ABSTRACT

Introduction: Fournier's gangrene is characterized by tissue necrosis, which requires treatment employing debridement and antibiotics with wounds of varying sizes. The objective is to standardize the surgical techniques of reconstructions with flaps used to treat wounds after Fournier's gangrene. Method: A study was conducted by searching the PubMed/Medline, SciELO, and LILACS databases. Results: In wounds with skin loss of 25% to 50%, a local advancement cutaneous flap or a pudendal flap from the thigh was used; in wounds, greater than 50%, a superomedial thigh flap or myocutaneous flap from the gracilis muscle was used, with the aim of to enable proper reconstruction. Conclusion: Advancement and pudendal thigh flaps were used for wounds with up to 50% loss of scrotal skin substance, while the myocutaneous gracilis flap and supero-medial flap of the thigh were indicated for wounds with more than 50% of the total scrotal surface affected, after Fournier gangrene.


Introdução: Gangrena de Fournier é caracterizada por necrose tecidual, que necessita de tratamento por meio de desbridamento e antibióticos, com feridas de dimensões variadas. O objetivo é padronizar as técnicas cirúrgicas de reconstruções com retalhos utilizadas no tratamento das feridas após gangrena de Fournier. Método: Realizou-se estudo por meio da busca nas bases de dados PubMed/Medline, SciELO e LILACS. Resultados: Nas feridas com perdas cutâneas de 25% a 50%, foram utilizados retalho cutâneo local de avanço ou retalho pudendo da coxa, nas maiores de 50% foram necessárias as confecções do retalho superomedial da coxa ou retalho miocutâneo do músculo grácil, com intuito de possibilitar a reconstrução adequada. Conclusão: Os retalhos de avanço e pudendo da coxa foram utilizados para feridas com perda de substância cutânea escrotal de até 50%, enquanto os retalhos miocutâneo de músculo grácil e superomedial da coxa foram indicados para as feridas com mais de 50% da superfície escrotal total acometida, após gangrena de Fournier.

Introducción: La fascitis necrotizante es una infección grave que involucra la piel, el tejido celular subcutáneo y las fascias superficiales-profundas, progresa muy rápidamente y si no se trata a tiempo puede causar la muerte. Reporte de caso: Presentamos el caso de una paciente adulta con antecedentes de anemia, a quien se le diagnosticó fascitis necrosante de origen odontogénico, por lo que fue intervenida quirúrgicamente y se realizó un drenaje intraoral y extraoral más escarectomía. Además, se le administró tratamiento farmacológico (antibióticos intravenosos) y se colocó una membrana amniótica como material de apósito para reconstruir la superficie hemorrágica dejada por la fascitis necrotizante. Conclusiones: La membrana amniótica como material de apósito presentó resultados favorables en la paciente al obtenerse una rápida regeneración de la secuela producida por la fascitis necrotizante.


Introduction: Necrotizing fasciitis is a serious infection that involves the skin, the subcutaneous tissue, and the superficial-deep fascia, it progresses very quickly and if not treated in time it can cause death. Report case: We present the case of an adult patient with a history of anemia, who was diagnosed with odontogenic necrotizing fasciitis, for which she underwent surgery and underwent intraoral and extraoral drainage plus escharectomy. In addition, pharmacological treatment (intravenous antibiotics) was administered, and an amniotic membrane was placed as a dressing material to reconstruct the hemorrhagic surface left by the necrotizing fasciitis. Conclusions: The amniotic membrane as a dressing material presented favorable results in the patient, obtaining a rapid regeneration of the sequel produced by necrotizing fasciitis.

