ABSTRACT

O autor discute a importância de se resgatar a morte como algo natural, tirando-a do interdito em que se encontra. Tenta mostrar o quanto este aspecto cria distorções para a vida, determinando uma série de distúrbios na conduta médica, em especial na abordagem do paciente terminal. Defende a ideia de que é uma doença da nossa cultura ocidental e a situação que leva a esta repressão é negação da morte. Dentro das referências teóricas da Psicologia Analítica de Jung, o autor propõe condutas para se tentar resgatar a vivência simbólica da morte restituindo-se a polaridade dialética Vida-Morte. Defende a proposição de que a vida só pode ter um sentido pleno se não negarmos a morte. Propõe que, como no parto, o progresso da Medicina se harmonize com respeito aos limites da Vida e da personalidade do paciente, frequentemente desrespeitados. Discute a morte como símbolo fundamental dentro do processo de individuação.


The author discusses the importance of readdressing death as something natural, retrieving it from the interdict in which it finds itself. He attempts to show the extent to which this aspect creates distortions to life, determining a series of disorders in medical conduct, especially in the approach to terminally ill patients. He defends the idea that it is a disease of our Western culture and that the situation that leads to this repression is the negation of death. In line with the theoretical references of Jung's Analytical Psychology, the author proposes conducts to try to rescue the symbolic experience of death and restore the life-death dialectical polarity. He supports the proposition that life can only have a full meaning if we do not negate death. He proposes that, as in childbirth, the progress of medicine should harmonize with respect to the limits of life and the patient's personality, which are often disrespected. He discusses death as a fundamental symbol within the individuation process.


El autor discute la importancia de rescatar la muerte como algo natural, sacándola del entredicho en que se encuentra. Intenta mostrar cuánto este aspecto crea distorsiones para la vida, determinando una serie de perturbaciones en la conducta médica, especialmente en el abordaje del paciente terminal. Defiende la idea de que es una enfermedad de nuestra cultura occidental y la situación que lleva a esta represión es la negación de la muerte. Dentro de los referentes teóricos de la Psicología Analítica de Jung, el autor propone conductas para tratar de rescatar la experiencia simbólica de la muerte, restituyendo la polaridad dialéctica Vida-Muerte. Defiende la proposición de que la vida sólo puede tener pleno sentido si no negamos la muerte. Propone que, como en el parto, el progreso de la Medicina debe armonizar respecto a los límites de la Vida y de la personalidad del paciente, que muchas veces son irrespetados. Se habla de la muerte como símbolo fundamental dentro del proceso de individuación.

ABSTRACT

Abstract Renal transplant remains the preferred therapy for end-stage renal disease (ESRD). Given the shortage of suitable donor kidneys, use of an expanded criteria donor (ECD) allows marginal kidneys to be transplanted; albeit at risk of increased graft failure due to lower nephron mass. To reduce the risk of graft failure, double kidney transplant (DKT) is advocated, with favorable outcomes. Transplant renal artery stenosis (TRAS) is one of the most common vascular complications following renal transplant. Unlike single kidney transplants, where TRAS usually presents with fluid overload, uncontrolled hypertension, and worsening kidney functions; it may be clinically silent in DKT patients since they have two functional transplanted kidneys. We hereby report a case of TRAS in a DKT patient who had 2 years of favorable clinical outcomes following successful endovascular stenting. He however recently died of COVID-19 associated pneumonitis.


Resumo O transplante renal continua sendo a terapia preferida para doenças renais em fase terminal. Dada a escassez de rins de doadores adequados, o doador com critérios expandidos permite que rins marginais sejam transplantados, embora haja um maior risco de falha do enxerto devido à diminuição da massa nefrótica. Para diminuir o risco de falha do enxerto, recomenda-se o transplante renal duplo (TRD), com resultados favoráveis. A estenose de artéria renal transplantada (EART) é uma das complicações vasculares mais comuns após o transplante renal. Ao contrário dos transplantes de rim simples, nos quais a EART geralmente se manifesta como sobrecarga de fluido, hipertensão descontrolada e piora das funções renais, ela pode ser clinicamente silenciosa em pacientes com TRD, pois eles têm dois rins funcionais transplantados. Relatamos aqui um caso de EART em um paciente com TRD que teve resultados clínicos favoráveis por dois anos após o sucesso do implante de stent endovascular. No entanto, ele morreu recentemente de pneumonite associada à covid-19.

