ABSTRACT

Objective:To analyze the incidence and the related factors of umbilical vein catheter (UVC) tip migration within 7 d after umbilical vein catheterization.Methods:This prospective study involved neonates who were successfully indwelled with UVCs in the Department of Neonatology of Gansu Provincial Women and Child-care Hospital from June 2020 to December 2021. The position of the UVC tip, length of umbilical stump, body weight, and abdominal circumference were recorded after the UVCs were inserted successfully, and the changes in these four data at 2, 24, 48, 72 h, and 7 d after catheterization were analyzed and compared. All the subjects were divided into the non-migrate group, inward migration group, and outward migration group. Chi-square test, Mann-Whitney U test, or Kruskal Wallis H test were used for statistical analysis. Results:A total of 157 newborns were enrolled, with 51 cases in the inward migration group, 62 cases in the outward migration group, and 44 cases in the non-migrate group. There were no significant differences among the three groups regarding gestational age, birth weight, gender, born through cesarean section, age at the time of catheterization, use of sedation, and feeding modes (all P>0.05). The migration rates of UVCs tip at 2, 24, 48, 72 h, and 7 d after catheterization were 0, 27.4% (43/157), 27.2% (31/114), 25.3% (21/83), and 29.0% (18/62), respectively. The cumulative migration rates at 24, 48, 72 h, and 7 d were 27.4% (43/157), 47.1% (74/157), 60.1% (95/157), and 72.0% (113/157), respectively. Compared with the non-migrate group, the inward migration group had a shorter umbilical cord stump at 24 and 48 h [0.5 cm (0.4-0.5 cm) vs 0.6 cm (0.5-0.8 cm); 0.4 cm (0.3-0.5 cm) vs 0.5 cm (0.5-0.6 cm), Z=-5.55 and -3.69, both P<0.05], less abdominal circumference increment at 48 and 72 h [0.6 cm (0.5-1.0 cm) vs 0.9 cm (0.7-1.2 cm); 0.6 cm (0.3-0.9 cm) vs 0.9 cm (0.7- 1.3 cm), Z=-2.03 and -2.09, both P<0.05)], and more weight loss percentage [-4.7% (-6.0%--3.6%) vs -3.1% (-3.7%--2.2%); -6.0% (-7.5%--5.0%) vs -3.9% (-5.1%--2.4%), Z=-3.75 and -2.96, both P<0.05]. The abdominal circumference increased more in the outward migration group at 24, 48, 72 h, and 7 d than those in the non-migrate group [1.6 cm (0.9-1.9 cm) vs 0.7 cm (0.5-0.9 cm); 1.5 cm (1.2-1.8 cm) vs 0.9 cm (0.7-1.2 cm); 1.7 cm (1.3-1.9 cm) vs 0.9 cm (0.7-1.3 cm); 1.6 cm (1.1-1.9 cm) vs 0.9 cm (0.6-1.3 cm), Z=-4.82, -4.79, -3.74, and -3.09, all P<0.05]. Conclusion:The incidence of UVC tip migration is high, which could be affected by dryness and retraction of the umbilical cord stump and the change in neonatal abdominal circumference and body weight.

ABSTRACT

La persistencia de lesiones perirradiculares luego del tra- tamiento endodóntico es un problema que requiere del clínico un conocimiento cabal de la histofisiología y de la histopato- logía del sistema de conductos radiculares del tejido pulpar y de los tejidos perirradiculares (periodonto y hueso); además de considerar siempre la posible existencia de enfermedades sistémicas que también pueden actuar como factores de in- fluencia. La presencia de bacterias remanentes a posteriori del tratamiento es considerada como una de las causas principales y más frecuentes para la perpetuación de las lesiones perirra- diculares. Sin embargo, existen otros factores causales, como la existencia de conductos laterales o accesorios infectados y no tratados, la reabsorción dentinaria interna, intercomunica- ciones, cul-de-sacs o istmos; que representan áreas de difícil acceso durante la instrumentación e irrigación. Cuando la cau- sa original se localiza en la zona perirradicular, como en los casos de actinomicosis, reacciones a cuerpo extraño, cristales de colesterol (CRCo) y granulomas o quistes con alto conte- nido de CRCo, la indicación más adecuada es el retratamiento y la cirugía periapical como complemento (AU)


