ABSTRACT

Background: Tuberculosis (TB) is a disease that has both medical as well as social dimensions. Stigma and associated discrimination experienced by persons affected by TB is a barrier to the interventions towards TB elimination. A better understanding of stigma will improve the effectiveness of the interventions aimed to al-leviate the effects of TB stigma. The objectives were to measure the prevalence of self, anticipated, experi-enced stigma and its associated factors and to explore the impact of tuberculosis related stigma among TB pa-tients. Methodology: A cross-sectional study was conducted in Puducherry district, Southern India among 420 adult drug sensitive non-HIV TB patients registered under NTEP. Data was collected by face-to-face interview using standardised questionnaire. Results: The mean(±SD) age of the study participants was 44.5 (±15.03) years. Majority were males (267, 63.6%) and were married (330, 78.6%). The prevalence of stigma among TB patients was found to be 69.3%. Perceived stigma was noted in 47.1%, 33.6% had self- stigma and 26.0% had experienced stigma. On measur-ing the impact of stigma, 52.6% reported participation restriction. Illiteracy and lower-socio economic status were found to be significantly associated with TB stigma. Conclusions: Stigma among TB patients was high. Tailored patient-centric interventions to address stigma and discrimination using culturally appropriate and locally available resources are necessary

ABSTRACT

To summarize Professor ZHANG Zhiyuan's experience in treating leukorrheal diseases based on syndrome differentiation. It is believed that the main pathogenesis of leukorrheal diseases includes mainly depression, dampness and deficiency. Leukorrheal diseases can be classified into four syndrome types: syndrome of stagnation of liver qi and spleen deficiency, syndrome of internal retention of damp-heat, syndrome of cold-dampness invading downward, and syndrome of kidney yang deficiency. The treatment approach focuses on restoring the ascending and descending opening and closing of qi, regulating the liver and spleen to promote yang qi respectively. The four types of syndromes correspond to the application of four different herbal formulas: By using the combination of Danggui Shaoyao Powder (当归芍药散) from the Synopsis of Golden Chamber (《金匮要略》) with wind-dispelling herbs, adjustments are made to harmonize the liver and spleen; By applying modifications to the Baxian Drink (八仙饮) from the Obstetrics Enlightenment (《产科发蒙》) to clear heat and promote diuresis; A self-designed Shiwei Wendai Decoction (十味温带汤) is adjusted to warm the yang and dispel cold; The Baji Tusi Decoction (巴戟菟丝汤) from the New Compilation of Effective Recipes (《验方新编》) is adjusted to tonify and warm the kidney yang.

ABSTRACT

ABSTRACT Despite the high sustained virologic response (SVR) rates of direct-acting antiviral (DAAs) therapy, a small number of patients does not eradicate the virus, and these patients represent a challenge. This study aims to compare the outcomes of three second-line regimens for DAAs-experienced patients with chronic hepatitis C (CHC). This prospective observational study was conducted at the Damanhur Viral Hepatitis Center from January 2017 to February 2020. We included patients with CHC who did not achieve SVR after the complete course of Sofosbuvir/Daclatasvir±Ribavirin (SOF/DAC±RBV). The primary endpoint was SVR-12 after re-treatment. This study included 360 patients (with a mean age of 51.53±11.38 years). Approximately 51.1% of the patients were males, and 65.5% had liver cirrhosis. All patients of group 1 (45 patients) received SOF/VEL/VOX over 12-weeks; SVR-12 was achieved in 44 patients (97.8%). Group 2 (28 patients) received SOF/DAC/RBV over 24-weeks; (one patient was lost during follow-ups and one patient discontinued treatment due to hepatic decompensation). SVR-12 was achieved in 25 patients (96.2%). Group 3 (287 patients) received SOF/Ombitasvir/Paritaprevir/Ritonavir/RBV) over 12-weeks. Eight patients were lost during follow-ups, and one patient discontinued treatment due to grade 4 adverse events. SVR-12 was achieved in 276 patients (99.3%). There was no difference between the groups regarding their age, gender distribution, baseline viral load or comorbidities. Adverse events (thrombocytopenia, anemia, hyperbilirubinaemia and prolonged INR) were significantly higher in group 3, while group 1 did not experience any. The three studied retreatment regimens can be used for DAAs treatment-experienced patients considering availability. The SOF/VEL/VOX combination had the least adverse events.

