Abstract Introduction: Posterior pharyngeal wall is the most rare subsite for hypopharyngeal carcinomas. Because of its rarity, there are few studies published in the literature specifically concerning posterior pharyngeal wall carcinoma. Objectives: To report our functional results in patients with the carcinoma of the posterior wall of the hypopharynx after surgical treatment by resection via a lateral or infrahyoid pharyngotomy approach, with the preservation of the larynx and reconstruction with a radial forearm free flap. Methods: The study included 10 patients who underwent surgery for a carcinoma of the posterior wall of the hypopharynx over a 6 year period. The associated postoperative morbidity was investigated and functional results were analyzed. Results: Nine patients had T3 lesions and one patient had a T2 lesion. The preferred approach to access the hypopharynx was a lateral pharyngotomy in 5 patients and lateral pharyngotomy combined with infrahyoid pharyngotomy in 5 patients with superior extension to oropharynx. The pharyngeal defects were reconstructed successfully with radial forearm free flaps. Four patients received adjuvant radiotherapy only, and 4 patients with N2b and N2c neck diseases received adjuvant chemoradiotherapy. The mean duration of hospitalization was 15.6 days (range, 10-21 days). All patients achieved oral intake in a median time of 74 days (range, 15-180). Decannulation was achieved in all patients and the median time fordecannulation was 90 (range, 21-300 days). The mean followup duration was 38.3 months (range, 10-71 months) and 8 patients survived. One patient died due to regional recurrence in the retropharyngeal lymph nodes and 1 patient died due to systemic metastasis. Conclusion: Primary surgery is still a very effective treatment modality for the carcinoma of the posterior wall of the hypopharynx and does not permanently compromise the swallowing and laryngeal functions if pharyngeal reconstruction is performed with a free flap.


Resumo Introdução: A parede posterior da faringe é o subsítio mais raro para carcinomas hipofaríngeos. Devido à sua raridade, há poucos estudos publicados na literatura especificamente sobre o carcinoma da parede posterior da faringe. Objetivo: Relatar nossos resultados funcionais em pacientes com carcinoma da parede posterior da hipofaringe após tratamento cirúrgico por ressecção via faringotomia lateral ou infra-hióidea, com preservação da laringe e reconstrução com retalho livre radial do antebraço. Método: O estudo incluiu 10 pacientes submetidos à cirurgia para carcinoma da parede posterior da hipofaringe por 6 anos. A morbidade pós-operatória associada foi investigada e os resultados funcionais foram analisados. Resultados: Nove pacientes apresentaram lesões T3 e um paciente apresentou lesão T2. Avia preferida para acessar a hipofaringe foi a faringotomia lateral em 5 pacientes e a faringotomia lateral combinada com a faringotomia infra-hióidea em 5 pacientes com extensão superior até a orofaringe. Os defeitos faríngeos foram reconstruídos com sucesso com retalhos livres radiais do antebraço. Quatro pacientes receberam apenas radioterapia adjuvante e 4 pacientes com doença cervical N2b e N2c receberam quimiorradioterapia adjuvante. A duração média da hospitalização foi de 15,6 dias (variação de 10 a 21 dias). Todos os pacientes retornaram à ingestão oral em um tempo médio de 74 dias (variação de 15 a 180). A decanulação foi possível para todos os pacientes e o tempo médio foi de 90 dias (variação de 21 a 300 dias). A duração média do seguimento foi de 38,3 meses (10 a 71 meses) e 8 pacientes sobreviveram. Um paciente foi a óbito devido a recorrência regional nos linfonodos retrofaríngeos e outro devido a metástase sistêmica. Conclusão: A cirurgia primária ainda é uma modalidade de tratamento muito eficaz para o carcinoma da parede posterior da hipofaringe e não compromete de forma permanente as funções de deglutição e da laringe se a reconstrução faríngea for feita com retalho livre.

ABSTRACT

El síndrome de apneas e hipoapneas obstructivas del sueño se asocia con una disminución de la calidad de vida, bajo rendimiento escolar y, hasta en el 40% de los niños, trastornos de conducta como hiperactividad, enuresis, ansiedad y depresión. Varios estudios demostraron que la adenoamigdalectomía es efectiva para mejorar o resolver los trastornos respiratorios del sueño. Si bien esta cirugía tiene resultados beneficiosos, no está exenta de riesgos. El dolor y el sangrado posoperatorio son las dos causas principales de morbilidad. Otras complicaciones de la cirugía son las náuseas y los vómitos posoperatorios, el retraso en la alimentación, la deshidratación, la otalgia referida, los cambios en la voz y, raras veces, la muerte.En este artículo se realizan recomendaciones sobre el cuidado posoperatorio de los niños con adenoamigdalectomía


Obstructive sleep apnea and hypopnea syndrome is associated with decreased quality of life, poor school performance and, in up to 40% of children, behavioral problems such as hyperactivity, enuresis, anxiety and depression. Several studies have shown that adenoamygdalectomy is effective in improving or resolving sleep-disordered breathing. While this surgery has beneficial results, it is not without risks. Postoperative pain and bleeding are the two main causes of morbidity. Other complications of surgery include postoperative nausea and vomiting, delayed feeding, dehydration, referred earache, voice changes, and, rarely, death. Recommendations on postoperative care for children undergoing adenoamygdalectomy are mentioned in this article.

Abstract The aim of the study is to evaluate pharyngeal airway dimensions and hyoid bone position according to different Class II malocclusion types in Turkish population. Materials and Methods: The retrospective clinical study consisted of patients divided into 3 subgroups with skeletal Class II malocclusion. A total of 221 individuals (131 females and 90 males) were included in the study. Individuals with skeletal Class II malocclusion were divided into three subgroups as maxillary prognathia, mandibular retrognathia and combined. In the cephalometric analysis; 8 nasopharyngeal, 7 oropharyngeal, 2 hypopharyngeal, 9 hyoid measurements and 4 area measurements were used. The distribution of sex and growth-development stages of the patients were compared with the Pearson chi-square test. One-way ANOVA was used to evaluate patients. Tukey Post-Hoc tests were used for bilateral comparisons for significant parameters. SPSS package program was used for data analysis. Results were considered statistically significant at p0.05). When the position of the hyoid bone was evaluated, a statistically significant difference was found between the three groups in the measurements of Hy-Pg (mm) (p<0.05). Conclusion: Linear and areal nasopharyngeal airway dimensions are similar in subgroups of Class II malocclusions, while the distance of the hyoid bone from the pogonion is less in the mandibular retrognathia group.