The persistence of periradicular lesions after endodontic treatment is a problem that requires the doctor to have a thor- ough knowledge of the histophysiology and histopathology of the root canal system, the pulp tissue and periradicular tis- sues (periodontium and bone); as well as always considering the possible existence of systemic alterations that can also be influencing factors. Persisting bacteria within the root canal system after treatment is one of the major and most frequent causes for the perpetuation of periradicular lesions. Howev- er, there are other possible causal factors such as the exist- ence of untreated lateral or accessory canals, internal dentin resorption, intercommunications, cul-de-sacs or isthmuses; areas that represent a difficulty in access during instrumen- tation and irrigation. If the original cause is located in the periradicular area, in cases like actinomycosis, foreign-body reactions, cholesterol crystals (CRCo) and granulomas or cysts with high content of CRCo, retreatment coupled with periapical surgery is the best approach to treatment (AU)

ABSTRACT

Traumas na região da face são comuns devido a sua topografia e à grande exposição do local. As lesões na região maxilofacial são normalmente causadas devido a traumas por acidentes de trânsito, agressão física, acidentes cotidianos como queda da própria altura e de nível. O objetivo deste trabalho é relatar um caso clínico de trauma facial com a presença de corpo estranho impactado na face, bem como o tratamento cirúrgico empregado. Paciente do sexo feminino, 17 anos, foi regulada para o Hospital Geral Cleristón Andrade com histórico de queda de cavalo, cursando com fragmento de madeira (galho de arbusto) tranfixado em região maxilofacial. A paciente foi encaminhada ao centro cirúrgico para a remoção do galho de arbusto transfixado. O procedimento foi rápido, e sua remoção se deu por meio do movimento contrário ao mecanismo de trauma. Lesão como esta possui aspectos singulares e devem ser avaliados multidisciplinarmente na emergência. A remoção desses objetos deve ser realizada de forma a preservar as estruturas dentro do possível, levando em consideração também os fatores estéticos e funcionais envolvidos(AU)


Traumas in the face region are common due to its topography and the great exposure of the place. Injuries in the maxillofacial region are usually caused due to trauma from traffic accidents, physical aggression, everyday accidents such as falling from height and level. The aim of this paper is to report a clinical case of facial trauma with the presence of a foreign body impacted on the face, as well as the surgical treatment used. A 17-year-old female patient was treated at the Cleristón Andrade General Hospital with a history of falling from a horse, taking a wood fragment (shrub branch) transfixed in the maxillofacial region. The patient was referred to the operating room for removal of the transfixed bush branch. The procedure was quick, and its removal took place through a movement contrary to the trauma mechanism. Injuries like this have unique aspects and must be evaluated multidisciplinary in an emergency. The removal of these objects must be carried out in order to preserve the structures as much as possible, also taking into account the aesthetic and functional factors involved(AU)

ABSTRACT

A hepatic abscess caused by a swallowed foreign body is a rare and challenging diagnosis. Most patients have nonspecific symptoms, and more than 90% of patients do not remember having swallowed it, which occurred accidentally. In this setting, fish bones, chicken bones, and toothpicks are the most found foreign bodies. We reported the case of a 54-year-old male patient admitted with abdominal pain and intermittent fever. He was diagnosed with liver abscess and treated successfully with antibiotics and a laparoscopic procedure; a rosemary twig was found during the abscess drainage procedure. Furthermore, a literature review of 22 cases of laparoscopic treated liver abscesses associated with a foreign body was made.

ABSTRACT

O edema tardio intermitente e persistente (ETIP) por implante de ácido hialurônico é uma reação inflamatória imunomediada decorrente de fenômenos imunogênicos ao próprio preenchedor bem como de sua capacidade em reter água, configurando assim o edema local. Pode ser desencadeado após infecções virais ou bacterianas. Assim como em muitas doenças infectocontagiosas, a COVID-19 pode vir a apresentar na pele diversos sinais e sintomas que ainda não são completamente compreendidos, porém muitas manifestações cutâneas associadas já foram descritas. Vimos, por meio deste relato de caso, apresentar o ineditismo de uma reação do tipo ETIP, desencadeada pela infecção por Sars-CoV-2.


