ABSTRACT

Objective@#To analyze the effect of ABC classification method on improving the drug management system of Urumqi Friendship Hospital.@*Methods@#The drug data of Urumqi Friendship Hospital from January 2017 to December 2017 were retrospectively analyzed.The in-hospital information system (HIS) was used to sort the annual consumption, and the antibacterial drugs were sorted by DDDS to analyze the data as control group.From January 2018 to December 2018, the drug data of Urumqi Friendship Hospital were retrospectively analyzed by the AIS analysis method in the hospital HIS system, which were selected as research group.The effect of ABC analysis on the management of hospital drugs was analyzed, and to compare the changes in drug management before and after the application of ABC analysis.@*Results@#Through the analysis of A, B and C drugs, the total amount of 921 drugs were found to be 127.358 million CNY, including 122 kinds of A drugs (13.25%), the amount of 97.975 million CNY (79.93%); 133 kinds of B drugs (14.44%), the amount of 18.56 million CNY (15.17%); 666 kinds of C drugs (72.31%), the amount of 6.0016 million CNY (4.90%). Among the ABC classification results, there were 24 kinds of cardiovascular system drugs in class A drugs, and the highest percentage was 28.57%.There were 29 kinds of Chinese herbal medicines in class B drugs, with a percentage of 23.33%.There were 52 kinds of oral drugs (42.62%), 64 kinds of injections (52.46%), and 6 kinds of other drugs (4.92%), while 66 drugs (49.62%) were classified as B drugs.Species (44.36%) and 8 other drugs (6.02%). Among the ABC classification results, the total amount of the top 20 drugs in class A drugs was 46 073 529.65 CNY, accounting for 47.11% of the A drugs, accounting for 37.65% of the total amount.Among them, the amount of clopidogrel tablets and atorvastatin calcium tablets was significantly larger.The inventory turnover index [(8.37±2.48)d] of the inventory management index of the research group after the ABC classification method was significantly higher than that of the control group [(6.22±3.61)d] (t=14.897, P<0.05), and the monthly average inventory amount [(4.5 354±0.5 024)million CNY] was significantly higher than that in the control group [(3.9 963±0.7 538)million CNY] (t=18.060, P<0.05), the slow-moving rate [(4.24±1.31)%] and the stock-out rate [(8.73±3.14)%] were significantly lower than those in the control group [(5.11±0.82)%, (9.39±2.67)%] (t=17.084, 4.859, all P<0.05).@*Conclusion@#The ABC classification method helps to improve the hospital drug management system, can promote rational drug use, dynamically manage drugs, ultimately reduce drug costs, and rationally utilize limited drug resources.

The inventory turnover index [(8.37 ±2.48)d] of the inventory management index of the research group after the ABC classification method was significantly higher than that of the control group [(6.22 ±3.61) d] ( t=14.897, P<0.05),and the monthly average inventory amount [(4.5 354 ±0.5 024)million CNY] was significantly higher than that in the control group [(3.9 963 ±0.7 538) million CNY] (t=18.060,P<0.05),the slow-moving rate [(4.24 ±1.31)%] and the stock-out rate [(8.73 ±3.14)%] were significantly lower than those in the control group [(5.11 ±0.82)%,(9.39 ±2.67)%] (t=17.084,4.859,all P<0.05).Conclusion The ABC classifica-tion method helps to improve the hospital drug management system ,can promote rational drug use ,dynamically manage drugs,ultimately reduce drug costs ,and rationally utilize limited drug resources.

ABSTRACT

Objective To understand the patterns of outpatient prescription medications in the hospital's elderly patients.Methods Data for all outpatient prescriptions at our hospital written for seniors (≥60 years old) between 2013 and 2014 were collected and medications were ranked based on the price.The top 100 medications were chosen for analysis.Results The most expensive and most frequently prescribed three therapeutic groups were drugs for cardiovascular,endocrine & metabolic,and bone & joint disorders.Medication numbers and prices varied among different age groups,with the 60-64 and 75-79 groups representing the highest numbers and prices.The average price (460.3 yuan) per prescription for male patients was higher than that (399.3 yuan) for female patients.Conclusions The cost of prescription drugs in elderly patients is higher than in middle-aged patients,with drugs for non communicable chronic diseases accounting for most of the expenses.Polypharmacy is also a prominent issue.Prescriptions for chronic illnesses in the elderly should follow well established guidelines and appropriately managed.

