ABSTRACT

Background Air pollution has gradually become a major environmental and public health problem faced by countries around the world. Hazy weather not only affects the health of the population, but also poses a threat to social and public safety. China has successively promulgated policies such as the "Ten Articles on Atmosphere" and the Three-year action plan to fight air pollution, aiming to improve ambient air quality. It is clear that the Beijing-Tianjin-Hebei region has accomplished the set targets and improved air quality according to the environmental monitoring data of 2017. Objective To assess air quality improvements through the evaluation of the disease burden due to fine particulate matter (PM2.5) pollution in Shijiazhuang City before and after the air quality improvement from 2014 to 2021, including fatalities and health economic losses attributed to PM2.5 pollution. Methods Data on causes of death, PM2.5 concentrations, the number of permanent residents at the end of the year, gross regional product, and disposable income per capita in urban areas of Shijiazhuang were collected from 2014 to 2021. Total, non-accidental, circulatory, and respiratory deaths due to PM2.5 pollution were estimated by global exposure mortality models (GEMM). Health and economic losses due to selected diseases were calculated by value of statistical life (VOSL). Results During the study period, the average annual concentration of PM2.5 in Shijiazhuang was highest in 2014, and began to decline year by year in 2017, but all exceeded the current national limit of the second level of ambient air quality standards (35 μg·m−3). The total deaths, non-accidental deaths, circulatory disease deaths, and respiratory disease deaths attributed to PM2.5 pollution from 2014 to 2021 were 41326, 40246, 21792, and 5022, respectively; the associated health economic losses were 37.362, 36.369, 19.695, and 4.535 billion yuan, respectively. From the perspective of improved air quality, both the number of attributed deaths and health economic losses had declined in a volatile manner since 2017, with a significant decrease in 2019. If the average annual concentration of PM2.5 reached the second-level limit of China's ambient air quality standard (35 μg·m−3), the total deaths, non-accidental deaths, and deaths from circulatory diseases and respiratory diseases due to PM2.5 pollution would deducted by about 17000, 16000, 9000, and 2000, respectively; the corresponding health and economic losses would decreased by 15.201, 14.761, 7.959, and 1.859 billion yuan, respectively. If the average annual concentration of PM2.5 reached the latest PM2.5 air quality guidelines (5 μg·m−3) proposed by the World Health Organization, the total deaths, non-accidental deaths, circulatory disease deaths, and respiratory disease deaths due to PM2.5 pollution would deducted by 36000, 35000, 19000, and 4000, respectively, and the corresponding health and economic losses would reduced by 32.673, 31.796, 17.211, and 3.969 billion yuan, respectively. Conclusion PM2.5 pollution can lead to severe mortality burden and economic loss. Under the implementation of the State Council's "Ten Articles on Atmosphere" and the Three-year action plan to fight air pollution, Shijiazhuang's PM2.5 concentration and health economic losses have been significantly reduced, and further control of PM2.5 pollution can achieve greater health benefits and economic gains, affirming the positive results of local air pollution prevention work.

ABSTRACT

The core of diagnosing and treating diseases in traditional Chinese medicine lies in syndrome differentiation. The eight principles of syndrome differentiation serve as guidance for syndrome differentiation. As one of the eight principles of syndrome differentiation, the differentiation of deficiency and excess is the basic and critical method. Ischemic stroke is currently the leading cause harming the health of Chinese residents. Although the hypotheses about the cause of ischemic stroke have evolved from external wind to the later internal wind and to the modern theory of toxin damaging the brain collaterals, they all believe that this disease is rooted in internal deficiency and external excess. According to available studies, although stroke is characterized by complex pathogenesis and rapid progression of syndromes, the key cause evolution has a regularity, that is, from excess to deficiency. This article analyzes the historical evolution of the etiology, pathogenesis, and syndrome differentiation schemes of stroke. There are diverse schemes for the syndrome differentiation of stroke, which make it difficult to choose in clinical practice. In view of this problem, this paper puts forward a new approach of staging sequential treatment of ischemic stroke based on the differentiation of deficiency and excess according to the evolution law of the key cause of stroke. Furthermore, we conducted a randomized controlled study on 100 patients with ischemic stroke to evaluate this new approach. The results showed that the staging sequential treatment of ischemic stroke based on the differentiation of deficiency and excess demonstrated definite clinical efficacy. In addition, this article reviews the previous research results of our team and the research achievements of other teams to preliminarily explore the relationship between stroke syndromes and biomarkers, aiming to provide an objective basis for unveiling the pathogenesis of stroke. In summary, according to the key cause evolution (from excess to deficiency), the treatment of ischemic stroke by stages based on differentiation of deficiency and excess can facilitate the rapid intervention and improve the clinical efficacy on ischemic stroke.

ABSTRACT

The objective of the present study is to point out tensions of the theoretical/practical universe that Bioethics is facing in Brazil, in the search for a praxis for the destination of surplus embryos. We consider that Bioethics analyzes the implications of such practices in society and in relationships between individuals. Brief data from other countries were presented to compare the Brazilian situation progressively from the conceptual point of view and the adoption of measures. The research is a scoping review on the main points that have been hindering the progress of discussions on the subject and consequently the respective solution. The legal status of the embryo was described from several perspectives and theories, with the resulting proposals for the destination of surplus embryos in their positive and negative aspects. The tensions of Bioethics were presented in the context of post-modernity and the consequent social and moral plurality, together with the difficulties of identifying a secular bioethical morality. In the end, we conclude the possibility of proclaiming a consensus on the destination of surplus embryos based on secular morality, supported by the figure of the "moral strangers".


