ABSTRACT

Introdução: os selantes resinosos possuem atividade preventiva e terapêutica da cárie dentária oclusal através da criação de uma barreira física que impede a adesão de biofilme. Não existem relatos destes materiais com propriedades antimicrobianas e, neste ensejo, a adição de princípios ativos obtidos de fitoterápicos configura como uma opção possível para resolver esta limitação. Objetivos: avaliar o potencial antimicrobiano do selante Fluroshield® enriquecido com o extrato do caule de Schinopsis brasiliensis (Braúna), bem como, a liberação do princípio ativo, grau de conversão, resistência de união imediata ao esmalte e padrão de falha dos materiais experimentais. Metodologia: trata-se de um estudo experimental in vitro. As cascas da Braúna foram secas a 40 ± 1 °C e o extrato etanólico foi obtido por percolação do pó da casca da Schinopsis brasiliensis. O selante comercial Fluroshield® (Dentsply) foi enriquecido com as proporções em massa (g) [0% (Controle), 20%, 10%, 5%, 2,5% e 1,25%]. Foi realizada a avaliação da Atividade antimicrobiana, a liberação do princípio ativo foi aferida através de Ultra Cromatografia Líquida de Alta Performance (UHPLC), o grau de conversão (GC) através do Micro-Raman, a resistência de união (RU) imediata ao esmalte foi avaliada através do microcisalhamento e padrão de fratura através de análise em estereomicroscópio. Os valores finais foram submetidos aos testes de normalidade Shapiro-Wilky (p > 0,05) e Levene. Os dados paramétricos foram submetidos a análise de variância (ANOVA one-way) com pós-teste de Tukey (p <0,05) e os não paramétricos através do teste de Kruskal-Wallis com pós-teste de Dunn's através do software GraphPad Prism 8 e Microsoft Excel 2018. Resultados: materiais com maior % de extrato apresentaram menor GC, não houve diferença estatisticamente significativa para RU entre os diferentes grupos, o padrão de fratura predominante foi adesiva, houve liberação do princípio ativo em meio aquoso em todos os materiais e os grupos com 20% e 10% de extrato em massa foram capazes de inibir a formação de biofilme de maneira semelhante ao controle positivo (clorexidina 0,12%). Conclusão: a adição do extrato de Schinopsis brasiliensis em concentrações entre 10% e 20% apresenta-se como uma alternativa promissora para a obtenção de propriedades antimicrobianas em selantes resinosos (AU).


Introduction: dental sealants have a preventive and therapeutic activity in the prevention and treatment of dental caries. However, there are no reports of these materials with antimicrobial properties and the addition of phytotherapy molecules would be an option to solve this limitation. Objectives: to evaluate the antimicrobial properties of the Fluroshield™ sealant with the addition of the stem extract of Schinopsis brasiliensis (Braúna), as well as the potential for releasing the active ingredient, degree of conversion and immediate enamel bond strength. Methodology: this is an experimental in vitro study. Braúna barks were dried at 40 ± 1 °C and the ethanolic extract was obtained by percolation of Schinopsis brasiliensis bark powder. The commercial sealant Fluroshield™ was enriched with the proportions by mass (g) [0% (Control), 20%, 10%, 5%, 2.5% and 1.25%]. The evaluation of the Inhibition of Biofilm Formation was carried out, the release of the active principle was measured with Ultra High Performance Liquid Chromatography (UHPLC), the immediate bond strength (BS) to the dental tissue was evaluated with microshear and the degree of conversion (DC) with Micro-Raman. Final values were submitted to Shapiro-Wilky (p > 0.05) and Levene normality tests. Parametric data were submitted to analysis of variance (one-way ANOVA) with Tukey's post-test (p <0.05) and nonparametric data through the Kruskal-Wallis test with Dunn's post-test using the GraphPad Prism software 8 and Microsoft Excel 2018. Results: materials with a higher % of extract had a lower DC, there was no statistically significant difference for BS between the different groups, there was release of the active principle in all materials and groups with 20% and 10 % of extract by mass were able to inhibit biofilm formation similarly to the positive control (0.12% chlorhexidine). Conclusion: the addition of Schinopsis brasiliensis extract in concentrations between 10% and 20% is a promising alternative for obtaining antimicrobial properties in resin sealants (AU).