Resumen El objetivo del estudio es evaluar las dimensiones de las vías respiratorias faríngeas y la posición del hueso hioides según los diferentes tipos de maloclusión de clase II en la población turca. Materiales y métodos: El estudio clínico retrospectivo consistió en pacientes divididos en 3 subgrupos con maloclusión esquelética de Clase II. Un total de 221 individuos (131 mujeres y 90 hombres) fueron incluidos en el estudio. Los individuos con maloclusión esquelética de Clase II fueron divididos en tres subgrupos como prognatismo maxilar, retrognatismo mandibular y combinados. En el análisis cefalométrico se utilizaron 8 medidas nasofaríngeas, 7 orofaríngeas, 2 hipofaríngeas, 9 medidas hioides y 4 medidas de área. La distribución del sexo y las etapas de desarrollo del crecimiento de los pacientes se compararon con la prueba de chi cuadrado de Pearson. Se utilizó un ANOVA unidireccional para evaluar a los pacientes. Las pruebas Tukey Post-Hoc se usaron para comparaciones bilaterales de parámetros significativos. El programa del paquete SPSS se usó para el análisis de datos. Los resultados se consideraron estadísticamente significativos a un nivel de significación p0.05). Cuando se evaluó la posición del hueso hioides, se encontró una diferencia estadísticamente significativa entre los tres grupos en las mediciones de Hy-Pg (mm) (p<0.05). Conclusión: Las dimensiones de las vías respiratorias nasofaríngeas lineales y areales son similares en los subgrupos de maloclusiones de Clase II, mientras que la distancia del hueso hioides al pogonion es menor en el grupo de retrognacia mandibular.

Abstract Pharyngeal tuberculosis is an extrapulmonary form of this disease which makes up less than 1% of cases, most of which are secondary. In Peru, there are no case reports of this form of extra-pulmonary tuberculosis. We present the case of a 42-year-old man who consulted due to a two-month history of sore throat and a pharyngeal lump, with no epidemiological or medical history. A biopsy of the lesion and Ziehl-Neelsen staining yielded results compatible with pharyngeal tuberculosis. A chest x-ray showed bilateral miliary lesions, confirming a secondary form of disseminated tuberculosis. This highlights the importance of ruling out tuberculosis in all cases of chronic pharyngitis which are unresponsive to conventional treatment.


Resumen La tuberculosis faríngea es una forma extrapulmonar de esta enfermedad que corresponde a menos de 1% de casos, mayormente de carácter secundario. En Perú, no hay datos de reportes de casos en esta forma de tuberculosis extrapulmonar. Presentamos el caso de un varón de cuarenta y dos años que consultó por dolor de garganta y tumoración en faringe de dos meses de evolución, sin antecedentes epidemiológico o patológico. Se realiza biopsia de la lesión y coloración Ziehl Neelsen dando compatible a tuberculosis faríngea. En la radiografía de tórax se encontró lesiones pulmonares diseminadas bilaterales de tipo miliar, confirmando forma secundaria a tuberculosis diseminada. Por ello, se resalta la importancia de descartar tuberculosis ante toda faringitis crónica sin respuesta al tratamiento convencional.

RESUMEN La estenosis faríngea es una complicación muy poco frecuente de la adenoamigdalectomía. Consiste en un estrechamiento de la vía aerodigestiva superior secundaria a la adhesión parcial o total de las estructuras que componen la orofaringe producto de una disección extensa al realizar amigdalectomía. El objetivo de este trabajo es presentar tres casos clínicos tratamiento y correspondiente técnica quirúrgica. Se describen tres pacientes operados durante la edad preescolar de adenoamigdalectomía, que cursaron en el posoperatorio con estenosis faríngea. Todos presentaron roncopatía severa, respiración oral y resonancia hiponasal. La nasofibroscopía evidenció estrechez faríngea en distintos grados. A todos se les realizó resección del tejido cicatricial y faringoplastía con colgajo miomucoso y posterior rehabilitación con bulbo faríngeo. El uso de colgajos faríngeos posterior a la liberación de adherencias posadenoamigdalectomía permite aportar tejido sano a zonas cruentas, evitando la formación de nuevas adherencias y restituyendo la permeabilidad oronasal. El bulbo faríngeo es fundamental para mantener un apropiado lumen y manejo de la cicatrización en el posoperatorio. La técnica de colgajo faríngeo lateral, que interpone mucosa faríngea sana para la cobertura del lecho cruento, constituye una alternativa quirúrgica eficaz para resolver la estenosis faríngea posquirúrgica.


ABSTRACT Pharyngeal stenosis is a very rare complication of adenotonsillectomy. It is caused by a narrowing of the upper aerodigestive pathway secondary to the total or partial adhesion of the retropharyngeal structures as a result of extensive dissection during tonsillectomy. Our aim is to present three consecutive clinical cases of pharyngeal stenosis after adenotonsillectomy, discuss its treatment and corresponding surgical technique. We present three infants with adenotonsillectomy with postoperative pharyngeal stenosis. All had severe snoring, oral breathing and hyponasal resonance. The nasofibroscopy showed pharyngeal obliteration in several degrees. All patients underwent scar tissue resection and pharyngoplasty with myomucosal flap and subsequent use of pharyngeal bulb. The use of pharyngeal flaps after release of pharyngeal adhesions allows to provide healthy tissue to row areas, avoiding new postoperative adhesions and providing adequate oronasal permeability. The pharyngeal bulb is essential to maintain permeability and management of scarring in the postoperative period. The technique of lateral pharyngeal flap, which aims to interpose healthy pharyngeal mucosa to cover the bloody bed, is an effective therapeutic alternative to solve post-surgical pharyngeal stenosis.

ABSTRACT

Adenoamigdalectomia é um procedimento realizado para a remoção das amígdalas e da adenoide, que é indicada para obstrução de vias aéreas superiores devido hipertrofia dessas estruturas. Com isso, o objetivo deste estudo é avaliar qual a influência da adenoamigdalectomia no crescimento de crianças com distúrbios respiratórios do sono, com base em marcadores bioquímicos e antropométricos. Pesquisa em bancos de dados virtuais MEDLINE (Medical Literature Analysis and Retrieval System on-line) acessado via PubMed e Scielo, com limites para idioma com os descritores "growth", "adenotonsillectomy", "sleep-disordered breathing" e "children". A pesquisa estendeu-se de agosto de 2015 a março de 2017. Foram considerados critérios de inclusão: artigos originais que avaliassem amostra de menores de 12 anos, submetidos à amigdalectomia diante do diagnóstico de distúrbio respiratório do sono e/ou hiperplasia/hipertrofia de amígdala e/ou adenóide. Foram selecionados 22 estudos. Observou-se que a adenoamigdalectomia é fator importante no crescimento pôndero-estatural em crianças com DRS, a curto e longo prazos. Existem ganhos significativos de peso, altura e em alguns casos, ocorrência de "catch-up" no crescimento. Há evidências da modificação da concentração sanguínea de mediadores químicos do crescimento (IGF-I e IGFBP-3) após a cirurgia. Adenoamigdalectomia promove ganhos de peso e altura, favorecendo o crescimento de crianças a ela submetidos.