El objetivo del presente estudio es señalar las tensiones del universo teórico/práctico que la bioética enfrenta en Brasil, en la búsqueda de una praxis para el destino de los embriones sobrantes. Consideramos que la bioética analiza las implicaciones de tales prácticas en la sociedad y en las relaciones entre los individuos. Se presentaron breves datos de otros países para comparar progresivamente la situación brasileña desde el punto de vista conceptual y de la adopción de medidas. La investigación es una revisión del alcance de los principales puntos que han obstaculizado el avance de las discusiones sobre el tema y, en consecuencia, la respectiva solución. Se describió el estatuto jurídico del embrión desde diversas perspectivas y teorías, con las consiguientes propuestas para el destino de los embriones sobrantes en sus aspectos positivos y negativos. Se presentaron las tensiones de la bioética en el contexto de la posmodernidad y la consecuente pluralidad social y moral, junto con las dificultades de identificar una moral bioética laica. Al final, se concluye la posibilidad de proclamar un consenso sobre el destino de los embriones sobrantes basado en una moral laica, apoyada en la figura de los "extraños morales".


O objetivo do presente estudo é destacar as tensões do universo teórico/prático que a Bioética está enfrentando no Brasil, na busca de uma praxis para a destinação de embriões excedentes. Nós consideramos que a Bioética analisa as implicações de tais práticas na sociedade e nas relações entre indivíduos. Dados resumidos de outros países são apresentados para comparar a situação brasileira progressivamente de um ponto de vista conceitual para a adoção de medidas. A pesquisa é uma revisão de escopo sobre os pontos principais que vem atrapalhando o andamento das discussões sobre o assunto e consequentemente a solução respectiva. O status legal do embrião foi descrito a partir de diversas perspectivas e teorias, com as propostas resultantes para a destinação dos embriões excedentes em seus aspectos positivos e negativos. As tensões da Bioética foram apresentadas no contexto da pós-modernidade e a consequente pluralidade social e moral, juntamente com as dificuldades de identificar uma moralidade bioética secular. Ao final, nós concluímos pela possibilidade de proclamar um consenso sobre a destinação de embriões excedentes baseado na moralidade secular, apoiado pela figura dos "estranhos morais".

Introducción. En los refrigerios escolares se ha incrementado el consumo de los productos industrializados y se ha observado una disminución en la ingesta de alimentos frescos. Objetivo. Determinar la prevalencia y tendencia de sobrepeso, obesidad y consumo de alimentos no recomendables en escolares del norte de México. Materiales y métodos. Estudio descriptivo y transversal, realizado del año 2015 a 2022 en una muestra de 468 niños de ambos sexos de 9 a 12 años de escuelas públicas, se les realizó mediciones antropométricas de peso, estatura y circunferencia de cintura. Para diagnosticar el estado nutricional se utilizó el puntaje Z del índice de masa corporal (IMC) por medio del software Anthro Plus. El consumo alimentario de los escolares se evaluó por medio de una lista de cotejo durante cinco días consecutivos por observación directa los alimentos y bebidas que los escolares llevaron de casa para consumir durante el horario escolar. Resultados. Se encontró que el sobrepeso (SBP) y la obesidad (OB) aumentó durante la pandemia, siendo mayor el SBP en las niñas (17,0%) y mayor la OB en los niños (46,8%). En cuanto a la ingesta alimentaria se observó que durante la pandemia aumentó el consumo de alimentos no recomendables asociándose con el IMC. Conclusiones. El sobrepeso y la obesidad aumentó durante la pandemia, siendo mayor el sobrepeso en las niñas y mayor la obesidad en los niños. En cuanto a la ingesta alimentaria se observó que durante la pandemia el consumo de alimentos no recomendables específicamente en las bebidas azucaradas aumentó y se asoció con el IMC(AU)


Introduction. School snacks have increased consumption of industrialized products and a decrease in the intake of fresh food has been observed. Objective. Determine the prevalence and trend of overweight, obesity and consumption of undesirable foods in schoolchildren in northern Mexico. Materials and methods. A descriptive and cross-sectional study was conducted in a sample of 468 children of both sexes aged 9 to 12 in public schools and anthropometric measurements of weight, height and waist circumference were made. The body mass index (BMI) Z score was used to diagnose nutritional status using the Anthro Plus software. Food consumption of schoolchildren was evaluated by means of a checklist for five consecutive days by direct observation of the food and beverages that schoolchildren took from home to consume during school hours. Results. Overweight (PBS) and obesity (OB) were found to increase during the pandemic, with higher PBS in girls (17.0%) and higher OB in boys (46.8%). About food intake, it was observed that during the pandemic consumption of undesirable foods increased in association with the BMI. Conclusions. Overweight and obesity increased during the pandemic, being more overweight in girls and obesity in boys. As for food intake it was noted that during the pandemic consumption of foods not specifically recommended in sugary drinks increased and was associated with BMI(AU)