ABSTRACT

Background: Risky sexual behavior (RSB) is common among youths which predispose them to sexually transmitted infections. This study sets out to identify the factors associated with RSB among undergraduates in Osun state. Methods: The study design was descriptive cross sectional and a total of 550 respondents from two universities in Osun state were sampled using a multistage sampling technique, out of which data from 266 sexually experienced respondents was further analyzed. Data collected via a semi­structured self-administered questionnaire and analyzed using univariate, bivariate and multivariate analyses. Results: The 266 respondents consisted of 54.5% males and 45.5% females and larger percentage of them were in the age group 15-24years. Seven out of ten respondents (69.9%) were sexually active while 65.8% were involved in at least one RSB. Of the 266 respondents, 28.6% ever had concurrent multiple sexual partners, 15.8% used alcohol/drugs at last sex while 48.1% did not use condom at last sex. More males (71.7%), alcohol users (76.8%), drug users (78.0%), pornography watchers (82.7%), internet users (71.2%) respondents "not in good terms with mum" (86.7%) and "those whose mum doesn't instruct them morally" (84.2%) were involved in RSB compared to their respective counterparts (p<0.05). However, there were no identifiable predictors of RSB on regression analysis. Conclusion: Risky Sexual Behavior is prevalent among undergraduates with males being more involved, among other associated factors. Concerned stakeholders should engage youths via behavioral change communication strategies so as to significantly reduce their involvement in RSB

ABSTRACT

This study is based on the investigation of Japanese ancient experienced agricultural techniques and historical and current exploration of strain and morphology of plants. We aim to dig up the tacit knowledge, which can guarantee the quality of production and profitability when they are applied for current medicinal plant cultivation. We focused on Paeonia lactiflora Pallas as the candidate of the farming products for the vitalization of mountainous area. Bon-ten (with white double-petaled flowers) is considered to produce highly qualified Paeoniae Radix in medical practice. We found several strains of the plant source different from Bon-ten as the origin of Yamato-Shakuyaku, bred after TOKUGAWA Yoshimune’s policy promoting the domestic cultivation of medicinal plants in Edo period. These strains include the cultivar with red single-petaled flower, which is drawn in ‘Matsuyama-Honzo’ by Morino Tosuke-Saikaku, who established private herb garden, ‘Morino herbal garden,’ supported by Tokugawa shogunate. Among Morino family documents, we also found the unpublished historical papers suggesting the various productions of crude drugs at Yamato region in Edo period, and considered the transition of domestic production area, cultivation strains and cultivation skills. We can find the descriptions of picking of buds and flowers among current production manuals, but it is suggested the possibility of flowers for ornamental or commercial use to improve economic efficiency by the investigation of regional cultures and interviews of experts of crude drugs.

ABSTRACT

Objective:We aimed to analyze the spectrum of diseases of Experienced Ten Acupoints in clinical studies, in order to provide reference for further clinical trials.Methods:The Chinese academic journals full-text database (CNKI),Chinese science and technology journals full-text database (VIP),Wanfang database and Chinese biomedical database (SinoMed), PubMed, Cochrane Library and Web of Science Database were searched from the incecption to October, 2021. The quantitative analyses were conducted on the characteristics of the clinical studies, including years of publication, journals, affiliations of authors, interventions, diseases, types of studies, evaluation of efficacy, rate of citations.Results:A total of 81 articles were included in 44 journals. The majority types of studies were RCTs, the main intervention was acupuncture as well as acupuncture combined with Traditional Chinese Medicine or medicine. For types of diseases, digestive system diseases account for the major part (66.67%), followed by nervous system diseases (17.28%).Conclusions:The studies associated with Experienced Ten Acupoints were relatively few, the corresponding spectrum of diseases was in limit range, and experimental studies lacks. This study suggested that the clinicians should pay attention to the classical acupoints prescriptions, and the research progress of clinical trials should be more standardized.

O romance Mrs. Dalloway, escrito pela inglesa Virginia Woolf (1882-1941), é um marco na literatura universal por apresentar um novo formato na arte da escrita intitulado fluxo de consciência. Esta forma estilística visa descrever as falas e os pensamentos dos personagens por meio do discurso indireto livre, integrado à preleção do narrador. O enredo se passa em um único dia da vida da protagonista Clarissa Dalloway, no qual são captados os ínfimos detalhes do que ela experiencia. Dentre os fenômenos vivenciados encontramos a noção de tempo, por meio da qual o narrador mistura homogeneamente o fluxo passado, presente e futuro. O objetivo deste artigo é discutir a vivência do tempo na referida obra literária pautada numa discussão fenomenológica. Para isto, utilizamos como método a revisão narrativa da literatura de autores que contribuíram para o desenvolvimento da noção de tempo vivido na fenomenologia. É no fluxo do tempo que se dão os significados das experiências na história dos personagens no seu contato ambíguo com o mundo. The novel Mrs. Dalloway, written by English writer Virginia Woolf (1882-1941), is a landmark in universal literature for presenting a new format in the art of writing named Stream of Consciousness. This stylistic form aims to describe the speeches and thoughts of the characters through free indirect speech, integrated to the narrator's lecture. The novel takes place in a single day of the life of the protagonist Clarissa Dalloway, capturing the minute details of what she experiences. Among the phenomena experienced we find the notion of time, whereby the narrator mixes the past, present, and future flux into one envelope. This article thus discusses the experience of time in this literary work based on a phenomenological discussion. We conducted a review of the literature including authors that contributed to the development of the notion of experienced time in phenomenology. In the flow of time lie meanings of the experiences of characters in their ambiguous contact with the world. We conclude that the descriptions elaborated by Virginia Woolf allow for an approximation to the experience of time that crosses the totality of the dimension of being...(AU)