Adenotonsillectomy (AT) is a procedure performed to remove the tonsils and adenoids, which is indicated when there is obstruction of the upper airway due to hypertrophy of these structures. Evaluate the influence of the AT on growth in children with sleep-disordered breathing, based on biochemical and anthropometric markers. Research in virtual databases Medline and SciELO, with limits of idiom with the descriptors "growth" , "adenotonsillectomy", "sleep -disordered breathing" and "children". The research extended from august 2015 to march 2017 were considered inclusion criteria: original articles that evaluated sample under 12 years who underwent tonsillectomy before the diagnosis of sleep-disordered breathing and/or hyperplasia/hypertrophy of tonsils and/or adenoids. 22 studies were selected. It was observed that adenotonsillectomy (AT) is an important factor in the weight and height development in children with SDB in the short and long term and observed significant gains in weight, height and in some cases, the occurrence of "catch-up" growth. There is even evidence of the change in concentration blood of the chemical mediators growth factor (IGF-I and IGFBP-3) after surgery. The adenotonsillectomy promotes gains in height and weight, and consequently growth in individuals subjected to it.

ABSTRACT Objective: to evaluate the speech sound production of children diagnosed with a palatine mouth breathing and / or hypertrophic pharyngeal tonsil and compare it to that of a group of children that do not show any respiratory alterations, besides associating with age and sex. Methods: a quantitative, cross-sectional, analytical and observational research. Children from five to twelve years old have took part of the study, 50 of them diagnosed as mouth breathers (research group - RG) and 50 with no respiratory alteration (control group - CG). Anamnesis and evaluation based on MBGR protocol was performed, focusing on the speech, supported by figures and with samples of automatic and spontaneous speech. Results: there were no differences between the groups, taking into account the parents' complaint. Speech alterations, such as phonetic deflection, lingual interposition and distortions, and occlusion alterations were more frequent in RG. Speech alterations prevailed for males in 83% and the average age related to speech did not show any significance. Conclusion: mouth breathing children present more alterations of speech sounds than those presented with no respiratory alteration, regardless of the age group, being more common in male children.


RESUMO Objetivo: avaliar a produção dos sons da fala de crianças com diagnóstico de respiração oral com hipertrofia de tonsilas palatinas e/ou faríngeas e compará-las ao grupo de crianças sem alterações respiratórias, além de associar a idade e sexo. Métodos: pesquisa do tipo observacional, analítica, transversal, quantitativa. Participaram crianças de cinco a 12 anos, sendo 50 com diagnóstico de respiração oral (GP) e 50 crianças sem alterações do modo respiratório (GC). Foi realizada anamnese e avaliação baseada no protocolo MBGR, com enfoque para a fala, com o apoio de figuras e amostras de fala automática e espontânea. Resultados: não houve diferença entre os grupos quanto à queixa dos pais. Alterações de fala como desvios fonéticos, interposição lingual e distorções, e alterações de oclusão foram mais frequentes no GP. As alterações de fala prevaleceram para o sexo masculino em 83% e a média de idade relacionada a fala não evidenciou significância. Conclusão: crianças respiradoras orais possuem mais alterações dos sons da fala do que crianças sem alterações respiratórias, independentemente da faixa etária e são mais comuns em crianças do sexo masculino.

La difteria es una enfermedad prevenible por vacunas. En la actualidad existe un brote en nuestro país que se inició en abril de 2016. Desde entonces se han descrito casos en diferentes sitios del país predominantemente de la forma faríngea. La difteria cutánea no ha sido reportada en este brote. A nivel mundial el compromiso simultáneo en piel y mucosas, no ha sido descrito. La paciente cuya enfermedad describimos aquí es un caso de difteria de presentación atípica que se inicia con expresión cutánea y evoluciona a compromiso nasal, ocular y faríngeo con complicaciones severas en la órbita ocular expresadas como un síndrome del vértex orbitario, en una paciente con diabetes mellitus tipo 2 no controlada, que tuvo una evolución tórpida que la llevó a la muerte. Diphtheria is a vaccine-preventable disease. Actually, there is an outbreak in Venezuela since April 2016. Cases in different parts of the country have been described, predominantly pharyngeal. Cutaneous diphtheria has not been reported in this outbreak. The simultaneous presentation of the cutaneous form with mucosal involvement has not been described in the revised literature. We present a case of diphtheria with atypical presentation that begins with cutaneous expression and evolves to nasal, ocular and pharyngeal compromise with severe complications of the orbit, expressed in an orbital vertex syndrome in a patient with type 2 diabetes mellitus, who had a torpid evolution that led her to death. ABSTRACT INTRODUCTION: Adenotonsillectomy is the most common surgery performed by otolaryngologists in pediatric age, and one of the most frequently asked questions about the postoperative period is whether there is a potential for change in vocal pattern of these children. OBJECTIVE: To evaluate the impact of adenotonsillectomy in the voice emission pattern of children with hypertrophy of palatine and pharyngeal tonsils. METHODS: This is a prospective study in which we carried out perceptual auditory assessments and acoustic analysis of 26 children with adenotonsillar hypertrophy at three time points: before surgery, one month and three months after surgery. The following acoustic parameters were estimated using the Praat software: fundamental frequency, jitter, shimmer, and harmonic-noise ratio. RESULTS: A statistically significant change was found between shimmer and harmonic-noise ratio during vowel /u/ production between the preoperative and 1st month postoperative time points. No significant differences were detected for acoustic parameters between preoperative analysis and that of the 3rd month post-operation. CONCLUSION: Transient changes in acoustic parameters occur in children with adenotonsillar hypertrophy submitted to adenotonsillectomy, progressing to normalization in the 3rd postoperative month.