RESUMEN INTRODUCCIÓN En 2020, primer año de pandemia (AP) de COVID-19, Argentina focalizó las acciones en la nueva enfermedad y dejó relegado el monitoreo de las otras (noCOV). El objetivo de este trabajo fue comparar la mortalidad por causas noCOV durante el AP con respecto a los 5 años del período previo a la pandemia (PPP) MÉTODOS Se realizó un estudio transversal con fuentes de datos secundarias y base poblacional nacional. Se utilizó la causa básica de muerte, desagregada a nivel de capítulo y grupos según la Clasificación Internacional de Enfermedades, 10a revisión. Se estimaron las tasas brutas (TBM) y ajustadas (TAM) de mortalidad, comparando con test t y considerando como significativo un valor p<0,05. Se calculó el exceso de mortalidad (EM) realizando la diferencia entre el número de defunciones noCOV del AP y el intervalo de confianza del 95% superior o inferior de la media de casos del PPP RESULTADOS La TBM disminuyó 9,1%, y el EM fue de -4,5%. Salvo en el caso de la diabetes mellitus y las enfermedades respiratorias sin especificar, la TAM disminuyó significativamente durante el AP. La mayor variación de TBM fue por infecciones respiratorias agudas bajas en menores de 19 años y por accidentes de transporte en adultos de 20 a 59 años y de 60 años o más (reducción de 46,3% y 48,8%, respectivamente) DISCUSIÓN Durante el AP hubo una disminución de la mortalidad por noCOV especialmente en el caso de las infecciones respiratorias agudas y los accidentes de tránsito.


ABSTRACT INTRODUCTION During 2020, the first year of the pandemic (YP), Argentina focused the actions on the new disease, leaving aside the monitoring of diseases other than COVID-19 (non-COV). The objective of this work was to compare mortality from non-COV causes during the YP with respect to the 5 years of the pre-pandemic period (PPP) METHODS A cross-sectional study was conducted using secondary data sources and national population-based data. The basic cause of death was disaggregated at the chapter and group level according to the International Classification of Diseases, 10th revision. Crude (CMR) and adjusted mortality rates (AMR) were estimated, comparing with t-test and considering significant a p-value<0.05. Excess mortality (EM) was calculated as the difference between the number of non-COV deaths in the YP and the upper or lower 95% confidence interval (CI95%) of the mean number of PPP cases RESULTS The CMR decreased by 9.1% and EM was -4.5%. Except for diabetes mellitus and unspecified respiratory diseases, the AMR showed a significant reduction during the YP. The greatest variation of CMR was due to acute lower respiratory infections in people under 19 years of age and due to transport accidents in adults aged 20 to 59 years and adults aged 60 or older (reduction of 46.3% and 48.8%, respectively) DISCUSSION During the YP there was a decrease in mortality due to non-COV causes, especially those related to acute respiratory infections and transport accidents.

ABSTRACT

Abstract This article aims to estimate the excess of deaths in the years 2020 and 2021 in Mato Grosso state, in the state capital and in the countryside, according to gender and age group. Data was extracted from DATASUS/Ministry of Health website for the period from 2015 to 2020 and from the website of the State Department of Health - Data Warehouse System (DW) for 2021. Non-fetal deaths by natural causes of residents in Mato Grosso were analyzed and the analyses were broken down into countryside and state capital (Cuiabá). The variables selected were age group, gender, month of occurrence, and underlying cause of death. Excess mortality was calculated using generalized additive quasi-Poisson model adjustments with correction for overdispersion. A 30% excess of deaths was identified in 2020, with the state capital recording the highest estimate, in older age groups, and between the months of July and September. In 2021, the expected number of deaths was 57% higher, with the double in the younger age groups in the countryside. The study showed different demographic profiles of excess deaths in the years 2020 and 2021 during COVID-19 pandemic and distinct patterns between countryside and state capital, suggesting inequalities that may have caused impact on different risks.


Resumo O objetivo deste artigo é estimar o excesso de óbitos nos anos de 2020 e 2021 no estado de Mato Grosso, na capital do estado e no interior, segundo sexo e faixa etária. Os dados foram extraídos do site do DATASUS/Ministério da Saúde para o período de 2015 a 2020 e do site da Secretaria de Estado da Saúde - Sistema Data Warehouse (DW) para 2021. Óbitos não fetais por causas naturais de residentes no Mato Grosso foram analisados ​​e as análises foram divididas em interior e capital do estado (Cuiabá). As variáveis ​​selecionadas foram: faixa etária, sexo, mês de ocorrência e causa básica do óbito. O excesso de mortalidade foi calculado usando ajustes generalizados do modelo aditivo quase-Poisson com correção para superdispersão. Identificou-se um excesso de 30% de óbitos em 2020, com a capital do estado registrando a maior estimativa, nas faixas etárias mais avançadas, e entre os meses de julho e setembro. Em 2021, o número esperado de óbitos foi de 57% superior, sendo o dobro nas faixas etárias mais jovens do interior do estado. O estudo mostrou diferentes perfis demográficos de excesso de óbitos nos anos de 2020 e 2021 durante a pandemia de COVID-19 e padrões distintos entre interior e capital do estado, sugerindo desigualdades que podem ter impactado em diferentes riscos.