Introdução: Gangrena de Fournier é uma infecção polibacteriana, geralmente causada por bactérias anaeróbias e aeróbias, sendo caracterizada por fasciite necrosante escrotal e perineal. Seu tratamento é embasado em intervenção cirúrgica com excisão da área necrótica e antibioticoterapia precoces. Diversas são as estratégias de reconstrução do defeito resultante do desbridamento, devendo ser salientado que perdas teciduais maiores do que 50% costumam ser reconstruídas com retalhos. Métodos: Análise retrospectiva da série de casos de reconstrução escrotal após gangrena de Fournier procedidos pelos autores ao longo de 2020, totalizando oito pacientes. Resultados: O retalho mais utilizado foi o fasciocutâneo de coxa, que apresentou taxa de necrose parcial de 14,29%, sem necrose total. Em um dos casos foi possível reconstruir uma uretra esponjosa com músculo grácil sem fistulização, evitando que o paciente fosse submetido a uma uretrostomia definitiva. Quanto às complicações, foi comum a ocorrência de intercorrências menores que necessitaram de procedimentos revisionais simples. Destaca-se a prevalência de 75% de diabetes mellitus em nossa casuística, o que pode ter interferido negativamente no processo cicatricial. Conclusão: A reconstrução escrotal com retalhos é importante para acelerar a cicatrização da ferida proveniente do desbridamento de gangrena de Fournier e para manter o aspecto de bolsa necessário para a termorregulação do testículo. Nossa opção primária foi o retalho fasciocutâneo de coxa, que se mostrou seguro. Pequenas intercorrências foram frequentes nesta série, sem comprometimento do resultado final.


Introduction: Fournier's gangrene is a polybacterial infection, usually caused by anaerobic and aerobic bacteria, characterized by scrotal and perineal necrotizing fasciitis. Its treatment is based on surgical intervention by excision of the necrotic area and early antibiotic therapy. There are several strategies to reconstruct the defect resulting from debridement, and it should be noted that tissue losses greater than 50% are usually reconstructed with flaps. Methods: Retrospective analysis of the series of cases of scrotal reconstruction after Fournier's gangrene performed by the authors throughout 2020, totaling eight patients. Results: The most used flap was thigh fasciocutaneous flap, which presented a partial necrosis rate of 14.29%, without total necrosis. In one of the cases, it was possible to reconstruct a spongy urethra with gracilis muscle without fistulization, preventing the patient from undergoing a definitive urethrostomy. As for complications, the occurrence of minor complications that required simple revision procedures was common. The prevalence of 75% of diabetes mellitus in our series is highlighted, which may have negatively interfered with the healing process. Conclusion: Scrotal reconstruction with flaps is important to accelerate wound healing from Fournier gangrene debridement and to maintain the pouch aspect necessary for testicular thermoregulation. Our primary option was thigh fasciocutaneous flap, which proved to be safe. And minor complications were frequent in this series, without compromising the final result.

Introdução: O ultrassom tem sido utilizado na atualidade na medicina intensiva. A fasciite necrosante quando não diagnosticada e tratada rapidamente apresenta progressão rápida e alta mortalidade. O objetivo é apresentar a importância da anatomia na fasciite necrosante e o uso do ultrassom no diagnóstico precoce. Métodos: Apresentou-se a aplicação do ultrassom point of care e a relevância da anatomia na fasciite necrosante. Resultados: As comunicações anatômicas entre as fáscias das regiões escrotal, perineal, peniana e abdominal permitem a disseminação do processo infeccioso decorrente da gangrena de Fournier para as regiões adjacentes. O ultrassom possibilitou o diagnóstico precoce na fasciite necrosante. Conclusão: As comunicações entre as fáscias das regiões escrotal, perineal, peniana e abdominal contribuíram para a progressão do processo infeccioso decorrente da gangrena de Fournier e o ultrassom possibilitou o diagnóstico precoce.


Introduction: Ultrasound is currently being used in intensive care medicine. When not diagnosed and treated quickly, necrotizing fasciitis has a rapid progression and high mortality. The objective is to present the importance of anatomy in necrotizing fasciitis and the use of ultrasound in early diagnosis. Methods: The application of point-of-care ultrasound and the relevance of anatomy in necrotizing fasciitis were presented. Results: The anatomical communications between the fasciae of the scrotal, perineal, penile and abdominal regions allow the spread of the infectious process resulting from Fournier's gangrene to the adjacent regions. Ultrasound enabled early diagnosis of necrotizing fasciitis. Conclusion: Communications between the fasciae of the scrotal, perineal, scrotal, penis and abdominal regions contributes to the progression of the infectious process resulting from Fournier gangrene and ultrasound permitted earlier diagnose.