La novela La Señora Dalloway, de la escritora inglesa Virginia Woolf (1882-1941), es un hito en la literatura universal por presentar un nuevo formato en el arte de la escritura titulado "flujo de conciencia". Esta forma estilística pretende describir las palabras y los pensamientos de los personajes por medio del discurso indirecto libre, integrado a la prelección del narrador. La historia de la novela se pasa en un único día de la vida de la protagonista Clarissa Dalloway, en el que se captan los ínfimos detalles de lo que ella experimenta. Entre los fenómenos vivenciados encontramos la noción de tiempo, por medio de la cual el narrador mezcla el flujo pasado, presente y futuro en un envoltorio único. El objetivo de este artículo es discutir la vivencia del tiempo en la referida obra literaria pautada en una discusión fenomenológica. OBJECTIVE@#To verify the clinical effect of acupuncture at "experienced ten acupoints" for functional dyspepsia (FD) of liver stagnation and spleen deficiency.@*METHODS@#A total of 96 patients with FD of liver stagnation and spleen deficiency were randomly divided into an observation group and a control group, 48 cases in each group. In the observation group, "experienced ten acupoint" prescription was applied,acupoints included Shangwan (CV 13), Zhongwan (CV 12), Xiawan (CV 10), Qihai (CV 6), Tianshu (ST 25), Zusanli (ST 36) and Neiguan (PC 6). In the control group, irrelevant acupoint prescription was applied, acupoints included Feiyang (BL 58), Heding (EX@*RESULTS@#Compared before treatment, the symptom score after treatment was decreased in the observation group (@*CONCLUSION@#Acupuncture at "experienced ten acupoints" can improve symptoms in patients with FD of liver stagnation and spleen deficiency, has the better clinical effect.

ABSTRACT

Background:Untreated maternal Human Immunodeficiency Virus (HIV) infection is associated with adverse pregnancy outcome including preterm birth, low birth weight, and mother-to-child transmission of the virus. This study aimed to compare the pregnancy outcome between HIV infected mothers who received ART in pregnancy and those who were ART-naïve. Methods:A cross-sectional study of HIV-infected mothers who brought their infants for follow up between November 2007 and May 2017 at the University of Port Harcourt Teaching Hospital, Port Harcourt, Nigeria.Relevant information obtained include: time of diagnosis, antiretroviral therapy (ART) regimen and when it was commenced, gestational age at delivery and birthweight of child, mode of delivery, infant feeding option and ARV prophylaxis. Infection status of the infant was determined by DNA PCR at 6weeks of age. Based on when ART was commenced, mothers were grouped into three [(HAART experienced (HE) if ART was started before pregnancy, HAART in pregnancy (HIP) and HAART naive (NH) if no HAART was taken in pregnancy].Main outcome measures were rates of prematurity, low birth weight, mean birth weight, birth defects and mother-to-child transmission Result:A total of 1,640 HIV-exposed infants were seen (716(43.6%) in HE, 360(22.0%) in HIPand 564(34.4%) in NH groups). There were 825(50.3%) males and 815(49.7%) females. Zidovudine/Lamivudine and Nevirapine/Efavirenz was the most frequently used combined ART in 724 (67.3%) mothers. The mean birthweight was 3.12±0.38Kg; range 1.2 –5.7Kg (3.11±0.58Kg in HE; 3.13±0.53Kg in HIP; 3.18±0.74Kg in NH) Table 3. A hundred and eighty (11.0%) babies were preterm [76(42.2%) in HE; 26(14.4%) in HIP; 78(43.3%) in NH](p=0.007), while 159(9.7%) were LBW [74(46.5%) in HE; 22(13.8%) in HIP; 63(39.6%) in NH](p=0.03). Fourteen (0.9%) babies had birth defects [5(35.7%) in HE; 9(64.3%) in HIP] (p=0.01). The commonest birth defects were neural tube defect 7(50%) and congenital heart defect 4(28.8%). Overall transmission rate was 21.4% [8% in the HE, 4.5% in HIP and 87.5% in NH groups] (p=0.001). The mean birth weights of uninfected babies were higher than their infected counterparts but was not significant (p>0.05).Conclusion:The benefits of early HAART in reducing mother-to-child transmissionmust be weighed against the risks of lower birthweight and potential teratogenic effects of drug exposure on the foetus