Resumo No cenário de calamidade assistido durante a pandemia de Covid-19, o ato de definir a causa básica de um óbito não foi trivial e o aumento da utilização dos códigos garbage (códigos mal definidos ou pouco específicos) traz preocupação em relação à qualidade da informação sobre as causas de morte nos estados do Brasil. Constatou-se um aumento significativo do número de óbitos no período pandêmico no estado da Paraíba, localizado na região Nordeste do Brasil, situação que se assemelhou ao contexto nacional. Em 2020 ocorreram 31.107 óbitos na Paraíba, enquanto no período de 2015 a 2019 foi identificada uma média de 27.000 óbitos. O objetivo do presente estudo é identificar e mensurar o excesso de óbitos classificados com códigos garbage em 2020, no estado da Paraíba, durante a pandemia de Covid-19. As séries temporais de óbitos foram obtidas do Painel de Monitoramento da Mortalidade por Causas Básicas Inespecíficas ou Incompletas (garbage codes) do Ministério da Saúde, para todos os meses de 2015 a 2020. Foram calculadas as projeções de óbitos totais e dos óbitos por códigos garbage (CG). O cálculo do excesso da proporção de óbitos classificados por códigos garbage no estado da Paraíba, em 2020, resultou em 8,58%, destacando-se o mês de junho com o maior valor (19,42%). Espera-se contribuir para avanços no conhecimento da realidade da qualidade da informação da notificação dos óbitos em uma área do país que almeja avanços nesse sentido e evidenciar a necessidade da investigação em outras localidades do Brasil.


Abstract In the disaster scenario witnessed during the pandemic caused by COVID-19, the act of defining the underlying cause of a death was no trivial matter and the increased use of Garbage Codes (poorly defined or not very specific codes) raises concerns regarding the quality of the information on causes of death in the states of Brazil. There was a significant increase in the number of deaths during the pandemic in the state of Paraíba, located in the Northeast region of Brazil, a situation similar to the national context. There were 31,107 deaths in 2020 in Paraíba, while an average of 27,000 deaths were identified for the period 2015 to 2019. Our goal was to identify and measure the excess of deaths classified with Garbage Codes in the COVID-19 pandemic in the geographic space of the state of Paraíba in 2020. The time series of deaths were obtained from the Mortality Monitoring Panel for Unspecific or Incomplete Basic Causes (Garbage Codes) of the Ministry of Health, for all months of the years 2015 to 2020. Projections of total deaths and deaths by Garbage Codes (GC) were calculated. The calculation of the excess proportion of deaths classified by Garbage Codes in the state of Paraíba resulted in 8.58%, highlighting the month of June 2020, which reached the maximum value equal to 19.42%. We expect to contribute to advance knowledge regarding the current reality of quality information on death notifications in an area of the country that seeks to make progress in this direction and to highlight the need for research in other locations in Brazil.


Resumen En el escenario de desastre que se vivió durante la pandemia de COVID-19, definir la causa subyacente de una muerte no fue baladí y el aumento del uso de códigos garbage (códigos mal definidos o poco específicos) genera preocupaciones acerca la calidad de la información sobre las causas de muerte en los estados de Brasil. En cuanto al aumento del número de muertes en el período de la pandemia en el estado de Paraíba, en la región Nordeste de Brasil, la situación fue similar al contexto nacional: 31.107 muertes en 2020, mientras que durante el período 2015-2019 el promedio fue de 27.000 muertes. El objetivo de este trabajo fue identificar y medir el exceso de muertes clasificadas con códigos Garbage en la pandemia de COVID-19 en el estado de Paraíba en 2020. Las series temporales de defunciones se obtuvieron del Panel de Seguimiento de Mortalidad por Causas Básicas Inespecíficas o Incompletas del Ministerio de Salud, para todos los meses de 2015 a 2020. Se calcularon proyecciones de muertes totales y muertes por códigos basura, para los que el cálculo de la proporción de exceso de muertes resultó en 8,58 %, con un valor de 19,42 % durante junio. Se espera contribuir a la calidad de la información sobre las notificaciones de muerte en un área del país que busca avances en esa dirección y resaltar la necesidad de investigaciones en otras localidades de Brasil.

ABSTRACT

Objective To explore the impact of temperature and environmental factors on the risk of deaths from cardiovascular and cerebrovascular diseases (CVD) in Wuhan during the course of high temperature. Methods The daily CVD death data and meteorological and atmospheric pollutant concentration data during the high temperature process in Wuhan from 2014 to 2019 were collected. The generalized additive models (GAM) were used to obtain the characteristics of meteorological factors, atmospheric pollutant concentrations, high impact factors and thresholds affecting the relative risk of CVD death. Results The analysis results showed that: (1) When the maximum temperature reached 36.7°C and the lowest temperature reached 25.3°C, the relative risk of CVD death increased significantly; (2) The risk of CVD death during the first high temperature process was the largest, and the average CVD excess mortality rate during the first high temperature process from 2014 to 2019 reached 21.7%; and (3) The average temperature, maximum temperature and PM10 during the course of high temperature were important environmental factors that increased the risk of CVD deaths, and the relative risks were 1.14 (95% confidence interval (95% CI): 1.11-1.17), 1.11 (95% confidence interval (95% CI): 1.08-1.15, and 1.06 (95% confidence interval (95% CI): 1.02-1.09), respectively. Conclusion Temperature and PM10 are important environmental factors that increase the risk of death from CVD during the high temperature process in Wuhan from 2014 to 2019, with the first annual high temperature process having the greatest impact on the risk of CVD death.