Persistent Intermittent Delayed Swelling (PIDS) due to hyaluronic acid implantation is an immune-mediated inflammatory reaction resulting from immunogenic phenomena to the filler itself, and its ability to retain water, thus configuring local edema. Viral or bacterial infections can trigger the condition. As with many infectious diseases, COVID-19 may have several signs and symptoms on the skin that are not yet fully understood, but many associated skin manifestations have already been described. Through this case report, we report the novelty of an ETIP-type reaction triggered by SARS-CoV-2 infection.

ABSTRACT

Introducción. La ingestión accidental de cuerpos extraños como espinas de pescado es muy poco frecuente; en ocasiones, pueden alojarse en el esófago y es posible su migración extraluminal. Este accidente se presenta principalmente en países asiáticos por el gran consumo de pescado. La migración extraluminal de una espina de pescado desde el esófago a la glándula tiroides es aún más infrecuente, con pocos reportes en la literatura.Objetivo. Presentar nuestra experiencia en el diagnóstico y manejo quirúrgico de un paciente con un cuerpo extraño (espina de pescado) alojado en la glándula tiroides y, asimismo, hacer una revisión de la literatura científica.Caso clínico. Se presenta una paciente de 53 años, sin antecedentes de importancia y con un cuadro clínico de tres meses de evolución consistente en odinofagia posterior a la ingestión accidental de una espina de pescado. En la ecografía practicada en otra institución se observó un cuerpo extraño alojado en el lóbulo tiroideo izquierdo en relación con la musculatura del esófago. En la tomografía de cabeza y cuello se apreciaba una imagen hiperdensa lineal de 2,6 cm de longitud en la misma localización. En la exploración quirúrgica se encontró tejido fibrótico en la región posterior del lóbulo tiroideo izquierdo y se procedió a practicar una tiroidectomía subtotal izquierda. La evolución posoperatoria fue adecuada.Conclusiones. La migración extraluminal de cuerpos extraños en el tubo digestivo es factible, sobre todo la de las espinas de pescado por su morfología. La tomografía es la herramienta diagnóstica más confiable para detectar cuerpos extraños alojados en la glándula tiroides. Es necesario conocer las posibles complicaciones y, asimismo, optimizar las posibles intervenciones, pues de eso depende el pronóstico clínico del paciente


Introduction: Accidental ingestion of foreign bodies such as fish bones is very rare; sometimes they may lodge in the esophagus, and extraluminal migration is possible. This accident occurs mainly in Asian countries due to the large fish consumption. Extraluminal migration of a fish bone from the esophagus to the thyroid gland is even more infrequent, with few reports in the literature.Objective: To present our experience in the diagnosis and surgical management of a patient with a foreign body (fish bone) lodged in the thyroid gland and to review the scientific literature.Clinical Case: A 53-year-old patient is presented, without a significant history and with a clinical picture of three months of evolution consisting of odynophagia after accidental ingestion of a fish bone. In the ultrasound performed at another institution, a foreign body was observed lodged in the left thyroid lobe in relation to the musculature of the esophagus. Head and neck tomography showed a linear 2.6 cm long hyperdense image at the same location. On surgical exploration, fibrotic tissue was found in the posterior region of the left thyroid lobe and a left subtotal thyroidectomy was performed. Postoperative evolution was adequate.Conclusions: Extraluminal migration of foreign bodies in the digestive tract is feasible, especially that of fish bones due to their morphology. Tomography is the most reliable diagnostic tool to detect foreign bodies lodged in the thyroid gland. It is necessary to know the possible complications and to optimize the possible interventions, since the clinical prognosis of the patient depends on it

ABSTRACT

Abstract Hip arthroplasty is a common and safe intervention in orthopedic surgery. However, the proximity of this joint to large vessels makes the occurrence of vascular injury a rare but serious and possibly lethal complication of this surgical technique. Acute vascular injuries in the context of a hip arthroplasty have variable etiologies and clinical presentations, and are more common in revision surgeries and in situations of medial intrapelvic migration and of chronic infection of the hip prosthesis. In the present article, the authors present a case of acute and late major vascular complication in the context of hip arthroplasty revision. The patient developed an acute laceration of the external iliac artery caused by chronic and progressive medial intrapelvic acetabular migration of the hip prosthesis associated with chronic infection.