Aim: Resin modified glass ionomer (RMGI) is class of material that can be used as sealant for preventing and arresting the progression of caries in pits and fissures. As these are hybrid materials, their properties can be affected by factors related to the polymerization process. Therefore, this study aimed to evaluate the influence of different generations of LED curing units (Elipar DeepCure-L and VALO Grand) on Knoop microhardness values (KHN) of RMGI sealants (Clinpro XT and Vitremer). Methods: Forty cylindrical specimens (6mm ø x 1 mm high) were prepared according to the manufacturer's instructions and divided into four groups (n=10) according to the type of RMGI and LED used. The KHN of the top surface of each sample was calculated 7 days after light-curing. Data were submitted to two-way ANOVA (α = 0.05). Results: Vitremer had higher KHN values than Clinpro XT after using both LEDs (p<0.0001), but especially when light-cured with the use of VALO Grand (p<0.0001). Whereas the KHN value of Clinpro was not influenced by the LED device (p>0.05). Conclusions: Top surface microhardness values of RGMI sealants were affected by both material composition and generations of LED curing units used. Third generation LED curing units seemed to be more efficient for the polymerization of RMGI-based sealants

ABSTRACT

Abstract In the routine clinical situation, the contamination by blood and/or saliva in restorative procedures can be happen in non-cooperation of the patient in dental office. The aim of the study was to assess in vitro shear bond strength of a resin sealant associated with two types of adhesives contaminated with saliva and blood. Healthy human molars were used and the specimens and the crowns were sectioned in the bucco-lingual direction, thus obtaining two segments of similar proportions (mesial and distal), totaling 60 surfaces, and the surfaces were randomly divided into 4 groups (n = 15). Group I (control) received no type of contamination and the sealant was applied. In group II, the surfaces were contaminated with 10 µl of saliva/blood and the sealant was applied. In group III, the surfaces were contaminated with 10 µl of saliva/blood and the Single Bond total-etch adhesive system was applied followed by application of sealant. In group IV, the surfaces were contaminated with 10 µl of saliva/blood and the Prime & Bond NT total-etch adhesive system was applied followed by the application of sealant. Samples were tested in the universal testing machine and the analysis of shear bond strength was performed. A difference between Group I (12.61MPa) and the other groups was found; Group II (2. 28MPa) was different than Groups III (7.07MPa) and IV (7.79MPa), but Groups III and IV were similar. The application of an adhesive system when there is contamination with saliva/blood is required prior to application of pit and fissure sealants. (AU)


Resumo Na situação clínica de rotina, a contaminação por sangue e/ou saliva em procedimentos restauradores pode ocorrer em pacientes que não colaboram no consultório odontológico. O objetivo do estudo foi avaliar a resistência ao cisalhamento in vitro de um selante de resina associado a dois tipos de adesivos contaminados com saliva e sangue. Foram utilizados molares humanos saudáveis e os espécimes e as coroas foram seccionados na direção bucal-lingual, obtendo assim dois segmentos de proporções semelhantes (mesial e distal), totalizando 60 superfícies, e as superfícies foram divididas aleatoriamente em 4 grupos (n = 15). O Grupo I (controle) não recebeu nenhum tipo de contaminação e o selante foi aplicado. No grupo II, as superfícies foram contaminadas com 10 µl de saliva / sangue e o selante foi aplicado. No grupo III, as superfícies foram contaminadas com 10 µl de saliva / sangue e o sistema adesivo Single-Bond foi aplicado seguindo a aplicação de selante. No grupo IV, as superfícies foram contaminadas com 10 µl de saliva / sangue e o sistema adesivo de ataque total Prime & Bond NT foi aplicado seguido da aplicação de vedante. As amostras foram testadas na máquina de ensaio universal e a análise da resistência à ligação ao cisalhamento foi realizada. Uma diferença entre o Grupo I (12,61MPa) e os outros grupos foi encontrada; O Grupo II (2,28 MPa) foi diferente dos Grupos III (7,07MPa) e IV (7,79 MPa), mas os Grupos III e IV foram semelhantes. A aplicação de um sistema adesivo quando existe contaminação com saliva / sangue é necessária antes da aplicação de selantes de fissura e fissura.