ABSTRACT

As one of notable works of HUANG Yuanyu, a renowned physician from the Qing Dynasty, The Indications of Classics by Four Medical Sage emphasize the theoretical paradigm of “one qi circulation” with a particular focus on the middle qi. Building upon disease differentiation and treatment, HUANG Yuanyu attaches great importance to understanding the underlying cause of the disease. It is believed that the imbalance of the six climatic factors is the fundamental cause of diseases. The concept of mutual interaction between the physiological and pathological aspects of the six climatic factors is proposed to grasp the basic pathogenesis. Based on the individual's specific condition of six climatic factors, excess or deficiency, specific prescriptions are formulated. The diagnosis and treatment system can be summarized as a “differentiation of diseases → disease etiology → differentiation of the condition of six climatic factors → individualized pathogenesis of traditional Chinese medicine (TCM) → prescription of TCM” model based on the holistic view. HUANG Yuanyu' s diagnosis and treatment system is highly practical and can provide reference for clinical diagnosis and treatment of diseases in TCM.

ABSTRACT

Objective: To explore the correction factors of lung cancer caused by radon exposure in a uranium mine, and estimate the excess relative risk (ERR) coefficient of lung cancer caused by radon in the uranium miners. Methods: Male miners who worked in a uranium mine more than one year in Hunan Province from 1958 to 2018 were selected. This study preliminarily estimated the ERR coefficient of lung cancer caused by radon in the miners with different corrections using the Possion regression model. Results: This study cohort included 4 851 uranium miners, with 187 miners died with lung cancer from 1958 to 2018, and cumulative follow-up of 207 251 person-year. The ERR coefficient of lung cancer caused by radon without correction factors was estimated to be 0.21%/WLM (95%CI: 0.04%/WLM-0.27%/WLM). In the final model, the exponential correction factors of radon-induced lung cancer were time since exposure and exposure rate. In this model, if time since exposure was 45 years and the average exposure rate was 0.14 WL, the estimated ERR coefficient was 1.73%/WLM (95%CI: 0.36%/WLM-3.11%/WLM). The ERR decreases by about 60.00% for every 10 years since exposure, and increases by about 30.00% for every one WL increase exposure rate. Conclusion: The correction factors of lung cancer caused by radon in uranium miners in this mine were the time since exposure and exposure rate. ABSTRACT

Hepatic osteodystrophy (HO) should be treated by stages based on the differentiation of root deficiency and branch excess. The root cause of HO is the insufficiency of the liver, spleen, and kidney, and the branch onset should be differentiated by stages that in the incubation, the pathogenesis is shaoyang (少阳) constraint and block and cardinal disturbance, while in the attack period, it is turbid toxin, static heat, scorching marrow and withering bones. In treatment, attention should be paid to regulating and tonifying the depletion of the liver, spleen, and kidney to cultivate the foundation. In the incubation period, it is suggested to put focus on unblocking cardinal disturbance of shaoyang liver and gallbladder so as to regulate and harmonize qi and blood. In the attack period, the focus should be on dissolving stasis, removing turbidity, and clearing the source to promote gallbladder function and bone strength. In clinical practice, medication should be modified according to individual symptoms, and the root and the branch, the primary and the secondary should be differentiated to closely follow the pathogenesis and be tailored according to the symptoms. At the same time, reasonable and safe medication should be emphasized to protect liver function.

ABSTRACT

Mercury is highly toxic and can be absorbed through skin contact. From December 5, 2020 to February 16, 2021, occupational disease laboratory of the First People's Hospital of Baiyin received 30 urine mercury test samples from a beauty salon in Lanzhou City. The test results showed that 28 samples exceeded the normal value (normal value: 4 μg/g Cr) . 15 patients were treated with sodium dimertopropyl sulfonate for mercury removal and tiopron for liver protection, and the prognosis was good.

ABSTRACT

A randomized, double-blind, placebo-controlled, multi-center phase Ⅱ clinical trial design was used in this study to recruit subjects who were in line with the syndrome of excess heat and fire toxin, and were diagnosed as recurrent oral ulcers, gingivitis, and acute pharyngitis. A total of 240 cases were included and randomly divided into a placebo group and a Huanglian Jiedu Pills group. The clinical efficacy of Huanglian Jiedu Pills in treating the syndrome of excess heat and fire toxin was evaluated by using the traditional Chinese medicine(TCM) syndrome scale. Enzyme-linked immunosorbent assay(ELISA) was used to determine and evaluate the levels of adenosine triphosphate(ATP), 4-hydroxynonenal(4-HNE), and adrenocorticotropic hormone(ACTH) in plasma of the two groups before and after administration and to predict their application value as clinical biomarkers. The results showed that the disappearance rate of main symptoms in the Huanglian Jiedu Pills group was 69.17%, and that in the placebo group was 50.83%. The comparison between the Huanglian Jiedu Pills group and the placebo group showed that 4-HNE before and after administration was statistically significant(P<0.05). The content of 4-HNE in the Huanglian Jiedu Pills group decreased significantly after administration(P<0.05), but that in the placebo group had no statistical significance and showed an upward trend. After administration, the content of ATP in both Huanglian Jiedu Pills group and placebo group decreased significantly(P<0.05), indicating that the energy metabolism disorder was significantly improved after administration of Huanglian Jiedu Pills and the body's self-healing ability also alleviated the increase in ATP level caused by the syndrome of excess heat and fire toxin to a certain extent. ACTH in both Huanglian Jiedu Pills group and placebo group decreased significantly after administration(P<0.05). It is concluded that Huanglian Jiedu Pills has a significant clinical effect, and can significantly improve the abnormal levels of ATP and 4-HNE in plasma caused by the syndrome of excess heat and fire toxin, which are speculated to be the effective clinical biomarkers for Huanglian Jiedu Pills to treat the syndrome of excess heat and fire toxin.

