ABSTRACT

Lipoma is a rare mesenchymal tumour of stomach (less than 1% of gastric tumours) to present as gastrointestinal bleed. We report a case of upper gastrointestinal bleed from a gastric lipoma in a 42 year male patient who underwent distal gastrectomy for resection of the large submucosal tumour situated in antropyloric region. Most common age of presentation of gastric lipoma is fifth or sixth decade of life and most of these are located in submucosal plane (90% cases) and in antropyloric region (75% cases). Gastric lipomas can be diagnosed by endoscopic means but most often with CT scan which shows characteristic fat attenuation. Small asymptomatic incidentally diagnosed can be safely observed while larger symptomatic tumours are treated by endoscopic or surgical resection which offers cure from this benign lesion.

El lipoma gástrico (LG), es un tumor benigno raro que representa el 5 % de los lipomas gastrointestinales y menos del 1 % de todos los tumores gástricos. Se localizan habitualmente a nivel de la submucosa y generalmente en la región antral. La endoscopia y la tomografía computarizada son las herramientas diagnósticas más utilizadas en el proceso diagnóstico. Dada su relativa rareza, quedan a menudo fuera del diagnóstico diferencial de las masas submucosas gastrointestinales superiores. El objetivo de este manuscrito, es reportar dos casos tratados de forma consecutiva y revisar la literatura existente en esta materia. Se trata de dos pacientes de sexo masculino, de 49 y 69 años de edad, a quienes se diagnosticó una lesión submucosa gástrica por endoscopia y fueron tratados quirúrgicamente y cuyos especímenes fueron estudiados histológicamente, concluyéndose en ambos casos la existencia de un LG. Ambos pacientes evolucionaron de forma correcta y permanecen en controles clínicos y endoscópicos hasta la actualidad, sin inconvenientes. El LG es una entidad poco común, que puede simular una enfermedad maligna. Presentamos dos casos tratados quirúrgicamente con resultados satisfactorios.


Gastric lipoma (GL), is a rare benign tumor that represents 5 % of gastrointestinal lipomas and less than 1 % of all gastric tumors. They are usually located at the level of the submucosa and usually in the antral region. Endoscopy and computed tomography are the diagnostic tools most used in the diagnostic process. Given their relative rarity, they are often left out of the differential diagnosis of the upper gastrointestinal submucosal masses. The aim of this manuscript was to report two cases treated consecutively and review the existing literature on this subject. Two male patients, 49 and 69 years of age, were diagnosed with a gastric submucosal lesion by endoscopy and were treated surgically. The specimens were studied histologically; concluding the existence of a GL in both cases. The two patients evolved adequately and have remained in clinical and endoscopic controls until now, without problems. GL is a rare entity, which can simulate a malignant disease. We present two cases treated surgically with satisfactory results.

Los lipomas gástricos representan el 5% de los lipomas gastrointestinales y entre el 1 a 3% de los tumores gástricos benignos. Suelen cursar asintomáticos y ser hallazgos incidentales o pueden manifestarse de distintas maneras, presentando desde dispepsia y dolor abdominal hasta hemorragia digestiva e intususcepción. La gastroscopia, TC y la endosonografía son útiles para el diagnóstico de estos lipomas. En caso de existir síntomas, se puede considerar la enucleación quirúrgica o endoscópica de la lesión. Gastric lipomas represent 5% of gastrointestinal lipomas, and between 1-3% of benign gastric tumors. They use to be asymptomatic and incidentally found, or may be manifested in different manners, presenting even dyspepsia and abdominal pain and up to digestive hemorrhage and intussusception. Gastroscopy, CT, and endosonography are useful for diagnosing these lipomas. In case of existing symptoms, surgical or endoscopic enucleation of the lesion might be considered. ABSTRACT

Gastric lipoma is a typical benign submucosal tumor that is usually asymptomatic and it is generally detected incidentally when performing upper GI endoscopy. However, depending on its size and location, an atypical gastrointestinal lipoma can cause abdominal pain, diarrhea, constipation, intestinal obstruction, intussuception and life-threatening gastrointestinal bleeding. This tumor is diagnosed and differentiated from other malignant and submucosal tumors on the basis of its characteristic findings at endoscopy and on computed tomography, magnetic resonance imaging and endoscopic ultrasonography. We report here on the case of a 58-year-old female with epigastric discomfort and melena; a 4.5x4 cm ulcero-fungating mass was detected on the anterior wall of the gastric antrum. Surgical subtotal gastrectomy was performed and the lesion was diagnosed as gastric lipoma.

ABSTRACT

Gastric lipomas account for less than 3% of benign gastric tumor arising from the submucosal layer. Gastric lipomas are usually asymptomatic, but occasionally diagnosed from epigastric pain, obstruction and bleeding by their size and location. A 68-year-old female with melena was diagnosed as gastric lipoma, having a bleeding focus at endoscopy. Endoscopy with biopsy revealed a yellowish fat containing lipoma which was located prepyloric antrum and protruded to the duodenal lumen, and synchoronous adenocarcinoma which was located at the gastric angle and distal body. Adenocarcinoma was confined to the mucosa and seperated from lipoma by normal stomach tissue. The patient received Billroth-II subtotoal gastrectomy, and gastric lipoma and early gastric cancer were resected, completely.

ABSTRACT

Gastric lipomas are rare benign submucosal tumors which can present the diagnostic and therapeutic problems. The preferred treattment is observation or local excision. Since preoperative diagnosis and differentation from malignant tumors can be difficult by use of the conventional diagnostic modalities such as X-ray or endoscopic examination, patients are sometimes subjected to more extensive surgical procedures than warranted. As to the diagnosis of submucosal tumors of the upper digestive tract, a newly developed diagnostic modality of endoscopic ultrasonography(EUS) allows us to visuialize the structures underlying the gastrointestinal wall in a noninvasive maneuver, and can contribute to make differential diagnosis and decision of management. Strip biopsy is an endascopic tissue resection technique which permits resection of both mucosal and submucosal tissue regardless of the morphological type of the lesion, because the submucosal saline injections during the procedures make the flat or depressed lesions to be elevated. Now, we report a 56-yr-old female which had a Yamada type I polypoid gastric submucosal lipoma in the antrum, which could be performed the different diagnosis and successful endoscopic removal by endoscopic ultrasonography and strip biopsy.