Introdução: O estudo foi realizado para comparar a eficácia da retenção hidrofóbica e hidrofílica de selante de fóssulas e fissuras entre crianças de 7 a 10 anos de idade escolar. Material e métodos: O presente estudo randomizado de boca dividida foi realizado nas superfícies oclusais dos primeiros molares inferiores permanentes para comparação e avaliação da retenção junto com outras variáveis de resultado, como a cor, descoloração marginal, adaptação marginal, forma anatômica, sensibilidade pós-operatória, formação de cárie e rugosidade de superfícia de selantes hidrofóbicos e hidrofílicos de fóssulas e fissuras no 3º e 6º mês de aplicação. Os dados foram analisados usando o software SPSS e os resultados foram obtidos. Resultados: No presente estudo, entre as 50 restaurações com selantes hidrofóbico de fóssulas e fissuras (Grupo I), 48 (96%) das restaurações ficaram retidas no final do terceiro mês e 45 (90%) ficaram retidas no final do sexto mês. Entre as 50 restaurações nos selantes de fóssulas e fissuras hidrofílicas (Grupo II), 49 (98%) restaurações ficaram retidas no final do terceiro mês e 46 (92%) no final do sexto mês ficaram retidas. Não houve diferença significativa no número de restaurações totalmente retidas ao final de seis meses (p = 1,00) entre os dois grupos. A avaliação de todas as outras variáveis entre as duas intervenções mostrou que a diferença não foi estatisticamente significativa. Conclusão: O estudo concluiu que o selante de fóssulas e fissuras hidrofílico era semelhante ao selante de fóssulas e fissuras hidrofóbicas em termos de retenção, combinação de cor, descoloração marginal, adaptação marginal, forma anatômica e rugosidade de superfície. Introduction: The study was done to compare the effectiveness of hydrophobic and hydrophilic pit and fissure sealant retention among 7-10 year old school children. Material and Methods: The present split mouth randomized trial was conducted on the occlusal surfaces of permanent first mandibular molars to compare and assess the retention along with other outcome variables like colour match, marginal discolouration, marginal adaptation, anatomic form, post-operative sensitivity, caries formation and surface roughness properties of Hydrophobic and Hydrophilic pit and fissure sealants at 3rdand 6th month. The data was compiled and analyzed using SPSS software and results were generated. Results: In the present study among the 50 restorations in the hydrophobic pit and fissure sealants (Group I), 48 (96%) restorations were retained at the end of third month and 45(90%) were retained at the end of sixth month. Among the 50 restorations in the hydrophilic pit and fissure sealants (Group II), 49(98%) restorations were retained at the end of third month, and 46(92%) at the end of sixth month were retained. There was no significant difference in the number of completely retained restorations at the end of six months (p = 1.00) among both the groups. The assessment of all the other outcome variables between both the interventions showed that the difference was not statistically significant. Conclusion: The study concluded that the hydrophilic pit and fissure sealant was similar to the hydrophobic pit and fissure sealant in terms of retention, colour match, marginal discolouration, marginal adaptation, anatomic form and surface roughness properties with (AU)

Introduction: Dental caries is an infectious mulifactorialdisease which can affect any tooth in the oral cavity. Assuch it is important to protect them from becoming carious.Resin sealant methods developed by Bowen in 1962 stillcontinue to form the basis of presently available sealants. Animportant factor for sealant success is its marginal integrityand retention. Hence this study was undertaken to assessthe retention and marginal discoloration of conventionalpit and fissure sealants, Glass ionomer sealant type VII andHelioseal-F. Study aimed to assess the retention and marginaldiscoloration of conventional pit and fissure sealants, glassionomer sealant type VII and helioseal -F.Material and methods: This study was conducted in a privatedental clinic in Bangalore from 2015 to 2016. Thirty childrenaged 6 to 8 years were selected. The children were divided into3 groups of 10 children each. First group, ortho Phosphoricacid etchant gel was applied with a disposable nylon applicatortip on to the pit and fissures. Conventional light-cured resinbased p.it-and-fissure sealant (Clinpro™ Sealant (3M ESPE)was cured with the light curing unit for 20 seconds. Secondgroup (Group II): Fuji VII cement was mixed according tomanufacturer’s instructions and applied to the occlusal surfaceusing a plastic-filling instrument and a disposable nylon brushto spread it into the pits and fissures. Third Group (Group III):Etching was done with 37% phosphoric acid for 20 seconds.Using the syringe needle tip, Helioseal F sealant was flowedinto the fissures for 20 seconds. First follow up examinationwas done at 3 months recall by using visual and tactileexamination. The sealants were examined for their integrity,retention and marginal discoloration. The data was obtainedat 3 months and 6 months intervals. The teeth were visuallyinspected for caries.Results: At 3-month evaluation 89.7% of retention was seenfor conventional resin sealant, 72.7% retention for HeliosealF and 65.3% for Glass ionomer sealant. At 6month followup 73.3% retention was seen for conventional resin sealant,52.4% retention for Helioseal F and 34.9% for Glass ionomersealant. There was no significant difference in terms ofmarginal discoloration between conventional resin sealantand helioseal F. However glass ionomer sealant showed asignificant marginal discoloration as compared to heliosealF. No significant differe-nce was seen in the development ofcaries between the three groups.Conclusion: Conventional sealants are better sealants withrespect to retention

Abstract Objective: To clinically evaluate the retention and marginal discoloration of pit and fissure sealants applied to primary and permanent teeth. Material and Methods: The study population encompassed of 5-15 years-old children. After consenting, a light-curing sealant was applied to etched pits and fissures of occlusal surfaces of selected sound teeth. The retention rate and marginal discoloration were assessed, 3 months after application of the sealants based on the criteria proposed by Simonsen's criteria (total retention: score 0, partial loss: score 1, and total loss: score 2). Each tooth was considered as an independent sample during analysis. Results: The achieved sample size was 43 children aged 5-15 years (mean age=10.0 years). Therefore, data of 100 teeth from 43 children were used for the final analysis. The percentage of completely retained sealants (59%) was higher than the percentage of partially retained sealants (23%) and completely missing sealants (18%) after 3 months follow up. Out of 100 sealed teeth, 60% were either had marginal discoloration or completely missing. Using the Mann-Whitney test, there was a statistically significant difference (p<0.05) between primary and permanent teeth in terms of retention. However, there was no statistical difference (p>0.05) between upper and lower teeth in terms of retention. Conclusion: The success rate of fissure sealants after 3 months follow-up was satisfactory.