ABSTRACT

Objective To observe the effect of excess oxygen supply for different time periods on the mitochondrial energy metabolism in alveolar epithelial type Ⅱ cells. Methods Rat RLE-6TN cells were assigned into a control group (21% O2 for 4 h) and excess oxygen supply groups (95% O2 for 1,2,3,and 4 h,res-pectively).The content of adenosine triphosphate (ATP),the activity of mitochondrial respiratory chain complex V,and the mitochondrial membrane potential were determined by luciferase assay,micro-assay,and fluorescent probe JC-1,respectively.Real-time fluorescence quantitative PCR was employed to determine the mRNA levels of NADH dehydrogenase subunit 1 (ND1),cytochrome b (Cytb),cytochrome C oxidase subunit I (COXI),and adenosine triphosphatase 6 (ATPase6) in the core subunits of mitochondrial respiratory chain complexes Ⅰ,Ⅲ,Ⅳ,and Ⅴ,respectively. Results Compared with the control group,excess oxygen supply for 1,2,3,and 4 h down-regulated the mRNA levels of ND1 (q=24.800,P<0.001;q=13.650,P<0.001;q=9.869,P<0.001;q=20.700,P<0.001),COXI (q=16.750,P<0.001;q=10.120,P<0.001;q=8.476,P<0.001;q=14.060,P<0.001),and ATPase6 (q=22.770,P<0.001;q=15.540,P<0.001;q=12.870,P<0.001;q=18.160,P<0.001).Moreover,excess oxygen supply for 1 h and 4 h decreased the ATPase activity (q=9.435,P<0.001;q=11.230,P<0.001) and ATP content (q=5.615,P=0.007;q=5.029,P=0.005).The excess oxygen supply for 2 h and 3 h did not cause significant changes in ATPase activity (q=0.156,P=0.914;q=3.197,P=0.116) and ATP content (q=0.859,P=0.557;q=1.273,P=0.652).There was no significant difference in mitochondrial membrane potential among the groups (F=0.303,P=0.869). Conclusion Short-term excess oxygen supply down-regulates the expression of the core subunits of mitochondrial respiratory chain complexes and reduces the activity of ATPase,leading to the energy metabolism disorder of alveolar epithelial type Ⅱ cells.

ABSTRACT

PURPOSE@#Hip fractures in elderly have a high mortality. However, there is limited literature on the excess mortality seen in hip fractures compared to the normal population. The purpose of this study was to compare the mortality of hip fractures with that of age and gender matched Indian population.@*METHODS@#There are 283 patients with hip fractures aged above 50 years admitted at single centre prospectively enrolled in this study. Patients were followed up for 1 year and the follow-up record was available for 279 patients. Mortality was assessed during the follow-up from chart review and/or by telephonic interview. One-year mortality of Indian population was obtained from public databases. Standardized mortality ratio (SMR) (observed mortality divided by expected mortality) was calculated. Kaplan-Meir analysis was used.@*RESULTS@#The overall 1-year mortality was 19.0% (53/279). Mortality increased with age (p < 0.001) and the highest mortality was seen in those above 80 years (aged 50 - 59 years: 5.0%, aged 60 - 69 years: 19.7%, aged 70 - 79 years: 15.8%, and aged over 80 years: 33.3%). Expected mortality of Indian population of similar age and gender profile was 3.7%, giving a SMR of 5.5. SMR for different age quintiles were: 3.9 (aged 50 - 59 years), 6.6 (aged 60 - 69 years), 2.2 (aged 70 - 79 years); and 2.0 (aged over 80 years). SMR in males and females were 5.7 and 5.3, respectively.@*CONCLUSIONS@#Indian patients sustaining hip fractures were about 5 times more likely to die than the general population. Although mortality rates increased with age, the highest excess mortality was seen in relatively younger patients. Hip fracture mortality was even higher than that of myocardial infarction, breast cancer, and cervical cancer.

Objective: to estimate excess deaths during the COVID-19 pandemic in the state of Santa Catarina and its macro-regions, Brazil, 2020-2021. Methods: this was an ecological study, using data from the Mortality Information System; excess deaths in adults were calculated by the difference between the observed number of deaths and expected number of deaths, taking into account the average of deaths that occurred between 2015 and 2019; the variables "macro-region of residence", "quarter", "month", "sex" and "age group" were analyzed; data were analyzed in a descriptive manner. Results: a total of 6,315 excess deaths in 2020 and 17,391 in 2021, mostly in males (57.4%) and those aged 60 years and older (74.0%); macro-regions and periods with the greatest excess deaths were those in which there were most deaths due to COVID-19; the greatest excess deaths occurred in March 2021 (n = 4,207), with a progressive decrease until the end of the year. Conclusion: there were excess deaths in the state of Santa Catarina and in all its macro-regions during the COVID-19 pandemic.