RESUMO INTRODUÇÃO: Adenotonsilectomia é o procedimento cirúrgico mais realizado pelos otorrinolaringologistas em pacientes pediátricos, e entre as dúvidas mais frequentes a respeito do pós-operatório, inclui-se a possibilidade de modificações no padrão vocal dessas crianças. OBJETIVO: Avaliar o impacto da adenotonsilectomia no padrão de emissão vocal de crianças com hipertrofia de tonsilas palatinas e faríngea. MÉTODO: Trata-se de estudo prospectivo, em que foram realizadas a avaliação perceptiva-auditiva e a análise acústica da voz de 26 crianças com hipertrofia adenotonsilar em três oportunidades: no pré-operatório e no 1° e 3° meses após o procedimento cirúrgico. Os parâmetros acústicos frequência fundamental, jitter, shimmer e proporção harmônico-ruído foram avaliados por meio do programa Praat. RESULTADOS: Houve uma alteração estatisticamente significante entre o shimmer e a proporção harmônico-ruído da emissão da vogal/u/entre o período pré-operatório e o 1° mês do pós-operatório. Não houve diferenças significantes dos parâmetros acústicos entre a análise pré-operatória e aquela realizada no 3° mês do pós-operatório. CONCLUSÃO: Crianças com hipertrofia adenotonsilar submetidas à adenotonsilectomia cursam com alterações transitórias dos parâmetros acústicos, evoluindo com a normalização dos mesmos no 3° mês do pós-operatório.

Abstract Introduction: Narrow airway dimensions due to mandibular deficiency can predispose an individual to severe respiratory distress. Hence, treatment with mandibular advancement devices at an early age might help improving the pharyngeal passage and reduce the risk of respiratory difficulties. Therefore, the aim of the current study was to evaluate the mean changes in the pharyngeal dimensions of children with mandibular deficiency treated with Clark's twin-block appliance (CTB) followed by fixed orthodontic treatment. Methods: Orthodontic records of 42 children with mandibular deficiency were selected. Records comprised three lateral cephalograms taken at the start of CTB treatment, after CTB removal and at the end of fixed appliance treatment, and were compared with 32 controls from the Bolton-Brush study. Friedman test was used to compare pre-treatment, mid-treatment and post-treatment pharyngeal dimensions. Wilcoxon signed rank test was used to compare the airway between pre-treatment and post follow-up controls. Mann-Whitney U test was applied to compare the mean changes in pharyngeal dimensions between treatment group and controls from T2 to T0. Post-hoc Dunnet T3 test was used for multiple comparisons of treatment outcomes after CTB and fixed appliances, taking a p-value of ≤ 0.05 as statistically significant. Results: Superior pharyngeal space (p < 0.001) and upper airway thickness (p = 0.035) were significantly increased after CTB, and the change in superior pharyngeal space remained stable after fixed mechano-therapy. Conclusion: CTB can have a positive effect in improving pharyngeal space and the resultant increase in airway remains stable on an average of two and a half years.


Resumo Introdução: a redução nas dimensões das vias aéreas causada pela deficiência mandibular pode predispor um indivíduo a dificuldades respiratórias severas. Assim, o tratamento com aparelhos de avanço mandibular na infância pode contribuir para melhorar a via aérea faríngea e reduzir o risco de problemas respiratórios. Objetivo: o objetivo do presente estudo foi avaliar as alterações médias nas dimensões da faringe de crianças com deficiência mandibular tratada com o aparelho Twin Block (TBC) seguido pelo tratamento ortodôntico fixo. Métodos: a documentação ortodôntica de 42 crianças com deficiência mandibular, consistindo de três telerradiografias de perfil - tiradas ao início do tratamento com TBC (T0), após a remoção do aparelho (T1) e ao final do tratamento ortodôntico fixo (T2) - foi selecionada e comparada à de 32 crianças controle do estudo Bolton-Brush. O teste de Friedman foi utilizado para comparar as dimensões da faringe antes, durante e após o tratamento. O teste de postos de Wilcoxon foi utilizado para comparar as vias aéreas antes do tratamento e depois do acompanhamento das crianças controle. O teste U de Mann-Whitney foi empregado para comparar as alterações médias nas dimensões da faringe entre o grupo tratado e as crianças controle, de T0 a T2. O teste T3 de Dunnett foi utilizado como post-hocpara realizar comparações múltiplas dos resultados do tratamento após o uso do TBC e dos aparelhos fixos, considerando-se como estatisticamente significativo um valor de p ≤ 0,05. Resultados: o espaço faríngeo superior (p < 0,001) e a espessura das vias aéreas superiores (p = 0,035) aumentaram significativamente após o uso do TBC, e a alteração no espaço faríngeo superior permaneceu estável após a mecanoterapia fixa. Conclusão: o TBC pode produzir um efeito positivo no espaço faríngeo, e aumento resultante nas vias aéreas permanece estável, em média, por dois anos e meio.

ABSTRACT

Introducción: la adenoidectomía y la tonsilectomía (amigdalectomía) son intervenciones quirúrgicas muy frecuentes en Otorrinolaringología. Sin embargo, muchas veces se las realiza sin un criterio valorable y de forma indiscriminada. Aunque en principio las indicaciones generales para estas intervenciones son los procedimientos infecciosos u obstructivos, en la actualidad también se contempla su utilidad en otros cuadros poco comunes como la sospecha de malignidad amigdalina, halitosis de origen amigdalino, nefropatía por IgA y Síndrome PANDAS (Síndrome de Desorden Neurosiquiátrico Pediátrico Autoinmune asociado con Estreptococo). A partir de septiembre de 2009, el Hospital Carlos Andrade Marín amplió la cobertura de atención a la población pediátrica, aumentando el número de estas cirugías en nuestro Servicio. El objetivo de este estudio fue, revisar las indicaciones y complicaciones de estos procedimientos basados en la experiencia de nuestro servicio en la población pediátrica y adulta y cotejarlas con los consensos internacionales. Materiales y métodos: se realizó un estudio descriptivo, retrospectivo utilizando los expedientes clínicos de los pacientes intervenidos en el Servicio de Otorrinolaringología del Hospital Carlos Andrade Marín en el período de mayo 2010 a diciembre 2012. Se analizaron datos demográficos, indicaciones para la realización de cirugía y complicaciones presentadas. Resultados: se intervinieron un total de 120 pacientes en este período. No hubo diferencia en el sexo de los pacientes. El promedio de edad en Tonsilectomía en adulto fue 26 años y en niños, 5 años; en la Adenoidectomía el promedio de edad fue 4.5 años y en los casos de adenoidectomía + tonsilectomía combinada, la edad promedio fue 5 años de edad. Se realizó tonsilectomía en el 45% de los pacientes, adenoidectomía en 30% y adenoidectomía + tonsilectomía combinada en 25% de los casos. La principal indicación para realización de tonsilectomía fue la amigdalitis recurrente (57% casos), mientras que para la adenoidectomía fue la hipertrofia adenoidea obstructiva (60% casos). La tasa total de complicaciones en tonsilectomía fue del 11% (6 / 54); correspondiendo a 4 casos de sangrado precoz (7%), y 2 casos de epistaxis posterior, secundarios a intubación nasotraqueal. En adenoidectomía, la tasa total de complicaciones fue del 5% (2/36), 1 caso por epistaxis severa, y 1 caso por desgarro de mucosa de nasofaringe que fue controlado durante cirugía. No hubo complicaciones en los casos de adenoidectomía + tonsilectomía combinada. No se presentaron decesos en ningún grupo. Conclusiones: la adenoidectomía y la tonsilectomía son procedimientos quirúrgicos eficaces y con baja tasa de complicaciones si se los emplea de acuerdo a los criterios establecidos en consensos internacionales. Nuestra experiencia quirúrgica está acorde a ello.