ABSTRACT

Objective:To retrospectively analyze the treatment of 25 cases of lower necrotizing fasciitis.Methods:A total of 25 patients with lower limb necrotizing fasciitis (13 males and 12 females), with mean age 63 years old (48-75 years old) in Dalian Municipal Central Hospital from September 2016 to December 2020. After admission, the patient′s general physical condition was strictly evaluated, the relevant preoperative examination was improved, and the necrotizing fasciitis laboratory risk index (LRINEC) score was performed. In the absence of surgical contraindication, multiple debridement was performed, leaving the necrotic tissue removed for general bacterial culture and drug sensitivity test in parallel. After debridement, eight patients showed a large area of skin necrosis, and amputation was selected. The other 17 patients chose limb protection treatment after debridement, and adopted debridement and free skin grafting. After surgery, patients were encouraged to strengthen rehabilitation exercise to restore limb function to the maximum extent.Results:With followed up 0.6 to 3.0 years, with an average of 1.8 years. Methods include outpatient return visit, WeChat contact or telephone inquiry. The skin survived in 17 patients with mean healing time (27.5 ± 6.9) d. Eighteen patients were multiple bacterial infections and seven patients were single bacterial infections. All patients had no joint dysfunction caused by scar contracture, and reinfection in the skin grafting area.Conclusions:Necrotizing fasciitis requires early diagnosis and early treatment, with correct choice of treatment method is closely related to the patient′s prognosis.

ABSTRACT

Three cases of vibrio vulnificus-associated necrotizing fasciitis are presented. All patients showed a rapidly progressing, swelling, redness in the limb after eating seafood. Additionally, they presented with multiple organ dysfunction syndromes affecting the heart, lung, liver, kidney, coagulation, and blood. Vibrio vulnificus related necrotizing fasciitis is a fatal soft-tissue infection with high mortality. These are essential to improve survival by early clinical suspicion and immediate surgical intervention.

ABSTRACT

Objective:To summarize the infective characteristics of Fournier's gangrene (FG) and evaluate the effect of negative pressure wound therapy (NPWT).Methods:A retrospective case control study was conducted to analyze the clinical data of 31 patients with FG admitted to Peking University First Hospital from May 2010 to September 2020, including 29 males and 2 females, aged 21-78 years [(55.2±2.0)years]. A total of 29 patients were caused by infectious diseases of the perianal and urinary system, and the rest two patients were caused by vulvar infection and retroperitoneal abscess. A total of 23 patients were treated with NPWT (Group A) and 8 patients were treated with conventional dressing (Group B). Characteristics of pathogen, drug-resistance rate, medical treatment and prognosis for all patients were summarized. The hospitalization duration, numbers of operation and wound healing time were compared between two groups.Results:Monomicrobial infection was identified in 14 patients, while polymicrobial infection in 15 patients, fungal infection in 1 and culture-negative in 1. Escherichia coli, Enterococcus faecalis, Enterococcus faecium, Klebsiella pneumoniae and Staphylococcus haemolyticus were the most common pathogenic bacteria. The resistance rate of gram-negative bacilli to third-generation cephalosporins was 37%. Staphylococcus haemolyticus were methicillin-resistant Staphylococcus. The carbapenem antibiotics combined with vancomycin antibiotics were used for all patients as the empirical anti-infection treatment. Three patients died, and the rest 28 patients were followed up for 3 to 12 months [(10.8±2.6)months] after discharge. All the wounds were healed well without recurrence. In Group A and Group B, the hospitalization duration was (37.4±15.0)days and (47.0±16.0)days, respectively ( P>0.05); the number of operation was 3(3, 6) times and 13(4, 17)times, respectively ( P<0.05); the wound healing time was (38.9±17.8)days and (61.8±14.2)days, respectively ( P<0.05). Conclusions:Enterobacteriaceae, Enterococcus and Staphylococcus haemolyticus are the most common pathogenic bacteria for FG, among which the proportion of drug-resistant bacteria is relatively high. NPWT is an effective adjuvant therapy for wound management with reduced operation times and short wound healing time compared to conrentional method.

ABSTRACT

Abstract The severe bacterial diseases discussed herein are those that present dermatological lesions as their initial manifestations, for which the dermatologist is often called upon to give an opinion or is even the first to examine the patient. This review focuses on those that evolve with skin necrosis during their natural history, that is, necrotizing fasciitis, Fournier gangrene, and ecthyma gangrenosum. Notice that the more descriptive terminology was adopted; each disease was individualized, rather than being referred by the generic term "necrotizing soft tissue infections". Due to their relevance and increasing frequency, infections by methicillin-resistant Staphylococcus aureus (MRSA) were also included, more specifically abscesses, furuncle, and carbuncle, and their potential etiologies by MRSA. This article focuses on the epidemiology, clinical dermatological manifestations, methods of diagnosis, and treatment of each of the diseases mentioned.