Objective To develop a symptom experience scale for stroke survivors and to test its reliability and validity. Methods The initial item pool was formulated by combining the results of a literature review with ex-isting stroke-related scales and interviews with stroke patients. Items were added, deleted or modified after expert con-sultation and a pilot study to set up the scale. Then 200 in-patients in the rehabilitation departments of two hospitals in Shanghai were surveyed using the scale. The scale′s reliability and validity were examined. Results The final scale consisted of 19 items. An exploratory factor analysis identified six principal factors which explained 62% of the total variance in the results. The average scale-level content validity index was 0. 95 and the item content validity index ranged from 0.80 to 1. The scale results correlated well with those using the Stroke Impact Scale. The Cronbach′s αcoefficient was 0.81 and the split-half coefficient was 0.76. Conclusion The symptom experience scale for stroke survivors has good reliability and validity. It can be used to evaluate the symptom experiences of stroke survivors and provide information for the management of symptoms.

OBJECTIVE@#To observe the clinical effect of acupuncture at "experienced ten acupoints" for postprandial distress syndrome.@*METHODS@#A total of 62 patients with postprandial distress syndrome were randomly divided into an observation group (31 cases, 5 cases dropping off) and a control group (31 cases, 6 cases dropping off ). Acupuncture was applied at Baihui (GV 20), Zhongwan (CV 12), Qihai (CV 6), Tianshu (ST 25), Neiguan (PC 6), Zusanli (ST 36), Gongsun (SP 4), Danzhong (CV 17) in the observation group. In the control group, 6 non-acupoint points were intervened with shallow puncture. The treatment was given 20 min each time, once every other day, 3 times a week for a total of 4 weeks in the two groups. Symptom index of dyspepsia (SID) and Nepean dyspepsia index (NDI) scores were compared before and after treatment, and the efficacy was evaluated in the two groups.@*RESULTS@#The effective rate in the observation group was 76.9% (20/26), which was higher than 28.0% in the control group (7/25, <0.01). After treatment, the SID and NDI scores in the two groups were lower than those before treatment (<0.01, <0.05), and the SID and NDI scores in the observation group were lower than those in the control group (<0.01, <0.05).@*CONCLUSION@#Acupuncture at "experienced ten acupoints" can significantly reduce the symptoms of dyspepsia and improve the quality of life in patients with postprandial distress syndrome.

ABSTRACT

The experienced prescriptions of famous prestigious Chinese physicians are effective prescriptions developed by prestigious Chinese doctors during their long-time clinical practice, which reflect the traditional Chinese medicine (TCM) understanding of the disease development regularity and core pathogenesis. These experienced prescriptions provide valuable experience for medication and prescription regularities, and represent the highest level of TCM treatment and the major sources of new drug research, development and technology innovation. It is of great significance to inherit academic thoughts and clinical experiences of prestigious Chinese physicians, explore and summarize experienced prescriptions, and develop new Chinese drugs. The researches of new drugs based on experienced prescriptions are the major direction encouraged by the government, with the maximum amount of new TCM drug applications but a low approval rate in recent years. The main reason for the low number of approved new TCM drug applications is that researchers know less about evaluation concepts and relevant techniques, leading to problems in research and development strategy. To facilitate a smooth advance of the new drug research and development and take full advantage of the experienced prescriptions, in this paper, we focus on the problems about new drug development of experienced prescriptions, lay emphasis on the new TCM drug research and development concepts of clinical value, history of human application and whole-process quality control, and deeply and systematically analyze concerns in such links as pharmacy, pharmacodynamics, toxicology and clinic application. The purpose of this article is to provide the reference in solving actual problems, the reliable basis of further researches on experienced prescriptions, and the important guarantee for developing more safe, effective and high-quality controllable drugs to meets clinical requirements, so as to achieve the strategy of a healthy China.