Introduction: adenoidectomy and tonsillectomy are very common surgeries in Otolaryngology. However, often they are done without a valuable criterion and indiscriminately. Although in principle the general indications for these interventions are infectious or obstructive procedures, currently its utility is also required by other rare cases as suspected tonsillar malignancy, halitosis of tonsillar origin, IgA nephropathy syndrome and PANDAS (Syndrome Pediatric Autoimmune Neuropsychiatric Disorder associated with Streptococcus). From september 2009, Carlos Andrade Marín Hospital expanded coverage for care in the pediatric population, increasing the number of these surgeries in our Service. The aim of this study was to review the indications and complications of these procedures based on the experience of our service in the pediatric and adult population and to align with international consensus. Materials and methods: a descriptive and retrospective study was conducted using the medical records of patients treated in the Department of Otolaryngology, Carlos Andrade Marín Hospital in the period from may 2010 to december 2012. Demographic data, indications for surgery and complications were analyzed. Results: a total of 120 patients were operated during this period. There was no difference in sex of patients. The average age in adult Tonsillectomy was 26, and in children, 5 years; the mean age in Adenoidectomy was 4.5 years and in cases of combined tonsillectomy and adenoidectomy, the average age was 5 years old. Tonsillectomy was performed in 45% of patients, 30% adenoidectomy, and combined tonsillectomy and adenoidectomy in 25% of cases. The main indication for performing tonsillectomy was recurrent tonsillitis (57% cases), while for adenoidectomy was obstructive adenoid hypertrophy (60% cases). The overall complication rate in tonsillectomy was 11% (6/54); corresponding to 4 cases of early bleeding (7%), and 2 cases of posterior epistaxis by nasotracheal intubation. In adenoidectomy, the overall complication rate was 5% (2/36), 1 case of severe epistaxis, and 1 case by tearing of the nasopharyngeal mucosa that was controlled during surgery. There were no complications in cases of adenoidectomy + combined tonsillectomy. No deaths occurred in either group. Conclusions: adenoidectomy and tonsillectomy are effective surgical procedures and have low rate of complications if they are used according to the criteria established by international consensus. Our surgical experience is accordingly.

ABSTRACT

Facial deformities are related to morphological differences and the mandible position shows differences in maxillomandible relation. The aim of this research was to compare the pharyngeal airway space (PAS) in subjects with class II and class III facial deformities We included 28 adult subjects with skeletal characteristics associated to class II or class III according to the SNA angle and dental overjet; subjects with facial asymmetry and other facial deformities and subjects with facial trauma or facial surgery history were excluded. Cone beam computed tomography was realized (CBCT) to asses the nasopharynx, oropharynx, hypopharynx, as well as the distance measured between the mandible genial spine and hyoid bone; data analysis were realized by descriptive analysis and statistical analysis using t test with 0.05 to show statistical differences. Class II subjects presented minor values in all of the measurements; in the oropharynx and the hypopharynx we observed the most important differences, with nasopharynx showing statistically significant differences (p<0.05). In conclusion class II subjects presented a minor pharyngeal airway space and it is suggested that this information should be used in the diagnosis process and prior to surgical treatment.


Las deformidades faciales son asociadas a diferencias en la posición mandibular evidenciando diferencias en la relación maxilomandibular. El objetivo de esta investigación fue comparar el espacio aéreo faríngeo en sujetos con deformidad facial clase II y clase III. Se incluyeron 28 sujetos con características esqueletales asociadas a clase II o clase III seguidos de la evaluación del angulo SNA y el resalte dentario; se excluyeron los sujetos con asimetría facial y otras deformidades faciales y sujetos con historia de trauma facial o historia de cirugía facial; se realizó la tomografía computadorizada cone beam para evaluar el área de nasofaringe, orofaringe, hipofaringe y la distancia entre la espina geni mandibular y el hueso hioides; los datos se analizaron con estadística descriptiva y la prueba t usando un valor de 0,05 para establecer significancia estadística. Se observó que los sujetos de clase II presentaron valores menores a los sujetos clase III en todas las mediciones realizadas; en el área de orofaringe e hipofaringe se observaron las diferencias mas importantes, estadísticamente significativas (p<0,05). Se puede concluir que los sujetos con deformidad facial clase II presentan un espacio de vía aérea faríngea más estrecho y se sugiere que este temática sea resuelta en la etapa de diagnóstico previo a la selección de tratamientos quirúrgicos o no quirúrgicos.

OBJECTIVE: To verify the reliability of adenoid hypertrophy diagnosis by cephalometric radiography. METHOD: Thirty male subjects, aged between 12 and 15 years, either mouth-breathers, or not, were selected. Diagnostic tests for adenoid hypertrophy were performed by radiological cephalometry based on lateral cephalometric radiographs and nasal endoscopy (gold standard). The CefX Cephalometric software program, version 2000 was used and the rhinoscopy was performed with a flexible endoscope. Blockage of 47% and 75% of the nasopharynx were taken as the cutoff points for cephalometric radiography and endoscopy, respectively. RESULTS: The correlation between the two examinations was considered moderately positive (0.5). Tests of validity and reliability reported a sensitivity of 100%; specificity 65.5%; positive predictive value of 9.1%; negative predictive value 100%, and exactness of 66.60%. CONCLUSION: Lateral cephalometric radiography was considered practical and comfortable for the patient; relatively efficient for detecting adenoid hypertrophy and obtaining the diagnosis of nasopharyngeal airway obstruction. .


