ABSTRACT

@#Introduction: Annually, over two million newborns die worldwide immediately after birth, mostly because of failure to initiate and sustain breathing. A significant decline in newborn deaths can be achieved by using proper essential newborn care (ENC) techniques. Competency-based education (CBE) could successfully build ENC skills. The purpose of the present study was to investigate the effect of applying CBE on Midwifery students’ knowledge and skills acquisition and retention of ENC. Methods: This quasi-experimental study recruited third-year under-graduate midwifery students (n=54). They were equally assigned to the interventional and control group. The interventional group was taught ENC by using CBE, whereas the control group was taught by using traditional methods. Students’ ENC knowledge was assessed three times using a multiple-choice question exam. The ENC skills were measured twice by using the Essential Neonatal Care Performance Checklist. Results: The intervention group exhibited significant ENC knowledge and skills, performance acquisition, and retention (p<0.001). Conclusion: CBE is a useful educational model for the acquisition and retention of ENC.

ABSTRACT

RESUMEN Objetivo: La incertidumbre de una posible transmisión posnatal por SARS-CoV-2 genera un desafío entre aplicar o evitar los cuidados esenciales del recién nacido para prevenir la transmisión. La revisión sistemática tiene como objetivo describir el impacto de los cuidados esenciales del neonato en la transmisión posnatal del SARS-CoV-2. Métodos: Se realizó una revisión sistemática en bases de datos electrónicas, se incluyeron estudios observacionales, serie de casos y reporte de casos. Dos investigadores de forma independiente evaluaron los criterios de inclusión, la calidad metodológica y la extracción de datos. Resultados: Se tamizaron 208 estudios para una inclusión final de 9 artículos relevantes. El reporte de transmisión del virus SARS-CoV-2 está centrado en la posibilidad de transferencia por la leche materna y ausencia de información acerca de la transmisión por otros cuidados esenciales, sin embargo, se observa un bajo uso en la práctica clínica de los cuidados esenciales del recién nacido hijo de madre con COVID- 19. Conclusiones: La información es limitada sobre la posible transmisión del SARS-CoV-2 a través de los cuidados esenciales del recién nacido, sin embargo, la pandemia ha impactado de forma importante los mismos. Según los hallazgos en la literatura, es mayor el beneficio de estas intervenciones y las recomendaciones científicas promueven su aplicación empleando las medidas de protección personal para la madre y el personal de salud.


ABSTRACT Objective: The uncertainty of a possible postnatal transmission by SARS-CoV-2 creates a challenge between applying or avoiding essential newborn care to prevent transmission. The objective of the systematic review is to describe the impact of essential neonate care on the postnatal transmission of SARS-CoV-2. Methods: A systematic review was carried out in electronic databases, observational studies, case series, and case reports. Two investigators independently assessed the inclusion criteria, methodological quality, and data extraction. Results: 208 studies were screened for a final inclusion of 9 relevant articles. The transmission report of the SARS-CoV-2 virus is focused on the possibility of transfer through breast milk and the absence of information about transmission by other essential care, however, a low use, in clinical practice, of essential care for the newborn child of a mother with COVID-19, is observed. Conclusions: Information is limited on the possible transmission of SARS-CoV-2 through essential newborn care; however, the pandemic has had a significant impact on them. According to the findings in the literature, the benefit of these interventions is greater, and the scientific recommendations promote their application, using personal protection measures for the mother and the healthcare personnel.

