ABSTRACT

PURPOSE: In second degree burn injuries of face, it is important to solve much discharge of early burn wound and shorten healing time. We introduce methodical and effective dressing in treatment of second degree facial burn at focus to solve much discharge of early burn wound and to shorten healing time. METHODS: From March, 2011 to December 2013, a study was done with 398 patients who had second degree burn wound on face. Initially we performed dressing with hydrofiber agent (Aquacel(R)). After cleansing with saline, Vasaline guaze was layed on face, and hydrofiber agent were layed over it, and Vasaline guaze was layed over there again, then saline wet guaze dressing performed. When we changed dressing daily, we evaluated burn wound degree. In superficial second degree burn wound like pinkish colored wound, we repeated hydrofiber agent and saline wet guaze dressing. In deep second degree burn wound like pale or mottled colored wound, we changed dressing with cultured allogenic keratinocyte (Kaloderm(R)). RESULTS: Number of patients with only superficial second degree burn wound was 272, with superficial second degree and deep second degree burn wound together was 113, with only deep second degree burn wound was 13. In only superficial second degree burn wound, the average healing time was 5.1 days with hydrofiber agent and saline wet dressing. In superficial second degree and deep second degree burn wound together, the average healing time was 7.2 days with hydrofiber agent and saline wet guaze dressing and cultured allogenic keratinocyte. In deep second degree burn wound, average healing time was 11 days with hydrofiber agent and saline wet guaze dressing and cultured allogenic keratinocyte. CONCLUSION: In this strategy, hydrofiber agent is good to be attached on irregular surface of facial burn wound, and we use hydrofiber agent to make wet environment to promotion wound healing, not as absorbable material. We use Saline wet guaze as absorbable material. And after evaluation of degree of burn, we use cultured allogenic keratinocyte to promote wound healing in case of deep second degree burn. We think this strategy using hydrofiber agent and saline wet gauze and cultured allogenic keratinocyte on facial burn wound is very effective dressing method.

ABSTRACT

Objective To study the efficacy of simple Chinese medicine steam spray and comprehensive rehabilitation nursing after facial burn deep Ⅱ degree.Methods 80 patients were randomized into the treatment group (40 cases) and the control group (40 cases).The treatment group was given the treatment of Chinese medicine steam spray.The control group was given the treatment of Chinese medicine steam spray,pressure therapy,physical therapy,rehabilitation training and psychotherapy.The clinical efficacy of two groups was evaluated respectively after 3 months.Results After 3 months,the skins of most of patients in the treatment group were nearly normal with no pigmentation and scar hyperplasia.The cure rate was 72.5 %,the obvious effective rate was 12.5 %,and the effective rate was 7.5 %.Some patients in the control group were lightly and inconspicuously pigmented,but the pigmentation area was reduced.The colour of their skins was not different from that of the normal skin.The cure rate was 42.5 %,the obvious effective rate was 12.5 %,and the effective rate was 25.0 %.The effects of the two groups were significant differences (P<0.05).Conclusions Comprehensive rehabilitation nursing after the facial burn can reduce the pigmentation and scar hyperplasia and improve the quality of life.

ABSTRACT

Objective To explore the effects of cognitive behavioral intervention in psychological nursing of young female patients with facial burn.Methods 64 cases of young women patients with facial bum were selected as the research object,and randomly divided into the observation group and the control group,with 32 cases in each group.The observation group was given the cognitive behavioral intervention and the control group was done by conventional nursing methods.The anxiety and depression scores between the two groups before and after the intervention were compared.Results After injury,the anxiety and depression scores of both the observation group and the control group were higher than those of the domestic norm; the anxiety and depression scores between two groups showed no statistic significance before intervention,and after intervention,the anxiety and depression scores between two groups had significant difference,scores of the observation group were lower than those of the control group.Conclusions Cognitive behavioral intervention can reduce anxiety and depression of young female patients with facial burn.It is a kind of effective psychological nursing method,and worthy of clinical promotion.

ABSTRACT

Las quemaduras faciales constituyen un grupo dentro de las quemaduras consideradas como en zona especial dada las potenciales secuelas estéticas y funcionales que pueden resultar de su tratamiento, por este motivo, un adecuado manejo reducirá estos riesgos. La cara posee una rica irrigación de tal forma que el manejo deberá ser más conservador que en otras zonas del cuerpo. Las quemaduras superficiales requieren de un tratamiento basado en ungüentos antibióticos y cobertura con apósitos sintéticos transitorios en esperade epidermización. En quemaduras faciales profundas el tratamiento consistirá en escarectomía química y quirúrgica una vez definida la quemadura y su posterior cobertura con tejido autólogo o sustitutos dérmicos definitivos respetando las unidades estético-funcionales de la cara. Este grupo de quemadura luego de la primera atención deberá ser derivadaa un centro de quemados para su manejo integral.


Facial burns constitute a group within the burns considered as in special area given the potential aesthetic and functional sequel that can result from their treatment. For this reason, an appropriate management reduced these risks. The face has a rich irrigation in such a way that management should be more conservative than in other areas of the body. Superficial burns require a treatment based on antibiotic ointments and coverage with transient synthetic dressings in hopes of epimerization. In deep facial burns the treatment will be chemical and surgical scarectomy once defined the burn and its subsequent coverage with autologous tissue or dermal substitutes respecting the aesthetic and functional units of the face. This group of burn after the first attention must be referred to a burn center for integral management.

ABSTRACT

PURPOSE: The face is very important body area. So, for burns on the face, early wound healing is important of all. Since B.C.1500, Greeks started using phototherapy for wound healing. Although phototherapy has been studied a long time, the mechanism and result of researches have not been clearly defined. So, the authors will evaluate the effectiveness of the phototherapy, spectrum of 633 nm and 830 nm, and the effectiveness in second degree facial burn. METHODS: We studied 30 burn patients with second degree burn wounds with symptoms of edema, discharge, and redness whom admitted from August 2006 to May 2008. Wound healing of phototherapy was compared to that with hydro-gel dressing. We evaluated each groups by the time it took for wound to epithelize, swelling disappearance, the amount of exudates, hospital days. Satisfaction question surveyed by patients ranging from 0 to 5 scales with 0 being the lowest and 5 being the highest. RESULTS: The duration of epithelization was 10+/-2.3 days in cases with phototherapy. Setting the factor's point becoming 1, the duration of cases with hydro-gel dressings was redness 5+/-2.2 days, discharge 6+/-1.8 days, and the average admission days were 11+/-1.6 days. The group with phototherapy was evaluated superior to the control group in satisfaction index by 30%. CONCLUSION: Simple hydro-gel dressing on facial burn is a routine job. Considering the fact that face is the most prominent sight-catching point, we should try to heal the wound as soon as possible. Phototherapy is the choice that we can use to shorten the duration of redness, the time it takes to epithelize and increase satisfaction index. So if possible, when got a burn on face, authors recommend the phototherapy.
