@#Introduction: Untreated iron deficiency (ID) can lead to severe anaemia, requiring blood transfusion, or increased mortality risk. Globally intravenous (IV) iron is increasingly recognised as a recommended option for patients. This study aims to evaluate the budget impact associated with introducing a new intravenous (IV) iron, ferric derisomaltose (Monofer® [IIM]) as one of the treatment options for the management of ID in the Ministry of Health Malaysia (MOHM) setting. Methods: A 5-year budget impact model was developed from 2020 to 2024 for patients with ID that require a high iron dose (≥500 mg), using the perspective of MOHM. The model was built with four external medical specialists, each with experience and deep knowledge of ID management, to support estimations on the future development of iron use in Malaysia. Results: Compared to the current market mix with the existing IV iron products (i.e., iron sucrose and iron dextran), a cost-saving of MYR 53,910 could be achieved with the introduction of IIM in 2020. The uptake of IIM into MOHM over five years is estimated to lead to an overall budget saving of MYR 11,837,524 over a 5-year time horizon. Conclusion: The use of IIM in place of the current IV iron products in MOHM resulted in a significant cost saving by reducing the number of visits required to achieve the targeted iron dose and the shorter IV infusion time with IIM.

ABSTRACT

Objective:To determine the content of ferric oxide in calamine powder. Methods:Flame atomic absorption spectrom-etry ( FAAS) was used to determine the content of ferric oxide. The determination results were compared with those of the titration method. Results:There was a good linear relationship between the absorbance and the concentration of iron within the range of 0. 5-4μg·ml-1 , the relative standard deviation of the repeatability test was 1. 2%, the average recoveries were 100. 4% and 99. 4%, the limit of quantitation and detection was 0. 081 μg·ml-1 and 0. 024 μg·ml-1 , respectively. Conclusion:The method of FAAS is ac-curate and quick with good specificity and high sensitivity, which can be used for the determination of ferric oxide in calamine powder.

ABSTRACT

This study aims at developing an analytical procedure capable of quantifying the ferric oxide present in the mixture of ferric oxide/lactose monohydrate (0.4% w/w). The analytical procedure was checked for specificity, linearity, precision (system repeatability, procedure repeatability and intermediate precision), accuracy, stability of solutions and robustness of the procedure. The concentration of Fe (III) was determined by spectrophotometry at 480 nm based on calibration curves. The specificity was verified. The linearity was obtained in the range of 11.2 to 16.8 µg of ferric oxide/mL. The relative standard deviation (RSD) of the system repeatability, procedure repeatability and intermediate precision, were not more than 2%. The RSD of the accuracy values were less than 0.75%. The stability of the samples was checked over a 24 hours assay. In the robustness evaluation, the wavelength and the concentration of hydrochloric acid varied. The maximum absorbance deviation due to wavelength variation was 0.14 percent, and the maximum deviation due to the hydrochloric acid concentration variation was 2.4%, indicating that the concentration of hydrochloric acid is critical to the analysis of ferric oxide. The procedure developed was validated and is suitable to the performance qualification of powder mixers..


O objetivo deste trabalho foi desenvolver um método analítico capaz de quantificar o óxido férrico presente na mistura óxido férrico/lactose monoidratada (0,4% w/w). Verificou-se a especificidade, linearidade, precisão (repetibilidade do sistema, repetibilidade do método e precisão intermediária), exatidão, estabilidade das soluções e robustez. A concentração de Fe(III) foi determinada por espectrofotometria em 480 nm com base em curvas de calibração. Verificou-se a especificidade verificada. A linearidade foi obtida na faixa de 11,2 a 16,8 µg de óxido férrico/mL. O desvio padrão relativo (DPR) da repetibilidade do sistema, método e precisão intermediária foram inferiores a 2%. Os valores de DPR da exatidão foram inferiores a 0,75%. A estabilidade das amostras foi verificada ao longo de 24 horas de ensaio. Na avaliação da robustez variou-se o comprimento de onda e a concentração de ácido clorídrico. O desvio máximo de absorbância ao se variar o comprimento de onda foi de 0,14%, enquanto que para a concentração de ácido clorídrico o desvio foi de 2,4% para a concentração de 0,8 M. Assim, a concentração de ácido clorídrico é crítica para a análise de óxido férrico. O método desenvolvido foi validado e é adequado à qualificação do desempenho de misturadores de pós e granulados...

ABSTRACT

Erythropoietin combined with parenteral iron sucrose therapy is an alternative to blood transfusion in anemic patients. It was shown to be effective in surgical patients in several previous studies when used in conjunction with other methods. However, there are no guidelines about safety limits in dosage amounts or intervals. In this study, we report a case of significant postoperative hemorrhage managed with high dose parenteral iron sucrose, low dose erythropoietin, vitamin B12, vitamin C, and folic acid. An 80-year-old female patient presented for severe anemia after a total hip arthroplasty and refused an allogenic blood transfusion as treatment. The preoperative hemoglobin of 12.2 g/dL decreased to 5.3 g/dL postoperatively. She received the aforementioned combination of iron sucrose, erythropoietin, and vitamins. A total of 1,500 mg of intravenous iron sucrose was given postoperatively for 6 consecutive days. Erythropoietin was also administered at 2,000 IU every other day for a total of 12,000 IU. The patient was discharged in good condition on the twelfth postoperative day with a hemoglobin of 8.5 g/dL. Her hemoglobin was at 11.2 g/dL on the twentieth postoperative day.

ABSTRACT

Objective To investigate the toxic effect of nona-ferroso-ferric oxide(Nano-Fe_3O_4),nano-silicon dioxide(Nano- SiO_2)and single walled carbon nanotubes(SWCNTs)on the lung and mechanism in rats.Methods Fourty-nine Wistar rats were randomly divided into 7 groups,the control group,low and high dose groups of three nanomaterials.The rats were exposed by intratracheal instillation once two days for 5 weeks,and then killed by abdominal aorta bloodletting.The pathology of lung,lactate dehydrogenase(LDH),total antioxidant capacity(T-AOC),superoxide dismutase(SOD),malondialdehyde(MDA),interleukin-1(IL- l),interleukin-6(IL-6)and tumor necrosis factor-?(TNF-?)in bronchoalveolar lavage fluid(BALF)were determined.Results The fibrous tubercle and the matrix inflammation of lung tissue was found in the experimental groups.The activities of T-AOC and SOD decreased while IL-6 concentration increased significantly in the experimental groups(P

ABSTRACT

Objective To evaluate the applied value superparamagnetic Ferric Oxide oral contrast agent in the dignosis of MR forstomach.Methods Ferric Oxide Suspension was administered before MR scanning of stomach in 36 cases,ranged in age form 21 to 60 years old.MR findings were analysed.Results The fullness of stomach was 100% after administration of Ferric Oxide Suspension.Stomach canal appeared as negative contrast in all MR sequences,the relation between lesions in stomach and around tissue were clearly showed.During MRCP,it could restrict the high signal in stomach canal.Conclusion Oral Ferric Oxide Suspension as contrast agent during MR examination of stomach is easily accepted by patients,it is a safe and steady negative contrast agent for gastrointestinal MR examination.