ABSTRACT

This study aimed to determine the frequency and distribution of infectious diseases diagnosed through necropsy examination and histopathological analysis in growing/finishing pigs along 12 years (2005-2016) in Southern Brazil. We evaluated 1906 anatomopathological exams of pigs at growing/finishing phases, of which the infectious diseases corresponded to 75.6% of the cases (1,441/1,906). Porcine circovirus type 2 (PCV2) infections were the most frequent, accounting for 51.3% of the cases (739/1,441) with a higher frequency from 2005 to 2007, characterizing an epidemic distribution, with a gradual decline after 2008. Infectious diseases affecting the respiratory system were the second major cause with 30.1% of the cases. Among these, necrotizing bronchiolitis caused by swine Influenza (15.1%, 218/1,441) and bacterial pneumonia (15%, 216/1,441) were the main conditions. Influenza was mostly diagnosed from 2010 to 2013, accounting for 43.1% (167/387) of the cases. After this period, both respiratory infectious diseases were endemic. Digestive system infectious diseases accounted for 10.5% of the diagnoses (151/1,441), with the following main conditions: Salmonella spp. enterocolitis (43.7%, 66/151), Lawsonia spp. proliferative enteropathy (41.7%, 63/151), and Brachyspira spp. colitis (14.6%, 22/151). The latter had a higher incidence from 2012 to 2014 with all cases detected in this period. Polyserositis and bacterial meningitis represented, respectively, 5.8% (84/1,441) and 2.3% (33/1,441) of the cases diagnosed, with a constant endemic character.(AU)


O objetivo deste estudo consistiu em determinar a frequência e a distribuição das doenças infecciosas diagnosticadas através de exame de necropsia e análise histopatológica em suínos nas fases de crescimento/terminação ao longo de 12 anos (2005-2016) no sul do Brasil. Foram avaliados 1906 laudos anatomopatológicos de suínos nas fases de crescimento/terminação, dos quais as doenças infecciosas corresponderam a 75,6% (1441/1906) do total. As infecções por circovírus suíno tipo 2 (PCV2) foram as mais frequentes, contabilizando 51,3% (739/1441) dos casos, com uma alta frequência de 2005 a 2007 caracterizando uma distribuição epidêmica neste período, e um declínio gradual após o ano de 2008. A segunda principal causa incluiu as doenças infecciosas que afetam o sistema respiratório (30,1% dos casos). Dentre essas, destacaram-se a influenza suína (15,1%; 218/1441) e pneumonias bacterianas (15%; 216/1441). O diagnóstico de influenza apresentou uma frequência elevada de 2010 a 2013, totalizando 43,1% (167/387) dos casos. Após este período, ambas doenças infecciosas respiratórias exibiram caráter endêmico. As doenças infecciosas do sistema digestório totalizaram 10,5% (151/1441) dos diagnósticos, com as seguintes principais condições: enterocolite por Salmonella spp. (43,7%; 66/151), enteropatia proliferativa por Lawsonia spp. (41,7%; 63/151) e colite por Brachyspira spp. (14,6%; 22/151). A colite por Brachyspira spp. apresentou uma alta incidência de 2012 a 2014 com todos os casos detectados no período. As polisserosites e meningites bacterianas representaram 5,8% (84/1441) e 2,3% (33/1441) dos casos diagnosticados, respectivamente, com um caráter endêmico constante.(AU)