RESUMEN Introducción: la fascitis necrotizante corresponde a una infección bacteriana de los tejidos blandos, caracterizada por necrosis difusa y rápidamente progresiva del tejido celular subcutáneo y sistema músculo aponeurótico, acompañado de un acentuado compromiso séptico sistémico. Presentación del caso: se presenta el caso de un paciente de 66 años de edad, masculino, negro, con antecedentes de una neoplasia de laringe sin diagnóstico histológico, que fue recibido en el cuerpo de guardia del servicio de Otorrinolaringología del Hospital Abel Santamaría Cuadrado, por presentar un cuadro de gran toma del estado general, fiebre, disnea inspiratoria, estridor; al examen físico se encontró lesión ulcero necrótica de gran extensión en la región anterior del cuello. Se estableció el diagnóstico de una fascitis necrotizante del cuello y una neoplasia de laringe causante de un síndrome disneico laríngeo. Se procedió a realizar necrectomía y traqueostomía e ingreso del paciente. Conclusiones: la fascitis necrotizante de la región anterior del cuello a punto de partida de una neoplasia laríngea es una infección poco común, rápidamente progresiva y de difícil diagnóstico en estadío temprano.


ABSTRACT Introduction: necrotizing fasciitis corresponds to a bacterial infection of the soft tissues, characterized by diffuse necrosis and rapidly progressive infection of the cellular subcutaneous tissue and muscular aponeurotic system, accompanied by an accentuated septic systemic compromise. Case report: a 66 year old, male, black patient with a history of a laryngeal neoplasia without histological diagnosis, who came to the emergency room of the otolaryngology service at Abel Santamaria Cuadrado General Teaching Hospital. He presented an inadequate chart of general condition, fever, inspiratory dyspnea and stridor; at physical examination, an ulcer necrotic lesion of great extension was found in the anterior region of neck and a laryngeal neplasia triggering a laryngeal dyspnoeic syndrome; performing necrectomy and tracheostomy as well as the admission of patient. Conclusions: necrotizing fasciitis of the anterior region of neck from the starting point of a laryngeal neoplasia is a rare infection, having a rapidly and progressive expansion of difficult diagnosis in early stages.

ABSTRACT

Fournier gangrene is a relatively rare clinical critical disease, and its clinical symptoms are not specific and easily unrecognized by some clinicians. It has the features of acute onset, quick development, severe illness, and often accompanied by infection shock which is seriously life-threatening. It is difficult in treatment with high medical costs and long length of hospitalization, which increases pain for patients and relatives and brings heavy economic and psychological burden on patients, society, and medical workers. By reviewing the literature home and abroad and combined with clinical practice, I summarize the researches on concept, epidemiology, clinical manifestation, diagnosis and treatment of Fournier gangrene, in order to provide reference for vast number of clinical workers.

ABSTRACT

Las infecciones de piel y tejidos blandos (IPTB) representan la tercera causa de consulta por enfermedad infecciosas a los servicios médicos, después de las infecciones respiratorias y urinarias. Se presenta una guía de práctica clínica (GPC) con 38 recomendaciones basadas en la evidencia, graduadas bajo el sistema SIGN, para el diagnóstico y tratamiento de pacientes adultos con IPTB en el contexto colombiano, posterior a un proceso de adaptación de GPC publicadas y la búsqueda sistemática y síntesis de literatura para la actualización de la evidencia científica. Además, se realizó un consenso de expertos para la evaluación de las potenciales barreras para la implementación de las recomendaciones y la evaluación del grado de recomendación en el contexto local.


Skin and soft tissue infections (SSTI) represent the third leading cause of infectious disease consultation for medical services after respiratory and urinary tract infections. This document generates a clinical practice guideline with 38 recommendations based on evidence, graduated under the SIGN system for the diagnosis and treatment for SSTI infections in adult patients in Colombia, following a process of adaptation of guidelines published, and the systematic search and synthesis of literature for the updating of scientific evidence. In addition, a consensus of experts was made for the evaluation of the potential barriers for the implementation of the recommendations and the evaluation of the degree of recommendation in the local context.

