ABSTRACT

Objective @#To investigate the clinical and molecular epidemiological characteristics of 10 patients with enterovirus (EV)-D68 infections in the respiratory tract in Shaoxing City, Zhejiang Province, so as to provide insights into formulation of control interventions for EV-D68 infections.@*Methods@#Clinical specimens were sampled from patients with acute respiratory tract infections (ARTIs) admitted to sentinel hospitals in Shaoxing City from 2021 to 2022, and EV-D68 was detected using real-time fluorescent PCR assay and sequencing of the VP1 region of enterovirus. The epidemiological and etiological characteristics of EV-D68 infected cases were descriptively analyzed. @*Results@#A total of 3 009 specimens were sampled from patients with ARTIs from 2021 to 2022, and the detection of EV-D68 was 0.33%. Of all EV-D68-infected patients, there were 6 men and 4 women, and 5 cases under 18 years of age, 2 cases at ages of 18 to 60 years and 3 cases at ages of over 60 years. EV-D68 infection predominantly occurred in summer (5 cases detected between May and July) and autumn (5 cases detected between September and October). The main clinical symptoms included fever (10 cases), sore throat (9 cases) and cough (8 cases), and all 10 cases recovered well, with no deaths reported. Sequencing identified D3 subtype in all 10 specimens positive for EV-D68. @*Conclusions@#The ARTIs caused by EV-D68 occurred predominantly among children under 18 years of age in Shaoxing City, and was highly prevalent in summer and autumn. D3 was the predominant enterovirus subtype.

ABSTRACT

Objective:To analyze the etiological and epidemiological characteristics of hand, foot and mouth disease (HFMD) in Xi′an from 2019 to 2021, so as to provide evidence for the prevention and control of HFMD.Methods:Stool specimens and anal swabs were collected from patients with HFMD. Enteroviruses (EVs) including enterovirus 71 (EV71), coxsackievirus A16 (CVA16), CVA6 and CVA10 were detected by RT-PCR. Excel 2007 and SPSS18.0 software were used for data collection and statistical analysis, respectively. The epidemiological data of HFMD cases were analyzed by descriptive epidemiology method. The VP1 gene sequence of the representative strain of each CVA6 genotype was downloaded. Phylogenetic trees were constructed using MEGA X software and the genetic characteristics were analyzed.Results:A total of 1 531 HFMD cases were involved and 1 365 were positive for EVs with a positive rate of 89.16%. The detection rates of EV71, CVA16, CVA6, CVA10 and other EVs were 1.31% (20/1 531), 32.46% (497/1 531), 38.47% (589/1 531), 5.09% (78/1 531) and 11.23% (172/1 531), respectively. There were significant differences in the pathogen composition in HFMD cases of different clinical types (χ 2=46.14, P<0.01) and occupations (χ 2=34.65, P<0.01) as well as in different years (χ 2=462.86, P<0.01). The average age was greater in patients with CVA16 infection than in those with CVA6 or CVA10 infection ( F=6.00, P<0.01). In 2019, the HFMD cases were mainly caused by CVA16, while in 2020 and 2021, the main pathogen was CVA6. Enterovirus-positive cases showed a bimodal distribution with the main peak from May to July and the secondary peak from September to November. CVA16 was the predominant pathogen in spring and summer, and CVA6 was the predominant pathogen in autumn. CVA6 was the dominant pathogen in eight districts and counties of Xi′an; CVA16 was the dominant pathogen in six districts and counties; CVA6 and CVA16 co-circulated in one district. The CVA6 isolates belonged to two evolutionary branches of D3a subtype. Conclusions:CVA6 and CVA16 were the prevalent pathogens of HFMD and CVA6 subtype D3a circulated in Xi′an from 2019 to 2021. The pathogen composition of HFMD cases showed obvious differences in population, time and regional distribution.