ABSTRACT

OBJECTIVE: The aim of this study is to estimate the trends in prevalence of end stage renal disease (ESRD) during the period 1999-2007 at one site in Trinidad, the Eric Williams Medical Sciences Complex (EWMSC), and to describe the epidemiological features, age, gender, ethnicity and co-morbidities associated with ESRD. DESIGN AND METHODS: A retrospective cohort study design was used. There was a count of patients on haemodialysis at the EWMSC centre from 1999 - 2007 in order to demonstrate trends in prevalence but more detailed data were collected and analysed for patients with ESRD attending the nephrology clinic between January 2002 and December 2007. The data that were collected from the patients' records included: demographic data (age, gender and ethnicity), medical history (diabetes mellitus, hypertension, end stage renal disease and autoimmune disorders), history of dialysis (type of vascular access, frequency of dialysis), mortality and its cause. RESULTS: Records of 81 patients were retrieved. Their age range was 10-79 years. The survey showed that patients most affected in the study population were: males, aged 50-59 years, who were hypertensive and/or diabetic and of African descent. CONCLUSIONS: In conclusion, we provide epidemiological evidence of ESRD and the associated contributing factors at one hospital in Trinidad.


OBJETIVO: El objetivo de este estudio es estimar las tendencias de la prevalencia de la enfermedad renal en fase terminal (ERFT) durante el periodo 1999-2007 en el Complejo de Ciencias Médicas Eric Williams, en Trinidad, y describir los rasgos epidemiológicos - edad, género, etnicidad - y co-morbilidades asociadas con la ERFT. DISEÑO Y MÉTODOS: Se usó un diseño de estudio de cohorte retrospectivo. Se llevó a cabo un análisis de todos los pacientes que asistieron a las clínicas de nefrología de EWMSC, de enero de 2002 a diciembre de 2007. Los datos recogidos de las historias clínicas de los pacientes al final del período de estudio incluyeron: datos demográficos (edad, género y etnicidad), historia médica (diabetes mellitus, hipertensión, enfermedad renal en fase terminal, trastornos autoinmunes), historia de diálisis (tipo de acceso vascular, frecuencia de diálisis), mortalidad y causa. RESULTADOS: Se obtuvieron las historias clínicas de 81 pacientes, cuya edad fluctuaba de 10 a 79 años. La encuesta mostró que los pacientes más afectados en la población del estudio fueron varones, de 50-59 años de edad, que eran hipertensos y/o diabéticos, de ascendencia africana. CONCLUSIONES: En conclusión, se ofrece evidencia epidemiológica de la ERFT en un hospital en Trinidad y se señalan los factores asociados que contribuyen a la enfermedad.

ABSTRACT

Se efectuó una intervención educativa dirigida a los cuidadores familiares de 20 pacientes con enfermedades en fase terminal, pertenecientes al área de salud del Policlínico Universitario José Martí Pérez de Santiago de Cuba, desde julio hasta diciembre del 2009, con vista a modificar sus conocimientos sobre los cuidados paliativos que debían brindar con calidad a las personas afectadas. Luego de aplicada la estrategia se logró cambiar positivamente el nivel de instrucción de los cuidadores acerca del tema, así como su modo de obrar en relación con los pacientes y sus familiares.


An educational intervention directed to the family care givers of 20 terminally ill patients, belonging to the health area of José Martí Pérez University Polyclinic in Santiago de Cuba was carried out from July to December, 2009, with the objective of modifying their knowledge on the palliative cares that they should give with quality to affected people. After the strategy was applied it was possible to change positively the instruction level of the care givers on the topic, as well as their way of working in relation to the patients and their relatives.

Se presentan los conceptos generales sobre la fase terminal de las enfermedades en los niños, el abordaje terapéutico que se realiza en una institución de tercer nivel de atención, la forma en que se proporcionan las malas noticias a los padres y niños, los aspectos bioéticos y el apoyo médico, emocional y espiritual que se requiere en esta compleja situación.


This paper presents the general concepts regarding the terminal phase of a child's illness and the therapeutic approach undertaken at a third-level care institution. We discuss how this information is transmitted to parents and children along with the bioethical aspects and the medical, emotional and spiritual support required during this complex situation.
