ABSTRACT

Objective To evaluate the significance of selective salpingography(SSG)and fallopian tube recanalization(FTR)in diagnosis and treatment for obstructive infertility. Methods Totally, 268women with infertility caused by obstruction of the fallopian tube were divided randomly into two groups, one (Group A)with a non-operative hysterosalpingeal system by insertion of co-axial cannula and guiding filament made by COOK Corporation in the US and another(Group B)with a self-prepared hysterosalpingeal system modified based on the apparatus of COOK Corporation, for SSG and FTR.Then, efficacy of SSG and FTR for the infertile women was evaluated by one-year cumulative pregnancy rate and reeanalization rate of the fallopian tube.Resnits One hundred and seventy of 198 fallopian tubes were recanalized in Group A, with a recanalization rate of 85.8%, and 290 of 320 tubes in Group B were recanalized.with a recanalization rate of 90.6%.reaching statistically significant difference.Furthermore, recanalization rate varied with different sites of occlusion of the fallopian tube.Pregnancy rate was 35.3%(36/102)in Group A, significantly higher than that in Group B(47.6%, 79/166).As the same, pregnancy rate varied with different sites of occlusion of the fallopian tube. Conclusion Selective salpingography and fallopian tube recanalization with self-prepared hysterosalpingeal system have dual effects on diagnosis and treatment for tubal infertility of women, which are simpler, safer and more reliable, worth to be clinically applied.

ABSTRACT

Objective To evaluate the clinical value of selective salpingography(SSG)and fallopian tube recanalization(FTR) for diagnosis and treatment of oviduct obstructive infertility with self-made simple coaxial microtube.MethodsSSG and FTR with self-made simple Coaxial Microtube were performed for 76 cases with 134 tubes of obstructive oviduct through wire-guided tubal recanalization,anti-inflammatory and anti-adhesion effect of recanalization fluid, and the patients were treated with anti-bacterial and hydrotubation after operations.ResultsSSG displayed that 81 tubes obstruction in interstitial tubal,25 tubes obstruction in isthmus,16 tubes obstruction in ampulla and 12 tubes obstruction in fimbria. The total recanalization rate of FTR was 76.1%,with the recanalization rate 96.3% in interstitial, the recanalization rate 68.0% in isthmus and the recanalization rate 43.8% in ampulla, and no serious complications occurred. All cases were followed up for one to two years postoperatively.Normal pregnancy pationes were 31 cases(40.8%),and 29 women were normal parturition,2 cases(2.6%) were extrauterine pregnancy.ConclusionThe advantage of SSG and FTR with self-made simple coaxial microtube is low cost, simplicity and excellent efficacy. It is worth popularizing in grass-roots hospitals.

ABSTRACT

Selective salpingogram(SSG)and hysterosalpingography(HSG)have achieved fairly reliable diagnostic value. SSG and transcervical fallopian tube recanalization(T-FTR)possess rather credible therapeutic value,long-term effect and low risk. Recanalization rate and pregnancy rate, combining with the measurement of tubal perfusion pressures(TPP)and development of ultrasonic technology, can evaluate the therapeutic effect and safety more generally and objectively. Researches in the future should lay emphasis on developing non-radiologic technique, promoting diagnostic veracity, reducing side effects, and establishing randomly control trials together with consolidating the standardized evaluation criteria.(J Intervent Radiol,2007,16: 714-717)
