ABSTRACT

Osteonecrosis in isolated fractures of the acetabulum without dislocation of hip seems to be a known complication, but to our knowledge it has not been reported adequately. The causative nature of post-traumatic femoral head osteonecrosis has not been studied critically. The pathophysiology of osteonecrosis in this case also eludes us. Striking evidence points towards the intra-operative blood loss and low mean arterial pressure possibly leading to hypo-perfusion of femoral head leading to osteonecrosis. Fractures of the acetabulum pose a difficult problem for the patient and the surgeon because of possible complications. Thus any surgeon involved in surgery for fractures of the acetabulum should be aware of the possibility of this potential complication. Here is a 61-year male, who sustained a complex fracture of the acetabulum without hip dislocation, subsequently was treated surgically with internal fixation using an anterior approach, 10 months after surgery patient developed osteonecrosis of the femoral head.

ABSTRACT

Fundamento: los traumatismos dentarios representan la segunda causa de atención odontopediátrica que afecta la población menor de 19 años. Objetivo: identificar el traumatismo dentario en la población de 15 a 18 años de la Escuela Militar Camilo Cienfuegos de Sancti Spíritus. Metodología: se realizó un estudio descriptivo. La población fueron 417 estudiantes, de ellos se obtuvo una muestra de 121 adolescentes con traumas dentarios, con previo consentimiento informado. Las variables: edad, sexo, etiología y tipo de trauma, así como el diente afectado, factores de riesgo y el tratamiento recibido. Para el procesamiento de la información se utilizó la frecuencia absoluta y relativa. Resultados: el grupo de edad más afectado fue el de 15-16 años con predominio del masculino. El diente más afectado fue el incisivo central superior derecho y la fractura no complicada de corona tuvo mayor incidencia seguido de la infracción o infractura del esmalte. Conclusiones: los traumatismos dentales se presentaron en la mayoría de los estudiantes de 15-18 años que a su vez no recibieron atención estomatológica lo que afecta la conservación del órgano dentario.


Backgroud: Dental traumas represents the second cause of pediatric dentistry care affecting the population under 19 years old. Objective: to identify the dental traumas in the population between 15-18 years old at Camilo Cienfuegos Military School from Sancti Spíritus city. Methodology: a descriptive study was carried out. The population was composed by 417 students with a simple of 121 adolescents with dental trauma with a previous consent. The variables were age, sex, etiology and type of trauma as well as the tooth affected, risk factors and treatment received. For the processing of the information the absolute and relative frequency were used. Results: the age group mostly affected was between 15-16 years old, prevailing males. The most affected tooth was the central right upper incisive and the non complicated fracture of crown had the most incidence followed by the infraction of the enamel. Conclusions: Dental traumas were present in the majority of the students between 15-18 years old and at the same time they did not receive dental care which affects greatly the preservation of the dental organ.

@#Most of patients received in hospital with the upper and lower extremity fracture are caused by the earthquake.These fractures have apparent characters,such as:the serious situation,complexity and lots of complications which can be divided two periods:the early and the late.These complications include shock,the nerve and vascular injury,the acute kidney failure,fat embolism,infection,deep venous thrombosis,decubitus ulcer and so on.These complications should be known completely and the therapy should be given early.Practice can not be taken because of pain and some other reasons,and this will lead to the muscle atrophy,adynamia and the joint disfuction.Finally the abilty to work was lost partly or completely.If some measures about rehabilitation were taken early,we can reduce the complication and the disability possibly.