Resumo A artroplastia da anca é uma intervenção frequente e segura na cirurgia ortopédica. No entanto, a proximidade dessa articulação com vasos de grande calibre faz com que a ocorrência de lesão vascular maior seja uma complicação rara, mas grave e possivelmente letal, dessa técnica cirúrgica. As lesões vasculares agudas no contexto de uma artroplastia da anca têm etiologia e apresentação clínica variáveis e são mais frequentes em cirurgias de revisão e situações de migração medial intrapélvica e de infecção crônica de próteses da anca. No presente artigo, os autores apresentam um caso com complicação vascular maior aguda e tardia em contexto de revisão de prótese da anca. Trata-se de um paciente que desenvolveu uma laceração aguda da artéria ilíaca externa em contexto de migração acetabular intrapélvica crônica progressiva da prótese da anca associada a infecção crônica.

ABSTRACT

The aim of this study was to characterize the tissue reactions triggered by the polypropylene mesh coated with chitosan and polyethylene glycol film, and if it's able to prevent the formation of peritoneal adhesions. Defects in the abdominal wall of rats were induced and polypropylene meshes coated with chitosan/polyethylene glycol (CPEG group, n= 12) and uncoated (PP control group, n= 12) were implanted. On the fourth and forty-fifth postoperative day the formation of adhesion and the tissue reaction to the biomaterial was evaluated through histological and histochemical analysis. The area (P= 0.01) and severity (P= 0.002) of the adhesion was significatively less in the CPEG group. On the fourth day the foreign body reaction was less intense in CPEG group (P= 0.018) and the production of collagen fibers was more intense in this group (P= 0.041). The tissue reactions caused by the biomaterials were similar on the 45th day, with the exception of the high organization of collagen fibers in the CPEG group. The CPEG meshes did not fully prevent the formation of adhesions, but minimized the severity of the process. The foreign body reaction promoted by polypropylene meshes coated with CPEG is less intense than that triggered by uncoated polypropylene meshes.(AU)


O objetivo deste estudo foi caracterizar as reações tissulares desencadeadas pela tela de polipropileno revestida com o filme de quitosana e polietilenoglicol e verificar se ela é capaz de prevenir a formação de aderências peritoneais. Um defeito na parede abdominal dos ratos foi realizado, e as telas de polipropileno revestidas com quitosana/polietilenoglicol (grupo CPEG, n= 12) e sem revestimento (grupo controle PP, n= 12) foram implantadas. No quarto e no 45º dia pós-operatório, avaliou-se a formação de aderências e a reação tecidual ao biomaterial por análise histológica e histoquímica. A área (P= 0,01) e a severidade (P= 0,002) da aderência peritoneal foram significativamente menores no grupo CPEG no 45º dia. No quarto dia, observou-se que a reação do corpo estranho foi menor no grupo CPEG (P= 0,018), e a produção de fibras de colágeno mais intensa (P= 0,041). As reações tissulares causadas pelos biomateriais implantados foram semelhantes no 45º dia, com exceção da melhor organização das fibras colágenas no grupo CPEG. As telas CPEG não impediram completamente a formação de aderências, porém minimizaram a gravidade do processo. A reação de corpo estranho promovida por telas de polipropileno revestidas com CPEG é menos intensa do que a desencadeada por telas de polipropileno não revestidas.(AU)

ABSTRACT

La migración intracraneal del aceite de silicona intraocular es una complicación rara en el tratamiento de las complicaciones de la retinopatía diabética. Se han postulado varios posibles mecanismos etiopatogénicos para explicar este fenómeno de migración. El aceite de silicona se presenta hiperdenso en la tomografía, siendo un desafío distinguirlo de la hemorragia subaracnoidea, resaltando la utilidad de la resonancia magnética. En pacientes con evidencia de retinopexia de silicona previa, la visualización de una lesión intraventricular con un artefacto de desplazamiento químico asociado aumenta la posibilidad de que se trate de una migración del aceite de silicona. Presentamos el caso de un paciente con antecedente de retinopexia con aceite de silicona y cefalea intensa, identificándose migración del aceite de silicona al sistema ventricular con estudios de tomografía y resonancia.


