Resumen OBJETIVO: Estimar la cantidad de pacientes con irregularidades menstruales que se embarazaron luego de una cirugía bariátrica (manga gástrica) y describir los desenlaces de los embarazos. MATERIALES Y MÉTODOS: Estudio retrospectivo de serie de casos de pacientes obesas y con irregularidades menstruales intervenidas mediante cirugía bariátrica (manga gástrica). Se operaron entre el 1 de junio de 2014 y el 31 de diciembre de 2019 en el Centro Médico Imbanaco Cali, Colombia. RESULTADOS: De 1418 cirugías bariátricas, tipo manga gástrica, 117 pacientes cumplieron con los criterios de inclusión, 80 de ellas se localizaron telefónicamente y aceptaron incluirse al estudio. Luego de la manga gástrica, el 86.25% (n = 69) de las pacientes logró la regularización de sus ciclos menstruales. Además, el 31% (n = 25) consiguió el embarazo, 80% (n = 20) de ellas tenían la intención de buscar el embarazo. Los desenlaces de esos embarazos fueron favorables, con partos a término, con adecuado peso y talla al nacer. CONCLUSIONES: Después de la cirugía bariátrica, tipo manga gástrica, en mujeres con irregularidades menstruales, 25 de las 80 pacientes se embarazaron y los desenlaces obstétricos fueron satisfactorios. Sin duda, hacen falta más estudios prospectivos para poder afirmar, con una mejor representación estadística, las repercusiones de la disminución del peso corporal en la tasa de fertilidad.


Abstract OBJECTIVE: To estimate the number of patients with menstrual irregularities who became pregnant after bariatric surgery (gastric sleeve) and to describe the pregnancy outcomes. MATERIALS AND METHODS: Retrospective study of a series of cases of obese patients with menstrual irregularities who underwent bariatric surgery (gastric sleeve). They were operated between June 1, 2014 and December 31, 2019 at the Imbanaco Cali Medical Center, Colombia. RESULTS: Of 1418 bariatric surgeries, gastric sleeve type, 117 patients met the inclusion criteria, 80 of them were located by telephone and agreed to be included in the study. After gastric sleeve, 86.25% (n = 69) of the patients achieved regularization of their menstrual cycles. In addition, 31% (n = 25) achieved pregnancy, 80% (n = 20) of them intended to seek pregnancy. The outcomes of these pregnancies were favorable, with full-term deliveries, with adequate weight and height at birth. CONCLUSIONS: After bariatric surgery, gastric sleeve type, in women with menstrual irregularities, 25 of the 80 patients became pregnant and the obstetric outcomes were satisfactory. Undoubtedly, more prospective studies are needed to be able to affirm, with a better statistical representation, the repercussions of the decrease in body weight on the fertility rate.

ABSTRACT

La hemorragia digestiva determinada por una fístula entre manga gástrica y seudoaneurisma de arteria esplénica o polo superior de bazo es infrecuente. Se presenta un caso clínico de una paciente de 52 años con antecedentes de cirugía de manga gástrica y fuga anastomótica. Ingresó por hemorragia digestiva alta grave. Se operó de emergencia y realizó punto hemostático sobre cara posterior de manga gástrica. Se reintervino por resangrado realizándose la gastrectomía y esplenopancreatectomía distal por solución de continuidad de arteria esplénica. Dada la inestabilidad hemodinámica se efectuó un esofagostoma y yeyunostomía, reconstruyéndose a los 8 meses con buena evolución.


Gastrointestinal bleeding caused by a fistula between the gastric sleeve and a pseudoaneurysm of the splenic artery or upper pole of the spleen is uncommon. A clinical case of a 52-year-old patient with a history of gastric sleeve surgery and anastomotic leak is presented. She was admitted for severe upper gastrointestinal bleeding. She underwent emergency surgery and performed a hemostatic stitch on the posterior face of the gastric sleeve. She underwent reoperation due to rebleeding, performing gastrectomy and distal splenopancreatectomy due to discontinuation of the splenic artery. Given the hemodynamic instability, an esophagostomy and jejunostomy were performed, reconstructing at 8 months with good evolution.


