Tumores de células gigantes en el antebrazo / Tumors of giant cells in the forearm

Se presentan los casos clínicos de 2 pacientes atendidos en la consulta de Ortopedia del Hospital General Docente Dr Juan Bruno Zayas Alfonso de Santiago de Cuba. Uno había sido operado de un tumor de células gigantes en el tercio proximal del radio izquierdo hacía 3 años, con evolución satisfactoria hasta que comenzó a presentar aumento de volumen y dolor en el tercio distal de dicho radio, que limitaba la flexoextensión de la muñeca; el otro había sufrido un traumatismo en el tercio distal del antebrazo izquierdo y presentaba aumento de volumen entre los límites de la cara dorsal y ventral del antebrazo y la muñeca. Se realizó la exéresis de los tumores, con resección en bloque y fijación externa con injerto de hueso del banco destinado a ello en esta provincia. Ambos egresaron sin complicaciones y se indicó seguimiento por ortopedia y oncología.

The case reports of 2 patients assisted in the out-patient Orthopedics Deparment of Dr Juan Bruno Zayas Alfonso Teaching General Hospital in Santiago de Cuba are presented. One had been surgically treated 3 years ago due to a giant cells tumor in the proximal third of the left radius, with a satisfactory clinical course until it began to present volume increase and pain in the distal third of his radio that limited the wrist's flexoextension; the other patient had suffered a traumatism in the distal third of the left forearm and it presented increase of volume between the egdes of the dorsal and ventral sides of the forearm and the wrist. The exeresis to the tumors, with resection in block and external fixation with bank bone implants dedicated to this in the province were carried out. Both patients were discharged without complications and Orthopedics and Oncology follow-up were indicated.

Giant cell tumor of right thumb: a rare case report.

Giant cell tumour of soft tissue (GCT-ST) resembling osseous giant cell tumour is a distinct and uncommon entity. Here we present a 58 year old woman with a 1 year history of swelling located on the base of right thumb diagnosed on histology as giant cell tumour of soft tissue. The surgeon should keep a differential diagnosis as GCT-ST for any swelling of the finger.

Tratamiento del tumor de células gigantes con curetaje e injerto con hidroxiapatita porosa coralina HAP-200® / Treatment of the giant cells tumor with curettage and graft of porous coral hydroxyapatite HAP-200®

Se realizó la presentación de un caso con un tumor de células gigantes en el extremo distal del fémur derecho a nivel del cóndilo femoral interno. Se retira quirúrgicamente, previo estudio, donde se confirmó el diagnóstico a través de la biopsia. En dicho proceder se efectuó un raspado extenso de la lesión respetando la superficie articular del cóndilo femoral, rellenando la cavidad con la hidroxiapatita HAP-200 ®, sin necesidad de apoyar dicho acto con una osteosíntesis interna o externa, sólo con una calza de yeso por 6 semanas. El seguimiento del paciente ha sido, hasta la fecha, de 6 años y no se ha reportado ninguna recidiva o metástasis, con una osteointegración positiva del biomaterial, lográndose la curación ósea y una función articular sin restricción.

We presented a case of a giant cells tumor in the distal end of the right thigh at the level of the internal femoral condyle. Previously studied, the tumor was surgically retired, and the diagnosis was established through the biopsy. In that procedure we carried out a lesion extended scrape respecting the join surface of the femoral condyle, filling the cavity with hydroxyapatite HAP-200®, being unnecessary to support it with an internal or external osteosynthesis, only with plaster wedge for 6 weeks. The patient´s follow-up has lasted for 6 years up to day, and no recidivism or metastasis has been reported, with a biomaterial positive osteointegration, achieving the bone healing and a join functioning without restriction.

Granuloma central de células gigantes / Giant cells central granuloma

El granuloma reparativo central de células gigantes es una lesión proliferativa no neoplásica de etiología desconocida. Se presenta un paciente masculino de 40 años de edad, portador de prótesis parcial superior. Fue remitido al Servicio de Cirugía Maxilofacial del Hospital V I Lenin por presentar aumento de volumen en reborde alveolar superior, de color rojo grisáceo y que provocaba expansión de corticales óseas. Una vez analizados los exámenes clínicos, radiográficos e histopatológicos se diagnosticó un granuloma reparativo central de células gigantes Se realizó exéresis quirúrgica de la lesión y extracción de dientes adyacentes con una evolución satisfactoria sin señales de recidivas luego de tres años del tratamiento. El granuloma reparativo central de células gigantes se presentó como respuesta a un trauma. La correcta interpretación de los datos clínicos, radiográficos e histopatológicos nos permitió llegar al correcto diagnóstico y plan de tratamiento(AU)

Giant-cell central reparative granuloma is non neoplastic proliferative lesion of unknown etiology. We report a 40 years old male patient who was admitted at the Maxillofacial Service of the V I Lenin Hospital. The patient had partial upper prosthesis and was complaining of red-grey volume increase lesion in upper alveolar ridge which led to the expansion of cortical bone. Having analyzed clinical, radiographic and histopathological findings the case was concluded as a giant-cell central reparative granuloma. Surgical exeresis and adjunct tooth extraction were done. After three years of treatment, satisfactory follow up without recurrence is reported(AU)

Tumor de células gigantes del extremo distal del radio y reconstrucción ósea / Tumeur de cellules géantes de l'extrémité distale du radius et réparation osseuse / Giant cells tumor of radius distal end and bone reconstruction

Se presenta el caso de una mujer de 40 años de edad, de la raza negra, con un tumor del extremo distal del radio derecho, con diagnóstico histológico de tumor de células gigantes de bajo grado de malignidad y con la propuesta de amputación del miembro. Se le realizó una cirugía conservadora con exéresis total de la lesión, respetando el margen oncológico, y practicada en dos tiempos. Se utilizó un injerto libre de peroné, se realizó una artrodesis de la muñeca y la fijación interna del injerto con el sistema AO. Se logró la consolidación del injerto y la incorporación activa de la paciente a la sociedad. Se expone el diagnóstico, tratamiento, seguimiento, rehabilitación y pronóstico del caso(AU)

This is the case of a black women aged 40 presenting with a tumor of distal end of right radium with histological diagnosis of low-grade malignancy giant cells tumor and proposal of limb amputation. A conservative surgery was performed with a two-steps total exeresis of lesion sparing the oncologic margin. A fibular free-graft was used and wrist arthrodesis and internal fixation of graft using AO system. There was a good graft consolitadion and an active incorporation of patient to social activities. The diagnosis, treatment, follow-up, rehabilitation and case prognosis are exposed(AU)

Le cas d'une femme âgée de 40 ans, de race noire, atteinte d'une tumeur de l'extrémité distale du radius droit, avec un diagnostic histologique de tumeur de cellules géantes peu maligne, et avec une proposition d'amputation du membre affecté, est présenté. Elle a subi une chirurgie conservatrice par exérèse totale de la lésion, pratiquée en deux temps et en respetant les limites de la tumeur. Une greffe libre de péroné a été utilisée, tandis qu'une arthrodèse du poignet et la fixation interne de la greffe par système AO ont été réalisées. La consolidation de la greffe et le retour de la patiente à la vie quotidiene sont réussies. Le diagnostic, le traitement, le suivi, la rééducation et le pronostic de ce cas sont exposés(AU)
