ABSTRACT

Objective:To explore the application effect of continuous nursing based on hybrid electronic platform and fragmentation time in the out-of-hospital rehabilitation of patients with anterior cruciate ligament reconstruction.Methods:This was a randomized controlled study. From April 2020 to October 2021, a total of 90 patients undergoing anterior cruciate ligament reconstruction in Weifang People′s Hospital were selected as the research objects by convenience sampling. Forty patients from April to December 2020 were selected as control group, and 45 patients from January to October 2021 were selected as intervention group. The control group implemented routine discharge guidance and follow-up, while the intervention group was given continuous nursing plan based on the concept of hybrid electronic platform and fragmented time. Before operation and 1, 3 and 6 months after operation, the knee range of motion and Lysholm score of patients in the two groups were analyzed. At 1, 3 and 6 months after operation, the rehabilitation exercise compliance score, self-efficacy score and knee stability examination of the two groups were analyzed.Results:A total of 41 patients in each group completed the study. At 1 month after operation, the knee range of motion, Lysholm score, rehabilitation exercise compliance score and self-efficacy score of intervention group were (96.46 ±3.58) ° and (53.54 ± 6.57), (69.17 ± 6.27), (4.01 ± 0.38) points, respectively. In the control group, they were (89.02 ± 4.22)° and (45.02 ± 7.61), (56.78 ± 8.45), (3.61 ± 0.42) points. At 3 months after operation, they were (136.83 ± 4.30)° and (72.76 ± 4.96), (60.71 ± 5.87), (4.97 ± 0.32) points in the intervention group, (133.54 ± 3.58)° and (69.83 ± 6.65), (56.73 ± 8.57), (4.83 ± 0.45) points in the control group, respectively. At 6 months after operation, they were (139.39 ± 1.99)° and (85.61 ± 6.11), (57.71 ± 7.41), (6.58 ± 0.96) points in the intervention group, (138.29 ± 2.65)° and (81.80 ± 6.38), (47.90 ± 6.02), (6.35 ± 1.23) points in the control group, respectively. The knee range of motion, Lysholm score and rehabilitation exercise compliance score of the intervention group were better than those of the control group at 1, 3 and 6 months after operation, and the differences were statistically significant ( t values were 2.12-8.61, all P<0.05). The self-efficacy score of the intervention group was higher than that of the control group at 1 month after operation, and the difference was statistically significant ( t=4.57, P<0.05). Conclusions:The continuous nursing program based on hybrid electronic platform and fragmented time can improve the knee function of patients with anterior cruciate ligament reconstruction in the early postoperative period, improve the compliance of rehabilitation exercise and the level of early self-efficacy.

ABSTRACT

OBJECTIVES@#To study the efficacy of family rehabilitation treatment performed by parents under the guidance of professionals in children with autism spectrum disorder (ASD).@*METHODS@#In the prospective study, 60 children with ASD, aged 24-60 months, were randomly divided into an observation group and a conventional group. The parents of the children in the conventional group received an online training on basic knowledge and rehabilitation training of ASD alone, and those in the observation group received the online training and performed family rehabilitation treatment under the guidance of a professional team. Psycho-Education Profile Third Edition (PEP-3) and Childhood Autism Rating Scale (CARS) were used to evaluate the changes in related abilities after intervention.@*RESULTS@#After 6 months of intervention, the scores of all dimensions of the PEP-3 scale in the observation group and most dimensions of the conventional group significantly increased (@*CONCLUSIONS@#An online training on basic knowledge and rehabilitation training of ASD for parents can improve the abilities and core clinical symptoms of children with ASD. The family rehabilitation treatment model with a team of professionals as the resource platform and parents as the performer has a more significant efficacy on improving the language, sports, and other abilities and alleviating the severity of the symptoms in children with ASD.

ABSTRACT

Objective Using the information - motivation - behavior model (IMB) to intervene with the alcohol dependence patients and provide a new strategy for implementing health education. Methods Eighty-nine patients with alcohol dependence, who clinically recovered or improved after the discharge, were randomly divided into a research group and a control group. Both groups received routine health education, rehabilitation guidance and a follow-up call. The research group implemented additional IMB health education. The implementation effect on alcohol dependence scale (ADS), Michigan alcoholism screening test (MAST), and alcohol dependence prevention and treatment knowledge (self-designed questionnaire) of the two groups before and after intervention was analyzed. Results The relapse rate of the research group was lower than that the control group, which was statistically significant (P<0.05). MAST scale and ADS scale in the research group were lower than those in the control group within 3 months and 6 months after the discharge, which was significant (P values of both scales <0.05). After the 6-month follow-up, the knowledge of alcohol dependence patients in the research group and their families was significantly higher than that in the control group (P<0.05). Conclusion IMB health education implementation for alcohol dependence patients significantly reduced the relapse rate, decreased physical and psychological dependence, and improved the ability of patients to prevent and control alcohol dependence.