The intracranial migration of intraocular silicone oil is a rare complication in the treatment of complications of diabetic retinopathy. Several possible etiopathogenic mechanisms have been published to explain this phenomenon of migration. Silicone oil appears hyperdense on tomography, being a challenge to distinguish it from subarachnoid hemorrhage, highlighting the usefulness of magnetic resonance imaging. In patients with evidence of previous silicone retinopexy, the visualization of an intraventricular lesion with a chemical displacement device presents the possibility that it is a migration of the silicone oil. To present the case of a patient with a history of retinopexy with silicone oil and intense headache, identifying the migration of silicone oil in the ventricular system with tomography and resonance studies.

ABSTRACT

Abstract The use of metal wires, called Kirschner wires, is a simple and effective fixation method for the correction of shoulder fractures and of dislocations in orthopedic surgery.Wire migration during the postoperative follow-up is a possible complication of the procedure. The authors present the case of a 48-year-old male patient, a business administrator, who suffered a fall from his own height during a soccer match resulting in right shoulder trauma. The patient was treated at a specialized orthopedics and trauma hospital and was diagnosed with a grade V acromioclavicular dislocation. Four days after the trauma, the acromioclavicular dislocation was surgically treated using ligatures with anchor wires, coracoacromial ligament transfer, and fixation with Kirshner wires from the acromion to the clavicle. At the follow-up, 12 days after the surgical procedure, migration of the Kirschner wire to the acromion edge was identified. The patient was oriented to undergo another surgery to remove the Kirshner wire, due to the possibility of further migration; nonetheless, he refused the surgery. Nine months after the surgical treatment, the patient complained of pain on the left shoulder (contralateral side), difficulty tomobilize the shoulder, ecchymosis, and protrusion. Bilateral radiographs demonstrated that the Kirschner wire, originally from the right shoulder, was on the left side. The patient then underwent a successful surgery to remove the implant.


Resumo O uso dos fios metálicos, denominados fios de Kirschner, é um método de fixação simples e eficaz para a correção de fraturas e luxações do ombro na cirurgia ortopédica. Uma das possíveis complicações é a migração do fio durante o acompanhamento pósoperatório. Os autores apresentam um caso de um paciente masculino de 48 anos, administrador, que sofreu uma queda de mesmo nível com trauma em ombro direito durante uma partida de futebol. Atendido emumhospital de referência de ortopedia e traumatologia, foi diagnosticada luxação acromioclavicular grau V. Quatro dias após o trauma, fez-se o tratamento cirúrgico da luxação acromioclavicular com amarrilhos com fios de âncora, transferência do ligamento coracoacromial e fixação com fio de Kirchner do acrômio à clavícula. No retorno, 12 dias após o procedimento cirúrgico, identificou-se amigração do fio de Kirschner do bordo do acrômio. Apesar de orientado a se submeter a cirurgia para remoção do fio, o paciente se recusou.Novemeses após o tratamento cirúrgico, o paciente apresentou dores no ombro esquerdo (lado contralateral), dificuldade para mobilizar o ombro, equimose e saliência. Foram feitas radiografias bilaterais e foi constatado que o fio de Kirschner, originalmente no ombro direito, estava no ombro contralateral. Fez-se então cirurgia para remoção do implante, com sucesso.

ABSTRACT

La perforación concomitante de esófago y aorta se puede presentar después de la ingestión de cuerpos extraños. El reparo aórtico por técnica endovascular, a pesar de ser reciente, es un tratamiento de primera línea por tratarse de un abordaje poco invasivo, rápido y que permite la estabilización hemodinámica, en comparación con la reparación abierta tradicional. Se presentan dos casos de perforación aórtica, en los cuales se llevó a cabo el reparo endovascular con éxito. El primer paciente sufrió una ruptura contenida de la aorta torácica, secundaria a la ingestión de un cuerpo extraño (espina de pescado), y presentó mediastinitis. El segundo paciente sufrió una ruptura aórtica en el arco distal a la arteria subclavia, la cual se corrigió por vía endovascular, pero desarrolló una fístula aorto-esofágica y, finalmente, murió


Concomitant esophageal and aortic perforation has been described in the literature as major complications of foreign body ingestion. Although it has not been widely studied, aortic endovascular repair is the first line of treatment, for it is less invasive, faster and allows early patient stabilization, as compared with the traditional open repair. We present two cases managed successfully with endovascular repair of the aortic perforation. The first case had a contained rupture of the thoracic aorta caused by the ingestion of a foreign body (fish bone) and developed mediastinitis. The second case had an aortic rupture in the arc distal to the subclavian artery, managed with endovascular but he developed an aortoesophageal fistula which was finally lethal.