ABSTRACT

Objective To analyze the epidemiological characteristics and prevalent pathogens of foodborn diseases in Anyang city, results of active surveillance of foodborne diseases in Anyang City in 2016-2021,and to provide references for prevention and control of foodborne diseasess. Methods Information on surveillancecases of foodborne diseases reported by sentinel hospitals in Anyang City from 2016 to 2021 was collected, and the collected stool specimens were tested for pathogens.Data was analyzed using descriptive epidemiological methods. Results A total of 1 535 foodborne cases were collected in Anyang City.The reported cases were mainly concentrated in June to August (53.03%, 814/1535), among which the 25-44 years old group (25.41%, 390/1 535) and 45-65 years old group (24.43%, 375/1 535) were the main cases; the proportion of home-made in the processing or packaging method was the highest, and the home was the main exposed places (46.45%, 713/1 535) . Gastrointestinal symptoms were the main clinical symptoms (94.07%, 1 444/1 535) , The  overall detection rate of pathogen in Anyang City was 6.97% , and The detection rates of pathogenic microorganisms were statistically significant in different years. The main serotype of diarrheagenic Escherichia coli was EAEC(73.81%), and the main type of Salmonella was Salmonella enteritidis (54.55%). Conclusion The incidence of food-borne diseases in Anyang City is high in summer and autumn from 2016 to 2021. The age groups of 25-44 years old and 45-65 years old are the main incidence groups. The exposed places are mainly family, and the pathogens cause Escherichia coli, Salmonella. It is necessary to further improve the active surveillance system of foodborne diseases in Anyang City, increase the monitoring of key high-risk groups, and strengthen food safety education for home-made food, especially in summer and autumn.

ABSTRACT

ObjectiveTo analyze the influenza surveillance data in Ezhou City, Hubei Province from 2016 to 2021, determine the epidemiological characteristics and etiological trend of influenza like illness (ILI), and to provide scientific evidence for influenza prevention and control. MethodsThe ILI surveillance data were reported by Ezhou influenza sentinel hospitals and etiological examination results were collected by network laboratory. Influenza surveillance data from 2016 to 2021 were analyzed. ResultsFrom 2016 to 2021, the percentage of ILI visits (ILI%) in Ezhou city was 2.81% and increased over years. Majority (55.55%) of ILI cases were 0‒4 years. A total of 7 716 ILI samples were examined from 2016 to 2021, of which 1 467 tested positive with a positive rate of 19.01%. Influenza A H1N1 was mainly concentrated in January-April, A H3N2 mainly in August-December, B Victoria mainly in April-July and December-March, and B Yamagata mainly in December-February. Influenza network laboratory isolated influenza virus from the 1 467 positive samples by using MDCK cells and SPF chicken embryos. The overall isolation rate was 32.78%, which was 26.93% by MDCK cells and 5.86% by SPF chicken embryos. From 2016 to 2021, a total of 13 ILI outbreaks were reported in Ezhou City. Temporally, the outbreaks mainly occurred in winter and spring. Spatially, they were mainly in primary schools, middle schools and kindergartens. ConclusionThe winter and spring are the key time period of influenza prevention and control in Ezhou City, as they are susceptible to influenza outbreaks. Children aged 0‒14 are the key population of prevention and control. Diverse subtypes of influenza virus alternate by years, which warrants continually strengthening monitoring. Additionally, certain countermeasures against COVID-19 may be recommended in the prevention and control of influenza.