Objetivo: estimar el exceso de muertes en Santa Catarina y macrorregiones durante la pandemia de COVID-19, en los años 2020 y 2021. Métodos: estudio ecológico utilizando Sistema de Información de Mortalidad. El exceso de defunciones en adultos se calculó por la diferencia entre defunciones observadas y esperadas, considerando el promedio de defunciones entre 2015 y 2019. Las variables analizadas fueron: macrorregión de residencia, trimestre, mes, sexo y grupo de edad. Los datos se analizaron descriptivamente. Resultados: el exceso fue de 6.315 defunciones en 2020 y 17.391 en 2021, mayor en varones (57,4%) y mayores de 60 años (74,0%). Las macrorregiones y periodos con mayor superávit fueron los que registraron más muertes por COVID-19. El mayor exceso de muertes ocurrió en marzo de 2021 (n=4.207), con una disminución progresiva hasta el final del año. Conclusión: hubo exceso de muertes en Santa Catarina y en todas macrorregiones durante la pandemia de COVID-19.

Introducción: En las últimas semanas de 2022 aparecen nuevas informaciones acerca del exceso de muertes durante la pandemia de COVID-19 en 2020 y 2021 en múltiples naciones. Objetivo: Comentar los aspectos de mayor interés en las últimas informaciones publicadas. Métodos: Análisis de documentos sobre COVID-19 publicados en 2020 y 2021 y síntesis de la información recopilada. Resultados: Un importante estudio de la OMS estima un exceso de muertes de 14,83 (13,23 ± 16,58) millones más de las esperadas en todo el mundo, durante 2020 y 2021. Estos estimados son más conservadores que los de otras instituciones que estimaron entre 16 y 18,2 millones. El informe de OMS estima que cuatro de cada cinco países con mayores excesos de muertes ocurren en estados de "medianos ingresos", con algunos de los peores 25 resultados en América Latina (Cuba no está incluida). No se documenta todavía lo ocurrido durante 2022 que ya termina. Conclusiones: En el futuro se debe mantener la vigilancia de la evolución de las cifras de mortalidad y la estructura por causas de muerte en las poblaciones.


Introduction: In the last weeks of 2022, new information appears about the excess deaths during the COVID-19 pandemic in 2020 and 2021 in multiple nations. Objective: To comment on the aspects of greatest interest in those cited documents. Methods: Analysis of documents on COVID-19 was conducted on those published in 2020 and 2021 and synthesis of the information collected. Results: A major WHO study estimates an excess of deaths of 14.83 (13.23 ± 16.58) million more than expected worldwide, during 2020 and 2021. These estimates are more conservative than those of other institutions that estimated between 16 and 18.2 million. The WHO report estimates that four out of five countries with the highest excess deaths occur in "middle-income" states, with some of the worst outcomes in Latin America. Cuba is not included. What happened during 2022, which is already ending, has not yet been documented. Conclusions: It is considered that monitoring of the evolution of mortality figures and the structure by causes of death in populations should be maintained in the future.

ABSTRACT

Background and objectives: children's food-related lifestyle brought changes in their weight status and eating patterns. We aimed to investigate the association between food consumption pattern and excess weight in preschoolers aged six. Methods: a cross-sectional analysis of a prospective birth cohort, in 2004, in Feira de Santana-BA. Food consumption frequency was verified, characterized by patterns and established through a food frequency questionnaire, validated in a previous study, in pattern 1: milk, vegetables, cereals, legumes, fruits and fish; pattern 2: snacks, soda/artificial juice, oils, sweets and coffee/tea; pattern 3: sausages, fast food, ketchup/mayonnaise and egg; pattern 4: red meat and chicken. Excess weight was defined according to the World Health Organization criteria. The main association was assessed using Pearson's chi-square test and Poisson regression. Results: a total of 618 children were investigated. Excess weight occurred in 28.6%, and pattern 3 frequency was 68%. In the bivariate analysis, only pattern 3 was associated with children's excess weight (PR: 1.23; 95%CI: 1.01-1.63). In the multivariate analysis, the prevalence of excess weight children who consumed pattern 3 was 50% (adjustedPR: 1.50; 95%CI: 1.01­1.93). Conclusion: moderate/high consumption of sausages, fast food, ketchup/mayonnaise and eggs was associated with excess weight among children in the sample.(AU)


Justificativa e objetivos: o estilo de vida das crianças relacionado à alimentação trouxe mudanças no estado de peso e no padrão alimentar. Objetivou-se averiguar a associação entre o padrão de consumo alimentar e o excesso de peso em pré-escolares aos seis anos de idade. Métodos: análise transversal, de uma coorte prospectiva de nascimento, em 2004, em Feira de Santana-BA. Verificou-se a frequência do consumo alimentar, caracterizado por padrões e estabelecido por meio do questionário de frequência alimentar, validado em um estudo prévio, em padrão 1: leite, verduras, cereais, leguminosas, frutas e pescados; padrão 2: salgadinhos, refrigerantes/sucos artificiais, óleos, doces e cafés/chás; padrão 3: embutidos, fast food, ketchup/maionese e ovo; padrão 4: carnes vermelhas e frango. O excesso de peso foi definido segundo os critérios da Organização Mundial da Saúde. A associação principal foi avaliada mediante Teste Qui-Quadrado de Pearson e regressão de Poisson. Resultados: investigaram-se 618 crianças. O excesso de peso ocorreu em 28,6%, e a frequência do padrão 3 foi de 68%. Na análise bivariada, somente o padrão 3 se associou com o excesso de peso da criança (RP: 1,23; IC95%: 1,01-1,63). Na análise multivariada, a prevalência de crianças que consumiam o padrão 3 com excesso de peso foi de 50% (RPajustado: 1,50; IC95%: 1,01­1,93). Conclusão: o consumo moderado/alto de embutidos, fast food, ketchup/maionese e ovo se associou com o excesso de peso entre as crianças.(AU)