ABSTRACT

Resumo A competência profissional representa a capacidade de mobilizar conhecimentos, habilidades, atitudes e valores para responder às demandas da situação de trabalho. O objetivo da pesquisa foi descrever e comparar a autopercepção de competências de professores de educação física. O modelo do estudo foi quantitativo, de caráter descritivo e comparativo. A amostra foi constituída de 30 professores experientes e 32 iniciantes. Os dados foram coletados através de questionário e analisados por meio da estatística descritiva e inferencial. A autopercepção dos professores experientes e iniciantes se diferenciou significativamente em seis das nove áreas de conhecimento relativas ao ensino. A experiência foi uma variável discriminativa das diferenças encontradas.


Abstract Professional competence is understood as the ability to mobilize knowledge, skills, attitudes and values to meet the demands of the work context. The aim of this study was to describe and compare beginners and experienced Physical Education teacher's self-perception of competence. The model study was quantitative, descriptive and comparative. The sample consisted of 30 experienced teachers and 32 beginning teachers. The self-perception of experienced teachers and beginning teachers differed significantly in six of nine teaching knowledge Areas. Teacher experience was discriminating the differences found.


Resumen La competencia profesional es la capacidad de movilizar conocimientos, habilidades, actitudes y valores para satisfacer las exigencias de la situación de trabajo. El objetivo fue describir y comparar la autopercepción de competencias del profesorado de educación física. El modelo de estudio fue cuantitativo, descriptivo y comparativo. La muestra estaba formada por 30 profesores experimentados y 32 principiantes. Los datos se recopilaron por medio de un cuestionario y se analizaron utilizando estadística descriptiva e inferencial. La percepción de los profesores experimentados y principiantes difiere sustancialmente en seis de las nueve áreas de conocimiento relativas a la educación. La experiencia fue una variable discriminativa entre las diferencias encontradas.

ABSTRACT

This qualitative-descriptive investigation examining the influence exerted by experienced soccer-players on the ranking obtained from the four best-ranked by their teams in the last four FIFA World Cups (WC). They were divided into experienced-players (at least one WC participation [+1WC]) or non-experienced players (no previous participation [1WC]). Thereafter, they were classified according to their contribution in each of the playoff matches in a given WC: STARTER (started all matches); START/SUBST (started at least one match); SUBST/BENCH (did not start any matches, but replaced a player in at least one match); and BENCH (did not participate in matches). The sample was composed of 33.7% experienced and 66.3% non-experienced players. The number of players +1WC in finalist teams was significantly higher than in the non-finalist teams (p<0.05). In addition, when compared with the non-finalist teams, the finalists presented significantly higher number of players STARTER with +1WC (p<0.05). Possibly, the selection of experienced players to participate in WC may be an effective strategy to achieve better competitive performance

ABSTRACT

Optimum research on famous TCM doctors’ experienced prescriptions can optimize the composition of prescriptions which originated from experienced prescriptions, clarify the functions of indications and efficacy features of experienced prescriptions. Case report forms which are normally adapted to clinical new medicine study cannot meet the demand of observational study to optimize experienced prescriptions. This article expounded design features, structure elements and collection timing and proposed requirements for information collection through practice of case report forms in the optimum research on famous TCM doctors’ experienced prescriptions, with a purpose to realize the optimization of experienced prescriptions through observational study on famous TCM doctors’ experienced prescriptions and provide evidence for further clinical research on experienced prescriptions.

ABSTRACT

The purposes of this study were to investigate the factors affection the Oral Impacts on Daily Performances for Children (C-OIDP) in elementary and middle school students, and identify the association between oral health-related behaviors, oral health condition and C-OIDP. A cross-sectional study was conducted in three schools in Incheon, Asan, Korea. A total of 175 selected children were interviewed by a trained examiner using a questionnaire. Oral Health Related Quality of Life was assessed by the Korean version of C-OIDP. Socio-economic characteristics, oral health-related behaviors, oral health condition and C-OIDP were verified using the questionnaire. ANOVA analysis was performed to determine the oral health and C-OIDP, and multiple regression analysis was performed to determine the factors affecting the C-OIDP. The activities with the greatest effect were eating (28.0%), cleaning teeth (22.9%), and smiling (18.9%). In the logistic regression model, the high item score of C-OIDP was associated with experiencing dental caries and gum pain in the past month. The more the C-OIDP prevalence item, the more the fillng deciduous tooth surface (fs) (p=0.024), caries experienced deciduous tooth surface (dfs) (p=0.049), total caries tooth surface (ds+DS) (p=0.021), and total caries experienced tooth surface (dfs+DMFS) (p=0.047). It can be concluded that the factors affecting C-OIDP are fs, dfs, dfs+DMFS, and gingival pain. Based on these results, we can improve C-OIDP to advance preventive practice.