Objective@#To illuminate the epidemic characteristics of Yersiniosis among children in the central city of Beijing and the accuracy of current clinical diagnosis towards Yersiniosis.@*Methods@#Etiological surveillance of diarrheal patients, a total of 3 493 cases, was performed in a children hospital in central area of Beijing from 2011 to 2018 continuously. Collected the epidemiological and clinical information of the cases, analyzed the clinical and etiological diagnosis for Yersiniosis and bacterial dysentery and compared the distribution of Yersiniosis cases with the different symptoms.@*Results@#A total of 3 493 acute diarrhea cases distributed from the age of 6 months to 13 years old, M (P25, P75) was 1.50 (0.75, 3.17) years old. The 28 cases were isolated Yersinia enterocolitica (isolation rate of 0.80%) and they could be diagnosed as Yersiniosis by etiology. The isolation peaked in May and February. A total of 85.71% (24/28) of Yersiniosis cases were under 5-year old. The children of 3-4 age group had the highest isolation rate (1.52%) while the rate (0.18%) of 0-1 age group was the lowest (P=0.025). The Yersinia enteroclitica isolation rates of diarrheal patients with the symptoms including mucus feces, fever, white blood cell (WBC) and red blood cell (RBC) in feces were higher than the patients without these symptoms (P>0.05). The 9 of 28 Yersiniosis cases by etiology diagnosis were clinical diagnosed as bacillary dysentery.@*Conclusion@#The infants and young children under 5-year old were the main population of Yersiniosis adolescent patients under 14-year old. The typical symptoms characterized with mucus stool, fever, WBC and RBC by routine microscopic examination. The preliminary clinical diagnosis of Yersiniosis is easily confused with bacterial dysentery.

ABSTRACT

Objective By comparing and analyzing the etiological monitoring results of influenza in Xicheng District general hos-pital and children′s Hospital ,to explore the epidemic regularity of influenza in children ,so as to provide the basis for developing in-fluenza prevention and control measures .Methods A total of 40 swabs of influenza samples were collected every week from two sentinel hospitals ,and the virus RNA was extracted and detected by RT-PCR method .Results There was a statistically significant difference in positive rate of influenza pathogen surveillance between general hospitals and children′s hospitals (21 .8% vs .14 .9% ) (P<0 .001) .In the general hospital ,the positive rate was highest in people aged ≥60 years old(31 .3% ) .In the children′s hospital , the positive rate was highest in people aged >4—14 years old(26 .7% ) .Both hospitals had an epidemic peak of influenza from No-vember to December of next year ,it was typical influenza epidemic of the North area .But the children′s hospital had a epidemic trough in January and February .In both hospitals ,the circulating strains were H3 ,H1N1 ,type B ,the predominant type was H3 and type B .Conclusion Taking the general hospital as a reference ,the quality of influenza surveillance in the children′s hospital was relatively stable and reliable .Lack of the people aged >24— <60 years old and aged ≥60 years old in children′s hospital ,is the key reason of the statistically significant difference of the positive rate ,between the general hospital and the children′s hospital .Chil-dren′s influenza vaccination rate is higher than the other age group ,and the protective effect of vaccine is better .

ABSTRACT

Objective To analyze the epidemiologic features of hand-foot-mouth disease (HFMD) in Haikou city from 2008 to 2015.Methods Descriptive methods on epidemiology and detection on pathogens were conducted in Haikou city from 2008 to 2015.Results A total of 71 611 patients were diagnosed as HFMD in Haikou city from 2008 to 2015,including 728 severe cases,accounting for 1.02% among all the cases.The average annual incidence was 458.89/100 000.A total of 11 deaths were caused by the disease,with the average annual mortality rate as 0.07/100 000.Two peaks of incidence were seen,from April to July and from September to November.Age of the patients mainly fell in children aged 5 and below,taking up 95.78% of the total cases.Among all the patients,1-year-olds presented the highest incidence as 12 881.24/100 000.The reported incidence for males was higher than that in females.There were 4 districts in Haikou city that reported the disease.Residential areas of the patients were scattered around,with a percentage of 79.89%.Spectrums of pathogens that causing the prevalence of HFMD were EV71 type,Cox A16 type and other enteroviruses,which prevailing in turns,since 2011.Conclusions Haikou city had been an area with high incidence of HFMD.The incidence started to show a rising trend recently.It is suggested that programs as surveillance,case management,health education and comprehensive prevention and control of disease on HFMD targeting on key population should be intensively implemented to reduce the mortality of the disease.