ABSTRACT

Introdução: Os selantes são considerados uma opção terapêutica minimamente invasiva no tratamento de lesões cariosas limitadas à metade externa da dentina. Nesta técnica, o tecido cariado é mantido coberto com um selante, promovendo a paralisação da lesão. Objetivo: analisar a utilização de selantes no tratamento de lesões de cárie, a sua eficácia, qual tipo de material é o mais ideal para essa finalidade e observar se essa terapêutica é a mais indicada para tratar lesões cariosas já cavitadas. Métodos: uma revisão da literatura foi realizada por meio de artigos científicos publicados nos últimos cinco anos (2015-2020) presentes em bases de dados eletrônicos, como PubMED/Medline, Lilacs e BBO. Resultados e discussão: a utilização dos selantes, antigamente, era apenas voltada para a prevenção, porém houve uma progressão nesse entendimento, passando a ser utilizado no tratamento de lesões cariosas que apresentam microcavidades oclusais de até três milímetros de perda da integridade superficial. Neste trabalho, todos os estudos descritos afirmaram que essa técnica é eficaz em paralisar o processo carioso, sendo indicada, principalmente, para crianças, pois reduz o tempo clínico e gera menos ansiedade. Em relação ao tipo de selante mais indicado para essa finalidade, as evidências científicas ainda são inconclusivas. Conclusão: o uso de selantes é um método eficiente na interrupção da lesão cariosa, porém depende da adequada retenção do material e da colaboração do paciente em continuar o acompanhamento pelo profissional. Além disso, é necessária a realização de estudos mais criteriosos que comparem os tipos de selantes e indiquem a opção mais adequada para obter o melhor selamento das lesões cavitadas.


Introduction: Sealants are considered a minimally invasive therapeutic option in the treatment of carious lesions limited to the outer half of the dentin. In this technique, the decayed tissue is kept covered with a sealant, promoting the paralysis of the lesion. Objective: to analyze the use of sealants in the treatment of caries lesions, their effectiveness, what type of material is most ideal for this purpose and observe whether this therapy is the most suitable for treating carious lesions already cavitated. Methods: a literature review was carried out through scientific articles published in the last five years (2015-2020) submitted to electronic databases, such as PubMED / Medline, Lilacs and BBO. Results and discussion: the use of sealants, in the past, was only aimed at prevention; however there was a progression in this understanding, being used in the treatment of carious lesions that present occlusal microcavities of up to three millimeters of surface integrity loss. In this research, all the studies described stated that this technique is effective in paralyzing the carious process, being indicated, mainly, for children, because it reduces the clinical time and generates less anxiety. Regarding the type of sealant most suitable for this purpose, the scientific evidence is still inconclusive. Conclusion: the use of sealants is an efficient method to interrupt the carious lesion; however, it depends on the adequate retention of the material and on the patient's collaboration in continuing the follow-up by the professional. In addition, it is necessary to carry out more careful studies that compare the types of sealants and indicate the most appropriate option to obtain the best sealing of cavitated lesions.

Objective@#To analyze the quality of the first permanent molars pit and fissure sealant for school-aged children in Xi'an from 2010 to 2018, and to provide a reference for preventing dental caries.@*Methods@#A total of 5 871 children were selected from different regions from 2010 to 2018 by cluster random sampling method for the quality inspection of pit and fissure sealant.@*Results@#The quality of pit and fissure sealant has been improved by year in Xi'an ( χ 2=1 326.39, P <0.01). The rate of intact sealant was 92.57% in 2018. The quality of project and reservation rate in rural areas(64.23%, 76.83%) were worse than those in urban (84.17%, 90.80%) and suburban areas(84.21%, 91.88%)( χ 2=310.58, 296.52, P <0.01); The quality of pit and fissure sealant was highest in tertiary hospitals(87.28%, 91.07%), followed by private hospitals(84.60%, 91.43%), primary hospitals(84.42%, 92.02%), secondary hospitals(72.79%, 84.45%)( P <0.01).@*Conclusion@#The quality of pit and fissure sealant in Xi'an reached the national requirement, but there is obvious gap between town and country, the quality of project varied substantially across medical institutions .Training and supervision should be strengthened in the future work.