OBJETIVO: Verificar a confiabilidade do diagnóstico da hipertrofia das adenóides por meio da cefalometria. MÉTODOS: Trinta indivíduos do gênero masculino, com idades entre 12 e 15 anos, respiradores bucais ou não, foram selecionados e neles realizados exames de diagnóstico de hipertrofia da adenóide por cefalometria radiológica, a partir da telerradiografia em norma lateral e exames rinoscópicos, a partir da nasofibroscopia (padrão ouro). A cefalometria utilizada foi a computadorizada e para isto se utilizou o programa CefX versão 2000 e a rinoscopia foi realizada com um nasofibroscópio flexível. Foi tomado como ponto de corte 47% e 75% de bloqueio da nasofaringe para a telerradiografia e nasofibroscopia, respectivamente. RESULTADOS: A correlação encontrada entre os dois exames foi considerada moderadamente positiva (0,5) e os testes de validade e confiabilidade registraram uma sensibilidade de 100%, especificidade de 65,5%, valor preditivo positivo de 9,1%, valor preditivo negativo de 100% e exatidão de 66,60%. CONCLUSÃO: A telerradiografia lateral foi considerada um meio prático, confortável para o paciente e relativamente eficiente na detecção da hiperplasia da adenóide e na obtenção do diagnóstico de obstrução nasofaringeana. .

Objective The objective of the present study was to evaluate current radiographic parameters designed to investigate adenoid hypertrophy and nasopharyngeal obstruction, and to present an alternative radiographic assessment method. Materials and Methods In order to do so, children (4 to14 years old) who presented with nasal obstruction or oral breathing complaints were submitted to cavum radiographic examination. One hundred and twenty records were evaluated according to quantitative radiographic parameters, and data were correlated with a gold-standard videonasopharyngoscopic study, in relation to the percentage of choanal obstruction. Subsequently, a regression analysis was performed in order to create an original model so the percentage of the choanal obstruction could be predicted. Results The quantitative parameters demonstrated moderate, if not weak correlation with the real percentage of choanal obstruction. The regression model (110.119*A/N) demonstrated a satisfactory ability to “predict” the actual percentage of choanal obstruction. Conclusion Since current adenoid quantitative radiographic parameters present limitations, the model presented by the present study might be considered as an alternative assessment method in cases where videonasopharyngoscopic evaluation is unavailable. .


Objetivo O objetivo deste estudo foi avaliar parâmetros radiográficos atuais destinados à verificação da adenoide e obstrução nasofaríngea e apresentar um método de avaliação alternativo. Materiais e Métodos Crianças (4 a 14 anos) que apresentavam queixas de obstrução nasal e/ou respiração oral foram submetidas ao exame radiográfico de cavum faríngeo. Cento e vinte registros foram avaliados por parâmetros radiográficos quantitativos, e estes dados foram correlacionados ao exame de videonasofaringoscopia, aqui considerado como padrão ouro, em relação à porcentagem de obstrução coanal. Posteriormente, uma análise de regressão foi realizada com os mesmos parâmetros quantitativos, de modo que um modelo original fosse criado com o objetivo de predição do percentual de obstrução coanal. Resultados Os parâmetros quantitativos atuais demonstraram correlações moderadas, quando não fracas, ao percentual de obstrução. O modelo de regressão desenvolvido (110.119*A/N) demonstrou capacidade satisfatória de “prever” o real percentual de obstrução adenóidea. Conclusão Uma vez que os parâmetros radiográficos atuais apresentam limitações, o modelo original aqui apresentado deve ser considerado como um método de avaliação adenóidea alternativo, a ser utilizado quando a videonasofaringoscopia estiver indisponível. .

ABSTRACT

Fundamento: La adenoiditis crónica, causante de la mayor cantidad de cirugías mayores electivas en niños, es una enfermedad frecuente en Cuba. Objetivo: describir las características clínicas y epidemiológicas de la adenoiditis crónica en niños. Métodos: estudio descriptivo con muestreo no probabilístico a 98 niños con adenoiditis crónica atendidos en el Hospital Pediátrico Universitario de Centro Habana, entre septiembre de 2009 y julio de 2011. Las variables analizadas fueron: edad, sexo, síntomas, signos y principales manifestaciones clínicas, principales antecedentes patológicos personales, antecedentes familiares, y principales factores de riesgos ambientales y sociales. Se realizó una encuesta para identificar factores de riesgo. Se efectuaron procesamientos estadísticos tales como: media, frecuencia relativa y tablas de frecuencia. Resultados: la mayor morbilidad se observó en los niños de 1 y 9 años de edad. Las principales características de la enfermedad, fueron la obstrucción nasal, mala oclusión dental, respiración oral e infección respiratoria. Los factores de riesgo más frecuentes fueron la asistencia al círculo infantil y la exposición al humo del cigarro. Los antecedentes personales y familiares más observados fueron el asma y la alergia respiratoria. Conclusión: la adenoiditis crónica en los pacientes pediátricos, es de origen multifactorial, y tiende a disminuir en la población infantil mayor de nueve años.


Background: chronic adenoiditis, which causes the greater number of elective major surgeries in children, is a common disease in Cuba. Objectives to describe the clinical and epidemiological characteristics of chronic adenoiditis in children. Methods: a descriptive study with non-probability sampling was conducted in 98 children with chronic adenoiditis treated at the University Pediatric Hospital of Central Havana, between September 2009 and July 2011. The variables analyzed were age, sex, symptoms, signs and main clinical manifestations, personal medical history, family history, and major environmental and social risk factors. A survey was conducted to identify risk factors. Statistical analysis such as: the mean, relative frequency and frequency tables were performed. Results: highest morbidity was observed in children aged 1 to 9 years. The main features of the disease were nasal obstruction, dental malocclusion, mouth breathing and respiratory infection. The most common risk factors were attendance to day-care centers and exposure to cigarette smoke. Personal and family history of asthma and respiratory allergies was the most frequently found. Conclusion: chronic adenoiditis in pediatric patients is multifactorial in origin, and tends to decrease in the child population older than nine years.