ABSTRACT

Background: The Ministry of Health and Family Welfare of India has come up with this essential newborn care services (ENBC) to reduce the neonatal mortality rates, and it is provided free of cost at all Government Healthcare facilities. This study was done to see the impact of ENBC services. Objectives: The objectives of this study are (1) to evaluate the provision of ENBC services in all maternity homes under the aegis of Ahmedabad Municipal Corporation (AMC) and (2) to assess the availability of adequate human resources, infrastructure, and equipment. Materials and Methods: A study design is cross-sectional. The study was conducted at all maternity homes in AMC. Sampling technique is the purposive type. Observation of health-care facility infrastructure under which assessment of availability of essential drugs, infrastructure and ENC equipment, observation of type of care, eye care and umbilical cord care at birth, infrastructure etc was done. Written permission from the Institutional Review Board, NHLMMC, Medical Officer of Health, AMC, Dean, was taken. Results: It was found out that a total number of rooms ranged from 3 to 20, number of beds ranged from 8 to 36, and deliveries per year ranged from 90 to 1646. Two of the maternity homes had assisted deliveries and cesarean section facilities. All the maternity homes had 100% availability of the medical. Resuscitator was present in 75% of the maternity homes.100% maternity homes were equipped with radiant warmers, 91.67 % had pump suction, weight scale was present in 100%, 83.33% had thermometer, and 91.67% had Hub Cutter. Cotton swabs, bag mask, gloves, mucus extractor, and needle syringe were present in all the maternity homes in our study. Conclusion: All maternity homes are providing sufficient ENBC. Only two maternity homes are providing cesarean section delivery. Essential infrastructure and service facilities are provided in all maternity homes except a few minor things like less nursing staff. Newborn care corner is present in all the maternity homes.

ABSTRACT

Health facilities in India are faced with the challenge of providing quality newborn care in the face of major skilled human resourceshortage. A possible solution is the concept of Mother-Neonatal ICU (M-NICU), where the mother has her bed inside the neonatalintensive care unit (NICU) by the side of baby’s warmer. Our observations in M-NICU of a public sector hospital in New Delhi, India,indicate that mothers can be easily trained to follow asepsis routines and monitor the neonates, and are better prepared for their post-discharge care. Incorporating space for both mothers and their newborns in level-II NICUs may provide quality and developmentallysupportive newborn care in coming years.

Introduction: Despite efforts by government and other agencies, neonatal mortality is still high in our country. Among all other reasons, lack of proper newborn care practices is one of the major contributors for such high rates of mortality. This study was undertaken to assess newborn care practices among mothers residing in slum areas. Material & Methods: A cross sectional study was carried out among 360 mothers of infants in slums of Kanpur. A pre designed and pre tested questionnaire was used. Percentages, chi-square & Fischer’s exact test was used to analyse data using SPSS. Results: Nearly 23.05% deliveries were conducted at home, mostly by untrained dais. Bathing the baby after birth was practiced in 44.45% of home deliveries. In case of home deliveries, eyes were cleaned in 69.95% cases , weight was taken in 29.02% newborns and cord was cut with unsterile blade in 19.2% cases. Turmeric powder with oil or ghee was applied on cord in 94% of home deliveries and in 17.3% of hospital deliveries after the child was brought home. A significant difference was seen in cord care practices in home and hospital deliveries. Early initiation of breast feeding and colostrum was given by 35.5% and prelacteal feed was given by 63.9%. Conclusion: Awareness regarding newborn care practices was good among mothers who delivered their babies in institutions, as depicted by various healthy practices followed by them which were significantly associated with institutional deliveries.

ABSTRACT

The main objective of the study is to identify the availability of infrastructure facility, human resources, investigative services, and facility based newborn care services with respect to Indian Public Health Standards (IPHS) at community health centers (CHC) of Bharatpur District of Rajasthan State. Data were collected from service providers at CHC through well structured questionnaire at thirteen CHCs situated at Bharatpur District of Rajasthan State. It was found that infrastructure facilities were available in almost all the CHCs, but shortage of manpower especially specialists was observed. Availability of investigative services was found quite satisfactory except ECG. It was also observed that none of the CHCs have fully equipped facility based newborn care services (including newborn corner and newborn care stabilization unit). As per IPHS suggested in the revised draft (2010) important deficiencies were revealed in the studied CHCs of Bharatpur district and by additional inputs such as recruiting staff, improving infrastructure facilities, CHCs can be upgraded.