Objective@#To examine the retention rate and caries prevention effect of fit-and-fissure sealants after 3 years of children oral disease comprehensive intervention program in Henan province.@*Methods@#For each of the five provincial regions attending the program in 2015, two classes from grade 2 in four primary schools were selected. Children in two schools were assigned into intervention group and the first permanent molars which were appriopriate for sealing were sealed with pit-and-fissure sealants. Children in other two schools were included in the control group and the first permanent molars were only examined for caries conditions. Three years later, retention rate of pit-and-fissure sealants and the dental caries prevalence was compared.@*Results@#Complete retention rate of sealants in the first permanent molars was 65.55% and the whole retention rate was 94.78%. The caries incidence and mean value of the first permanent molars in the intervention group children was 9.71%, (0.12±0.40) respectively whereas those in control group was 21.90%, (0.39±0.78) respectively. The differences between two groups were statistically significant(χ2/t=27.76, 6.91, P<0.01). The relatively effective rate of sealing for caries reduction was 63.43% and the actural effective rate was 6.51% and the pure yield rate of sealing was 12.19%. Higher sealant retention rate was associated caries prevalence(χ2=153.28, P<0.01).@*Conclusion@#Childhood oral disease intervention project in Henan Province shows positive effects. Sealing of the first permanent molars effectively prevent dental pit and fissure caries.

Abstract Objective: To compare the immediate microleakage of carious fissures sealed with a caries infiltrant covered by a flowable composite or solely with a flowable composite. Material and Methods: Extracted carious molars (n=20) were selected and paired among the experimental groups according to caries progression scores. Experimental groups (n=10) were divided according to the following sealing techniques: 1) caries infiltrant (Icon) + flowable composite (Z350 flow); 2) flowable composite (Z350 flow). Specimens were immersed in 3% methylene blue and evaluated in a stereomicroscope. Microleakage scores were attributed independently by two calibrated evaluators and the mode value was considered as the mean for the specimen. Binomial tests were used to test differences between two independent sample proportions at 5% significance level. Results: In the flowable composite group, a higher prevalence of dye penetration along the full depth of the fissure was observed, compared with caries infiltrant + flowable composite (p<0.05). For specimens showing dye penetration up to half of the fissure, in caries infiltrant + flowable composite group, all specimens showed dye penetration into the sealant, but not over the infiltrant. Conclusion: Using caries infiltrant as sealing material under a flowable composite cover is effective to improve the immediate sealing ability in carious fissures compared to sealing with flowable composite.

ABSTRACT

Objective: To compare retention, preventive effectiveness and longevity of two different highviscosity glass-ionomer sealants Ketac Molar® and Maxxion R® in the sealing of erupting and out-of-occlusion first permanent molar in children aged 5-7 years. Material and Methods: Children with past caries experience with healthy erupting first permanent molar were included in the study. Teeth 16 and 46 were sealed with Ketac Molar® and teeth 26 and 36 were sealed with Maxxion R®. After 8 months, the retention of sealants was evaluated using the following criteria: total retention; presence of sealant in two thirds of the occlusal surface; presence of sealant in one third of the surface; and total absence of the sealant. Results: Statistical difference between materials was observed, and Ketac Molar® retention was superior to that of Maxxion R® (p<0.05). Regarding the development of caries in sealed first permanent molar, none of the teeth sealed with Ketac Molar® or Maxxion R® developed caries lesion. Conclusion: Ketac Molar® retention is significantly superior to that of Maxxion R®, but the preventive effectiveness in the development of carious lesions is similar for both, which is extremely interesting for the field of public health, since the cost of the material is significantly lower.

ABSTRACT

Abstract The aim of this study was to evaluate the influence of polyhexamethylene guanidine hydrochloride (PHMGH) in the physico-chemical properties and antibacterial activity of an experimental resin sealant. An experimental resin sealant was formulated with 60 wt.% of bisphenol A glycol dimethacrylate and 40 wt.% of triethylene glycol dimethacrylate with a photoinitiator/co-initiator system. PHMGH was added at 0.5 (G0.5%), 1 (G1%), and 2 (G2%) wt.% and one group remained without PHMGH, used as control (GCTRL). The resin sealants were analyzed for degree of conversion (DC), Knoop hardness (KHN), and softening in solvent (ΔKHN), ultimate tensile strength (UTS), contact angle (θ) with water or α-bromonaphthalene, surface free energy (SFE), and antibacterial activity against Streptococcus mutans for biofilm formation and planktonic bacteria. There was no significant difference for DC (p > 0.05). The initial Knoop hardness ranged from 17.30 (±0.50) to 19.50 (± 0.45), with lower value for GCTRL (p < 0.05). All groups presented lower KHN after immersion in solvent (p < 0.05). The ΔKHN ranged from 47.22 (± 4.30) to 57.22 (± 5.42)%, without significant difference (p > 0.05). The UTS ranged from 54.72 (± 11.05) MPa to 60.46 (± 6.50) MPa, with lower value for G2% (p < 0.05). PHMGH groups presented no significant difference compared to GCTRL in θ (p > 0.05). G2% showed no difference in SFE compared to GCTRL (p > 0.05). The groups with PHMGH presented antibacterial activity against biofilm and planktonic bacteria, with higher antibacterial activity for higher PHMGH incorporation (p < 0.05). PHMGH provided antibacterial activity for all resin sealant groups and the addition up to 1 wt.% showed reliable physico-chemical properties, maintaining the caries-protective effect of the resin sealant over time.