O respirador bucal utiliza a cavidade oral como principal via durante a respiração. Dentre as principais causas, destacam-se: as hipertrofias adenoamigdalianas e as doenças inflamatórias como a rinite alérgica. OBJETIVO: Verificar a presença de atopia, os principais alérgenos envolvidos e verificar a coexistência de atopia com o grau de hipertrofia das tonsilas faríngeas e palatinas, em pacientes respiradores bucais. MÉTODO: Estudo de coorte histórico com corte transversal com revisão de 308 prontuários de pacientes acompanhados em um centro do respirador bucal de um hospital terciário, no período de 2008 a 2010. Foram coletados dados sobre a história clínica de respirador bucal e realizados exames clínico otorrinolaringológico, nasofibroscópico e teste cutâneo de leitura imediata aos aeroalérgenos. RESULTADOS: Dos 308 pacientes, 36% apresentaram positividade no teste alérgico, sendo que dos atópicos 95% foram positivos para ácaros. Do total de pacientes, 46% apresentaram hipertrofia adenoideana. Destes, 37% são atópicos e 47% apresentaram hipertrofia amigdaliana e, destes, 33% são atópicos. CONCLUSÃO: Nenhuma correlação direta entre atopia e o grau de aumento das tonsilas palatinas e faríngeas foi observada nos pacientes respiradores bucais avaliados. .


Mouth breathers use the oral cavity as their principal breathing route. The main causes include: adenotonsillar hypertrophy and inflammatory diseases such as allergic rhinitis. OBJECTIVE: To look for atopy, the main allergens involved and to check for atopy as a comorbidity with the degree of hypertrophy of the tonsils and adenoids in mouth breathers. METHOD: A historical cohort study with cross-sectional review of 308 medical charts of patients treated at a mouth breather care center of a tertiary hospital in the period of 2008-2010. We collected data on the mouth breather's clinical history and we ran otolaryngological exams, flexible nasal endoscopy and skin prick test to aeroallergens. RESULTS: Of 308 patients, 36% were positive on allergy testing, with 95 % of atopic patients being positive for mites. Among all patients, 46% had adenoid hypertrophy; of these, 37% were atopic and 47% had tonsillar hypertrophy, and among these, 33% were atopic. CONCLUSION: We found no direct correlation between atopy and the degree of tonsils and adenoid hypertrophy observed among the mouth-breathing patients assessed. si. .

As alterações de mastigação e de deglutição em crianças com hipertrofia adenoamigdaliana precisam ser melhor caracterizadas. OBJETIVO: Avaliar a frequência das alterações miofuncionais autorreportadas pelos pais, e se há diferenças entre os padrões de alterações entre crianças com hipertrofia adenoamigdaliana e as com hipertrofia apenas adenoideana. MÉTODO: Aplicação de questionário e avaliação clínica fonoaudiológica em crianças com hipertrofia de tonsilas com idade entre 3 e 6 anos. Os dados reportados pelos pais foram comparados com os dados obtidos pela avaliação fonoaudiológica; além disso, os dados das crianças com hipertrofia adenoamigdaliana foram comparados com as com hipertrofia adenoideana. Desenho científico: coorte transversal. RESULTADOS: As alterações miofuncionais observadas pela fonoaudióloga foram muito mais frequentes do que as reportadas pelos pais e não houve concordância entre os dois achados. As crianças com hipertrofia adenoideana e as com hipertrofia adenoamigdaliana apresentaram o mesmo padrão de alterações miofuncionais. CONCLUSÃO: Os pais relacionam pouco a hipertrofia de tonsilas a alterações na mastigação e na deglutição. A causa da obstrução respiratória parece não interferir no padrão de alteração miofuncional.


The changes in mastication and deglutition in children with adenotonsillar hypertrophy need to be better characterized. OBJECTIVE: To evaluate the frequency of parent-reported myofunctional changes and to determine if there are differences in the alteration patterns of children with adenotonsillar hypertrophy and subjects with adenoid hypertrophy. METHOD: Questionnaire and assessment by a speech therapist of children aged between three and six years with tonsillar hypertrophy. The data reported by the parents were compared to the data obtained from the speech therapist's evaluation; additionally, data from children with adenotonsillar hypertrophy were compared to findings from subjects with adenoid hypertrophy. Study Design: cross-sectional cohort. RESULTS: The myofunctional changes observed by the speech therapist were more frequent than the alterations reported by the parents, and there was no correlation between the two findings. The children with adenoid hypertrophy and the individuals with adenotonsillar hypertrophy had the same pattern of myofunctional alteration. CONCLUSION: Parents cannot clearly correlate tonsillar hypertrophy with changes in mastication and deglutition. The cause of the respiratory obstruction does not seem to interfere in the pattern of myofunctional change.

ABSTRACT

La fisura labio palatina corresponde a una malformación congénita, caracterizada por la interrupción en la continuidad de los tejidos. La cirugía primaria del velo del paladar permite reconstituir la morfología perdida del velo palatino, sin embargo ésta no siempre considera la devolución de la anatomía del músculo de la úvula, por lo cual, el velo operado adquiere una anatomía anómala impidiendo el cierre óptimo entre cavidad oral y nasal; esta condición se conoce con el nombre de insuficiencia velo faríngea. El objetivo de este trabajo fue asociar la morfología del velo palatino con la insuficiencia velo faríngea en individuos fisurados operados. Se realizó un estudio analítico, teniendo como unidad de muestra al velo del paladar; La muestra se seleccionó entre aquellos pacientes que cumplían con los criterios de inclusión. A los individuos seleccionados se les realizó un examen clínico y un registro fotográfico con el objetivo de clasificar la anatomía del velo del paladar. Posteriormente se les realizó una evaluación fonoaudiológica para determinar el grado de insuficiencia velo faríngea. Los datos obtenidos fueron tabulados y evaluados estadísticamente. La clasificación morfológica del velo del paladar determino que el 52% de los velos fueran clasificados como cóncavos, los velos convexos y planos se distribuyeron ambos en un 24%. Según la evaluación velo faríngea, el 12% se presentaron como insuficientes, el 52% como límite insuficiente, el 24% como límite suficiente y tan solo el 8% como suficientes. Al relacionar la morfología del velo del paladar con evaluación velo faríngea se obtuvo un p = 0,3. Existe una variabilidad tanto en la morfología del velo del paladar como en la evaluación de la insuficiencia velo faríngea en individuos fisurados operados, siendo esta ultima desfavorable. No se estableció una relación estadística entre morfología del velo del paladar y la evaluación velo faríngea.


