Index weight analysis of long-term care insurance disability rating scale / 中国卫生政策研究

Objective:To establish index weight system of the disability rating scale for the long-term care in-surance. Methods:On the basis of the index system of disability rating scale constructed by Delphi method,the im-portance of each index of the scale was evaluated and its weight was calculated by analytic hierarchy process. Re-sults:The weights of the 4 first-class indicators in the long-term care insurance disability rating scale were as follows:sensory perception was 0.2408,basic activities of daily living was 0.5899,cognitive ability was 0.1204, emotional& behavior was 0.0489;and the weights of 22 second-class indicators were calculated simultaneously. Conclusions:The determination of the index weight further improves the system of disability rating system,and provides reference for the development of long-term care insurance system.

Evidence-based research on the selection method system for China's essential medicine list / 中华医院管理杂志

Objective To find a selection method system appropriate for China's essential drug list.Methods Collection and analysis of technical paper literature on essential drug list developement,adjustment technical papers,and literature on evidence-based medicine,pharmacoeconomics evaluation,and medical insurance budget analysis from WHO and other countries.On such basis,a method system for selecting China's essential medicine list can be proposed,with its feasibility analyzed and demonstrated.Results The GRADE assessment proposed by WHO was used to evaluate the efficacy and safety of the drug;the pharmacy economics evaluation is added to assess the economic efficiency;these were aided by the pharmacoeconomics evaluation for a comprehensive evaluation of the pharmacoeconomics of the drug in question,supplementing analysis of the medical insurance,and ensuring its affordability of essential medicine so selected.The theory of this system is well developed,and supporting software ready for application.Examples in Zhejiang Province regarding essential medicine for diabetes have proved this method feasible.Data acquisition constitutes a major roadblock for scientific selection due to barriers against medical insurance data sharing.Conclusions Theoretic basis and method tools are fully available for essential medicine selection,and the selection method system as proposed in this study prove feasible to some extent.Decision makers are recommended to scientifically select the essential drugs,and to encourage data sharing,in order to make the list more scientific and pragmatic.

Study on disabled grade assessment scale for long-term care / 重庆医学

Objective To explore the establishment of disabled grade assessment scale for long-term care insurance (LTC).Methods Based on the results of literature review and long-term care needs survey,an initial scale of disabled grade assessment was formulated,then the Delphi method was combined to conduct the consultation on 22 specialists.Results After two rounds of expert consultation,the unanimous disabled grade assessment scale included four first-class indexes of sensation perception,daily activity ability,recognition ability and emotion behavior,the importance mean (M) was 4.64-5.00 points,which ≥4.0 points,the variable coefficient (CV) was 0.00 ％-13.86 ％,which ≤25 ％,the full mark rate (Fr) was 72.73 ％-100.00 ％,which ≥50 ％,recognition rate (Ar) was 90.91％-100.00％,which ≥80％,which indicated that the experts reached a consensus opinion.Among the 22 second-class indexes,the M was 4.27-4.95 points,which ≥4.0 points,CV was 4.20％-25.62％,Fr was 63.64％-95.46％,Ar was 77.27 ％-100.00 ％.The two items of touch sense and getting up from sitting position were newly added,the transferring of table and chair,recent memory and procedural memory were modified into sitting down on the bench and chair,immediate memory and short term memory,the experts reached the consistent opinion.Conclusion The replenished and modified disabled grade assessment scale for LTC has higher authority and expert consistency,which can provide a theoretical reference for the disabled grade assessment work of LTC.

The hospital grade assessment is an effective approach to measure the comprehensive strength and the overall level of a hospital, and it is also an effective carrier which can promote the standardized management , improve the quality of connotation and accelerate the development of the hospital .This article aims to expound the effective and concrete methods for preparation in the hospi -tal grade assessment by introducing the practice and experience of Jinling Hospital in grade assessement .It also points out that:defi-ning the purpose of the assessment is the fundation , having a thorough grasp of the assessment criteria is the basis , having a compre-hensive knowledge of the hospital is the premise , establishing and implementing the plan strictly is the crux .All these might be helpful to promote the hospital grade assessment .