ABSTRACT

RESUMO Dentre as infecções causadas por Streptococcus β hemolyticus do grupo A de Lancefield, talvez a síndrome do choque tóxico seja a mais grave, com alto índice de mortalidade. A semelhança clínica com outras formas de choque, principalmente séptico, pode, muitas vezes, confundir o avaliador e interferir na escolha da terapêutica mais adequada. Esse relato tem o objetivo de auxiliar seus leitores quanto à necessidade de adicionar tal síndrome como diagnóstico diferencial, frente a quadros de choque, principalmente aqueles que não apresentam manifestações clínicas bem definidas. Para isso, apresentamos o quadro de um lactente com sintomas gripais comuns, que evoluiu rapidamente com exantema, rebaixamento do nível de consciência, sinais clínicos e laboratoriais de choque, com necessidade de suporte intensivo. Além de culturas indicando o agente etiológico, o aparecimento de exantema e fasciíte necrosante levou ao diagnóstico, mas, em menos de 50% dos casos temos sinais clínicos clássicos dessa entidade. As penicilinas em terapia combinada com aminoglicosídeos ainda são a terapia de escolha e possuem alto nível de evidência. Apesar da gravidade a evolução foi satisfatória.


ABSTRACT Among the infections caused by Streptococcus β hemolyticus from the Lancefield serogroup A, toxic shock syndrome is perhaps the most severe, and its mortality rate is high. Its clinical similarity to other forms of shock, especially septic shock, can often confuse the evaluator and interfere with the selection of the most appropriate therapy. This report aims to inform readers of the need to add this syndrome as a differential diagnosis in cases of shock, especially those with no well-defined clinical manifestations. For this purpose, we present the case of an infant with common flu-like symptoms who progressed rapidly with a rash, a reduced level of consciousness and clinical and laboratory signs of shock that required intensive support. In addition to cultures indicating the etiological agent, the appearance of exanthema and necrotizing fasciitis led to the diagnosis. However, less than 50% of cases present classic clinical signs of this entity. Penicillins combined with aminoglycosides are still the therapy of choice and are supported by a high level of evidence. Despite the severity of this patient's presentation, the progression was satisfactory.

ABSTRACT

INTRODUCCIÓN: La Infección Necrotizante de tejidos blandos (INTB) tiene una elevada morbimortalidad. El objetivo de este trabajo es describir del manejo perioperatorio de menores de 15 años que cursaron con INTB durante 15 años en un Hospital pediátrico. MATERIAL Y MÉTODOS: serie de pacientes identificados INTB entre 2000 y 2015 en el Hospital Roberto del Río. Se describen variables demográficas, clínicas, vacuna, tratamiento, cirugías, complicaciones, microorganismos, seguimiento, y fallecimientos. RESULTADOS: 22 pacientes, con mediana de 2 años y 9 meses de edad. 50% estaban cursando con una varicela. Dos fallecieron. Ninguno era previamente vacunado contra el virus varicela zoster. La localización fue tronco (14), extremidades (7), cuello (1). Los pacientes conscientes presentaron hiperestesia cutánea. Se realizó aseo quirúrgico con una mediana de 6,8 horas desde el inicio de la hiperestesia y 2 horas desde la sospecha diagnóstica. Los microorganismos fueron: S. pyogenes (38%) y E. coli (31%). Los antibióticos más frecuentes fueron penicilina más clindamicina. La herida se manejó con cierre primario, injertos y/o colgajos. Cinco evolucionaron con secuelas que necesitaron tratamiento. CONCLUSIÓN: Se identificó que la INTB puede presentarse con varicela. Debe sospecharse en lesiones cutáneas e hiperestesia desproporcionada. Resección amplia y precoz son fundamentales para el tratamiento.