ABSTRACT

Objetivo: avaliar a efetividade dos selantes aplicados por estudantes do curso de graduação em Odontologia da Universidade do Sul de Santa Catarina (Unisul), Grande Florianópolis, após 6 meses da aplicação, e estimar a prevalência de cárie dos participantes. Sujeitos e método: estudo observacional descritivo com indivíduos das clínicas de Odontologia. A efetividade dos selantes oclusais foi avaliada por roteiro validado na literatura, e a prevalência de cárie, dicotomizada. Os dados foram analisados por estatística descritiva e as associações pelo teste Qui-Quadrado. Resultados: a taxa de resposta do estudo foi de 94,7% (n=18), 2/3 da amostra eram do sexo feminino (72,2% IC 95% 49,1;87,5), 60% dos pacientes recebiam 3 salários mínimos ou menos, maiores proporções de indivíduo tinham selados seus dentes há pelo menos 9 meses. Pouco menos de 2/3 da amostra receberam Fluroshield como material restaurador em seus dentes. Dentre os 57 dentes avaliados, 70,2% apresentavam retenção to-tal. Os dentes mais acometidos pela perda de retenção foram os segundos molares superiores (40%). Mais de 2/3 da amostra não apresentaram cárie (88,9% IC 95% 67,2; 96,9). O isolamento relativo foi o mais prevalente (72,2% IC 95% 49,1;87,5). Conclusão: sob a perspectiva das práticas dos estudantes de Odontologia, metade da amostra de pacientes apresentou alguma perda de retenção dos selantes, mesmo que um número maior de retenções totais e a ausência de recidiva de cárie tenham sido observados. Isso reflete a importância da relação professor-aluno no planejamento e na correta aplicação das técnicas de selamento por estudantes de Odontologia. (AU)


Objective: to evaluate the effectiveness of pit and fissure sealants applied by undergraduate dental students of the University of Southern Santa Catarina (Unisul), SC, Brazil, after 6 months, and to estimate the prevalence of caries in the participants. Subjects and method: observational and descriptive study with individuals from the dental clinics. The effectiveness of pit and fissure sealants was evaluated using a script validated in the literature, and the prevalence of caries was dichotomized. The data were analyzed by descriptive statistics and the associations were made using the chi-square test. Results: the response rate of the study was 94.7% (n=18), 2/3 of the sample consisted of women (72.2% 95%CI 49.1, 87.5), 60% of the patients received three Brazilian minimum wages or less, and a great portion of the participants had sealed their teeth at least 9 months earlier. A little under 2/3 of the sample received Fluroshield as restorative material for their teeth. Among the 57 teeth evaluated, 70.2% presented complete retention. The teeth most affected by loss of retention were maxillary left second molars (40%). More than 2/3 of the sample did not present dental caries (88.9% 95%CI, 67.2, 96.9). Relative isolation was the most prevalent (72.2% 95%CI 49.1, 87.5). Conclusion: from the perspective of the practices of dental students, half of the sample of patients presented some loss of sealant retention, despite the greater number of complete retentions and absence of recurrent caries. This reflects the importance of the teacher-student relationship in the planning and correct application of sealing techniques by dental students. (AU)

ABSTRACT

The study aims to evaluate the bond strength of sealants on deciduous molars. Clinpro ™ XT, Vitremer ™, Fluroshield and Optibond FL materials were applied to the occlusal surface of 40 deciduous lower molars (n = 5). The teeth were prepared for the micro tensile test, obtaining a specimen in a stick format with a cross-sectional area of 0.8 mm2 . After 24 hours and 6 months of restorative procedures, the specimens were traversed in universal test machine. The statistical analysis used the tooth as experimental unit, considering the adhesive and mixed fractures data, through Variance Analysis of repeated measures and Tukey tests (α = 0.05). There was a difference among the adhesion of the materials in 24 h, with higher union strength for Optibond FL (31.20 ± 1.36 MPa), significantly higher than that of Clinpro ™ XT (20.23 ± 1.16 MPa), Fluroshield (24.61 ± 2.76 MPa) and Vitremer™ (21.31 ± 2.32 MPa), which were similar. After 6 months of storage, the Clinpro ™ XT bond strength remained (22.18 ± 2.91 MPa), Optibond FL decreased (20.77 ± 1.53 MPa), but remained similar to that of Clinpro ™ XT in 6 months. The lowest adhesion values at 6 months were of Fluroshield (11.14 ± 1.98 Mpa) and Vitremer (5.29 ± 0.58 Mpa). It was concluded that the bond strength of the sealants to the occlusal surface of the deciduous molars was influenced by the material, with Clinpro ™ XT being the only material that maintained the bond strength values after 6 months. O presente estudo teve como objetivo avaliar a adesão de selantes em molares decíduos. Os materiais Clinpro™ XT, VitremerTM, Fluroshield e Optibond FL foram aplicados na superfície oclusal de 40 molares inferiores decíduos (n=5). Os dentes foram preparados para o ensaio de microtração, com obtenção de corpo-de-prova em formato de palito com área de seção transversal de 0,8 mm2 . Após 24 horas e 6 meses da realização dos procedimentos restauradores, os espécimes foram tracionados em máquina de ensaio universal. A análise estatística usou como unidade experimental o dente, considerando os dados de fraturas adesivas e mistas, pelos testes de Análise de Variância de medidas repetidas e Tukey (α=0,05). Houve diferença entre a adesão dos materiais em 24 h, com maior resistência de união para o Optibond FL (31,20 ±1,36 MPa), significativamente maior que a do Clinpro™ XT (20,23 ±1,16 MPa); Fluroshield (24,61 ±2,76 MPa) e VitremerTM (21,31 ±2,32 MPa) foram semelhantes. Decorridos 6 meses de armazenamento, a resistência de união do Clinpro™ XT se manteve (22,18 ±2,91 MPa) e a do Optibond FL diminuiu (20,77 ±1,53 MPa) mas se manteve semelhante à do Clinpro™ XT em 6 meses. Os menores valores de adesão em 6 meses foram do Fluroshield (11,14 ±1,98 Mpa) e do Vitremer (5,29 ±0,58 Mpa). Concluiu-se que a resistência de união dos selantes à superfície oclusal dos molares decíduos foi influenciada pelo material, sendo o Clinpro™ XT o único material que manteve os valores de resistência de união após 6 meses. ABSTRACT