The reason for lip and cleft palate is due to a congenital malformation characterized by the interruption in the continuity of tissue. The primary surgery of the palate velum allows us to reconstruct the lost morphology in the velum palate, however this doesn´t always consider the return of the uvula muscle anatomy, whereby the operated velum becomes an anomalous anatomy thus preventing an optimum closing between the nasal and oral cavity; this condition is known as pharyngeal velum insufficiency. The aim was to associate palate velum morphology with pharyngeal velum insufficiency on cleft palate operated patients. An analytical study was done, using the velum palate as sample; the sample was selected amongst patients who met the inclusion criteria. A photographic record and a clinical exam were done on those patients who were chosen in order to classify the anatomy of the velum palate. Subsequently a phonoaudiological evaluation was done in order to establish the degree of pharyngeal velum insufficiency. The data obtained were statistically tabulated and evaluated. The morphological classification of the palate velum determined that 52% of the vela were classified as concave; the flat and convex vela both had a 24% share each. According to the evaluation of the pharyngeal velum, 12% were shown to be insufficient, 52% as borderline insufficient, 24% as borderline sufficient, and only 8% proved to be sufficient. When relating the palate velum morphology with pharyngeal velum evaluation a p = 0.3 was obtained. There is a variability in both the palate velum morphology and in the evaluation of the pharyngeal velum insufficiency on operated cleft palate patients, with the latter shown as being unfavorable. No statistical relation was established between palate velum morphology and pharyngeal velum evaluation.

ABSTRACT

Fundamento: la adenoamigdalitis crónica en la infancia es una enfermedad frecuente en Cuba y ocupa el tercer lugar en la morbilidad quirúrgica en la especialidad de otorrinolaringología pediátrica. Objetivos: describir el comportamiento clínico-epidemiológico de la adenoamigdalitis crónica infantil. Métodos: se realizó un estudio descriptivo, prospectivo y transversal con un muestreo no probabilístico a 191 niños con adenoamigdalitis crónica, en un universo de 1 050 niños con enfermedades otorrinolaringológicas que asistieron al Hospital Pediátrico Universitario de Centro Habana entre septiembre de 2009 y julio de 2011. Para el procesamiento de datos se realizó una encuesta de factores de riesgo de padecer la enfermedad. Se efectuaron procesamientos estadísticos tales como: media, riesgo relativo porcentual y tablas de frecuencia por medio del software Excel. Resultados: la mayor morbilidad fue en varones de dos a cuatro años. La enfermedad se caracterizó por episodios de amigdalitis recurrente con obstrucción nasal y rinitis. Los factores de riesgo modificables más frecuentes fueron: ausencia o destete precoz, exposición involuntaria al humo del cigarro y la asistencia al círculo infantil. Los antecedentes personales y familiares más frecuentes fueron: alergia respiratoria o asma bronquial. Conclusiones: la enfermedad es de origen multifactorial y disminuye su morbilidad a partir de los seis años de edad.


Background: chronic adenotonsillitis in childhood is a very frequent disease in Cuba, and it occupies the third place in the surgical morbidity in the specialty of Pediatric Otorhinolaryngology. Objective: to describe the clinical-epidemiological behavior of chronic adenotonsillitis in childhood. Methods: a descriptive, prospective and cross-sectional study was conducted with a non-probabilistic sampling to 191 children with chronic adenotonsillitis in a universe of 1 050 children with otorhinolaryngological diseases treated in the Pediatric Hospital of Centro Habana, Cuba, from September 2009 to July 2011. Data processing was carried out using statistic tools such as: mean age, percentage relative risk and frequency tables by means of Excel software. Results: the biggest morbidity was in males from two to four years. The disease was characterized by episodes of recurrent tonsillitis with nasal obstruction and rhinitis. The most frequent modifiable risk factors were: absence or early suspension of maternal nursing, involuntary exposure to smoke cigar and the attendance to kindergarten. The most common personal and family antecedents were: respiratory allergy or bronchial asthma. Conclusions: this disease has a multifactorial origin, and it decreases its morbidity from the six years.

ABSTRACT

OBJETIVO: Verificar os aspectos fonoaudiológicos do sistema estomatognático, incluindo as estruturas fonoarticulatórias (lábios, língua e palato mole) e as funções de deglutição, respiração, fala e fonação (qualidade vocal e ressonância), pré e pós adenotonsilectomia. MÉTODOS: Participaram 22 crianças, 17 do gênero masculino e cinco do gênero feminino, na faixa etária entre 5 e 10 anos. Todas apresentavam hipertrofia de tonsilas palatina e faríngea com indicação cirúrgica de adenotonsilectomia e nenhuma delas havia passado por fonoterapia prévia. A avaliação fonoaudiológica foi realizada no período pré-operatório e entre um e seis meses após a adenotonsilectomia e constou de avaliação das estruturas (lábios, língua e palato mole), das funções de deglutição (líquido), respiração (modo), fala e fonação (qualidade vocal e ressonância). Para avaliar a qualidade vocal e a ressonância, participaram 15 sujeitos com avaliação pós-operatória realizada no período de um a dois meses. RESULTADOS: Houve diferença quanto ao modo respiratório nasal, postura de lábios ocluídos em repouso, tônus de língua alterado, mobilidade adequada de palato mole, postura de língua alterada na deglutição de líquido, e ausência do mecanismo compensatório de interposição de lábios na deglutição. Houve redução na frequência dos processos de distorção. Em relação à fonação, foi observada discreta melhora na qualidade vocal e ressonância. CONCLUSÃO: Após a adenotonsilectomia algumas estruturas e funções podem se readaptar ou apresentar melhora espontaneamente. No entanto, foi necessário encaminhar a maioria das crianças para o atendimento fonoaudiológico na busca da readaptação das estruturas estomatognáticas e funções avaliadas.


PURPOSE: To verify the speech therapy aspects of the stomatognathic system, including phonoarticulatory structures (lips, tongue, and soft palate) and swallowing, respiratory, speech, and phonation (vocal quality and resonance) functions, before and after undergoing adenotonsillectomy. METHODS: The study included 22 children, 17 males and 5 females, aged between 5 and 10 years, suffering from hypertrophy of palatine and adenoid tonsils, with surgical indication for adenotonsillectomy and with no previous speech therapy. The speech-language pathology evaluation was performed before surgery and during the period between 1 and 6 months after adenotonsillectomy. It consisted of an evaluation of structures (lips, tongue, and soft palate) and of swallowing (liquid), respiration (mode), speech, and phonation (voice quality and resonance) functions. To evaluate vocal quality and resonance, 15 participants with the postoperative evaluation carried out in a period from 1 to 2 months were considered. RESULTS: There were differences regarding nasal respiratory mode, lips closed at rest posture, changed tongue tonus, adequate mobility of the soft palate, changed tongue posture during liquid swallowing, and absence of interposition compensatory mechanism of lips in swallowing. Reduction in the frequency of distortion processes was also found. With regard to speech, little improvement in vocal quality and resonance was seen. CONCLUSION: Following adenotonsillectomy, some structures and functions can spontaneously readapt or improve. However, most children needed to be referred to speech therapy for readapting stomatognathic structures and the assessed functions.