ABSTRACT

Objective To investigate the symptoms of diarrhea and the behavior of medical treatment after diarrhea in 2013 in Tianjin City, and provid background information for the future population monitoring. Methods A stratified sam?pling method was used to investigate the symptoms, medical treatment and self medication behavior of 4028 residents in Tianjin. Results The number of people who had diarrhea in 2013 was 324, and the incidence rate of diarrhea was 8.04%. Diarrhea was mainly occurred in May-October (n=241, 74.38%). The incidence of diarrhea was the highest in 60-74 years old group, followed by 75-years old and 0-14 years old groups. The incidence of diarrhea was the lowest in 15-29 years old grouop. A total of 121 patients with diarrhea chose to go to hospital (47.08%), 78 patients chose to buy medicine for treat?ment (30.35%) and 58 patients did not do any disposal (22.57%). Among the patients with diarrhea, the highest proportion of patients chose to go to the first grade and the following medical institutions (68.59%, 83/121). There were 14.05%(17/121) and 17.36%(21/121) patients chose to go to the second and the third levels of medical institutions for the treatment. The av?erage cost for patients with diarrhea was 60 (20, 200) yuan. The purchasing expenditure median was 20 (11,50) yuan. The medical spending was more than the purchase of medicines (Z=2.412, P<0.05). The average cost of medical treatment was more higher in the second and the third levels of medical institutions than that of the first medical institution ( Z=50.709,P<0.05). The average cost of medical treatment was in turn increased for patients with diarrhea treated in Baodi county, Xiqing county, Hangu county and Heping county (Z=74.282,P<0.05). There was no statistical significance in medical expenses be?tween patients with different ages and patient with or without medical insurance. Conclusion There are high incidence of diarrhea in patients under 15 years old and patients over 60 years old. Residents suffering from diarrhea mainly choose their own medicine and the treatment in the first and following medical institutions.

ABSTRACT

Objective To understand the diabetes drug application of 2012-2014 in Wujin People's Hospi-tal of Changzhou,and to provide reference for clinical medicine.Methods The information system in Wujin People's Hospital of Changzhou was used,anti diabetic drug use data were retrieved from 2012 to 2014 in our hospital.The sales volume of drugs,defined daily dose( DDD) ,drug DDDs,defined daily cost of DDDc and sorting ratio were ana-lyzed as index.Results Consumptions of antidiabetic drugs as insulin and its analogs in our hospital were between 400-500 million yuan,occupied about 70%of the proportion,and played a leading role.Alpha glucosidase inhibitors and metformin were the main component in the oral hypoglycemic drugs,accounted for 35% and 10% respectively;biguanides of DDDs value ordering three years in a row were in the first place,the DDDs of alpha glucosidase inhibi-tors value ordering three consecutive annual were averagely in the third and fourth.Conclusion The clinical medica-tion of anti diabetic drugs in Wujin People's Hospital of Changzhou is reasonable,safe and economic.

ABSTRACT

Objective To analyze the cost-effectiveness of the treatment of type 2 diabetes by 3 different drugs.Methods Through the retrospective analysis of 198 cases with T2DM and drug economics,to analyze the costeffectiveness of the Yamo Li(glimepiride),repaglinide(repaglinide) and Avandia(rosiglitazone).Results No significant difference was observed in the total effective rate among the 3 drugs.The cost-effectiveness ratios in Group A,B and C were 2.99,1.43 and 5.76.The cost-effectiveness ratios increments in Group A,B and C were 42.15,18.32 and 214.74.Conclusion Based on individual differences of the patients,doctor should choose the drug with best treatment,lowest price and highest safety.