Objective: To evaluate the clinical performance of three preventive materials on the occlusal surfaces of primary molars over a 36-month period. Material and Methods: The study was conducted with a split-mouth design involving four experimental groups: G1 ­ resin-modified glass ionomer cement (Vitremer®); G2 - resin sealant (Alpha Seal Light®); G3 - silver diamine fluoride (Cariostatic®); and G4 - control group without material. Thirty-two children aged 36 to 60 months, attending a pediatric clinic, with a deft index ≥ 1 and four primary second molars participated in the study. The material retention was evaluated in G1 and G2, and the presence of incipient caries was evaluated in all groups. Data were submitted to the Wilcoxon test and survival test (α = 5%). Results: At three, six, 12, 24 and 36 months, G2 exhibited greater material loss (maximum loss: 44%) in comparison to G1 (maximum loss: 35%), but this difference did not achieve statistical significance (p = 0.214). The highest incidence of incipient caries occurred in G3 (20%), but no significant differences were found between groups at any evaluation time (p = 0.154). Conclusion: There were no statistically significant differences between the evaluated products regarding material retention and dental caries, although there was more material loss in teeth that received the Alpha Seal® sealant resin, and a greater number of carious lesions in the Cariostatic® group (silver diamine fluoride).

ABSTRACT

Objective: To verify the effectiveness of sealing with glass ionomer cement and transversal brushing in the prevention and treatment of initial caries lesions in erupting permanent molars of children aged 5-7 years. Material and Methods: Healthy teeth with ICDAS scores 1 or 2 were selected. One hundred and thirty-one children (79 teeth in the control group - 75 teeth in the test group) were randomly assigned into the two experimental groups: (1) transversal brushing technique for plaque control with conventional flat bristles and fluoridated dentifrice or (2) sealing the occlusal surfaces with glass ionomer cement (Vitro Molar®). Follow-up was performed for 18 months and the survival analysis was applied to test the occurrence of lesion progression. Results: The mean age of children was 5.4 years and it was verified that, by examining the initial tooth condition, the probability of progression was 66% lower when the tooth had initial caries lesions than when it was healthy; the probability of molar progression was about 74% lower in the test group compared to the control group. The mean time for lesion development in the test group (16.8 months; SD = 0.5) was greater than that in the control group (16.5 months; SD = 0.5; p=0.008). Conclusion: Sealing reduces the progression of caries disease in initial lesions when compared to the transversal brushing technique.

ABSTRACT

The consumption of beverages among children is rising. The purpose of this study was to examine the effect of kid's drink on dental resin-based pit and fissure sealant. Pororo, I-kicker, Sunkist kids were included in the experimental groups, and Samdasu was included in the control group. A conventional dental sealant material (Clinpro™Sealant®) was selected for this study. Resin specimens (8 mm in diameter and 1 mm in thickness) were prepared according to manufacturers' instructions and the initial roughness (Ra) was then measured. The pH of all the four groups was measured using a pH meter. The specimens were individually immersed in 5 ml of the experimental solutions and stored at 37℃ for 72 hours. Following this, the surface roughness of the resin specimens was measured by Surftest. The concentration of residual monomer released was determined by high performance liquid chromatography (HPLC). The surface morphology of the resin specimen was evaluated before and after storage by scanning electron microscopy (SEM). Data were statistically analyzed using Kruskal-Wallis and Duncan's test. The results showed that all the children's beverages examined in this study contained citric acid. The pH of I-kicker was the lowest (3.03±0.01), followed by that of Sunkist kids (3.26±0.02) and Pororo (3.47±0.02). We observed an increase in the surface roughness of resin specimens after 72 h of immersion in all the beverages tested (p < 0.05). There was matrix degradation after immersion, visualized on SEM image, in all the beverage groups. Bisphenol-A-glycidyl methacrylate was not detected after 72 hours, but triethylene glycol dimethacrylate levels were increased in all the beverages tested during the 72 hours by HPLC. These results suggest that intake of beverages containing acid can cause degradation of the resin-based pit and fissure sealants in children.