ABSTRACT

BACKGROUND: Hip fracture can result in a high functional disability rate in elderly patients. Traditional rehabilitation pays more attentions to rehabilitation training during hospitalization, instead of family rehabilitation training out of hospital. Intensive family rehabilitation training for 6 months has been proven to be an effective method, but its longer-term effect still needs further study. OBJECTIVE: To explore the effect of long-term intensive family rehabilitation training on postoperative functional recovery in elderly hip fracture patients. METHODS: Eighty-nine elderly hip fracture patients who were treated with surgery were prospectively enrolled, and randomized into an experimental group (n=42) and a control group (n=47). The control group patients were treated with traditional rehabilitation training, and the patients in the experimental group were treated with long-term intensive family rehabilitation training. Functional exercises in the experimental group were guided, and the rehabilitation scheme was adjusted in time using network tools. All the patients were followed up for 12 months, and their functional recovery was assessed at 3, 6, and 12 months postoperatively by the activity of daily living score, 6-minute walking test, the hip Harris score, the functional independent measure score and the MOS 36-Item Short-Form Health Survey (SF-36) score. The study protocol was implemented in line with the ethic requirements of the Sixth People’s Hospital of Chengdu, and each patient was fully informed of the study procedures. RESULTS AND CONCLUSION: Patients’ function gradually recovered in both group postoperatively. The activity of daily living score, functional independent measure score, 6-minute walking test, Hip Harris score, and SF-36 score of the experimental group were significantly better than those in the control group at 3, 6, and 12 months postoperatively (P<0.05). To conclude, long-term intensive family rehabilitation training for elderly hip fractures can improve patients’ daily life ability and quality of life; however, the rehabilitation program and its efficacy still require further research.

Objective@#To explore the effect of cognitive-behavioral therapy combined with family rehabilitation therapy on improving depression patients.@*Methods@#Totally 82 depressive patients admitted to our hospital from February 2016 to January 2017 were randomly divided into the control group and the observation group according to the random number table method, with 41 cases in each group. Patients in the control group were treated with family rehabilitation therapy, while the patients in the observation group were treated with cognitive behavioral therapy on the basis of family rehabilitation therapy. After 12 months of intervention, the patients in the observation group were treated with cognitive behavioral therapy. The treatment efficiency, psychological status, coping style and medication compliance of the two groups were compared.@*Results@#After intervention the scores of Self-rating Depression Scale (SDS) and Hamilton Depression Scale (HAMD) in the observation group were (31.08±4.83), (14.14±3.52) points respectively, which were significantly lower than (36.97±2.73), (16.68 ±2.14) points of the control group (t=-6.789,-3.948, P < 0.01). The effective rate of the observation group was 97.56% (40/41), which was significantly higher than 70.05% (32/41) of the control group, and the difference was statistically significant (χ2=5.581, P<0.05). The scores of "problem solving" and "help seeking" items in the observation group were (1.66±0.18) and (1.81±0.21) respectively, significantly higher than (1.39±0.11), (1.48±0.13) of the control group; "avoidance" and "self-blame" items were (1.33±0.12) and (1.47±0.14) respectively, significantly lower than (1.58±0.19), (1.48±0.13) in the control group, and the difference was statistically significant (t=-8.349-8.555, P < 0.05). After treatment, the compliance of the observation group was significantly better than that of the control group (χ2= 8.902, P < 0.05).@*Conclusion@#Cognitive behavioral therapy combined with family rehabilitation therapy for depression patients can effectively improve the bad mood of patients, improve their medication compliance, and achieve better clinical treatment results.

ABSTRACT

Objective To observe the effects of happy family cognitive training on patients with vascular cognitive impairment. Meth-ods From April, 2015 to April, 2016, 80 patients with vascular cognitive impairment were divided randomly into happy family cognitive training group (n=40) and control group (n=40). Both groups accepted routine medicine and rehabilitation exercise, while the happy family cognitive training group received happy family cognitive training additionally. They were assessed with Montreal Cognitive Assessment (MoCA) and Activities of Daily Living (ADL) before and after 12 weeks of treatment. Results The scores of MoCA and ADL improved more in the family cognitive training group than in the control group after treatment (t>5.454, P<0.001). Conclusion Happy family cogni-tive training is effective on vascular cognitive impairment, for cognitive function and activities of daily living.