INTRODUCTION: Necrotizing soft tissue infection (NSTI) has a high morbidity and mortality. The objective of this study is to describe the perioperative management of children under 15 years of age who have had this condition for 15 years in a pediatric hospital. MATERIAL AND METHODS: cases series of patients identified by Pathological Anatomy with NSTI during 2000 and 2015 at the Dr. Roberto del Río Hospital. Demographic variables, treatment, antecedents of vaccination, surgeries, complications, microorganisms, follow-up, and deaths are described. RESULTS: 22 patients were identified, with a median of 2 years 9 months of age (interquartile range: 13 months to 5 years y 10 months). 50% of the cases were associated to chickenpox. Two died. No patient was previously vaccinated against varicella zoster virus. The location was trunk (14), extremities (7) and neck (1). All conscious patients presented cutaneous hyperesthesia. Surgical debridement was performed with a median of 6.8 hours from the onset of hyperesthesia and 2 hours from diagnostic suspicion. The most frequent microorganisms were: Streptococcus pyogenes (38%) and Escherichia coli (31%). The most frequent antibiotics used were penicillin plus clindamycin. The wound was handled with primary closure, grafts and / or flaps. Five patients evolved with sequelae that needed treatment. CONCLUSION: It was identified that STNI in children is associated with chickenpox. It should be suspected in cutaneous lesions and disproportionate hyperesthesia. Aggressive resection is essential for treatment

ABSTRACT

INTRODUÇÃO: A Síndrome de Fournier consiste em uma fasciite necrosante que afeta tecido subcutâneo e pele do períneo e genitais externos. Caracterizada como uma urgência cirúrgica, seu tratamento é baseado em três pilares: debridamento de tecidos necróticos e infectados; controle sistêmico e antibioticoterapia; e reparação dos tecidos afetados. OBJETIVO: Identificar o perfil clínico de pacientes diagnosticados com Síndrome de Fournier em um hospital de urgências. MÉTODOS: Trata-se de um estudo descritivo, retrospectivo e de abordagem quantitativa. A amostra consistiu de pacientes diagnosticados com Síndrome de Fournier acompanhados pela comissão de curativos do hospital no período de agosto de 2016 a agosto de 2017, que receberam alta ou foram a óbito. RESULTADOS: A amostra do estudo foi composta por 14 pacientes, sendo em sua totalidade pacientes do sexo masculino, entre 21 e 82 anos e idade média de 55 anos. Em 50% dos casos, foi necessário internação em Unidade de Terapia Intensiva (UTI). Quanto ao desfecho, 78,6% (11) receberam alta hospitalar e 21,4% (3) evoluíram para óbito. CONCLUSÃO: A assistência a pacientes com Síndrome de Fournier ocorre de forma despadronizada, o que ocasiona altas taxas de mortalidade. A elaboração de protocolos específicos é necessária.


INTRODUCTION: Fournier's Syndrome consists of a necrotizing fasciitis that affects subcutaneous tissue and skin of the perineum and external genitalia. Characterized as a urological urgency, its treatment is based on three pillars: debridement of necrotic and infected tissues; systemic control and antibiotic therapy; and repair of the affected tissues. OBJECTIVE: To identify the clinical profile of patients diagnosed with Fournier's Syndrome in an emergency hospital. METHODS: This is a descriptive, retrospective and quantitative study. The sample consisted of patients diagnosed with Fournier's Syndrome and attended by the hospital curative committee from August 2016 to August 2017, who were discharged or died. RESULTS: The study sample consisted of 14 medical records, all of them were male, aged between 21 and 82 years and mean age of 55 years. In 50% of the cases admittance to the Intensive Care Unit (ICU) was necessary. Regarding the outcome, 78.6% (11) were discharged from hospital and 21.4% (3) died. CONCLUSION: Assistance to patients with Fournier Syndrome is poorly standardized, resulting in high mortality rates. Development of specific protocols is necessary.