O sangramento gastrointestinal causado por uma fístula entre a manga gástrica e um pseudoaneurisma da artéria esplênica ou pólo superior do baço é incomum. Apresenta-se o caso clínico de um paciente de 52 anos com história de cirurgia de manga gástrica e fístula anastomótica. Ele foi internado por hemorragia digestiva alta grave. Uma operação de emergência foi realizada e um ponto hemostático foi realizado na face posterior da manga gástrica. Foi reoperado por ressangramento, realizando gastrectomia e esplenopancreatectomia distal por descontinuação da artéria esplênica. Dada a instabilidade hemodinâmica, foi realizada esofagostomia e jejunostomia, reconstruindo aos 8 meses com boa evolução.

ABSTRACT

El objetivo de este estudio es revisar una de las opciones quirúrgicas para el tratamiento de la obesidad en pacientes con situs inversus total. Se presenta el caso de una paciente de 39 años de edad con situs inversus total (diagnóstico transquirúrgico) que fue sometida a gastrectomía en manga laparoscópica, con resultados postquirúrgicos óptimos; además, en la revisión de la literatura encontramos 14 casos publicados a nivel mundial de procedimientos quirúrgicos metabólicos.


The objective of this study is to perform a review of one of the surgicaloptions for the treatment of obesity in patients with total situs inversus. We present the case of a female patient of 39 years of age with total situs inversus (trans-surgical diagnosis) and who underwent a laparoscopic sleeve gastrectomy, with optimal postoperative results, in addition a review of the literature is made, finding 14 cases published to global level of metabolic surgical procedures.

ABSTRACT

Gastrointestinal stromal tumors (GISTs) are the most common non-epithelial tumors of the gastrointestinal tract. The most usual location is the stomach, followed by the small intestine, where it may cause digestive bleeding and anemia. Surgical resection of the tumor is the gold standard treatment, and definitive diagnosis is based on immunohistochemical analysis of the surgical specimen. We report the case of a 53-year-old man with gastric GIST presenting with endophytic and exophytic growth, located at the posterior wall of the stomach, in the antrum-body transitional zone, treated with gastric sleeve. Background: Bariatric surgery is seen as one of the most successful option for the control of morbid obesity and obesity related complications. Although, there are many surgical options available the Lincey Alexida, Xiaohua Qi, Patrick B. Asdell, José M. Martínez Landrón, Samarth B. Patel, Faustino Allongo. Frederick Tiesenga. Laparoscopic Adjustable Gastric Band as a Revision Surgery for Failed Vertical Gastric Sleeve or Roux-en-Y Gastric Bypass. IAIM, 2017; 4(12): 37-42. Page 38 laparoscopic vertical sleeve gastrectomy and the Roux-en-Y gastric bypass are among the most selected treatment methods. Even though the sleeve gastrectomy and the gastric bypass has proven to be efficacious in weight reduction in morbid obesity there are still some reported failures. Aim: This study main objective is to see if an adjustable gastric band can be offered as a sensible option for patients who have had a previously failed vertical sleeve gastrectomy or the Roux-en-Y gastric bypass surgery. The procedure will be classified as a revision surgery to increase the possibilities of additional weight loss not achieved with the sleeve gastrectomy or gastric bypass surgery alone. Method: A retrospective review of the charts for all the bariatric patients from April 2012 to April 2017 was conducted. The chart review yielded 12 patients who underwent either adjustable band over a previously failed vertical sleeve procedure or the adjustable band over a previously failed Roux-enY gastric bypass procedure. The patients were divided into two groups, group A and B. Group A is comprised of the 8 out 12 patients who had a previous failed gastric sleeve procedure. Group B has the remaining 4 patients who had a failed gastric bypass procedure. Observation: Both groups who underwent adjustable gastric band laparoscopically as a revision procedure after a two year follow up appointment showed Group A had a mean estimate weight loss of 30.75 lbs. (11%) with a mean Body Mass Index of 40.7 kg/m2 and Group B had a mean estimate weight loss of 42 lbs. (15%) with a mean Body Mass Index of 36.77 kg/m2 . Group A had an Excess Body Weight Loss of 27% at 1 month and 33% at the 2 year follow up and Group B had an Excess Body Weight Loss of 42.2%. Group B had an Excess Body Weight Loss of 15.1 % at 1 month and 42.2% at the 2 year follow up. Conclusion: Group A had an average of 27% Excess Body Weight Loss and Group B had an Excess Body Weight Loss of 15.1% after 1 month on follow up. Our study was limited by the small sample size. We suggest that further investigational studies, with greater and more diverse sample sizes, be conducted to assure the benefit of using the adjustable gastric band as a revision surgery. Based on our results we conclude that the adjustable gastric band as a revision surgery is a promising and sensible alternative treatment option for patients with a previously failed laparoscopic vertical sleeve gastrectomy or a failed Roux-en-Y gastric bypass.