ABSTRACT

Objective To investigate the health-related demands in main caregivers for patients with Parkinson's disease after surgery. Methods From March to December, 2014, twelve main caregivers were investigated with a semi-structured interview, and analyzed with Co-laizzi's analysis. Results and Conclusion Four aspects of demand were found, including knowledge about the disease, rehabilitation nursing skills, economic support and family support. Medical professionals can provide the targeted knowledge and technical guidance to meet their demands.

@#Objective To apply the WeChat in family rehabilitation after anterior cruciate ligament (ACL) reconstruction. Methods 126 patients accepted ACL reconstruction using quadrupled semitendinosus and gracilis tendons were divided into observation group (n=69) and control group (n=57). The control group received conventional rehabilitation program, and the observation group joined the WeChat group. They were assessed with knee range of motion (ROM), knee stability test, thigh perimeter, Lysholm score and the International Knee Documentation Committee (IKDC) score 3 and 6 months after operation. Results There was a significant difference between groups in ROM of knee flexion (P<0.05) 3 and 6 months after operation, but not in ROM of knee extension (P>0.05). There was no significant difference in thigh perimeter 3 months after operation, but it was significant 6 months after operation. There was significant difference in Lysholm score and IKDC score (P<0.05), but not in the joint stability (P>0.05). Conclusion Direction of rehabilitation with the WeChat Group can significantly improve the compliance, accelerate the rehabilitation process and reduce complications in patients after ACL reconstruction.

ABSTRACT

Objective To investigate the sleep status, quality of life, activities of daily living and the serum levels of interleukin-1β (IL-1β) and tumor necrosis factor alpha (TNF-α) in insomnia patients after meridian-collateral stroke treated by the new family rehabilitation program, a combined therapy including traditional family rehabilitalion, acupoint massage and ear point application. Methods A total of 80 cases of insomnia patients after meridian-collateral stroke were enrolled. Subjects were evenly divided into treatment group ( treated with new family rehabilitation program) and control group ( treated with traditional hospital health education and family rehabilitation). Clinical effects of the two groups were evaluated with the Pittsburgh Sleep Quality Index (PSQI), the brief version of WHO Quality of Life Assessment (WHOQOL-BREF) and activities of daily living ( ADL) . The serum levels of IL-1β and TNF-α were monitored before and after therapy and measured by enzyme-linked immunosorbent assay ( ELISA). Results ( 1) After treatment for 28 days, the scores of total PSQI, sleep time, sleep efficiency and sleep disorder incidence in the treatment group were significantly lower than those in the control group ( P<0.01) . ( 2) The scores of physiological, psychological, social relation, environmental dimensions of WHOQOL-BREF in the treatment group were significantly higher than those in the control group ( P<0.01) . ( 3) ADL scores were significantly higher in the treatment group than those in the control group (P<0.01) . (4) Serum levels of IL-1βand TNF-αwere increased in the treatment group as compared with those in the control group (P<0.01). Conclusion Compared with the traditional hospital health education, the new family rehabilitation program can effectively improve the quality of sleep, quality of life, as well as the activities of daily living. And the mechanism may be associated with the up-regulation of the serum levels of IL-1βand TNF-α, which are relevant to the sleep status.

Methods Family Rehabilitation Needs Survey Questionnaire was used to investigate 186 families of the children with disability from 5 rehabilitation institutions in 4 provinces. Results Most families accepted family rehabilitation, including family support, developed rehabilitation scheme, telephone counseling, real-time guidance and appointment for family rehabilitation. Most families would spent 1-2 hours a day for the family rehabilitation, and prefer the rehabilitation of gross motion. However, the rehabilitation agencies played less importance on supports of family rehabilitation. Conclusion The family rehabilitation supports needs to be improved in specification of rehabilitation process, developing rehabilitation scheme and quality control.

ABSTRACT

Objective To explore the effect of family rehabilitation on growth and development of children with hypoxic ischemic en-cephalopathy (HIE). Methods 80 children with moderate HIE were divided into control group (n=40) and observation group (n=40). The control group received routine intervention and nursing, and the observation group accepted family rehabilitation in addition. Mental devel-opment index (MDI) and psychomoter developmental index (PDI) were compared when they were 3 and 6 months old with intellectual de-velopment testing, and their development quotients (DQ) were compared at the age of 1 year. Results The MDI and PDI scores were higher (P<0.01) in the observation group than in the control group when they were 3 and 6 months old. The DQ were higher (P<0.05) in the obser-vation group than in the control group when they were 1 year old. Conclusion Family rehabilitation can promote the development of intelli-gence in children with HIE.