Objective@#To summarize the experience of applying cervical double parallel incision combined with mediastinoscope or thoracoscope in the treatment of cervical necrotizing fasciitis (CNF) accompanied with descending necrotizing mediastinitis (DNM), so as to provide a reference for clinical practice.@*Methods@#The clinical data of six patients with CNF accompanied with DNM who were admitted to the Department of Stomatology and the Department of Otolaryngology Head and Neck Surgery, The First People′s Hospital of Changzhou from September 2014 to September 2018 were retrospectively analyzed. All of the six patients were confirmed by CT of neck and chest, among whom there were two males and four females aged from 48 to 73. Three patients were treated with cervical double parallel incision combined with mediastinoscope to be combined with cervical and thoracic drainage under general anesthesia while the other three with cervical double parallel incision combined with thoracoscope to be combined cervical and thoracic drainage under general anesthesia. The CT of neck and chest as well as infectious indicators including hematology, C-reactive protein (CRP) and procalcitonin (PCT) were reexamined during the postoperative period.@*Results@#The cervical and thoracic combined drainage was unobstructed in all of the six patients, no secondary surgery was performed, and the infectious indicators gradually decreased. All patients had off-bed activities on the first day after the operation, were all cured and discharged after an average of 21 days (16 to 36 days) in hospital and followed up for an average of 18 months (4 to 30 months) after the operation. None of them experienced infection relapse, and they were all satisfied with the appearance of the cervical incision.@*Conclusions@#Cervical double parallel incision combined with mediastinoscope or thoracoscope for the treatment of CNF accompanied with DNM has the advantages of complete drainage, small trauma, excellent efficacy and aesthetic operative area, thus being deserved to be clinically popularized.

ABSTRACT

Ten patients with perianal necrotizing fasciitis were admitted to our department from June to December 2016. There were 8 men and 2 women among the patients, aged 42 to 69 years. Early and complete debridement surgery and comprehensive supportive treatment during perioperative period were carried out to quickly stabilize the patient′s overall condition, and wounds were sutured directly or repaired with autologous scalps and or adjacent local random flaps. After debridement, wound areas ranged from 10 cm×8 cm to 54 cm×21 cm, and area of the flap was about 8 cm×5 cm. The donor site of flap was sutured directly. After the operation, all skin grafts and the flap survived, and wounds of all patients healed. During follow-up of six months to one year, there was no recurrence of perianal necrotizing fasciitis, and functions of the involved lower extremities didn′t be influenced.

Las infecciones necrosantes de partes blandas constituyen un grupo heterogéneo de afecciones, poco frecuentes pero potencialmente letales, caracterizadas por la infección de la piel, el tejido celular subcutáneo y el músculo. La fascitis necrosante es una rara enfermedad. En Cuba no existen estudios de incidencia. Su etiología es generalmente polimicrobiana. El Streptococcus pyogenes es el germen más frecuente. Las toxinas bacterianas, mediante su actividad citopática y protrombótica, son las responsables de la necrosis tisular. Su curso clínico no es uniforme, los médicos solo logran hacer un diagnóstico inicial correcto del 15 % al 30 % de los casos. Si los signos y los síntomas iniciales se detectan a tiempo su índice de mortalidad puede lograr reducirse hasta el 12 %. El objetivo de este trabajo es hacer una revisión del tema y presentar los criterios actuales de diagnóstico y tratamiento.


Necrotizing lesions in soft tissues are a heterogeneous type of illnesses. infrequent but potentially fatal, characterized by skin infection, of the subcutaneous tissue and muscle. Necrotizing fasciitis is an unsual disease. There no incidence studies in Cuba. Its etiology is generally polymicrobial. Streptococcus pyogenes is the most frequent germ. Bacterial toxins, through its cytopathic and thrombotic activity, are responsible of tissue necrosis. Its clinical development is not uniform, doctor only achieve an initial correct diagnosis in the 15 to 30 % of the cases. If initial sings and symptoms are identified its mortality may be reduced to 12%. The objective of this work is to review the topic and present the current diagnosis and treatment criteria.

ABSTRACT

Primary necrotizing fasciitis (NF) of the breast is extremely rare. It progresses rapidly and can lead to sepsis and multi-organ failure without prompt medical and surgical management. Herein we describe the case of a healthy 42-year-old woman on the 10th day after cesarean section, who was admitted to the intensive care unit (ICU) with a painful and swollen right breast. It evolved rapidly in the ICU into septic shock and nipple discharge. She underwent immediate resuscitation followed by muscle-sparing right mastectomy. She was managed postoperatively in the ICU with intravenous antibiotic therapy. Complications included acute renal failure and anuria, leading to death on the fourth day after surgery. Prompt resuscitation and an aggressive surgical approach are critical to the successful management of this life-threatening pathology. Despite this, NF still carries a high mortality rate