El objetivo fue investigar la asociación entre el patrón de consumo de alimentos y el sobrepeso en preescolares de seis años. Métodos: análisis transversal de una cohorte prospectiva de nacimiento, en 2004, en Feira de Santana-BA. Se verificó la frecuencia de consumo de alimentos, caracterizada por patrones y establecida a través del cuestionario de frecuencia de alimentos, validado en un estudio previo, en el patrón 1: leche, verduras, cereales, legumbres, frutas y pescado; patrón 2: bocadillos, refrescos/jugos artificiales, aceites, dulces y café/té; patrón 3: salchichas, fast food, salsa de tomate/mayonesa y huevo; patrón 4: carnes rojas y pollo. El exceso de peso se definió según los criterios de la Organización Mundial de la Salud, la asociación principal se evaluó mediante la prueba de chi-cuadrado de Pearson y regresión de Poisson. Resultados: 618 niños fueron investigados. El exceso de peso se presentó en el 28,6% y la frecuencia del patrón 3 fue del 68%. En el análisis bivariado, solo el patrón 3 se asoció con el exceso de peso del niño (RP: 1,23; IC95%: 1,01-1,63). En el análisis multivariado, la prevalencia de niños con sobrepeso que consumieron el patrón 3 fue del 50% (RPajustado: 1,50; IC95%: 1,01­1,93). Conclusión: el consumo moderado/alto de salchichas, fast food, ketchup/mayonesa y huevos se asoció con sobrepeso entre los niños de la muestra.(AU)

ABSTRACT

Screen-based media usage among young people is blooming rapidly due to technological and digital revolution. We conducted community-based cross-sectional study to determine the prevalence of excess screen time and its association with sociodemographic and behavioral patterns in a rural block of Haryana, India. Asemi-structured interview schedule was administered by trained physicians to ascertain screen time in a typical day and various socioeconomic and behavioral factors among a random sample of 860 young men aged 18–24 years. The prevalence of excess screen time was 61.8% (95% confidence interval [CI] 58.4–65.1). It was significantly associated with education (adjusted odds ratio [AOR] 1.7, 95% CI 1.1–2.6) and occupation (AOR 2.2, 95% CI 1.2–3.9) of the father and their sleep duration of ?8 h (AOR 1.6, 95% CI 1.2–2.3). Limiting the screen time as per international standards and behavioral interventions are needed for this young population.

Introducción: La corrección del exceso vertical del maxilar traerá no solo cambios verticales en el perfil, sino también sagitales y transversales. La ausencia clínica de mordida cruzada posterior puede enmascarar deficiencias del ancho y provocar planes de tratamiento incorrectos. Objetivo: Describir los aspectos más importantes en la atención de un caso con exceso vertical y deficiencia transversal del maxilar sin mordida cruzada posterior. Presentación de caso: Paciente femenina de 26 años de edad acude a consulta por "dientes hacia adelante". El examen físico y el estudio de los medios diagnósticos permitieron concluir la existencia de exceso vertical y deficiencia transversal del maxilar. Clínicamente no se observó mordida cruzada posterior. El tratamiento por etapas incluyó ortodoncia, expansión e impactación quirúrgica del maxilar. Conclusiones: El ascenso quirúrgico del maxilar debe planificarse a partir de un análisis holístico. Incluye los cambios sagitales del perfil y de la relación transversal entre las arcadas, provocados por el efecto de rotación mandibular. La ausencia de mordida cruzada posterior no excluye la existencia de deficiencia transversal del maxilar. Asimismo, la dimensión definitiva de la expansión debe establecerse una vez que la arcada inferior alcance sus dimensiones definitivas(AU)


Introduction: The correction of the vertical excess of the maxilla will bring not only vertical changes in the profile, but also sagittal and transverse ones. The clinical absence of posterior crossbite can mask width deficiencies and lead to incorrect treatment plans. Objective: Describe the most important aspects in the care of a case with vertical excess and transverse deficiency of the maxilla without posterior cross bite. Case presentation: A 26-year-old female patient comes to the consultation for "teeth forward". The physical examination and the study of the diagnostic means allowed to conclude the existence of vertical excess and transverse deficiency of the maxilla. Clinically, no posterior crossbite was observed. Staged treatment included orthodontics, expansion and surgical impaction of the maxilla. Conclusions: The surgical ascent of the maxilla should be planned based on a holistic analysis. It includes the sagittal changes of the profile and the transverse relationship between the arcades, caused by the effect of mandibular rotation. The absence of posterior crossbite does not exclude the existence of transverse maxillary deficiency. Likewise, the definitive dimension of the expansion must be established once the lower arcade reaches its definitive dimensions(AU)