ABSTRACT

The aim of this study was to evaluate the effect of etching time and bonding agents on bond strength of sealant applied to the occlusal surface of primary molars.Forty non-carious exfoliated human primary molars were included in the study. The teeth were randomly divided into 4 groups for measurement. For group I, no acid etching treatment was used. For group II, III, and IV, acid etching gels were applied on the occlusal surface for 15, 30, and 60 seconds, respectively. Each group was divided into 2 subgroups; one group was treated with bonding agents on the enamel while the other was not. Microtensile bond strength was evaluated using a universal testing machine.There were no statistically significant differences in bond strength with varying duration of etching among groups. The results revealed that the use of bonding agents prior to application of fissure sealant increased the bond strength(p < 0.05).It could be concluded that etching time greater than 15 seconds does not significantly enhance the bond strength, but the use of bonding agents as an intermediate layer between the primary molar and fissure sealant would be beneficial in increasing the bond strength.

Objective@#To evaluate and compare the caries prevention effectiveness of a resin-based sealant and a glass ionomer sealant on permanent first molars in 7- to 9-years-old school children.@*Methods@#A randomized controlled, examiner-blinded, 5-year clinical trial was conducted in 19 primary schools in Dalian, Liaoning province. Totally 419 children (with 664 first molars) who had at least one sound permanent first molar with deep fissures or fissures with signs of non-cavitated lesions were recruited and stratified and allocated randomly into three parallel groups. A resin-based sealant was used in Group R (136 children with 219 first molars) and a glass ionomer sealant was used in Group G (130 children with 218 first molars). The third group (Group N, 153 children with 227 first molars) with none of the two sealants applied was served as blank control. Follow-up examinations were conducted at 0.5, 1, 2, and 5 years after sealing in order to assess the retention of the sealants and the incidence of caries occurrence. Chi-square test was adopted to compare the differences of caries incidence among the 3 groups. Generalized estimating equations (GEE) was used to analyze the effective factors of caries incidence.@*Results@#During the 5-year follow-up period, 13.4% (23/172) of the teeth in Group R, 22.5% (40/178) of the teeth in Group G and 34.5% (57/165) of the teeth in Group N were diagnosed as having developed cavitated lesions. And the differences in the incidence of caries among the three groups was statistically significant (P<0.05). According to GEE, the resin-based sealant and the glass ionomer sealant were found to be the protection factors and the non-cavitated lesion before sealing was found to be the risk factor. The resin-based sealant was found to be the protection factor to the glass ionomer sealant when the blank control group was removed.@*Conclusions@#Both resin-based sealant and glass ionomer sealant could effectively prevent the permanent first molars from the occurrence of carious lesions in 5 years. The resin-based sealant was superior to the glass ionomer sealant in caries prevention.

Abstract Objectives To evaluate the efficacy of a conservative treatment to prevent the progression of caries using an infiltrant on non-cavitated pit and fissures. Material and Methods This controlled clinical trial selected 23 volunteers with clinically and radiographically non-cavitated occlusal caries among patients presenting a "rather low" to "very high" caries risk. Eighty-six teeth were randomly divided into two experimental groups: teeth receiving a commercial pit-and-fissure sealant (Alpha Seal-DFL) and contralateral teeth receiving Icon infiltrant (DMG). Caries progression was monitored by clinical (laser fluorescence caries detection) and radiographic examination at 12-month intervals over a period of 3 years of monitored caries progression. Probing the sealing materials to detect areas of retention was also used to evaluate marginal integrity. Results Statistical analysis showed no difference in caries progression using laser fluorescence caries detection when both materials were compared, regardless of the evaluation times (p>0.05). No significance was observed when the marginal sealant integrity of both materials was compared, regardless of the evaluation time (p<0.05). Marginal integrity significantly reduced after 1 year for both materials (p<0.05), but remained stable after 2 and 3 years of evaluation, compared with 1-year results (p>0.05). SEM analysis exhibited a more homogeneous sealing for the infiltrant than obtained by the sealant. Conclusions The infiltrant was effective to prevent the caries progression in non-cavitated pit-and-fissures after 3 years of clinical evaluation, comparable with the conventional sealant. The infiltrant also presented better results in terms of caries progression at the 3-year evaluation time using the radiographic analysis.