A fasciíte necrosante primária (FN) da mama é extremamente rara. Ela progride rapidamente e pode levar à sepse e à falência de múltiplos órgãos sem pronto atendimento médico e cirúrgico. Aqui descrevemos o caso de uma mulher saudável de 42 anos de idade no décimo dia pós cesárea, que foi admitida na unidade de terapia intensiva (UTI) com a mama direita dolorida e inchada. O quadro evoluiu rapidamente na UTI a choque séptico e descarga mamilar. Ela foi submetida a ressuscitação imediata seguida de mastectomia de mama direita com preservação muscular. Foi administrada terapia endovenosa com antibióticos na UTI durante o período pós-operatório. Complicações incluíram insuficiência renal aguda e anúria, levando à morte no quarto dia após a cirurgia. A reanimação imediata e uma abordagem cirúrgica agressiva são fundamentais para o sucesso do manejo dessa patologia que é ameaçadora. Apesar disso, a FN ainda carrega uma alta taxa de mortalidade.

La técnica Meek es útil en pacientes con sitios donantes limitados, ya que es relativamente eficiente en cuanto a la expansión de la piel; llegando a expandirse hasta 9 veces en comparación al tamaño original, se realizó la evaluación de la cicatrización entre dos técnicas de expansión de tejidos de 1:3 veces entre la técnica Meek y la técnica mallado convencional. Cabe destacar que, con los avances de la bioingeniería, la técnica Meek se integra como un método completo para el tratamiento de autoinjertos. Presentamos un caso de un paciente varón de 34 años con una enfermedad infecciosa poli bacteriana "fascitis necrotizante"; con pérdida de tejidos que comprometen el tórax, abdomen y región inguino perineal, con un 14% de superficie corporal total comprometida. En la valoración cualitativa con la escala de Vancouver y POSAS se observaron mejores resultados de cicatrización con la técnica Meek.


The Meek technique is useful in patients with limited donor sites. It is a relatively efficient technique due to its ability to expand the skin up to nine times its original size. Scarring was assessed using two tissue expansion techniques, i.e. the Meek technique and the conventional mesh technique, which achieved an expansion ratio of 1:3. It should be noted that, with advances in bioengineering, the Meek technique is integrated as a complete method for autografting.1,2 We present the case of a 34-year-old male patient with a polymicrobial infectious disease called "necrotizing fasciitis". He had lost tissues of the thorax, abdomen and inguinal-perineal region, affecting 14% of his total body surface area. A qualitative assessment using the Vancouver scale and the Patient Objective Scar Assessment Scale (POSAS) showed better scarring results with the Meek technique

ABSTRACT Necrotizing fasciitis is a severe infection of the subcutaneous tissue characterized by necrosis of the superficial fascia and overlying skin and is usually associated with previous trauma and comorbidities. Periorbital necrotizing fasciitis is rare and commonly causes visual loss and soft tissue defects. A better prognosis relies critically on early diagnosis, prompt medical treatment, and timely surgical intervention. We describe a rare case of periorbital necrotizing fasciitis in the absence of an inciting event. A 55-year-old female patient presented with acute painful swelling and redness of the right upper eyelid that spread to both eyelids bilaterally within 24 h. We swiftly started the patient on intravenous antibiotic therapy, and we surgically debrided the necrotic tissue the following day. We performed two further procedures to improve eyelid closure and appearance. Despite the severe presentation, timely antibiotic therapy and proper surgical interventions led to a successful outcome in this case.


RESUMO Fasciite necrosante é uma infecção grave do tecido subcutâneo, caracterizada pela necrose da fáscia superficial e da pele sobrejacente. Traumas prévios e cormobidades geralmente estão associados à fasciite necrosante. Fasciite necrosante periorbital é rara. Perda visual e defeitos em tecidos moles são as morbidades mais comuns. Diagnóstico precoce, tratamento clínico rápido e intervenção cirúrgica oportuna levam a um melhor prognóstico. Reportamos um caso incomum de fasciite necrosante periorbital bilateral sem eventos desencadeantes. Uma paciente de 50 anos apresentou edema e eritema na pálpebra superior direita, que progrediu em 24 horas para ambas pálpebras bilateralmente. Ela era previamente hígida. A paciente foi submetida a debridamento cirúrgico do tecido necrótico, no mesmo dia. A paciente foi submetida a outras duas cirurgias, o que melhorou o fechamento palpebral e a aparência. Apesar da gravidade da doença, antibioticoterapia e cirurgias oportunas foram cruciais para o desfecho bem sucedido deste caso.