ABSTRACT

Objective To observe the effects of family cognitive training on patients with vascular cognitive impairment but without dementia.Methods Sixty patients with non-dementia type vascular cognitive impairment were divided at random into a group which received family cognitive training (30 cases) and a control group (30 cases).The 2 groups all took routine drugs and exercise.The family cognitive training group received cognitive training additionally.Before treatment and after 1 and 6 months of treatment,all of the patients of both groups were assessed using the mini-mental state examination (MMSE),the Montreal cognitive assessment (MoCA) and the modified Barthel index (MBI).Results After 1 month of treatment there was no significant difference between the 2 groups in any of the assessments.After6 months the scores on each item of the MMSE,MoCA and MBI had improved significantly more in the family cognitive training group than in the control group.Conclusion Family cognitive training is effective in treating non-dementia type vascular cognitive impairment.It can delay disease progression and improve cognitive function and ability in the activities of daily living.

ABSTRACT

Objective To explore the effect of health education path on care and household rehabilitation in children with cerebral palsy (CP). Methods 100 CP children from June to December, 2012 were divided into experimental group (n=50) and control group (n=50). The control group was given conventional health education, while the experimental group along with the health education path. Mastery rates of disease knowledge and family rehabilitation methods, cooperation rate, and care satisfaction rate of parents was assessed before discharge. Results Mastery rates of disease knowledge, family rehabilitation methods, cooperation rate, and care satisfaction rate of parents were high-er in the experimental group than in the control group (P<0.05). Conclusion Application of health education path can rule the health educa-tion of nurses, and improve the CP patient's satisfaction and compliance.

ABSTRACT

@#ObjectiveTo observe the effect of rehabilitation training on family caregiver of schizophrenic in rural areas.Methods60 schizophrenic patients in rural area were divided into rehabilitation group and control group. The rehabilitation group and their caregivers received training of family rehabilitation for 12 months. The caregivers in the control group only received interview. Brief Psychiatric Rating Scale (BPRS) and Social Disability Screening Schedule(SDSS) were used to assess the patients before, 6 months and 12 months after training. The self-made caregiver questionnaire was to assess mental knowledge and family harmony of the caregivers.ResultsPsychiatric knowledge and family harmony for caregivers significantly changed in the rehabilitation group after the family rehabilitation training. There were significant differences in the BPRS and SDSS score between the rehabilitation group and the control group.ConclusionThe family rehabilitation training can benefit the rural schizophrenic patients and their caregivers.

ABSTRACT

Objective To analyze the economic effects of different rehabilitation patterns for children suffering from cerebral palsy. Methods A total of 153 cerebral palsy patients were divided into a hospital-community-family rehabilitation group(n = 52), a hospital rehabilitation group (n = 50) and a non-intervention control group (n = 51). Those in the first group were provided with a hospital-community-family rehabilitation therapy pattern, those in the sec-ond only hospital rehabilitation and the third no intervention. All the patients were evaluated using the Gross Motor Function Measure-88 (GMFM-88) Scale and the Cost Measure Scale at admission, and at the end of the 3rd and 6th months of treatment. Results There were no significant differences in gross motor function among the three groups at admission. At the end of the 3rd month and the 6th month there were significant differences between the children in the hospital-community-family rehabilitation program and those in the hospital rehabilitation program in terms of gross motor function. Their general percentage, monthly percentage and monthly relative percentage results were all significantly different. But there was no significant difference in the non-interventian control group since admission. Every unit of improvement in gross motor function cost $101.87±97.59, $75.11±45.75 in the hospital-community-family reha-bilitation program and $387.21±54.76, $170.31±123.16 in the hospital rehabilitation program at the end of the 3rd and the 6th month respectively. So the cost of the former was only about 30% of the latter. Conclusion Hospital rehabilitation is suitable for the early rehabilitation of cerebral palsy children. Hospital-community-family rehabilitation is better for long-term rehabilitation of cerebral palsy children, and what is more, it can decrease the rehabilitation ther-apy cost substantially. So a hospital-community-family rehabilitation pattern is more compatible with China's national situation.

@#Stroke is a hi-attack rate,hi-multilation rate and hi-death rate disease.More than 70 percent patients with stroke remain functional disturbance,which destroy their quality of life and bring them heavy burden.The early multidisciplinary rehabilitation treatment,rehabilitation care,family rehabilitation training and application of new technology are the major strategies in kinds of rehabilitation treatment on stroke patients.The clinical researches have showed that the effect of multidisciplinary rehabilitation treatment is obviously better than simple treatment.So these therapeutic regimens have wonderful prospect and valuable generalization.