ABSTRACT

Los casos de apendicitis aguda debida a cuerpos extraños es una condición muy rara, con una prevalencia de 0,0005 %, en la cual se observa un papel crucial de la obstrucción de la luz apendicular en el inicio de sintomatología. La mayoría de los cuerpos extraños que se ingieren son expulsados sin complicaciones y sin necesidad de intervención quirúrgica, observándose perforaciones en menos del 1 %. Se presenta el caso de un hombre de 25 años de edad con un cuadro clínico de apendicitis aguda que necesitaba de intervención quirúrgica, en la que se halló un cuerpo extraño (gancho 'nodriza'), con perforación proximal apendicular y peritonitis localizada


Cases of acute appendicitis due to foreign bodies are a very rare condition, with a prevalence of 0.0005%, in which an obstruction of the appendiceal lumen is seen as a crucial role in the onset of the symptomatology. The majority of ingested foreign bodies are expelled without complications with no need of surgical intervention; perforations are observed in <1% of cases. We present the case of a 25-year-old man with a clinical acute appendicitis requiring surgical intervention, where a foreign body (safety pin) with proximal appendiceal perforation and localized peritonitis was found

ABSTRACT

Foreign body aspiration in children is a frequent cause of consultation in the pediatric emergency department and can be a life-threatening event. It occurs mainly during the first 4 years of life and is more frequent in males. It is possible to find the clinical triad of cough, localized wheezing and decrease of respiratory noises in 50 percent of cases. The diagnosis is based on an adequate clinical history to detect an asphyxiation event. Chest radiography is an important diagnostic tool although it is possible to find normal results in 12 to 25 percent of cases. Several studies have reported the usefulness of flexible bronchoscopy in extracting foreign bodies; however, rigid bronchoscopy remains the best choice for foreign body aspiration


La aspiración de cuerpo extraño en niños es una causa frecuente de consulta en el departamento de urgencias pediátricas y puede ser un evento potencialmente mortal. Se presenta principalmente durante los primeros 4 años de vida y es mas frecuente en el género masculino. Clínicamente es posible encontrar en un 50 por ciento de los casos la triada clínica de tos, sibilancias localizadas y disminución de ruidos respiratorios. El diagnóstico se basa en una historia clínica adecuada en la cual se busca intencionadamente el antecedente de un evento de asfixia. La radiografía de tórax es una herramienta importante para el diagnóstico aunque es posible encontrarla normal del 12 al 25 por ciento de los casos. Diversos estudios reportan la utilidad de la broncoscopia flexible en la extracción de cuerpos extraños, sin embargo la broncoscopia rígida continua siendo el estándar de oro para el manejo de la aspiración de cuerpo extraño

ABSTRACT

During laparoscopic cholecystectomy, a surgical clip is used to control the cystic duct and cystic artery. In the past, metallic clips were usually used, but over recent years, interest in the use of Hem-o-lok clips has increased. Surgical clip migration into the common bile duct (CBD) after laparoscopic cholecystectomy has rarely been reported and the majority of reported cases involved metallic clips. In this report, we describe the case of a 53-year-old woman who presented with abdominal pain caused by migration of a Hem-o-lok clip into the CBD. The patient had undergone laparoscopic cholecystectomy 10 months previously. Abdominal CT revealed an indistinct, minute, radiation-impermeable object in the distal CBD. The object was successfully removed by sphincterotomy via ERCP using a stone basket and was identified as a Hem-o-lok clip.

ABSTRACT

A 58-year-old man underwent laparoscopy-assisted distal gastrectomy (LADG) with Billroth I gastroduodenostomy due to early gastric cancer. During surgery, the perigastric vessels were ligated with Hem-o-Lok clips. Esophagogastroduodenoscopy (EGD) 6 months later showed a fungating mass at the anastomosis site. Repeat EGD 1 year after LADG showed a Hem-o-Lok clip at the fungating mass lesion. Because the patient was asymptomatic, with no major abnormalities on clinical examination, and endoscopic removal of the clip would have been difficult due to the presence of adhesions and inflammation, no attempt was made to remove the clip. The patient remained well after the exposed Hem-o-Lok clip was identified. A third EGD 6 months later showed that the clip had disappeared from the anastomosis site, and that this site was covered with normal mucosa surrounding the scar.

