ABSTRACT

Introduction: Garre’s osteomyelitis is mainly caused due to long standing periapical infection, recent infection of soft tissue, unhealed infected extraction areas etc. It mainly affects the mandible than the maxilla and is seen rarely in children. Case presentation: 10 year old patient with a long standing untreated dental infection of lower left molar is treated with root canal treatment. Inconsistent visit of the patient lead to the progression of the pathology to Garre’s osteomyelitis. Management and prognosis: Extraction of the offending tooth followed by targeted antibiotic therapy after a culture and sensitivity test. Lesion completely healed at the end of 2 months and skin changes started to appear. Conclusion: Management of osteomyelitis is children are crucial as they are in the growing phase. Timely management is nec- essary to reduce the deleterious effects caused by it in the bone.

ABSTRACT

Introduction: Garrè's osteomyelitis is a rare, chronic infection associated with proliferative periostitis that induces reactional bone neoformation. Objective: To report the clinical case of successful treatment of Garre's osteomyelitis in a young patient. Case report: An 11-year-old girl visited a dental clinic with complaints of volume increase in the mandibular border accompanied by pain and facial asymmetry. Computed tomography revealed extensive tooth destruction, a periapical lesion, and several radiopaque bone laminations parallel to the mandible cortex, all findings suggestive of Garrè's osteomyelitis. The patient was treated by tooth removal and antibiotic therapy, and the condition was completely resolved, with decreased facial asymmetry at the 6-month follow up. Conclusion: Once the diagnosis of Garrè's osteomyelitis is established and proper treatment is provided, the prognosis tends to be extremely favorable and bone remodeling should be considered until the affected surface returns to its normal anatomy.


Introdução: A osteomielite de Garrè é um tipo raro e crônico de infecção óssea associado a periostite proliferativa que induz neoformação óssea reacional. Objetivo: relatar o caso clínico de osteomielite de Garrè em uma paciente jovem tratada com sucesso. Relato de caso : Paciente do gênero feminino, 11 anos compareceu ao serviço odontológico com queixa de aumento de volume em borda de mandíbula acompanhada de dor e assimetria facial. A tomografia computadorizada revelou extensa destruição dentária associada a lesão periapical e a presença de várias laminações ósseas radiopacas paralelas ao cortical da mandibula, sugerindo o diagnóstico de osteomielite de Garrè. O caso foi tratado com a remoção do elemento dentário associado a antibioticoterapia, o que resolveu completamente a infecção e diminuiu a assimetria facial conforme constatado após 6 meses de proservação. Conclusão: Uma vez estabelecido o diagnóstico de osteomielite de Garrè, e a infecção adequadamente tratada, o prognóstico tende a ser extremamente favorável e a remodelação óssea deve ocorrer continuamente até que a superfície retorne à sua anatomia normal.

ABSTRACT

Garre’s sclerosing osteomyelitis is a specific type of chronic osteomyelitis that mainly affects children and young adults. This disease is commonly associated with an odontogenic infection resulting from dental caries. This article describes a case report of 14 year old boy with Garre’s osteomyelitis of mandible caused by infection of lower left first molar. The case was managed by non- surgical endodontic treatment and the healing was achieved.

Introducción: La osteomielitis esclerosante crónica de Garré (OECG) es una entidad rara que afecta a niños y adultos jóvenes. El proceso de OEG se caracteriza por una respuesta osteoblástica marcada, que en general puede aparecer en huesos largos o mandíbula, si bien la descripción original fue en mandíbula. Caso clínico: Masculino de 14 años con historia de tumefacción en cara de una semana de evolución con signos inflamatorios y drenaje espontáneo de líquido purulento. Inicialmente fue tratado con antibióticos, debido a la presencia de un área de celulitis. Además se realizó drenaje del área abscedada y exodoncia. No hubo aislamiento microbiológico posterior. Dada la mala respuesta a la medicación antibiótica, se realizó una gammagrafía ósea que arrojó por resultado la hipercaptación de la rama maxilar, confirmándose luego el diagnóstico de OEG en la biopsia de hueso. Discusión: Pocos casos clínicos de Osteomielitis Crónica de Garré han sido reportados. La mayoría de los reportes coinciden que la OECG afecta a niños y adultos jóvenes. Las localizaciones más frecuentes son las regiones metafisiarias de los huesos largos y la mandíbula. La causa se desconoce, pero los hallazgos microscópicos suponen el posible origen infeccioso .

ABSTRACT

O presente artigo objetivou atualizar o tema e relatar dois casos clínicos de Osteomielite de Garré em crianças. No primeiro caso clínico, havia história de cárie no dente 36, extraído havia 10 dias. Extrabucalmente observava-se tumefação dolorida em região submandibular esquerda. A radiografia oclusal evidenciou expansão da tábua óssea vestibular em camadas com aspecto de “casca de cebola”. O tratamento instituído foi antibioticoterapia após cultura e antibiograma. No segundo caso havia tumefação em região submandibular esquerda e restauração metálica no dente 36 com imagens radiolúcidas periapicais circunscritas. Foi realizado tratamento endodôntico e antibioticoterapia. A condição evoluiu favoravelmente em ambos os casos, não havendo sinais de recidiva após 3 meses.

ABSTRACT

Garré's osteomyelitis is generally considered to be synonymous with chronic osteomyelitis with proliferative periostitis and occurs most commonly in the first molar region of the mandible. We report a case of Garré's osteomyelitis caused by the infected tooth-germ of a wisdom tooth. A 12-year-old boy had a swelling of the right cheek and his right mandibular second molar was covered by gingiva with pus retention. X-ray examination showed a radiolucent area around the impacted tooth-germ of the wisdom tooth and extracortical new bone at the angle of the mandible. After preoperative treatment with antibiotics, the tooth-germ and extracortical bone were removed. The antibiotics treatment was continued for 18 days postoperation. No recurrence of pain or swelling has been observed thereafter.