La obesidad se ha convertido en la epidemia del siglo XXI, incrementado las patologías asociadas a la misma como: diabetes mellitus tipo 2, síndrome metabólico, hipertensión arterial, enfermedades cardiovasculares, entre otras, afectando negativamente la calidad y expectativa de vida. La manga gástrica ha ganado popularidad en el tratamiento quirúrgico primario de la obesidad. Pacientes y métodos: Se realizó un estudio prospectivo con 730 casos sometidos a manga gástrica en el Instituto Clínico La Florida años 2006 - 2012. Se analizó: sexo, edad, índice de masa corporal, porcentaje de exceso de peso perdido, tiempo quirúrgico, complicaciones, y respuesta postoperatoria de pacientes diabéticos 2 e hipertensos. Resultados: Femenino 533 (73%), masculino 197 (27%), edad promedio 40,1 años, tiempo quirúrgico 79 min, índice masa corporal promedio pre-operatorio 40,7; promedio postoperatorio 28,6 en 60 meses. Promedio pérdida del exceso de peso 72,1 % en 60 meses; complicaciones: disrupción de la línea de engrapado 1,09 %, Sangrado 1,2 %, neumonía 0,5 %, deshidratación 1,3 %, infección herida 5 casos (0,6%), estenosis 0 %, obstrucción 0 %, conversiones: 0 %, mortalidad: 0 %. De 42 pacientes diabéticos remisión en 34 (81 %) y mejoría en 8 (19 %) y 112 pacientes hipertensos remisión en 70 (62,5 %), mejoría 28 (25 %) y sin respuesta 14 (12,5 %). Conclusión: Se concluye que la manga gástrica es una técnica segura y efectiva para el tratamiento primario de la obesidad y la resolución de comorbilidades como la diabetes 2 e hipertensión arterial(AU)


Obesity has become the epidemic of the 21st century, increasing pathologies associated with it such as diabetes mellitus type 2, metabolic syndrome, hypertension, and cardiovascular diseases, among others, negatively affecting the quality of life and life expectancy. Sleeve gastrectomy has gained popularity in the primarily surgical treatment of obesity. Patients and methods: A prospective study of 730 cases that underwent sleeve gastrectomy was performed at the Instituto Clínico la Florida from 2006 to 2012. Gender, age, corporal mass index, percentage of excess of weight loss, duration of surgery, complications, postoperative reaction of diabetes-2 and hypertensive patients were analyzed. Results: 533 females (73%), 197 males (27%), Age Average: 40.1 years, surgery duration: 79 minutes; corporal mass index: preoperative average: 40.7; post-operative average: 28.6 in 60 months. Average weight excess loss: 72.1 % in 60 months. Complications: disruption of staple line 1.09 %, bleeding: 1.2%, pneumonia: 0.5 %, dehydration: 1.3 %, wound infection 5 cases (0.6 %), stenosis 0 %, obstruction 0 %, conversions: 0 %, mortality: 0 %. In 42 diabetic patients, 34 were in remission (81 %), 8 showed improvement (19 %). In 112 hypertensive patients, 70 were in remission (62.5 %), 28 showed improvement (25 %) and 14 showed no results (12.5 %). Conclusion: It is concluded that sleeve gastrectomy is a safe and effective technique for the primary treatment of obesity and the resolution of comorbidities like diabetes-2 and hypertension(AU)
