have the different degrees of anxiety and depression after surgery. By carrying out the public welfare training of esophageal speech on laryngectomees, the psychological state of the laryngectomees can be effectively improved.

ABSTRACT

Objective To identify the effect of specialized language training on the speech rehabilitation of total laryngectomy (TL)patients.Methods Twenty-seven TL patients were trained for esophageal speech by specialized speech-language pathologists from October 2005 to December 2006.Then the participants were followed and evaluated their esophageal speech level after one cycle of training program,1 month,3 months,6 months and 12 months after training program.Results The score of participants' esophageal speech was steadily improved during the 12 months after participating the training program.The score of esophageal speech greatly increased in the third month,and this significant improvement was kept until 12 months after the training program.The esophageal speech quality was negatively correlated with age and surgery area of patients.Conclusions Specialized speech-language training showed positive effect on esophageal speech rehabilitation,and esophageal speech training by specialists is worthy of wide application.

ABSTRACT

BACKGROUND AND OBJECTIVES: Advanced laryngeal cancer is frequently treated by total laryngectomy. This operation is effective but results in gross functional disability because of the permanent loss of voice. As an alternative using voice, esophageal speech has been employed as a natural and satisfactory means of communication for laryngectomized patients. Unfortunately, during past decades the success rate has ranged 40-60%. The purpose of this study was to determine which factors contribute to the proficiency of esophageal speech. MATERIALS AND METHOD: Videofluoroscopy and voice analysis of fourteen alaryngeal male patients who had trained esophageal speech were performed. RESULTS: Aerophagia and air ejection were impossible in poor esophageal speakers. Fluent esophageal speakers had short pseudoglottis, longer maximum phonation time, more efficient jitter, shimmer and harmonic-to-noise ratio. CONCLUSION: Aerophagia and air ejection are essential for esophageal speech. Short pseudoglottis (less than 2 cm) affords better esophageal speech. Natural repetitive movements of aerophagia and air ejection with accurate articulatory motion can improve the quality of esophageal speech.

ABSTRACT

Forty laryngectomees were divided into 4 groups. Each group had 10 patients, who speech with Esophageal, Tracheo- esophageal, Electrolaryngeal and Pneumatic artificial laryngeal respectively. Ten normal speakers were included as control group. The average fundamental frequencies (F0) were included as calculated after speaking a sentense. The result showed that it was similar between normal speakers and Tracheo-esophageal speech,the average F0 of Esophageal and Pneumatic artifcial Laryngeal speech were highest, and that of Electrolaryngeal speech was lowest.

ABSTRACT

Objective To compare the alaryngeal speakers'voice acoustic parameters and intra-tracheal pressure when they spoke different vowels.Methods 24 patients after tracheo-esophageal shunt phonation by the anastomosis of the membranous portion of the tracheal section with the anterior wall of esophagus after total laryngectomy and 16 esophageal speakers enrolled in this study. The voice acoustic parameters were measured when they spoke soft /a/ and soft /i/. The intra-tracheal pressure of 20 TE speakers was measured when spoke /a/ and /i/.Results There were no significant differences between vowel /a/ and /i/ in the two alaryngeal speaker group. The pressure of TE speakers after Shang's procedrue for comfortable /i/ was higher than that for /a/, and there was significant difference.Conclusion There were the same clinical meanings in evaluating alaryngeal voice acoustic aspect. when using vowel /a/ and /i/. However when using different vowel, the intrtracheal pressure was different in TE speakers.