ABSTRACT

Recently, dermal fillers have been used with increasing frequency to improve facial contours for cosmetic purposes. The appreciable increase in such filler procedures has led to more adverse events and complications. Herein, we report the clinical significance and differential diagnosis of delayed multiple granulomas that occurred in a patient with a history of an injection of an unknown substance as a filler 20 years previously. She was also taking oral steroids and tacrolimus for immune suppression after a kidney transplant that she received 10 years before she presented with granulomas.

OBJECTIVE: The aim of this study was to evaluate the diagnostic value of integrated 18F-fluoro-2-deoxyglucose-positron emission tomography/computed tomography (18F-FDG-PET/CT) for suspected recurrence of epithelial ovarian cancer (EOC) with non-disseminated lesions. METHODS: We retrospectively reviewed the medical records of recurrent EOC patients who underwent secondary cytoreduction from January 2000 to December 2013. A total of 134 patients underwent secondary cytoreduction after imaging with either 18F-FDG-PET/CT or contrast-enhanced computed tomography (CECT). RESULTS: In a patient-based analysis of 134 patients, 124 (92.5%) were confirmed to be positive for malignancy. Among 72 patients with suspected non-disseminated recurrence on 18F-FDG-PET/CT, 65 (89.0%) were confirmed to have recurrence, giving 98.5% sensitivity, 87.7% accuracy, and 88.9% positive predictive value (PPV). In the 65 patients with recurrence, residual tumor remained in 14 patients, giving an accuracy of patient selection for secondary cytoreduction of 69.4% (50/72) and it is higher than that of CECT (64.0%). In 169 lesions removed from patients who underwent preoperative 18F-FDG-PET/CT, 135 (79.9%) were confirmed to be positive for malignancy and 124 were accurately detected by 18F-FDG-PET/CT, giving 91.9% sensitivity, 81.1% accuracy, and 85.5% PPV. Foreign body granuloma was found in 33.3% of 21 lesions with false-positive 18F-FDG-PET/CT findings (7/21). The mean preoperative cancer antigen 125 (CA-125) level in false-positive patients was 28.8 U/mL. CONCLUSION: Compared with CECT, 18F-FDG-PET/CT shows higher sensitivity in lesion-based analysis and better accuracy of patient selection for secondary cytoreduction. However, there is still a need for integration of the results of 18F-FDG-PET/CT, CECT, and CA-125 levels to aid treatment planning.

Abstract Embolization by a dislodged projectile is a rare complication that may occur in cases of gunshot cardiac injuries. We report a case of a firearm projectile cardiac injury that evolved, with dislocation of the projectile during cardiac surgery, into embolization of the right external carotid artery.

Imperícia durante a prática odontológica pode, ocasionalmente, gerar iatrogenias durante o ato transcirúrgico, tais como a deglutição ou aspiração de materiais dentários ou instrumentos de uso diário. A falta de planejamento, falhas humanas durante o tratamento, diagnóstico incorreto e acidentes podem vir a se tornar potenciais complicações ao paciente, que além da morbidade podem causar complicações secundárias, podendo até mesmo levar o indivíduo a óbito. É importante o clínico reconhecer a sintomatologia apresentada pelo paciente e tomar atitudes cabíveis no momento em que a complicação é estabelecida, desde os níveis mais baixos de suporte até os mais complexos, proporcionando conforto físico e emocional, com o intuito de evitar problemas imediatos ou futuros à saúde como um todo


Malpractice during dental practice may occasionally generate iatrogenies during the transsurgical act, such as swallowing or aspiration of dental materials or instruments for daily use. Lack of planning, human failures during treatment, incorrect diagnosis and accidents may become potential complications for the patient, which in addition to the morbidity can cause secondary complications and may even lead to death. It is important for the clinician to recognize the symptoms presented by the patient and to take appropriate actions when the complication is established from the lowest levels of support to the most complex, providing physical and emotional comfort in order to avoid immediate or future health problems as a whole
